결핵은 다른 종류의 질환과 달리 완치까지 여러 가지 약제들을 동시에, 6개월 이상 장기간 복용해야 한다. 그러므로 약물치료의 표준요법을 정하고 이에 따라 결핵약을 복용하게 함으로써 잘못된 치료 처방으로 인한 치료실패를 예방하고 있다. 그런데 일부 결핵환자들은 간질환, 신장질환, 에이즈와 같은 질환을 동반하거나 임산과 같은 특수 상황에 직면할 수 있는데, 이 경우 통상적인 방법으로 결핵을 치료했을 때 심각한 부작용, 치료효과의 감소, 태아에 대한 기형유발 등을 초래할 수 있다. 이와 같은 특수 상황에서의 결핵치료 방법과 유의사항에 대해서 알아보도록 한다.

인류는 수많은 연구들을 통하여 결핵의 원인균(결핵균:M.tuberculosis)과 발병기전을 밝혔으며, 결핵퇴치사업을 범세계적으로 추진하였음에도 불구하고 결핵은 아직도 전 세계적으로 만연하고 있다. 결핵균이 현대의학의 발전에도 불구하고 우리나라를 포함하여 전 세계적으로 맹위를 떨치고 있는 것은 대부분의 미생물들과는 달리 결핵균만이 가지는 아주 독특한 생존전략을 가지고 있기 때문일 것이다.

폐결핵이 발생한 환자가 기침할 때 나오는 결핵균들은 새로 태어난 사람들을 포함하여 결핵균에 감염되지 않았던 다른 사람들을 새롭게 감염시키면서 인류 사회에 깊숙이 침투해서 번창하여 왔다. 그러나 1940년대에 접어들면서 인류는 결핵균에게 치명적인 무기들을 하나씩 개발하고 결핵균의 전략을 파악하면서 대대적인 반격을 시작하였다. 세월이 흘러 결핵퇴치사업이 성과를 보이면서 선진국들을 중심으로 결핵균의 세력이 급격히 위축되면서 위기를 맞고 있다.

다제내성 결핵은 치료 성공률이 낮은데 그 원인으로 사용할 수 있는 효과적인 항결핵 약제의 숫자가 제한되어 있고, 약제 부작용이 많고, 장기간의 치료기간으로 인해 환자의 순응도(compliance)가 낮기 때문이다. 치료에 실패한 다제내성 결핵환자는 본인의 고통뿐만 아니라 지속적으로 결핵균을 배출하는 만성배균자가 되어 수 많은 사람들에게 다제내성 결핵균을 퍼뜨리는 전염원이 된다.

The purpose of this study is to observe the effects of Chaenomelis Fructus Herba Chelidonii Clematis Florida Thunb mixed water extract on the inflammatory edema induced by Found's complete adjuvant and on different blood volume, and then to demonstrate its efficiencies as well as its safety. We injected Chaenomelis Fructus Herba Chelidonii Clematis Florida Thunb mixed water extract on the inflammatory edema induced by Found's complete adjuvant once daily at the same time for a week, then observed the paw-edema, the counts of the blood WBC, Serum total protein, Serum total bilirrubin, SGOT and SGPT The result obtained is as follows : Inflammatory edema on the right plantar in both sample groups showed a significant reduce compared with that in the control group. The counts of WBC in both sample groups showed a significant decrease compared with that in the control group. The volume of serum total protein in the Sample A group showed a significant decrease compared with that in the control group. The volume of serum total bilirrubin, GOT and GPT in both sample groups was not changed compared with that in the control group. As a result of this experiment, it is concluded that Chaenomelis Fructus Herba Chelidonii Clematis Florida Thunb mixed water extract showed that therapeutic effect on the anti-inflammation on arthritis; otherwise it showed non-toxic effect on the liver function.

Caerulein으로 유발된 흰쥐의 급성 췌장염에 대한 수념산(手拈散)의 효과

Objectives : This study was designed to investigate the effects of SuJeom-san(SJS) extract in rats with caerulein-induced acute pancreatitis (AP). Methods : We examined changes of pancreatic weight, histological, immunohistochemical and gene expression of cyclooxygenase (COX-2). Thirty-six adult male Sprague-Dawley rats were divided into six groups as follow: normal(Nor), caerulein-induced (Con), caerulein + cefotaxime sodium(CT), caerulein + SJS 3 mg/kg(SJSA), caerulein + SJS 6 mg/kg(SJSB) and caerulein + SJS 12 mg/kg(SJSC) groups. Pancreatic tissues of rats from all groups were removed for histological observation and light, and electron microscopic examination. Platelet activating factor(PAF) and Interleukin-6(IL-6) levels were determined spectrophotometrically. Results : The ratio of pancreas/body weight was significantly(p<0.05) increased in the Con compared with Nor, but significantly(p<0.05) decreased in SJSA, SJSB, SJSC and CT groups compared with Con. Caerulein administration significantly increased(p<0.05) the levels of amylase, but SJSA, SJSB, SJSC and CT significantly(p<0.05) reduced the levels of these enzymes. The levels of platelet activating factor(PAF) increased in Con compared with Nor, but decreased in SJSA, SJSB, SJSC and CT groups compared with Con. Interleukin-6(IL-6) levels increased significantly in all groups compared to Nor at 6 hrs, but significantly(p<0.05) reduced in SJSA, SJSB, SJSC and CT groups compared with Con at 24 hrs. The levels of tumor necrosis factor(TNF)-${\alpha}$ levels increased in all groups compared to Nor at 6 hrs, but significantly(p<0.05) reduced in SJSA, SJSB, SJSC and CT groups compared with Con at 24 hrs. The COX-2 positive materials were observed in the pancreas of the Con, but these positive materials were decreased in the SJS extract treatment group. Conclusion : SJS is potentially capable of limiting pancreatic damage during AP by restoring the fine structure of acinar cells and tissue; therefore, we conclude that SJS may have beneficial effects in the treatment of caerulein-induced AP.
