        여호와 증인 환자에서 산부인과적 무혈 수술의 임상적 고찰

        목적 : 여호와의 증인은 자신들의 교리에 따라 혈액이나 기타 혈액제재의 사용을 거부하는 종파이다. 이들에 대한 산부인과적 무혈 수술의 임상상 및 수술후 결과를 후향적으로 검토하였다. 연구방법 : 1995년 2월부터 1999년 8월까지 세종병원에서 입원 기간 중 어떠한 혈액이나 혈액제재를 투여 받지 않았던 여호와의 증인 환자 50예에 대해 무혈 수술의 성적을 후향적으로 조사하였다. 결과 : 환자의 연령 분포는 9세와 58세 사이로 평균 36.7 ± 8.7세 이었다. 무혈 수술의 가장 흔한 사례는 제왕 절개술이었다. 인간 조혈촉진인자(EPO)를 제왕 절개술 5예, 전자궁 적출술 2예, 난소 종양 2예에 투여하였다. EPO를 투여 받은 9예에서는 평균 혈색소 수치가 수술 전에 비해 수술 후 오히려 의미있게 증가하였다. 조직학적 결과로는 자궁 선근증 9예, 평활근종 8예, 생식 세포종 3예, 자궁외 임신 2예, 자궁 상피내 종양 1예, 상피성 난소 종양 1예, 출혈성 황체 낭종 1예, 성기삭 간질성 종양 1예로 총 27예이었다. 무혈 수술로 인한 술 후 사망은 없었으며 이환율도 적었다. 결론 : 여호와의 증인 환자에 대한 산부인과적 수술이 요구될 때 세심한 수술 기법과 다양한 혈액 보존법 등을 이용한다면 비교적 양호한 무혈 수술의 결과를 얻을 수 있었다. 그러나 향후 여호와의 증인 환자에 대한 특수화된 프로그램의 개발이 필요하다고 사료되었다. Objective : Jehovah's Witnesses are members of a religious denomination whose beliefs prohibit the use of blood or blood products. Plasma volume expanders and extracorporeal hemodilution of the patients's own blood are theologically acceptiable. And so we evaluated effectiveness of major operative procedures carried out on Jehovas's Witnesses patients without blood transfusions or blood products. Methods : We retrospectively studied 50 cases of Jehovas's Witnesses who underwent obstetric and gynecologic bloodless surgery from February 1995 to August 1999. No patients recieved any blood or blood products during hospitalizaion. Results : The age distribution of patients was between 9 years and 58 year, average 36.7±8.7 years old. The most common cases for bloodless surgery was Cesarean section. The recombinant human erythropoietin(EPO) was treated in 5 cases of Cesarean section, 2 cases of TAH and 2 cases of ovarian tumor perioperatively. EPO-treated 9 cases were increased postoperative mean Hb levels compared with preoperative mean Hb levels. Histology revealed 9 cases of adenomyosis, 8 cases of leiomyoma, 3 cases of germ cell tumor, 2 cases of ectopic pregnancy, 1 case of CIS, 1 case of Krukenberg tumor, 1 case of epithelial cell tumor, 1 case of hemorrhagic corpus tumor and 1 case of sexcord stromal tumor, total 27 cases. There was no operative death and postoperative morbidities were few. Conclusions : We concluded that if we preformed the surgeries with careful operative techniques and various blood conserving procedures we could do the bloodless surgery of Jehovah's Witnesses with relative safety. But the establishment of a well programmed special medical supports for the Jehovah's Witnesses community will be needed in future.

        치환된 알킬 사슬 혼합물의 자기조립 단분자막 구조지 STM 연구

        p-iodo-phenyl octadecyl ether (I-POE)와 p-iodo-phenyl docosyl ether (I-PDE)의 분자의 흑연표면에서의 자기조립과 이 두 분자로 이루어진 혼합물의 자기조립을 주사 터널링 현미경을 이용하여 연구하였다. 각 분자 시스템은 흑연 표면에서 head-to-tail 배향의 안정된 단분자막으로 자기조립한다. 혼합물 시스템에서는 I-POE와 I-PDE 분자들은 표면에서 섞이지 않고, 고립된 단분자막 도메인을 형성한다. 특히 I-POE 분자는 흑연 표면에서 단분자막 구조를 우선적으로 형성하는데 이는 알킬 사슬 길이와 작용기를 가진 헤드 그룹의 효과에 기인한다. Here, the I-POE molecules are preferentially adsorbed on the graphite surface, due to the effects of alkyl chain length and the functional group on the monolayer structure.

        Particle Balance Study for the Fueling Efficiency in KSTAR Experiments 2013

        The efficiencies of the fueling system in the KSTAR have been studied for the 2013 experimentalresults. The absolute amount of injected fuel or deuterium has been calibrated in advance of themain plasma experiments. This includes the primary actuator of fueling, i.e., gas puffing (GP)with piezoelectric valves located on mid-plane, and supersonic molecular beam injector (SMBI). In addition, neutral beam injectors (NBIs) and their base facility chamber also contribute to theamount of fuel injection whenever they are operated. Low fueling efficiency combined with particleloss due to wall retention essentially results in poor performance of density feedback control magneticfusion devices such as the KSTAR.

        무방향성 규소강판에서 냉간압연이 자기적 성질에 미치는 영향

        The effect of cold rolling on the magnetic properties of nonoriented electrical steel sheet containing 3% silicon was examined by the observation of microstructure and texture analysis. Grain size of each specimen was maximum when the reduction rate of 2nd cold rolling was 5% and it gradually reduced as the rate increased. It was thought that grain growth of 2nd cold rolled specimen was occurred by random, abnormal grain growth. In view of the texture of final product, specimens processed by single cold rolling method had (222) texture, which is unfavorable to magnetic properties, but the ones processed by double cold rolling method had nearly random texture. Specimen with 2nd cold reduction of 5%, thus, had the minimum iron-core loss.

        Authors presented a statistical observation and brief reviews of 99 cases of abdominal injuries who were admitted to St Paul's hospital, Catholic Medical Center, during the period from Jan 1963 to May 1967. Among the 99 patients of abdominal trauma, non-penetrating wounds were 84 cases (84.8%) and penetrating wounds 15 cases (15.2%). In 60 patients(60. 6 %) the cause of trauma was car accident. Kidney was the most common organ of injured (42.5 % ), spleen (26.0 %) and liver (18.1% ). The high frequency of associated injury was spine froceure (18. 1 % ). The common complication after operation for abdominal injuries were wound infection (5.3%), evisceration and pulmonary complication. Treatment for the rupture of abdominal solid organs were also discussed.

        Nadroparin is a low-molecular-weight heparin and has been effective in the prophylaxis and treatment of thromboembolism. Nadroparin is usually used for prevention of hepatic venoocclusive disease in SNUCH. Hepatic veno-occlusive disease (VOD, a common complication of stem cell transplantation) is a result of intensive conditioning by chemo-radio-therapy. The pathogenesis is probably multifactorial, but the key event is endothelial damage, which triggers the coagulation cascade, inducing the deposition of fibrin in the affected venules. Thus, the use of antithrombotic therapy is a logical prophylactic approach to VOD. Since immune function and platelet number is decreased after stem cell transplantation, nadroparin in 5% dextrose solution is continuously infused for 24 hours instead of subcutaneous injection but it is failed to cite data on the stability of nadroparin with diluent. The stability of nadroparin in 5% dextrose solution was studied. Duplicate solutions of 5,700 IU/0.6 ml of nadroparin and 100 ml of 5% dextrose solution were prepared. Samples were taken at 15 minutes, 2 hours, and 24 hours. Inspections for color change and precipitation performed with naked eyes. Samples were evaluated for pharmacologic activity by anti-factor Xa assay according to European Pharmacopoeia 4th. Throughout the 24-hour study period, the nadroparin admixtures were free of color change, precipitates. The pharmacologic activity of nadroparin remained 99.8~100.5% of activity measured immediately preparation. In conclusion, nadroparin 57 IU/ml in 5% dextrose solution based on 50 kg child was stable for up to 24 hours at 15~25°C.
