Multiple bilobar liver matastases (MBLM) are the main cause of low resectability in the colon cancer liver metastases. The authors experienced one case of initially non-resectable colon cancer liver metastases. He was curatively and safely treated with a two-stage hepatectomy using the new method of future remnant liver volume growing. A 54-year-old man was referred to our department with the sigmoid colon cancer combined with MBLM, which were checked in two small metastatic lesions in the left lobe and five large sized lesions in the right lobe in the computed tomogram (CT). A laparoscopic assisted anterior resection was primarily performed. We performed the 1^(st) stage hepatectomy 3 weeks after the colon resection. Intra-operative Ultrasonogram (US) found 9 small superficial metastatic lesions in the left lobe. All that lesions were completely removed by non-anatomical wedge resection. An occlusion ballon catheter was placed in the right portal vein through a small branch of the inferior mesenteric vein at that time. The future remnant liver volume was sufficiently increased 3 weeks after the 1^(st) hepatectomy. A right hepatectomy was safely performed 22 days after the 1^(st) hepatectomy. The patient received a regional chemotherapy (interleukin2 based immuno-chemotherapy through hepatic artery) for 4 months, then received 9 cycles of systemic chemotherapy (biweekly Oxaliplation, leucovorin, plus 5-fluorouracil) without any recurrence evidence.

본 저자들은 인위적인 인슐린 투여로 인한 인간성 저혈당증 1예에서 보존적인 치료로 의식기능은 회복하였으나, 현저한 인지능력 저하를 하였기에 문헌고찰과 함께 보고하는 바이다. Factitious hypoglycemia is a covert attempt to lower the blood glucose concentration with insulin or a sulfonylurea agent. This attempt is mainly performed by the patients themselves. In a large series of diabetics who had overdosed on drug, less than 5% had used insulin for suicidal or homicidal purposes. Additionally, 4 of 204 hypoglycemic episodes were due to a suicide attempt with insulin. The onset of insulin action, its peak level, and duration, and the degree of hypoglycemia, are determined by insulin's pharmacokinetics. However, the final outcomes are not associated with the amount of insulin used. The complications related to insulin overdose are cerebral damage and hypokalemia. Other complications include, pulmonary edema attributed to congestive heart failure and hypertensive crisis, as well as respiratory insufficiency. We report this case with a review of the literature.

Abnormal motion control mechanism sometimes performs false detection of the direction, thus makes target point runs off the track, also shows abnormal speed control or power control. This paper explains development of the tools that make scientific measure and various analysis available. This paper also explams how we get the motion control ability data from the horizontal and vertical line drawn on the touch screen by the patient, analysis of deviation analysis, and Turn-Amplitude study. We built a complete system based on this algorithm.

This paper suggests a fuzzy control algorithm which manipulates the quantity of gas in a gas storage tank to a given reference value. Fuzzy control rules proposed in this paper are divided into two groups. The first is to treat the quantity of gas in a gas storage tank, and the second is to manipulate the quantity in pipe which is utilized in moving gas from gas source to a gas storage tank. Fuzzy control rules to treat two control problems mentioned above are designed independently by investigating several special points having inherent features for given assumed possible response characteristics in the sense of common-sense and engineer's technical knowledge. In order to observe the validity of fuzzy control algorithm for manipulating the quantity of gas in a gas storage tank suggested in this paper, computer simulations are done for three cases, which the quantity of generated gas and the required quantity in load have time-varying patterns. Computer simulation results show that fuzzy control rules designed in this paper are well operated for every case.
Design of Brushless DC Motor Controller for an Electric Wheelchair

We introduce a design and implementation of brushless DC (BLOC) motor controller for an electric wheelchair. The control architecture consists of two layers of feedback control, namely the wheelchair posture control and the wheel velocity control. In the higher level of control architecture, the posture controller works as a reference velocity generator for the two wheels. We realize the joystick interface and the posture controller in an 8-bit microcontroller. It is on the lower layer that the velocity controller performs four quadrant operations. For the driving system, a DSP-based BLDC motor controller with three-phase inverter module is especially designed using only three Hall-effect sensors and a single dc-link current sensor. The functions of this motor controller include wheel velocity control and torque control. The performance of the control system has been verified experimentally.

<P>This paper presents a new learning method for Gaussian mixture models (GMMs) to improve their generalization ability. A traditional maximum a posterior (MAP) parameter estimate is used to achieve regularization based on conjugate priors. Plus, a model order selection criterion is derived from Bayesian-Laplace approaches, using the conjugate priors to measure the uncertainty of the estimated parameters. As a result, the proposed learning method avoids the possibility of convergence toward the boundary of the parameter space, and is also capable of selecting the optimal order for a GMM with more enhanced stability than conventional methods using a flat prior. When applying the proposed learning method to construct a GMM classifier for electromyogram (EMG) pattern recognition, the proposed GMM classifier achieves a high generalization ability and outperforms conventional classifiers in terms of recognition accuracy.</P>
