We made use of a neurosurgical care unit(NCN) from May 1986. The NCU differs from the intensive care unit (ICU) in several points. The NCU has 16 beds for neurosurgical patients who require continuous observation while the intensive therapy is not likely to be required. Wall units for oxygen and suction are equipped for each bed. One of the patient's family or relatives could attend the patient in the NCU. Neurosurgical nurse education has been offered monthly according to a scheduled curriculum to enhance an ability of neurosurgical close observation. The NCU has several unique advantages. The NCU can solve the bed availability problem caused by not only increased requirement for beds but also patients who were stabilized but still dependant or had some risk of sudden deterioration. Patients in the NCU require more obsevation than therapy. Thus, the NCU requires less equipments than the ICU, so it is cheap to set up. Regular neurosurgical nurse education fills up the efficiency of the NCU. Attendance of the family not only adds man-power but also provides a skinship and more intimate therapeutic environment. It also potentiates ability to care at home by their family. Our model of NCU has brought so significant advantages that we may advocate the efficacy of such an intermediate care facility, and present here our model of NCU.
[$Ca^{2+}-activated\;K^+$ Currents of Pancreatic Duct Cells in Guinea-pig

There are numerous studies on transepithelial transports in duct cells including $Cl^-$ and/or $HCO_3^-$. However, studies on transepithelial $K^+$ transport of normal duct cells in exocrine glands are scarce. In the present study, we examined the characteristics of $K^+$ currents in single duct cells isolated from guinea pig pancreas, using a whole-cell patch clamp technique. Both $Cl^-$ and $K^+$ conductance were found with KCI rich pipette solutions. When the bath solution was changed to low $Cl^-$, reversal potentials shifted to the negative side, $-75{\pm}4\;mV$, suggesting that this current is dominantly selective to $K^+$. We then characterized this outward rectifying $K^+$ current and examined its $Ca^{2+}$ dependency. The $K^+$ currents were activated by intracellular $Ca^{2+}$. 100 nM or 500 nM $Ca^{2+}$ in pipette significantly (P<0.05) increased outward currents (currents were normalized, $76.8{\pm}7.9\;pA$, n=4 or $107.9{\pm}35.5\;pA$, n=6) at +100 mV membrane potential, compared to those with 0 nM $Ca^{2+}$ in pipette $(27.8{\pm}3.7\;pA,\;n=6)$. We next examined whether this $K^+$ current, recorded with 100 nM $Ca^{2+}$ in pipette, was inhibited by various inhibitors, including $Ba^{2+}$, TEA and iberiotoxin. The currents were inhibited by $40.4{\pm}%$ (n=3), $87.0{\pm}%$ (n=5) and $82.5{\pm}%$ (n=9) by 1 mM $Ba^{2+}$, 5 mM TEA and 100 nM iberiotoxin, respectively. Particularly, an almost complete inhibition of the current by 100 nM iberiotoxin further confirmed that this current was activated by intracellular $Ca^{2+}$. The $K^+$ current may play a role in secretory process, slnce recycling of $K^+$ is critical for the initiation and sustaining of $CI^-$ or $HCO_3^-$ secretion in these cells.

Eight compounds, squalene (1), friedelin (2), b-sitosterol (3), b-sitosterol-3-O-glucoside (4), a-tocopherol (5), betulinic acid (6), trilinolein (7) and 1-O-(9Z,12Z-Octadecadienoyl)-3-nonadecanoyl glycerol (8), were isolated from the barks of Tilia amurensis. Their chemical structures were identified by comparing their physicochemical and spectral data with those published in the literature. These isolated compounds were examined for their inhibitory activities against topoisomerase I and II. Compound 7 showed significant inhibition of DNA topoisomerase I and II activities, with percent decreases in activity of 87 and 95%, respectively at a concentration of 100 mM. Compound 6 exhibited cytotoxicity against the human colon adenocarcinoma cell line (HT-29), the human breast adenocarcinoma cell line (MCF-7) and the human liver hepatoblastoma cell line (HepG-2), with IC50 values of 20, 59 and 16 mM, respectively.

Recently, DDS (Distributed Database System) has been often implemented on VAN (Value Added Network) as we know the amazing expansion of information network. DDS can yield significant cost and response time advantages over centralized systems for geographically distributed organizations. However, inappropriate design can result in high cost and poor response time. In a DDS design, the main problem is 1) how to select proper computer, and 2) how to allocate data fragment into proper nodes. This paper addresses DDS design problem of selecting the proper class of computers and the allocating data files on VAN. Also, the formulated model includes two objectives, the operating and investment cost. GA (Genetic Algorithm) is developed to solve this mathematical formulation. A numerical experiment shows that the proposed method arrives at a good solution.

The purpose of this study was to confirm whether the urban regeneration area coincides with the indigenous local community area. This study used the urban area community detection method based on the urban spatial network to derive the boundaries of the local community area in Seoul City and confirmed the geographical relationship with the urban regeneration area currently underway in Seoul. As a result of applying the above method to Seoul City, inconsistent urban regeneration zones with 10 local communities out of a total of 52 urban regeneration zones were found. In regeneration planning for these urban regeneration areas, it is necessary to plan to properly harmonize the local characteristics of the surrounding areas.
Risk factors for high neonatal serum magnesium concentrations in prenatal maternal magnesium therapy

Objective: The main purpose of this study is to analyze the risk factors for high neonatal serum magnesium concentrations in prenatal maternal magnesium therapy. Methods: This was a retrospective study that employed the electronic medical record systems of seven tertiary care centers under the College of Medicine, the Catholic University of Korea, from January 2009 to March 2021. This study was approved to use registry data and documentation of informed consent was waived by our Institutional Review Board (XC20WIDI0103/2020-2158-0020). Mothers who received antenatal magnesium sulfate for neuro-protection or eclampsia prophylaxis were included. This study evaluated mother-neonate pairs with each having one documented magnesium concentration measured from the blood. Results: A total of 877 mother -neonate pairs of magnesium concentration were obtained. Mothers of neonates with high neonatal magnesium concentrations (>4.5mg/dL) had high maternal magnesium concentrations, lower GFR, lower gestational age at delivery, and were more like to have preeclampsia. Conclusion: We have identified several predictors of neonatal serum magnesium concentrations that can guide maternal magnesium dose.

Senescence marker protein 30 (SMP30) is identified as an important aging marker molecule and known to play multifunctional roles as an intracellular calcium regulatory protein in the signaling process. To elucidate the functional significance of SMP30, we established the stably transfected P19 cell line with SMP30 expression vector. Overexpression of SMP30 slightly suppressed the proliferation of P19 cells. However, SMP30 overexpression was cytoprotective against calcium-mediated stress such as calcium ionophore (A23187), and thapsigargin. We found that SMP30 overexpression reduced the elevated intracellular calcium levels induced by A23187, but not by thapsigargin. In addition, SMP30 transfected P19 cells were more protective to tert-butylhydroperoxide induced cytotoxicity, indicating the antioxidative properties of SMP30. Taken together, our results suggest that external calcium regulation and antioxidant properties are involved in the cytoprotective mechanism of SMP30.
