        Tidal currents had been reduced remarkably since the constructions of Youngsan River Seadike and Youngam/Keumho Seawalls at the semi-enclosed Mokpo coastal zone. This deteriorates the water quality of the zone seriously, because reduced currents prevent pollutants flushing out or diluting easily. Three cases are simulated numerically with HD and AD modules of the MIKE21 software; continuously pollutants discharging case at Mokpo Harbor, intermittent discharging case at the gate of the Seadike, and continuously pollutants discharging case at Chungkye Bay. The results of all the cases show that the majority of the pollutants does not flush out but remains at Mokpo coastal zone. Especially, the release from gates of the Seadike may cause that fresh water concentration prevails near Mokpo Harbor.

        Streptococcus에 대한 chlorhexidine dihydrochloride와 불화나트륨의 효과

        Chlorhexidine dihydrochloride와 불화나트륨은 구강 내 세균을 억제하고 치태의 형성을 억제하기 위하여 많이 사용되고 있는 물질이다. 그러나 구강에는 여러 종류의 세균들이 상주하며 이들 물질에 대한 감수성도 다르다. 본 연구에서는 chlorhexidine dihydrochloride와 불화나트륨에 의해 Streptococcus mutans의 인공치태 형성이 억제되었을 때 Streptococcus mutans, Streptococcus sobrinus, Streptococcus oralis, Streptococcus salivarius의 증식에 미치는 이들 물질의 영향을 관찰하여 다음과 같은 결과를 얻었다. Streptococcus mutans를 8시간 배양하면 와이어에 형성된 인공치태 무게는 106.1±18.1㎎이었으나, 1.0μM chlorhexidine dihydrochloride를 첨가하면 5.1±1.5㎎으로 감소되었다(p<0.05). 이 때 배양액의 흡광도도 감소되었다. Streptococcus sobrinus의 배양액 흡광도는 감소되었으나 Streptococcus oralis에서는 감소되지 않았고 Streptococcus salivarius에서는 배양 8시간에 감소되었다가 배양 24시간에 대조군과 차이가 없었다. 배지에 3.0 mM 불화나트륨을 첨가하여 Streptococcus mutans를 8시간 배양하면 와이어에 인공치태 무게는 26.7±8.3㎎으로 감소되었다(p<0.05). 이 때 배양액의 흡광도도 감소되었다. Streptococcus sobrinus와 Streptococcus oralis에서의 배양액의 흡광도도 감소되었으나 Streptococcus salivarius에서는 거의 감소되지 않았다. 이상의 결과는 Streptococcus mutans의 인공치태 형성을 억제하는 chlorhexidine dihydrochloride와 불화나트륨 농도에서 Streptococcus의 여러 종의 감수성이 각각 다르다는 것을 시사하였다. Chlorhexidine dihydrochloride and sodium fluoride have been used as agents inhibiting the replication of oral bacteria and the formation of dental plaque. There are various kinds of bacteria with different sensitivity against these agents. In this study, chlorhexidine dihydrochloride and sodium fluoride were studied about their effects on the replication of Streptococcus mutans, Streptococcus sobrinus, Streptococcus oralis and Streptococcus salivarius at their concentrations inhibiting the formation of artificial plaque by Streptococcus mutans. When Streptococcus mutans was cultured for 8 hours in the media added with l.0μM chlorhexidine dihydrochloride, the weight of formed artificial plaque was decreased to 5.1 ±1.5mg compared with 106.1±18.1mg of the control(p〈0.05). At the same time, the optical density of cultured media was decreased. The optical density of cultured media was slightly decreased in Streptococcus sobrinus. but was not decreased in Streptococcus oralis. The optical density of Streptococcus salivarius was decreased at 8 hours-incubation, was not decreased at 24 hours-incubation. When Streptococcus mutans was cultured for 8 hours in the media added with 3.0 mM sodium fluoride. the weight of formed artificial plaque was decreased to 26.7i8.3 mg(p〈0.05). At the same time. the optical density of cultured media was decreased. The optical density of cultured media was decreased in Streptococcus sobrinus and Streptococcus oralis, but was slightly decreased in Streptococcus salivarius. These results suggest that at the concentration of chlorhexidine dihydrochloride and sodium fluoridehe inhibiting the formation of artificial plaque by Streptococcus mutans, different species of genus Streptococcus show the different sensitivity against these agents.

        Tidal characteristics have changed severely after constructions of seadike/seawalls near Mokpo Harbor. Tidal velocity of three layers, buoy paths and suspended sediment distributions are observed near there. The results can be useful database for calibrations of various numerical simulations for coastal and estuarine studies near Mokpo coastal zone.

      • 해수유통시설에 따른 대흑산도항 오염확산양상 변화

        This study was performed to predict some aspects of tidal currents and pollutant dispersion according to the facilities for seawater exchange in order to improve the water quality in Daeheuksando. A numerical simulation was executed, using the hydrodynamic module and advection dispersion module in MIKE 21 program. As a result of hydrodynamic simulation, it did not show change greatly in all cases. But, cutting part of Jungang-breakwater increased greatly than other area. When cut inside part in Jungang-breakwater, pollutant density of Yeri area decreased greatly by the ratio of 0.40 as compared with other cases. Also, dispersion extent diffused whole area of inside sea area of Daeheuksando. This case is the most effective for seawater exchange inside sea area of Jungang-breakwater.

        Muan coastal zone consists of Hampyong bay, Tando bay and Chungkye bay, each shows unique tidal current characteristics. Tidal amplification by constructions of the sea-dike and sea-walls has an effect on Chungkye bay. This brings about increase of tidal flat area, which makes the ebb dominance at Chungkye bay more seriously. This study was performed to investigate the environmental changes by tidal current characteristics at Muan coastal zone.

        Multi‑tissue lipotoxicity caused by high‑fat diet feeding is attenuated by the supplementation of Korean red ginseng in mice

        Background Excessive intake of fat, one of the causes of obesity, is associated with low-grade infammation in various susceptible organs and eventually causes tissue toxicity. This study examines the multifaceted suppressive efects of Korean red ginseng extract (KRG) on high-fat diet (HFD)-induced lipotoxicity and infammatory responses in the aorta, liver, and brain. Methods Male C57BL/6 mice were fed HFD with or without KRG for 12 weeks. The improvement efect in KRG on lipotoxicity and infammatory potential was determined in the blood and the aorta, liver, and brain tissues. Results KRG signifcantly inhibited 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase activity by >20% in vitro. KRG supplementation suppressed HFD-associated body weight gain, lipid profle changes, and excessive fat deposition in the liver and increased leptin, insulin, and ALT levels in the blood. Infammatory markers in the aorta, liver, and brain were also signifcantly reduced by KRG treatment. In microvascular endothelial cells, the 15% cyclic stretch-mediated upregulation of ICAM-1 and vascular cell adhesion protein-1 (VCAM-1) expression was signifcantly attenuated in the presence of KRG. Conclusion KRG supplementation attenuates HFD-mediated body weight gain, lipid profle changes, and multi-tissue infammatory responses.

        Streptococcus의 유당분해에 대한 자일리톨의 효과

        자일리톨은 탄소 5개가 있는 탄수화물로 치아우식증을 억제할 목적으로 사용되는 자당 대체물이다. 본 논문은 구강 중요 세균인 Streptococcus mutans, Streptococcus oralis, Streptococcus salivarius에서 유당분해에 대한 자일리톨의 작용을 관찰하고자 하였다. 세균에 대한 유당과 자일리톨의 병합효과를 보기 위하여 분광광도계를 이용한 흡광도를 측정하였고 생균수 검사를 실시하였다. Streptococcus mutans 또는 β-galactosidase에 의한 유당분해에 대한 자일리톨 효과를 보기 위하여 thin layer chromatography와 lactose-PTS activity test를 실시하여 다음과 같은 결과를 얻었다. 1. 유당과 자일리톨을 병합 첨가한 배지에 Streptococcus mutans를 배양하면 배양액 흡광도가 배양 8시간에 증가하지 않다가 배양 24시간에 증가하였다. 배양 8시간에서의 생균수도 유당 단독보다 유당과 자일리톨 병합 첨가 시 적게 나타났다. 2. 유당과 자일리톨을 병합 첨가한 배지에 Streptococcus oralis, Streptococcus salivarius를 배양하면 배양액 흡광도가 배양 8시간에 증가하지 않다가 배양 24시간에 증가하였다. 3. 배지에 유당 단독으로 첨가할 때보다도 유당과 자일리톨을 병합 첨가한 배지에 Streptococcus mutans를 8시간 배양하면 thin layer chromatography상 남아있는 유당이 많았으나, 배양 24시간에는 모든 유당이 분해되었다. 4. 배지에 유당 단독으로 첨가할 때보다도 유당과 자일리톨을 병합 첨가한 배지에서 Streptococcus mutans의 lactose-PTS 활성도가 낮았다. 5. 배지에 유당 단독으로 첨가할 때보다도 유당과 자일리톨을 병합 첨가한 배지에 β-galactosidase 사용시 thin layer chromatography상 유당이 많았다. 이상의 결과는 자일리톨이 Streptococcus의 유당 이용을 억제함을 시사하였다. Xylitol is a 5-carbons carbohydrate, which can be replaced with sucrose for preventing dental caries. To study the effect of xylitol on the fermentation of lactose in bacteria, the important oral bacteria such as Streptococcus (S.) mutans, S. oralis and S. salivarius were studied. The optical density using spectophotometer and the cell concentration were assessed to evaluate the combined effect of lactose and xylitol against the bacteria. Thin layer chromatography and lactose-PTS activity test were performed to evaluate the effect of xylitol on the fermentation of lactose in S. mutans and by β-galactosidase with the following results. 1. The optical density of Streptococcus mutans culture was not increased for 8 hours-incubation in the media added with lactose and xylitol, but was increased at 24 hours-incubation. The number of viable cells at 8 hours-incubation was smaller in the media containing lactose and xylitol in comparison with lactose only. 2. The optical densities of Streptococcus oralis culture and Streptococcus salivarius culture were not increased for 8 hours-incubation in the media added with lactose and xylitol, but were increased at 24 hours-incubation. 3. When Streptococcus mutans was incubated for 8 hours in the media added with lactose and xylitol. the amount of remained lactose was larger compared with the media added with lactose only. But all lactose was fermented in both media after 24 hours-incubation. 4. When Streptococcus mutans was incubated in the media added with lactose and xylitol, the activity of lactose-PTS was higher compared with the media added with lactose only. 5. When β-galactosidase was incubated in the media added with lactose and xylitol, the amount of remained lactose was larger compared with the media added with lactose only. These results indicated that xylitol inhibited the fermentation of lactose by Streptococcus.

        주의력결핍/과잉행동장애와 양극성장애 공존 환아군과 주의력결핍/과잉행동장애 환아군의 비교연구

        Objective : Attention deficit hyperactivity disorder (ADHD) is frequently comorbid with a variety of psychiatric disorders. Among them, bipolar disorder (BPD) has especially attracted growing interest. This is partly due to the fact that early onset BPD has been frequently misdiagnosed as ADHD because of symptomatic overlap. The aim of the present study is to find the differences in demographic data and clinical features of ADHD+BPD and ADHD in children and adolescents. Method : Children and adolescents patients participating in the present study have been enrolled at the child psychiatric clinic since 2004. 14 patients meeting the DSM-IV chteria for ADHD+BPD and 19 patients meeting the criteria for ADHD and 15 healthy comparison subjects were recruited. All groups were evaluated through the Schedule for Affective disorders and Schizophrenia for School-Age Children-Present and Life Time version-Korean version (K-SADS-PL-K). Demographic data and clinical charactehstics of the subjects were also collected. Parents were asked to complete the Child Behavior Check List(CBCL) and the Toddler Temperamental Scale (TTS) clinical ratings were obtained using the Young Mania Rating Scale(YMRS), the Children's Depression Iventory (CDI) and the Dupaul ADHD scale. Clinical variables between ADHD+BPD and ADHD were analyzed using the Mann-Whitney U test. YMRS, CDI, Dupaul ADHD scale (inattention and hyperactivity), CBCL, and TTS among the three groups were analyzed using the Kruskal-Wallis test with post-hoc Mann-Whitney U test. Results : 1) ADHD+BPD group had an earlier onset age of ADHD than ADHD group 2) ADHD+BPD group had more co-morbid psychiathc disorders than ADHD group. 3) Compared to ADHD group, ADHD+BPD had more psychiathc family history, especially mood disorders. 4) ADHD+BPD group had prodromal symptoms such as irritability, anger dyscontrol and academic dysfunction, compared to ADHD group who rarely showed prodromal symptoms. 5) ADHD+BPD group had higher ADHD scores than the ADHD group in the Dupaul ADHD inattentive scale. 6) In global functions of CBCL, ADHD+BPD group showed more impaired functions at home and school than ADHD group. 7) In Attention diagnostic system (ADS), ADHD+BPD group had more omission and commission errors than ADHD group, especially in the visual system. 8) In the subtests of the IQ profile, ADHD+BPD group had lower arithmetic and block design scores than ADHD group. Conclusion : Clinicians have to rule out underlying bipolarity in children and adolescents with ADHD who show earlier age of onset and severe clinical features. Bipolarity should be explored intensively in ADHD children and adolescents who have early onset of symptoms and severe functional impairment.

      • 비소세포 폐암 환자의 이차 치료로서 Gemcitabine과 Vinorelbine 혼합 요법의 효과

        Background : Both gemcitabine and vinorelbine are effective anticancer drugs with mild toxicity on non-small cell lung cancer, and monotherapy of these drugs are effective as a second-line chemotherapy. The aim of this trial was to assess the response and toxicity of a combination of gemcitabine and vinorelbine in patients of previously treated for non-small cell lung cancer. Materials and Methods : 24 patients, initial stage III A/B,IV and previously treated with platinium and taxane based regimens, were enrolled from June 2000 to March 2004. The regimens consisted of vinorelbine 25mg/㎡ followed by an infusion of gemcitabine 1000mg/㎡ on day 1 and day 8 every three weeks. This course was repeated more than twice. Results : Twenty-four patients were analyzed for the response, survival rate, and toxicities. The overall response was 17% with a complete remission rate of 4%. The median time-to progression (TTP) was 3.1 months (95%, CI l-10months), and the survival time was 8.2 months (95%, CI 1-23 months). The grade 3/4 toxicities encountered were neutropenia (12.5%), anemia (0%), thrombocytopenia (0%). Non-hematological 3/4 toxicities were not observed. Conclusion : A combination of gemcitabine and vinorelbine in patients previously treated for non-small cell lung cancer provides a relatively good response rate, and a low toxicity profile. However, further study will be needed to confirm its effectiveness. (Tuberc Respir Dis 2005; 58: 344-351) 배경 : Gemcitabine, paditaxel, docetaxel, vinorelbine, ir-rinotecan 새로운 항종양제의 출현으로 일차 치료의효과가 증대 되고 있고 재발시에도 좋은 신체 활동도를 보이고 있어서 이차 치료의 대상군도 늘어나는 효과를 보이고 있다. 치료의 필요성은 증대 되고 있지만 현재까지 표준 치료가 확립되지 않은 상황에서 Gem-citabine과Vinorelbine모두 독성이 강하지 않아서 혼합 요법이 가능한 장점이 있고 비소세포 폐암에 대한효과도 입증이 되어있어서 본 연구는 반응이 없거나 반응을 보인후에 재발된 진행된 비소세포 폐암에 ge-mdtabine과 vinorelbine 흔합 요법을 시행하여서 치료 반응률과 생존율 그리고 부작용을 평가하였다. 대상 및 방법 :2000년 6월부터 2004년 3월까지 충남대학교병원에내원하여 진행성 비소세포 폐암 IIIA/IIIB, IV로 진단을 받고 일차 항암화학요법치료를 받은 환자중에 초기 치료에 반응이 없거나, 치료에 반응이 있었으나 병이 진행된 환자로 추적 관찰 기간이 6개월 이상인 환자를 대상으로 생존율과 반응률 그리고 독성을 분석하였다. 결과 : 총 치료 반응률은 17%. 반응 유지기간의 중앙값은 3.1개월(1-10개월)이었고 생존기간의 중앙값은 8.2개월 (1-23개월) 그리고 1년 생존율은 35%였다. 항암화학요법에 의한 독성은 3도 이상의 중성구 감소가 12%, 오심과 구토가 12.5% 였다. 결론 : 일차 치료에 반응이 없거나 재발한 비소세포 폐암환자의 이차 치료로 gemcitabine과 vinorelbine 혼합요법은 효과적이라고 생각되며 향후 3상 연구를 통한다른 약제와의 비교 연구가 필요하다고 생각된다.

      • cis-platin에 의한 급성구토예방의 Dexamethasone의 4가지 정주량의 비교

        B5-hydroxytryptamine _3수용체(5-HT_3) 길항제와 dexamethasone의 동시투여가 cis-platin에 의한 급성 구토를 막는데 가장 효과적이다. 그러나 현재까지 가장 적절한 정주dexamethasone의 용량이 알려져 있지 않았다. 이에 dexamethasone의 4가지 다른 용량을 투여하여 그 효과를 비교하였다. 환자는 임의 추출되어 cis-platin 투여 45분전에 15분 동안 정주로 각각 dexamethasone 5, 10, 15, 20 mg을 투여 받았다. cis-platin 투여 30분전에는 ondansetron 8 mg이 부가하여 정주 되었다. 1999년 3월부터 2000년 2월까지 54명의 환자가 연구에 등록되어졌고 53명의 환자가 연구 대상으로 실험에 4군 (dexamethasone 5 mg 13명, 10 mg 14명, 15 mg 13명, 20 mg 13명)으로 나뉘어 평가 되어졌다. 급성구토와 구역질의 완전한 예방은 dexamethasone 5 mg을 투여 받은 환자에서 각각 69.2%, 60.9%, dexamethasone 10 mg을 투여 환자에서 69.1%, 61%, dexamethasone 15 mg 투여 환자에서 78.5%, 66.9%, dexamethasone 20 mg을 투여 환자에서 83.2%, 71.0%로 나타났다. 구토로부터 완전한 예방은 dexamethasone 20 mg을 투여 환자에서 5, 10 mg을 투여 환자와 비교하여 높았고, dexamethasone 15 mg을 투여 군에 비교하여서는 약간 우수한 효과만 있었다. 구역질으로부터의 완전한 예방도 월등한 것은 아닐지라도 20 mg을 받은 환자에서 높았다. 항 구토 치료는 특별한 불편 없이 조절되었고, 부작용의 발생에서 4가지그룹간에는 커다란 차이가 발견되지 않았다. Dexamethasone의 20 mg 정주양이 cis-platin으로 인한 급성구토를 예방하는데 가장 효과적인 예방량으로 사료되어진다. Background and objective: A 5-hydroxytryptamine _3(5-HT_3) receptor antagonist plus dexamethasone is the most efficacious antiemetic prophylactic treatment for the prevention of cis-platin induced acute emesis, but the optimal intraveous (Ⅳ) dose of dexamethasone is unknown. This prompted us to perform a randomized, double-blind, dose-finding study that compared four different doses of dexamethasone. Materials and Methods: Patients were randomized to receive dexamethasone, either 5, 10, 15, 20 mg, administered by 15-minute Ⅳ infusion 45 minutes before cis-platin. Ondansetron 8 mg was added to dexamethasone and was administered Ⅳ 30 minutes before cis-platin. From March 1999 to February 2000, 54 patients were enrolled onto the study and 53 were assessable according to the intention-to-treat principle (13 patients received 5 mg; 14 patients, 10 mg; 13 patients, 15 mg and 13 patients, 20 mg of dexamethasone). Results: Complete protection from acute vomiting and nausea was achieved by 69.2% and 60.9% of patients, respectively, who received 5 mg of dexamethasone, by 69.1% and 61.0% of those who received 10 mg, by 78.5% and 66.9% of those who received 15 mg, and by 83.2% and 71.0% of those who received 20 mg of dexamethasone. Complete protection from vomiting was significantly superior in patients who received 20 mg compared with those who received 5 and 10 mg of dexamethasone (P<05) and was superior, but not significantly, compared with those who received 15mg. Complete protection from nausea was superior, but not significantly, in patients who received 20 mg of dekamethasone. Multifactorial analysis confirmed these results. Antiemetic treatment was well tolerated, and no significant difference was found among the four groups in the incidence of adverse events. Conclusion: A 20mg single Ⅳ dose of dexamethasone should be considered the most efficacious prophylactic dose for the prevention of ois-platin induced acute amesis in treatment of cancer.
