      • 쇄골 중간부 골절후 불유합 및 지연 유합의 원인

        쇄골 중간 1/3 부위의 골절은 치료 결과가 양호하여 보존적 치료가 원칙적으로 받아 들어지고 잇으며 불유합에 대한 빈도도 낮은 것으로 되어 있다. 그러나 최근 전위 및 단축의 정도가 심할 경우 높은 불유합의 결과와 불량한 임상 결과를 보고하고 있어 이런 예에 대해선 수술적 치료를 권장하고 있다. 저자들은 단축 및 전위의 정도와 불유합의 연관성을 알아보고자 하였다. 연구 대상 및 방법 : 1993년 2월부터 2002년 1월까지 본원에 내원 했던 성인 중간부 쇄골 골절 환자 194명 중 완전 전위를 보인 78례를 조사하여 이중 63례를 대상으로 분석하여 후 향적 연구를 시행하였다. 4개월 추시 후에도 방사성 사진상 유합 소견이 관찰되지 않으면 불유합의 유의성을 알아보고자 하였다. 결 과 : 63례 중 불유합 및 지연 유합을 소견을 보인 환자는 15(23%)례였다. 골유합을 보인 48례의 단축은 평균 8.6㎜(2㎜-17㎜)였고, 전위는 평균 9.7㎜(2-22㎜)였다 .불유합 및 지연 유합을 보였던 15례에서 단축은 평균 14.5㎜(3㎜-37㎜), 전위는 평균 17.3㎜(4-25㎜)였다. 18㎜이상 단축이 있었던 경우 불유합의 발생과는 통계학적으로 유의성을 나타내었고(Fisher's exact test, p<0.01), Chi-square test상 16㎜ 이상의 전위를 보였던 경우 불유합 발생과 통계학적으로 의미가 있었다(p<0.01). 결 론 : 쇄골 골절 특히 중간 1/3의 골절은 비교적 보존적 치료에 결과가 좋은 것으로 되어 있으나, 전위 및 단축이 심한 경우 불유합에 대한 가능성이 높이 때문에 보다 경과 관찰시 유합의 진행이 없으면 적극적인 치료를 고려해야한다. Purpose : Because the prognosis of the mid 1.3 clavicle fracture is good, the conservative treatment with a figure of 8 bandage is the gold standard and the nonunions are rare. However, recently surgical treatment is recommended when the shortening and displacement is severe because of the high nonunion rate and the poor clinical result. This study was undertaken to evaluate that the shortening and displacement at fracture site are associated with the development of nonunion. Materials and Methods : We analysed the 194 fractures of mid 1/3 clavicle in adults which had been treated conservatively from February 1993 to January 2002 and did the retrospective study. Of these, 78 cases were originally in the middle third of the clavicle and had been completely displaced. We reviewed 63 of these cases. The shortening and displacement at the fracture site was measured on the initial roentgenogram. And the analysis of the patients' chart was done for another predisposing nonunion factors. Nonunion and delayed union are considered to be present when there has been little or no progression of clinical or radiographic healing at a minimum of 4 months after injury. Results : 15 of the 63 cases had developed nonunion.. The average 8.6㎜(2㎜-17㎜) shortening and average 9.7㎜(2-22㎜) in the union patients. The average 14.5㎜(3㎜-37㎜) shortening and average 17.3㎜(4-25㎜) in the nonunion patients. We found that initial shortening ≥18㎜(Fisher's exact test, p<0.01) and initial displacement ≥ 16㎜(Chi-square test. p<0.01)at the fracture site were significantly associated with the development of nonunion. Conclusion : The conservative treatment with figure-80-bandage is the gold standard in the clavicle middle one third fracture. However, the nonunion is commonly occurs in the cases of more of severely shortened and displaced fractures. If there are no signs of callus formation and the patient complains of pain after several week, osteosynthesis should be considered.

        Muscle proteins of the Korean gudgeon of the subfamily Gobioninae were investigated by the polyacrylamide gel electrophoresis to determine the relationships among the species and genera studied. The number of bands were varied between 8∼13, and the mobiltiy and density or width of staining were appeared to be specificity. Judging from the genera Pseudorasbora and Puntungia are closely related each other, but Gnathopogon and Squalidus which were previously known to be cospecific genus were conspicuously difference between them. The muscle protein electrophoretic patterns was provided to be confirmed the phyletic groups adopted for the subfamily Gobioninae by Banarescu and Nalbant in 1973.

        자기연마를 이용한 STS304 파이프 내면의 초정밀 가공

        The magnetic abrasive polishing is the useful method to finish using magnetic power of a magnet. It's not a long time this method was introduced to korea as one of precision finishing techniques. The magnetic abrasive polishing method is not so common for machine that it is not spreaded widely. There are rarely researcher in this field because of non-effectiveness of magnetic abrasive. The mechanism of this R&D is dealing with the dynamic state of magnet-abrasive. This paper deals with mediocritizing magnetic polishing device into regular lathe and this experiment was conducted in order to get the best surface roughness at low cost. We need to continue the research on it. This paper contains the result of experiment to acquire the best surface roughness, not using the high-cost polishing material in processing. The average diameters of magnetic abrasive are the particles of 150㎛, 250㎛.

        수면-무호흡 중후군 증례보고

        Sleep apnea syndrome is characterized by loud snoring, excessive daytime somnolence and multiple attack of apnea during sleep which can cause sudden nocturnal death, cor pulmonale, hypertension, personality change and morning headache. We report a typical case of obstructive sleep apnea syndrome in a 30 year old Korean woman. She was very obese and complained loud snoring, daytime hypersomnolence. Nocturnal sleep study showed repetitive attack of apnea and fall of arterial oxygen tension and saturation. Bilateral severe tonsilar hypertrophy was thought to be the mechanism of obstruction.

        Ba-Ferrite를 이용한 자기 연마재 개발

        The magnetic polishing is the useful method to finish some machinery fabrications by using magnetic power. This method is one of the precision techniques and has an aim for clean technology in the transportation of the pure gas in the clean pipes. The magnetic abrasive polishing method is not so common in the field of machine that it is not known to widely. There are rarely researcher in this field because of non-effectiveness of magnetic abrasive. Therefore, in this paper we deals with the development of the magnetic abrasive with the use of Ba-Ferrite. In this development, abrasive grain WA has been made by using the resin bond fabricated at low temperature. And magnetic abrasive powder was fabricated from the Ba-Ferrite which was crushed into 200 mesh. The XRD analysis result shows that only WA abrasive and Ba-Ferrite crystal peaks were detected, explaining that resin bond was not any more to contribute chemical reaction. From SEM analysis, we found that WA abrasive and Ba-Ferrite were strongly bonding with each other.

      • 폐경후 여성에서 폐경기간에 따른 호르몬 보충요법이 혈중 지질 농도에 미치는 영향

        폐경 증후군 여성 88명을 대상으로 호르몬 보충요법을 시행하여 다음의 결과를 얻었다. 1. 폐경 이후 5년 및 10년이 지난 여성에서 혈중 총 콜레스테롤 및 저밀도 지단백의 농도가 폐경전후의 여성에 비해 유의하게 높았으나, 고밀도 지단백의 농도는 차이가 없었다. 2. 호르몬 보충요법 6 주기 후의 혈중 저밀도 지단백의 농도는 폐경 후 5년 및 10년이 지난 여성에서 유의하게 높았으나, 총 콜레스테롤 및 고밀도 지단백의 농도는 유사 하였다. 3. 호르몬 보충요법 12 주기 이후의 혈중 지질 농도는 폐경 이후의 시기와 관계없이 유사한 결과를 나타내었다. 4. 혈중 총 콜레스테롤의 농도는 폐경 전후의 여성에서 호르몬 보충요법 12 주기 이후에 증가하는 양상을 보였고, 폐경 이후 5년 및 10년이 지난 여성에서는 별로 변화가 없었다. 5. 혈중 고밀도 지단백의 농도는 폐경 전후의 여성에서는 호르몬 보충요법 6 주기 이후부터 유의한 증가를 보였으나, 폐경이 우래된 여성에서는 호르몬 보충요법 12주기 이후부터 유의한 상승을 나타내었다. 6. 혈중 저밀도 지단백의 농도는 폐경이후의 시기에 관계없이 12주기의 호르몬 보충요법 이후에는 유의한 감소를 보였다. 이상의 결과를 볼때 폐경 이후 10년이 지난 여성에서도 12 주기 이상의 호르몬 보충요법 후에는 폐경 직후에 보충요법을 시행한 여성과 유사한 혈중 지질 농도의 양상을 보임으로써 폐경 이후 수년이 지난 여성에서도 적극적으로 호르몬 보충요법을 시행하는 것이 바람직 하다고 생각된다. As the evaluation of the effect of HRT on blood lipid levels in 88 postmenopausal women, we obtained following results. 1. In women with 5-year & 10-year passed menopause, total cholesterol and LDL cholesterol levels were significantly higher than women with just menopause(p<0.05), but HDL cholesterol levels had no significant differences. 2. After 6 cycles of HRT, LDL cholesterol levels were higher in women with 5 & 10-year passed menopause(p<0.05), but total and HDL cholesterol levels were not different. 3. After 12 cycles of HRT, lipid profiles showed similar levels in all 3 group. 4. After 12 cycles of HRT, total cholesterol levels were increased in women with just menopause(p<0.05), but no significat changes in women with 5 & 10-year passed menopause. 5. HDL cholesterol levels showed increased pattern since after 6 cycles of HRT in women with just menopause, but showed more slow pattern in the women with 5 & 10-year passed menopause. 6. LDL cholesterol levels were significantly decreased after 12 cycles of HRT in all 3 group(p<0.05). On the basis of above results, lipid profiles of women with 10-year passed menopause were similar with women with just menopause after 12 cycles of HRT. Active recommendation of HRT are needed even in women with long passed menopause.

        Genotype $\times$ Environment Interaction for Yield in Sesame (Sesamum indicum L.)

        Application of genotype by environment ($G\;{\times}\;E$) interaction would be used for identifying optimum test condition of the varietal adaptation in the establishment of breeding purpose. Yield and yield components were used to perform additive main effect and multiplicative interaction (AMMI) analysis. Significant difference for $G\;{\times}\;E$ interaction were observed for all variable examined. For yield, 0.18 of total sum of squares corresponded to $G\;{\times}\;E$ interaction. Correlation analysis was carried out between genotypic scores of the first interaction principal component axis (IPCA 1) for agronomic characters. Significant correlations were observed between IPCA 1 for yield and capsule bearing stem length (CBSL), number of capsule per plant (NOC). The biplot of grain yield means for IPCA1 which accounted for 34% of the variation in total treatment sums of squares showed different reaction according to $G\;{\times}\;E$ interaction, genotypes and environments. Taegu showed relatively lower positive IPCA1 scores, and it also showed smaller coefficient variation of yield mean where it is recommendable as a optimal site for the sesame cultivar adaptation and evaluation trial. In case of variables, Yangbaek and M1 showed relatively lower IPCA1 scores, but the score direction showed opposite each other on the graph. Ansan, Miryang1, Miryang4, and Miryang6 seemed to be similar group in view of yield response against IPCA1 scores. These results will be helpful to select experimental site for sesame in Korea to minimize $G\;{\times}\;E$ interaction for the selection of promising genotype with higher stability.
