        매복된 상악 중절치의 교정적 견인을 이용한 치험례 : CASE REPORT

        치아 매복이란 여러 가지 이유로 구강 점막이나 악골내에서 치아가 맹출하지 못하고 있는 상태를 의미한다. 상악 중절치 매복의 원인은 치아종, 과잉치, 공간의 상실, 선행유치의 만기잔존 또는 조기상실, 선행유치의 외상으로 인한 치관이나 치근의 기형, 치배의 이소위치 등이 있다. 매복된 상악 중절치의 경우, 측절치가 빠르게 근심으로 이동하여 공간을 상실하고, 정중선의 변이가 발생하며, 낭종을 형성할 가능성도 있다. 따라서, 매복치의 조기 진단과 그에 따른 적절한 처치가 즉각적으로 이루어져야 한다. 일반적으로 매복의 정도가 심하지 않거나, 각화조직에 의해 매복이 초래된 경우 외과적인 노출만으로도 맹출을 유도할 수 있지만, 외과적인 노출후에 일정기간의 관찰에도 맹출이 되지 않는다거나, 치아의 맹출 방향이 자가 교정될수 없을 정도로 심하게 변위되어 있는 경우, 매복의 위치가 너무 심부에 있는 경우에는 교정적 견인을 시도하는 것이 바람직하다. 본 증례는 매복된 상악 중절치 중 일정기간의 주기적 관찰후 맹출이 기대되지 않았기에, 교정적으로 견인하여 치료하였으며, 치료후 다소의 지견을 얻었기에 보고하는 바이다. A tooth impaction means a state that a tooth does not erupt out of oral mucosa or alveolar bone for many reasons. The reasons for an impaction of the Maxillary central incisor are an odontoma, supernumerary tooth, space loss, prolonged remaining or early loss of a preceding deciduous tooth, abnormalities of crown or root caused by trauma of a deciduous tooth and an ectopic position of a tooth germ. In the case of the impacted maxillary incisor, a rapid mesial movement of a lateral incisor leads a space loss and a midline deviation can be happened. Furthermore, it can cause a cyst. When we treated a patient with an impacted central incisor early, we could see a better prognosis. It means an early diagnosis and an exact treatment are very important. Generally if the impaction is not severe or it is caused by a keratinized covering tissue, a surgical exposure can induce an eruption easily but an orthodontic force is recommended when an eruption does not happen after a surgical method, when the eruption path is too transpositioned to be corrected spontaneously and when an impacted tooth is located so deeply. In the treatment using an orthodontic force, careful considerations about a root length, pulp, and a periodontal tissue can improve the periodontal and esthetic prognosis for the long follow-up results. This case is using an orthodontic traction following a periodic observation and in no expectation of spontaneous eruption. After treatment of this case, I have got some knowledges, so I report this case.

        The purpose of this study was to analyze the competition techniques traits and of international contest levels each category individual scoring - losing techniques of world elite judokas who. were awarded in the international judo tournaments (I,J.T.) (part 1: ∼O.G'%, part 2 :from O.G'% to July ,2000), and who are expected to participate in the 27th Olympic Games(O.G.) Sydney 2000 to prepare the 27th O.G, which will be held from 16. to 22 September, 2000 at Exhibition Center, Darling Harbour, in Sydney, Australia. The records of the results and contents of competition were obtained out of 97 for women I.J,T., world elite judokas. To decide individual contest levels, groups were devided into 3(A,B,C)groups and points were graded by 3 ways. The I.J.T. that were devided into 3 groups are shown in the table 1.

        Mouse의 치아 발육시 Runx2의 발현 양상

        Runx2는 runt gene family에 속하는 전사조절 인자로써 뼈의 형성과 골아세포의 분화에 중요한 역할을 담당하고 있다. Runx2-haploinsufficency는 쇄골의 저형성 및 두개 봉합의 지연을 특징으로 하는 쇄골두개 이형성증을 일으키며, 치아에 있어서는 법랑질의 저형성, 영구치 맹출지연 등을 보인다. 이에, 치아의 발육 및 맹출에 미치는 Runx2의 영향을 알아보기 위해 in situ hybridization 방법으로 태생 1, 4, 7, 14. 21일 된 쥐의 하악 및 제1대구치를 사용하여 실험을 실시하였다. Runx2-full length는 태생 1일과 4일에 치낭 및 주위조직에 보이지만 Runx2-typeⅡ는 보이지 않았다. Runx2-full length는 태생 7일에 치관 교합면 부위의 법랑모세포에 발현하였고, 1주일 후인 태생 14일에는 백악법랑경계 상방의 치관인 접면 법랑모세포에서 발현되었다. 이에 반해 Runx2-typeⅡ는 법랑모세포에서 발현하지 않았다. 또한 태생 21일에서는 두가지 이성질체가 모두 하악골에서 발현을 보였다. 이런 결과를 종합해볼 때, Runx2-full length는 치아의 맹출과 연관이 있으며, 법랑모세포의 분화 및 이로 인한 법랑질형성에 영향을 주지만 Runx2-typeⅡ는 하악골의 형성에만 영향을 미치는 것으로 사료된다. Runx2 is a transcription factor in homologous with Drosophila runt gene and it is essential for bone formation during embryogenesis and a critical gene for osteoblast differentiation and osteoblast function. Runx2-haploinsufficency causes cleidocranial dysplasia (CCD). CCD is an autosomal-dominant inherited disorder characterized by hypoplastic clevicle and delayed ossification in fontanelles and wormian bones. Dental defects are possibly shown to CCD patients : multiple supernumerary teeth, irregular and compressed permanent tooth crowns, hypoplastic and hypomineralized defects in enamel and dentin, an excess of epithelial root remnants, the absence of cellular cementum, and abnormally shaped roots. In addition, delayed eruption of the secondary dentition is a constant finding. The aim of this study is to evaluate the role of Runx2 in the tooth development and eruption through analyzing the expression pattern of Runx2 by in situ hybridization during crown (late bell stage) and root formation of tooth, using postnatal day 1, 4, 7, 14 and 21 mice mandibular molar teeth. mRNA of Runx2-full length is expressed in dental follicle and surrounding tissue at postnatal day1 and 4. At post-natal day 7, it is expressed in ameloblasts of occlusal surface of enamel and bone area surrounding the tooth. In comparison with previous stage, at postnatal day 14, it is expressed in ameloblasts of proximal surface of enamel. At postnatal day 21 it's expression is observed only in bone area. mRNA of Runx2-typeⅡ is not expressed At postnatal day 1 and 7. At postnatal day 14 and 21, it's expression is observed in the bone area. In this study, we suggest that Runx2 have a relation of ameloblasts differentiation and an important role to tooth eruption made by dental follicle during intraosseous eruption stage. Also we can confirm that Runx2 has a role to bone formation.

        In order to establish the optimum process parameters and diamond saw blade composition for machining natural stone, the chip formation process and the wear process must be understood. Diamond saw blade is one of the most effective, versatile, and extensively used methods of processing rock and other hard materials, such as granite, marble, concrete and asphalt. For many years, it has been known that chip thickness is one of the most significant in the understanding of the sawing process, and other variables such as force and power have been correlated with it. In this study, mathematical relations of a material chipped by a single grit of the saw blade will be undertaken. The material chipping geometries have been mathematically defined and derived as maximum chip thickness.

        The purpose of this study was to reform practically a Korean Judo's Doechigi-Bon(Forms of Counterattack, judo's Gaeshi no Kata, Judo's Uradori no Kata, Judo's Gonosen no Kata) that was established in 1955 Korea, according to changing of techniques by Judo's modernization, in order to have Judo's carefulness and systematic diffusion.       • 산지하천에서 빛과 초식에 의한 부착조류의 생체량변화

        빛과 초식에 의한 부착조류의 생체량 변화에 대한 영향을 평가하기 위하여 자연기질과 인공기질(유약을 칠하지 않은 타일: 3.7×9.5×2cm)을 이용하여 부분적으로 음지화된 2차 산지하천에서 실험하였다. Open canopy(상대적 빛 투과율: 20∼50%, chl. a: 9.5±3.0㎎/㎡, n=25)에서는 closed canopy(<6% 상대적 빛 투과율, 3.2±0.9㎎/㎡, n=17)보다 높은 부착조류 생체량을 유지하였다. 하천 주변식생의 잎 성숙과 잦은 강우에 의해서 여름의 부착조류 생체량은 봄과 겨울에 비하여 낮았다. 봄에 인공기질에 부착조류가 부착하는 속도는 겨울보다 빨랐으며 겨울에는 자연기질 수준(8∼9㎎/㎡)에 이르기 까지 최소 5∼6주 정도 소요되었다. 초식자인 다슬기는 출입을 제한한 실험구에서는 출입이 허용된 대조구보다 약간 높은 생체량(AFDW, chl. a)을 보였다. 결론적으로 빛이 부착조류 생체량에 증가에 미치는 영향은 초식 보다는 더 큰 것으로 사료된다. Effects of light and grazing on the biomass development of periphyton were evaluated using natural and artificial substrata (unglazed tile: 3.7×9.5×2cm) in a partially shaded 2nd order mountain stream. In open canopy (relative light input 20∼50%), a relatively high biomass (chl. a: 9.5±3.0㎎/㎡, n=25) was maintained than that of closed canopy (<6% of light input, 3.2±0.9㎎/㎡, n=17). Due to shading and frequent rainfalls, biomass was lower during the summer than in the spring and winter. During the spring, colonization of periphyton on artificial substrata occurred faster than in winter and it took a minimum of 5∼6 weeks to reach the biomass of natural substrata (8∼9㎎/㎡). When snails were allowed to freely enter the chamber (mean density: 114 ind./㎡) with artificial substrata, biomass accumulation (chl. a, AFDW) was low compared to the snail excluded enclosure (50∼90% of ungrazed substrata). It was concluded that the light input on periphyton biomass in a partially shaded stream played a more important role than grazing.

      • 서울지역 여대생들의 떡에 대한 인식 및 섭취 실태 조사

        This study conducted a survey of the improvement and popularization for Korea Rice Cakes, a Korean traditional food, on two hundred women students in five colleges in Seoul, Korea. It was investigated to get the information of the recognition, preference, and consumption patterns for the succession and improvement of Korean Rice Cakes. The results are as follows: 1. A portion of 41.5% among responders knows from three to six kinds of Korean Rice Cakes, and of 28% can make at least one among the kinds of Korean Rice Cakes. 2. A portion of 91% recognizes that Korean Rice Cakes are one of well-nourished food, but of 42% answers that Korean Rice Cakes cannot be replaced with the principle food. Also, there are no significant differences in the recognition of Korean Rice Cakes as the kinds of religions. 3. The frequency of purchase is about one or twice in a month (35.5%). Consumers (36.5%) intend to purchase Korean Rice Cakes for the commemoration of national holidays or anniversaries. 4. The reasons for the preference of other foods more than Korean Rice Cakes are the easiness of purchase (36.0%), the deliciousness (21.5%), the diverse kindness (17.5%), and the proper package (9.5%). It is important that constant and particular school / home education is need to improve and success Korean Rice Cakes.

        외과적 노출술을 이용한 매복된 하악 제 1 대구치의 자발적 맹출 유도

        하악 제 1대구치 매복의 발생 빈도는 전체 인구의 0.01%로 드물게 나타난다. 매복의 원인으로는 맹출 공간의 부족, 과잉치, 치성 종양이나 낭과 같은 맹출로상의 장애물, 유전적 요인, 내분비 질환, 외상 등이 있다. 하악 제 1대구치의 매복으로 인해 하안면 고경의 감소, 낭의 형성, 치관 주위염, 인접치의 치근 흡수, 부정 교합 등의 부작용이 생길 수 있으며, 치료 방법으로는 외과적 노출술, 교정적 견인, 외과적 재위치술, 발거 등이 있다. 다음의 두 증례는 하악 제 1대구치의 미맹출을 주소로 내원한 환아에게 외과적 노출술을 시행하여 원심 경사 매복된 하악 제 1대구치의 자발적 맹출을 유도한 증례들이다. Impaction of mandibular first molar is relatively rare and its overall frequency has been reported to be 0.01%. The etiology of impaction are lack of eruption space, physical obstacles such as supernumerary teeth, odontomas or odontogenic tumors, hereditary factors, functional disturbances of endocrine glands and traumas. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The treatment options available for impacted teeth include surgical exposure, orthodontic forced eruption, surgical repositioning and surgical removal of unerupted molar. This report presents two cases of distally tilted and impacted mandibular first molars which were treated by surgical exposure. In these cases, we could observe spontaneous eruption of the impacted mandibular first molars after surgical exposure.

        출생후 흰쥐 구치의 발육에 따른 치수의 신경지배 발달에 관한 면역조직화학적 연구

        The purpose of this study was to investigate the postnatal development of pulpal innervation with the tooth development and eruption process in developing rat mandibular molars (postnatal 1, 5, 10, 15, 20, 25, 35day-old and adult rats). Immunohistochemical method was carried out for the detection of nerve fibers, using the antibody against calcitonin gene-related peptide(CGRP). The results were as follows: The feature of CGRP-IR nerve fibers were shown in a bead-like appearance. The time of nerve entering into the dental papilla of tooth follicle began at the occured advanced dentinogenesis. The development of Raschkow plexus began at the formative stage of the roots and was accelerated at the stage of the crown emerged into the oral cavity. The number of nerve fibers entering the odontoblastic layer increased with the tooth eruption and mastication. The development of innervation was shown to be related with the stage of the development of individual teeth rather than the chronological age of the rat.

        Lidocaine, Thrombin, Epinephrine 의 항균효과

        목적 : 기관지내시경 검사에서 흔히 사용되는 국소마취제인 lidocaine과 내시경시 지혈목적으로 사용되는 thrombin과 epinephrine이 각종 균주에 미치는 항균효과에 대해 알아보고자 하였다. 재료 및 방법 : 균주는 가톨릭대학교 성모병원에서 2004년 3월부터 2004년 9월까지 임상검체 에서 동정된 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 각각 42, 42, 42, 43주를 대상으로 하였다. Lidocaine, thrombin, epinephrine 감수성 검사는 National Committee for Clinical Laboratory Standards (NCCLS, 2002)의 기준에 따랐다. 결과 : Lidocaine은 S. aureus, S. pneumoniae, P. aeruginosa에서 MIC_(50), MIC_(90) 모두 20,000 ㎍/mL 이었다. K. pneumoniae는 각각 10,000 ㎍/mL이었다. Thrombin은 S. aureus와 P. aeruginosa에서 MIC50 500 lU/mL 과 MIC_(90) 500 IU/mL 이상이었고, K. pneumoniae에서는 MIC_(50)과 MIC_(90)이 모두 500 lU/mL이상이었으나 S. pneumoniae에서는 MIC_(50)과 MIC_(90)은 125 IU/mL이었다. Epinephrine은 K. pneumoniae, S. pneumoniae에 대한 MIC_(50), MIC_(90)가 모두 >500 ㎍/mL이었고, S. aureus와 P. aeruginosa에 대한 MIC_(50), MIC_(90)가 모두 500 ㎍/mL이었다. 결론 : 기관지 내시경 검사에서 흔히 쓰이는 lidocaine, thrombin, epinephrine 등의 약제들이 호흡기 질환의 흔한 병원균인 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 균주들에 대해 항균 효과가 있을 수 있어 세균배양검사에 영향을 미칠 수 있겠다. Background : We performed this stody to find out about antimicrobial effect of lidocaine which is commonly used local anesthetic, and thrombin and epinephrine used for hemostasis during bronchoscopic procedures. Materials and Methods : The microorganisms that were cultured from specimens obtained during bronchoscopy were Staphylococcus aureus (n=42), Streptococcus pneumoniae (n=42), Klebsiella pneumoniae (n=42), and Pseudomonas aeruginosa (n=43) collected from St. Mary's Hospital, from March to Sep 2004 were used for susceptibity testing. Susceptibility to lidocaine, thrombin, and epinephrine were tested according to the National Committee for Clinical Laboratory Standards. Result : MIC_(50) and MIC_(90) of lidocaine for S. aureus, S. pneumoniae, P. aeruginosa were all 20,000 ㎍/mL and that for K. pneumoniae were 10,000 ㎍/mL. MIC_(50) and MIC_(90) of thrombin for both S. aureus and P. aeruginosa was 500 IU/mL and above 500 IU/mL, respectively; that for K. pneumoniae were all above 500 IU/mL and for S. pneumoniae they were 125 IU/mL, MIC_(50) and MIC_(90) of epinephrine for K. pneumoniae and S. pneumoniae were above 500 ㎍/mL; that for S. aureus and P. aeruginosa were 500 ㎍/mL. Conclusion : We observed possible antimicrobial effect of lidocaine, thrombin, and epinephrine in vitro against pathogens such as S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa, which are common respiratory microorganisms. The use of these agants could affect the result of bacterial culture.
