Discordance in Spirometric Interpretations Based on Korean and Non-Korean Reference Equations

Objectives: Korean regression models for spirometric reference values are different from those of other ethnic groups. The purpose of this study was to evaluate discordance in spirometric interpretations based on using Korean reference equations versus non-Korean reference equations. Methods: Spirometry was performed on 825 Korean male workers from April 2009 to November 2011. The spirometric patterns and disease severity were evaluated using two Korean equations (Choi’s and Lee’s) and three equations for Caucasians (NHANES III, Morris’s, and Knudson’s), and the results of Choi’s equation were compared with the non-Korean equations. The spirometric patterns were defined as normal, restrictive, and mild and moderate obstructive. Results: The mean differences in the FEV1% and FVC% between the two Korean equations were 2.0 ± 1.3% and 3.5 ± 2.2%, respectively. Morris’s equation had the greatest difference in the FEV1% from Choi’s equation: 32.9 ± 8.5%. Knudson’s equation had the greatest difference in the FVC% from Choi’s equation: 10.5 ± 6.5%. Conclusions: The two Korean equations shared similar characteristics of spirometric interpretation. The spirometric interpretations of Choi’s equation were significantly different from those of Morris’s equation and Knudson’s equation.

문제: 조직병리검사의 처방이 복잡하고 전문화됨에 따라 오류처방이 빈번하게 발생되고 이를 해결하기위한 프로세스가 복잡하여 부서간의 갈등과 업무의 가중, 또한 병원 수입 감소가 초래되었다. 목적: 발생된 오류처방의 유형과 발생율, 오류처방을 해결하기 위한 프로세스를 파악하여 이에 대한 문제점을 개선하여 업무의 효율성과 관련부서간의 부가업무 감소와 갈등해소, 더불어 병원수익을 향상시키는 것에 목표를 두고 활동하였다. 의료기관: 서울시에 소재한 종합병원 질 향상 활동: 조직병리검사 처방의 오류처방 발생 원인과 유형, 발생율을 조사하여 원인을 파악하고 개선을 위한 프로세스를 구축하여 질 향상을 도모하였다. 개선효과: 수술실에서 의뢰한 외과 입원환자의 일반조직병리검사 오류율은 개선 전 7.8%에서 개선 후 3.5%, 동결절편조직검사 오류율은 개선 전 28.6%에서 개선 후 0%로 이전보다 모두 개선되었다. 또한 수술실 간호사의 업무 만족도는 일반조직병리검사의 처방시 95%, 동결절편조직검사 처방시 90%로 높은 만족도를 보였다. 단 병리과의 경우 업무가 증가됨에 따라 초과근무시간이 증가하였다.
혼합액정(80CB+50CB)의 네마틱-스멕틱 A 상전이 근처의 네마틱상에서 휨 탄성상수(K₃)에 관한 광산란 실험연구

액정 8OCB(octyloxycyanobiphenyl)에 액정 5OCB(pentyloxycyanobiphenyl)를 무게비로 혼합하여 혼합액정 (8OCB+5OCB)을 만들었다. 이 혼합액정은 5OCB의 무게비에 따라 McMillan지수 (??)가 감소하였다. 그리고 Rayleigh 광산란 실험방법을 이용하여, 순수 8OCB와 혼합액정의 네마틱-스멕틱 A 상전이 근처의 네마틱상에서 산란된 빛의 세기를 온도와 함수로 측정하였다. 실험데이타 분석결과는 휨(bend) 탄성 상수에 비례하는 세로방향 상관거리(??)는 McMillan 지수에 관계없이 임계지수 ??=0.66±0.03을 갖고 발산하였다. 이 값은 de Gennes의 모델에서 제시된 ??=0.67과 일치함을 알수있다. By utilizing Rayleigh scattering technique, we have measured the temperature dependence of scattered light intensity near nematic to smectic-A phase transition point in the pure 8OCB(octyloxycyanobiphenyl) and its mixtures contaning 5, 10, 15 weight percentage of 5OCB(pentyloxycyanobiphenyl). From analysis of these data. it is found that the longitudinal correlation length (??) diverges with critical exponent ??=0.66±0.03 independent on McMillan parameter (??). This result is consistent with de Gennes model (??).

이 연구의 목적은 지역4-H조직의 내적 적합성을 탐색하고 이를 기술하는데 있었다. 2006년 8월부터 10월까지 약 3개월 동안 지역4-H후원회 회장 및 임원진 3명, 지역4-H연맹 회장 1명, 학교4-H 지도교사 4명, 지역의 영농4-H회장 1명, 시·군 농업기술센터 소장 및 4-H담당자 9명을 주요 연구 참가자로 하여 심층면담을 통해 자료를 수집하였다. 수집한 자료를 분석한 결과는 다음과 같았다. 첫째, 지역4-H조직 관련자들이 인식하는 지역4-H조직의 주요성과 격차로, ① 민간4-H조직을 중심으로 한 자율적이고 독립적인 활동 부재, ② 지역4-H조직 간 다양한 종류의 협력 부재, ③ 지역사회에서 4-H활동에 대한 가치 인식 부족이 나타났다. 둘째, 지역4-H조직의 핵심 과업으로 ① 4-H의 가치 및 이념에 부합하고, 지역의 실정에 맞는 프로그램 확보, ② 지역4-H 조직 간 협력적 네트워크 확립, ③ 4-H에 대한 지역사회의 관심 및 지지 확보가 나타났다. 셋째, 지역4-H조직의 내적 부적합성으로는 ① 인원 확보의 어려움과 핵심과업에 대한 전문성 결여, ② 관료화되고 경직된 낡은 문화, ③ 조직적 자원의 축소 및 조직 간 낮은 네트워크의 수준이 도출되었다. The purpose of this study was to describe and understand internal fit in Local 4-H Organization. The main participants of this study were one leader and two members of Local 4-H Society for the support, one leader of Local 4-H Association, four School 4-H teacher, one leader of Local farm 4-H, 9 local agricultural extension workers. The data were collected through in-depth interview with them from August, 2006 to October, 2006. The Results from this study were as follow: 1. Key performance gaps of local 4-H organizations were identified: ① lack of local 4-H based activities, ② lack of various types of partnership between local 4-H organizations, ③ lack of understanding value of 4-H activities in the communities. 2. Key tasks of local 4-H organizations were identified: ① developing local 4-H programs based on 4-H's values and ideas, ② establishing the cooperative networks among local 4-H organizations, ③ ensuring the interest and support about 4-H in the communities. 3. Internal misalignments of local 4-H organizations were identified: ① difference of recruitment and lack of expertise, ② bureaucratic and rigid cultures, ③ decrease in organizational support and low level of network among organizations.

Cardiac tumors, especially the primary tumors involving any part of the heart are extremely rare and its relative incidence has been reported to be approximately 0.02%. We report a patient with huge cardiac hemangioma who complained of shortness of breath, general weakness, and dizziness, Imaging study by echocardiography and computed tomography of thorax revealed a huge lobulating mass like a bunch of grapes in the right chamber of heart. The surgical exploration of thorax was performed and a histological diagnosis of spindle cell hemangioma was obtained by microscopy. The patient was treated by surgical resection of the tumor and doing well after surgery. Our experience indicated that prompt diagnosis and treatment of cardiac hemangioma is imperative for patients' prognosis.
M1 and M2 polarization of macrophages: a mini-review

The tissue microenvironment regulates differentiation of precursor monocytes into macrophages. In response to the presence of microbial products, stimulated lymphocytes, or damaged cells in the microenvironment, macrophages differentiate into distinct functional populations. The M1 macrophages are characterized by the generation of high levels of pro-inflammatory cytokines, antimicrobial properties, increased production of reactive nitrogen and oxygen intermediates, and induction of Th1 response. In contrast, M2 macrophages are characterized by their involvement in tissue remodeling, immune regulation, tumor promotion, and efficient phagocytosis. In this minireview, we discuss the stimulation, markers, cytokines, and signaling molecules involved in macrophage polarization.
Synergistic Induction of iNOS by IFN-g and Glycoprotein Isolated from Dioscorea batatas

Dioscorea species continue to be used in traditional Chinese medicine, and represent a major source of steroid precursors for conventional medicine. In the previous study, We isolated glycoprotein (GDB) from Dioscorea batatas, characterized, and demonstrated immunostimulating activity in C57BL/6 mice. The aim of this study was to investigate the mechanism whereby GDB activates macrophages. Macrophages activation by GDB was investigated by analyzing the effects of GDB on nitric oxide (NO) production, iNOS expression, mitogen activated protein kinase (MAPK) phosphorylation, and transcription factor activation. In the presence of IFN-Ճ, GDB strongly stimulated macrophages to express iNOS and produce NO. Furthermore, the activation of p38 was synergistically induced by GDB plus IFN-Ճ, but SB203580 (a p38 inhibitor) inhibited GDB plus IFN-Ճ-induced p38 activation. This study indicates that GDB is an important activator of macrophages. Furthermore, due to the critical role that macrophage activation plays in innate immune response, the activation effects of GDB on macrophages suggest that GDB may be a useful immunopotentiating agent.
