        The Influence of AH-26 and Zinc Oxide-Eugenol Root Canal Sealer on the Shear Bond Strength of Composite Resin to Dentin

        AH-26 근관실러가 상아질에 대한 복합레진의 전단결합강도에 미치는 영향을 평가하기 위하여 144개의 발거된 대구치를 실린더형의 몰드에 매식한 다음 치관부의 상아질 표면을 노출시킨 후 편평하게 연마하고 AH-26을 도포한 군, ZOE paste를 도포한 군 및 실러를 도포하지 않은 대조군으로 나누어 One-step 상아질 접착제를 처리한 후에 Charisma (Heraeus Kulzer, Germany) 복합레진을 적용하고 광중합시켰다. 시편을 분리하여 37℃ 항온조에 24시간 보관후 Instron test machine (Model 4202, Instron Corp., USA)을 이용하여 시편의 전단 결합 강도를 측정하고 one-way ANOVA 및 Tukey's studentized rank test로 통계 분석하였다. AH-26 근관실러로 처리한 군과 대조군은 ZOE 근관실러 처리군에 비해 유의하게 놓은 접착강도를 나타내었다 (p<0.05). AH-26 근관실러 처리군과 아무 처리하지 않은 대조군 사이에는 결합강도에 있어 유의한 차이를 나타내지 않았다 (p>0.05). 본 연구의 조건에서는 ZOE 근관실러는 상아질에 대한 복합레진의 전단결합강도를 저하시키는 반면 AH-26 근관실러는 유의한 영향을 미치지는 않는 것으로 보인다. 따라서 임상에서 근관실러로 AH-26을 사용한 근관충전 후에는 즉시 복합레진 코어를 해 주어도 결합력에 저해가 없을 것으로 여겨진다. The purpose of this study was to evaluate the influence of the AH-26 root canal sealer on the shear bond strength of composite resin to dentin. One hundred and forty four (144) extracted, sound human molars were used. After embedding in a cylindrical mold, the occlusal part of the anatomical crown was cut away and trimmed in order to create a flat dentin surface. The teeth were randomly divided into three groups; the AH-26 sealer was applied to the AH-26 group, and zinc-oxide eugenol (ZOE) paste was applied to the ZOE group. The dentin surface of the control group did not receive any sealer. A mount jig was placed against the surface of the teeth and the One-step dentin bonding agent was applied after acid etching. Charisma composite resin was packed into the mold and light cured. After polymerization, the alignment tube and mold were removed and the specimens were placed in distilled water at 37℃ for twenty four hours. The shear bond strength was measured by an Instron testing machine. The data for each group were subjected to one-way ANOVA and Tukey's studentized rank test so as to make comparisons between the groups. The AH-26 group and the control group showed significantly higher shear bond strength than the ZOE group (p<0.05). There were no significant differences between the AH-26 group and the control one (p>0.05). Under the conditions of this study, the AH-26 root canal sealer did not seem to affect the shear bond strength of the composite resin to dentin while the ZOE sealer did. Therefore, there may be no decrease in bond strength when the composite resin core is built up immediately after a canal filling with AH-26 as a root canal sealer.

      • Helicobacter pylori와 대장균의 Shuttle Vector 개발

        In this study, a vehicle vector using cryptic plasmids was constructed for gene transfer in Helicobacter pylori. pHP51(3.9 kb) and pHP489(1.2 kb) were selected for constructing vectors from cryptic plasmid of H. pylori isolates in Korea. The HindⅢ-digestedDNA fragment(1.2kb) of pHP489 and 1.6kb DNA fragment of pHP51 were ligated with a kanamycin resistance gene(aph3'-Ⅲ) from C. jejuni to produce the recombinant plasmids pHP489K and pHP51K, respectively. Transformation frequency of pHP51K by electroporation was low. But pHP489K could be effectively transformed into various H. pylori strains. In order to design an intermdiate vehicle vector for gene transfer into H. pylori, pBlueHP489K was prepared by recloning pHP489K DNA into pBluescript and pTZ19R vector. This vector permitted the DNA fragment containing pHP489 sequence, aph3'-Ⅲ, and cloned DNA to be cut and self-ligated in the SacⅠ site after cloning. ureA and ureB gene were inserted into pBlueHP489K, resulting in pBlueHP489K/AB. The DNA fragment containing pHP489, kanamycin resistance gene(aph3'-Ⅲ), and urease structural gene was cut away from pBlueHP489K/AB and self-ligated to generate pBlueHP489K/AB. pBlueHP489K/AB made urease-negative H. pylori strains restore their urease activity. By this experiment, pBlueHP489K was confirmed to be the vehicle system for transferring H. pylori genes.

        Carcinoid tumors have been reported in a wide range of organs but most commonly involve the lungs, bronchi and gastrointestinal tract. Within the gastrointestinal tract, appendix is the most common location for carcinoid, followed by the distal small intestine, the rectum, and the stomach. Among these, primary involvement of the ampulla of Vater is extremely rare. We report a case of carcinoid tumor of ampulla of Vater. A 62-year-old man presented with epigastric soreness. Gastroduodenal endoscopy showed hyperemic bulging mass on ampulla of Vater and diagnosed as carcinoid tumor by histologic finding, with immunohistochemical study. In ^(111)In-octreoscan, tumor had regional lymph node metastasis, but no evidence of distant metastasis. Pancreatoduodenectomy with lymph node dissection was performed.

      • Ranitidine, Sucralfate, Bismuth Subcitrate 복합제의 흰쥐 위궤양에 대한 치료효과

        Gastric ulcer disease results when there is an imbalance between aggressive factors(acid secretion) and protective factors(mucosal defence). H_2-receptor antagonists are the most frequently used agents for treatment of gastric ulcer disease. However, use of these products should be determined by potential for adverse effects, drug interactions and cost. Ranitidine, H_2 - receptor antagonist, competitively block the H_Z-receptor on the parietal cell, causing inhibition of gastric acid secretion. Sucralfate and bismuth subcitrate coat ulcer craters and then enhance mucosal defence. Bismuth subcitrate also causes detachment of Helicobacter pylori from the gastric epithelium with subsequent lysis of the bacteria. To estimate effects of the mixture of ranitidine 20mg/kg, bismuth subcitrate 40mg/kg and sucralfate 120mg/kg on aspirin - HCl induced gastric ulcer in rats, gastric ulceration, pH, gastric secretion, free and total acidity, pepsin activity, gastric mucosal hemorrhage were investigated. Results obtained were as follow : 1) The mixture showed remarkable suppression of gastric ulceration, pH, gastric secretion, free and total acidity, pepsin activity, gastric mucosal hemorrhage compared with ranitidine alone. 2) With elapse of time, the mixture showed more increasing inhibitory effects on gastric ulceration, pH, gastric secretion, pepsin activity, gastric mucosal hemorrhage than sucralfate and bismuth subcitrate.

      • 海洋學的 要素의 年週變化가 海洋生物에 미치는 影響 : 莞島 海域의 海苔에 關하여 on the laver in the Wan-Do coast area

        This paper aims to contribute to the increase of laver production with the scientific raising and management of laver, after axamining closely the various surroundings of the inside ocean and the outside of Wan-Do, the central farming place of laver, physically and chemically and comparing and investigating the laver productivity of it. As a result of investigating and studying many important factors thrice a year, that is, in the months of September, Movember an February from 1981, I measured the following factors : water temperature, salinity, dissolved oxygen, pH, Nitrite-N. Phosphate-P, and transparency. The results of study are as follows. The change of water temperature in February 1982 Varied from 5.0℃ to 8.5℃ and in February 1983 Varied from 6.5℃ to 8.5℃. The Salinity in February 1982 fluctuated with in the range between 33.41‰ and 34.03‰ and in February 1983. fluctuation with in the range between 33.31‰ and 33.60‰. pH in general showed a fluctuation between 7.89 and 8.1. The range of dissolved oxygen in 1981 was from 4.40㎖/ℓ to 7.22㎖/ℓ and in 1982, was from 4.20㎖/ℓ to 6.50㎖/ℓ. The change of dissolved oxygen showed the maximum of 1.22㎖/ℓ in February 1981 at station 3, and a minimum 4.20㎖/ℓ at statimon 3 in Septemer 1982. Transparency showed a fluctuation between 0.25m and 1.5m. The change of Nitrite-N in 1982 or 1983, Varied from 1.19㎍-at/ℓ to 0.66㎍-at/ℓ. The phosphate-P in 1982 or 1983 fluctuated with in the range between 0.35㎍-at/ℓ and 1.08㎍-at/ℓ. As a result of the study of the annual variation on oceanographical factors of Wan-Do coast area. Water temperature, more than anything else, was showed up as having an important effects to the laver growth condition and the situation of laver production of the outside ocean was showed up better than that of the inside ocean. I believe the cause of this is that the situation of seawater exchange of inside ocean is poor. And as a result of the overconcentration of laver is also poor. I think the quantity of laver temperature etc., the production of laver is also landwater is little, the exchange of seawater is good, and the quantity of dissoled oxygen is abundant.

      • 만성 화농성 중이염의 세균학적 고찰

        Background and Objectives : Chronic suppurative otitis media is one of the frequent diseases in otolaryngologic field. It is important to choose of antibiotics in the management of infectious disease. But the organisms in infections have been changed and resistance to antibiotics has been grown as the development of antibiotics has been achieved. And so it has been necessary to recognize the changes of organisms and resistance in antibiotics. Our study was performed to identify the pathogens isolated from discharges in chronic otitis media and evaluate the antibiotic agents, to recognize the change of to resistance to·antibiotic agents and to use the appropriate anitiotics. Materials and Methods : Retrospectively, authors reviewed the backeriologic study of 114 cases of chronic suppurative otitis media who visited the Department of Otolaryngology, Soonchunhyang University in Seoul from Nov. 1996 to Oct. 1999 and analyzed 101 strains and its sensitivity test to various antibiotics. Result : In 93 cases in which pathogenic organism was isolated, single infection was 85 cases(91.4%) and mixed infection was 8 cases(8.6%). The most frequent pathogenic organism was Staphylococcus aureus(59.4%), Pseudomonas aeruginosa(13.9%), Providencia(4.8%), Alcaligenes(3.0%) and Streptococcus pneumoniae(2.9%) were the next. Methicilline-Resistance Staphylococcus aureus was 34 cases(57.6%) of Staphylococcus aureus. Staphylococcus aureus was sensitive to Vancomycin(96.7%), Trimethoprim-Sulfamethoxazole (57.6%), Cefuroxime(42.4%) and Ciprofloxacin(40.7%) but resistant to Penicillin(94.9%). Pseudomonas aeruginosa was sentitive Amikacin(100%), Ciprofloxacin(100%) and Gentamicin(67%). Conclusion : As MRSA recently was wide spread in community accquired infection as well as nosocomial infection, we must consider strict control of MRSA.

      • 光陽灣과 駕莫灣의 海洋學的 特性

        The aim of this paper is to study and analyze the physical, chemical, and biological elements in Kwangyang Bay and Kamak Bay from 1984 to 1985. For this study the author established three stations in Kamak Bay, two stations in Kwangyang Bay and investigated water temperature, salinity, pH, dissolved oxygen, chemical oxygen demand, phosphate-P, nitrite-N, nitrate-N, ammonium-N, total-N, and plankton in February, April, June, October, September, and November. Their results are as follows. The water temperature of Kamak Bay is higher than that of Kwangyang Bay. In February, the st. 1 of the inner bay is lower about 2.7℃ than the st. 5 of the outer sea. Therefore this phenomenon shows that water temperature increases in proportion to air temperature. Salinity is 24.5‰ lowest in Kwangyang Bay in September because of the influx of lots of land water, and it is 34.O‰ highest in Kamak Bay in February. pH is low in summer when the amount of precipitation is large. The range of pH is narrow, and the average pH of the two sea areas is almost the same as that of sea water. DissoIved oxygen is 7.57㏄/ℓ in February, which is the highest amount, and 4.71㏄/ℓ in September, the lowest amount. This phenomenon seems to have resulted from the consumption of a large amount of dissolved oxygen that is related to oxidization of organic matter and breathing of plants and animals. Chemical oxygen demand is higher in September than in April, and it seems to be related to Precipitation and water temperature. The amount of phosphate-P is 1.52㎍-at/ℓ in Kwangyang Bay, 0.86㎍-at/ℓ in Kamak Bay, and is larger in February than in November. This results from water temperature and the influx of land water. Phosphate-P, tota1-N, and nutrients prevail in Kwangyang Bay, this phenomenon seems to have resulted from the influx of a large amount of organic matter. The amount of plankton is large in Kamak Bay, and is largest in winter. This fact shows that plankton increases very much in winter when the water temperature is lowest, and dissolved oxygen is highest amount.

        좌새골하정맥을 통하여 중심정맥도관 삽입 3 일 후에 발생한 상대정맥천공과 우흉수 : 증례 보고 A case report

        A 67-year-old male patient had coronary artery bypass graft. Before the induction of general anesthesia, 16 gauge central venous catheter was inserted via left subclavian vein. Initial chest anteroposterior (AP) radiograph showed adequate position, and the central venous pressure measurement through it was satisfactory. This patient's progress was satisfactory, and he was extubated on the second postoperative day. Daily chest AP radiograph had been unremarkable until the third postoperative day, when a right-sided pleural effusion was seen. A day later, the patient complained severe chest pain and dyspnea. We suspected perforation of superior vena cava wall by tip of central venous catheter on the chest computerized tomograph (CT). The injection of radiopaque dye through the central venous catheter confirmed spillage into the mediastinum on the chest CT. Right chest tube was inserted. The patient's condition dramatically improved after chest tube insertion. (Korean J Anesthesiol 2002; 43: 525~530)

      • 榮山江 河口 댐 建設前後의 海洋環境 比較硏究

        In order to find out the effect of the enclosure at Yeongsan River Estuary on the surrounding aquatic envionments, a study on the physical, chemical and biological changes in this area had been carried out during from February, 1979 to November, 1985. Since November, 1981 when the dam was completed, there have been the considerable changes in the enclosed estuary in salinity, dissolved oxygen density, COD, nutrients, suspended particulate matter(SPM), transparency and biological compositions, with reduced water flow and appearance of immovable water masses. The relationship between the density of SPM and the transparency was calculated as S=86.4982-71.4837T(r=-0.8610) in 1979, but this was changed after the enclosure of the tam to S=22.0384-6.1865(r=-0.7488) in 1984. The fishes collected during the investigation are consisted of 12 species incuding Fugu vermicularis porphyreus, Epinephelus septemfasciatus and Hippocampus coronatus, which are newly found from this area. New establishments of clear water-algal species in this area such as Codium fragile, Hizikia fusiforme, Sargassum fulvellum and Undaria pinnatifida implies that a great change of marine flora have been occurred after the enclosure of the dam. benthic animals are 11 species including Mytilus edulis, Chlamys farreri, Musculus senhausia and Hemicentrotus pulcherrimus, which are newly inhabited in this area. In paricular M. edulis attached abundantly to the shore, and this implies that there was a great deal of ecological change after the enclosure of dam in this area.
