        This study is purposed to analyse the effects of long-term & one-time 10km running exercise from bone metabolic marker change on bone metabolism. 7 male members(18.6±1.08) from a marathon club were subjected to the study. They were allowed to do long-term 10 km running training once a week for 6 months. And bone metabolism mark was determined according to the respective times : before/after 6 months training, before/after 10 km race, one day after the race, 3 days after the race. The examination items of bone metabolism mark were serum calcium, phosphorus, paratyroid hormone, osteocalcin, calcitonin, alkaline phosphatase in blood and calcium, phosphorus, deoxypyridinolin in urine. The results of study were as follows; 1) There was no change in deoxypyridinolin and alkaline phosphatase but there was significant change in calcitonin, paratyroid hormone and osteocalcin(p<.01). 2) There was no change of Calcium in serum and urine but there was significant change of phosphorus in blood(p<.01). To make a conclusion, it is determined that long-term 10 km running at low intensity and low frequency tends to restrict bone metabolism circulation, but exercise at a little high level activates bone metabolism into building up bones and absorbing bones and finally shows up high bone metabolism circulation, which is inferred to have an effective influence on bone health. It is considered there is a possibility that different exercise intensity, even same kind of exercise, influences on bone metabolism sdifferently.

        Anaplastic Thyroid Cancer Successfully Treated With Paclitaxel and Radiation: A Case Report

        Anaplastic thyroid cancer (ATC) is rare and aggressive, with an extremely low cure rate and poor prognosis. Here, we report a case of treating patients with ATC through multimodal treatment and review the related literature. We report the case of a 56-year-old woman who was referred to our hospital for getting treated for ATC, which extended to the external right tracheal wall and muscular layer of the esophagus, with lateral lymph node metastasis. The patient underwent concurrent chemoradiotherapy. Three cycles of weekly paclitaxel administration were conducted, along with intensity-modulated radiotherapy. In response evaluation after three cycles of paclitaxel treatment, the main mass showed a partial response, and 2 additional cycles of paclitaxel administration were included in the treatment regimen. After the 5 cycles, the patient showed a partial response and was considered operable. The patient underwent bilateral total thyroidectomy with central compartment neck dissection and bilateral modified radical neck dissection. More than 43 months following diagnosis, the patient is still alive without evidence of local recurrence or distant metastases. Paclitaxel-based concurrent chemoradiation therapy has demonstrated potential as an alternative and effective treatment option in patients with unresectable ATC, with a durable response of 43 months and mild toxicity.

        Adrenal Schwannoma : Rare Entity of Adrenal Incidentaloma

        Purpose: Adrenal schwannomas are very rare and are usually incidentally found on autopsy and imaging. The aim of this study was to describe our experience of adrenal schwannoma and review the literature regarding this rare tumor. Methods: To identify patients with adrenal schwannoma, the MEDLINE database was searched via the major electronic database PubMed using the medical subject heading terms “adrenal” and “schwannoma”. Thirty-nine adrenal schwannoma cases, including two from our institution were included. Results: The mean age at diagnosis was 47.95 years (range, 11∼89 years). The female:male ratio was 21:17. Sixteen patients had tumors on the right side, 19 on the left, while one patient had bilateral tumors. The mean tumor size was 6.12 cm (range, 0.6∼14.5 cm), and the mean tumor weight was 161.3 g (range, 31.5∼600.0 g). Sixteen tumors were solid, four cystic, one solid and cystic, and 18 were not described. Histologically, 14 cases were Antoni A, two were Antoni B, and 12 had concomitant Antoni A and B. Conclusion: Adrenal schwannoma is usually an incidental finding, and the clinician must have a high index of suspicion to recognize it when imaging reveals suggestive features.

        프라이빗 블록체인 및 스마트 컨트랙트 기반 고신뢰도 크라우드센싱 보상 메커니즘

        크라우드센싱 시스템을 현실화하기 위해서는 서비스 제공자 서버와 사용자간의 신뢰도 구축이 선행되어야 한다. 서비스 제공자 서버는 지급하는 보상을 최소화하기 위해 센싱 데이터에 대한 평가를 조작할 수 있다. 또한 사용자는 부정한 보상을 얻기 위해 거짓 데이터를 제공할 수 있다. 본 논문에서는 크라우드센싱 시스템에 프라이빗 블록체인을 도입함으로써 서버가 제공한 보상과 사용자가 제공한 데이터를 불가변적으로 기록해 서버와 사용자가 서로의 감시자 역할을 하도록 한다. 또한 스마트 컨트랙트를 통해 센싱 데이터에 대한 평가를 자동화하고 그 과정을 사용자에게 공개하여 서버 신뢰도를 구축하는 방법을 제시한다. 성능 평가 및 타 시스템 비교를 통해 제안된 크라우드센싱 보상 시스템의 실현 가능성을 보인다. To implement crowdsensing system in reality, trustiness between service provider server and user is necessary. Service provider server could manipulate the evaluation of sensing data to reduce incentive. Moreover, user could send a fake sensing data to get unjust incentive. In this paper, we adopt private blockchain on crowdsensing system, and thus paid incentives and sent data are unmodifiablely recorded. It makes server and users act as watcher of each others. Through adopting smart contract, our system automates sensing data evaluation and opens to users how it works. Finally, we show the feasibility of proposing system with performance evaluation and comparison with other systems.

        벌나무 열수 및 에탄올 추출물의 항산화 활성 및 알코올 분해능

        벌나무의 에탄올 및 열수 추출물을 각각 제조하여 추출물별 항산화 활성을 비교하고, in vitro 및 in vivo에서의 알코올 분해능을 검토하였다. 벌나무 열수 및 에탄올 추출물의 총 폴리페놀 함량은 각각 171.93±2.2 μg/mg, 152.69±1.25μg/mg, 총 플라보노이드 함량은 각각 7.51±1.34 μg/mg,5.01±0.83 μg/mg이었고, FRAP값은 각각 1.75±0.32 μM/μg, 1.67±0.28 μM/μg으로 나타났다. 벌나무 열수 및 에탄올 추출물은 DPPH 및 ABTS에 대해 높은 소거활성을 보였고, 지질과산화 억제활성이 높음을 알 수 있었다. 벌나무 열수추출물은 HepG2 세포에서 알코올에 대한 in vitro에서의 간세포 보호 효과가 있었고, 또한 알코올을 투여한 rat에서 벌나무 추출물을 투여하면 알코올 투여 30분이 경과한 후 알코올 투여군보다 혈중 알코올 함량이 유의적으로 저하되었다. 따라서 벌나무 추출물은 높은 항산화 작용을 하며, 알코올분해 작용 및 알코올에 대한 간 보호 효과가 높은 것으로 생각된다. The purpose of this study is to evaluate the antioxidant activities of extracts from Acer tegmentosum Maxim. (AT) and the ability of these extracts to reduce the serum alcohol concentration in rats administered alcohol. The total amount of polyphenols in hot water and ethanol extracts from AT were 71.93±2.2 μg/mg and 152.69 ±1.25 μg/mg, respectively, while the total amount of flavonoids in hot water and ethanol extracts from AT were 7.51±1.34 μg/mg and 5.01±0.83 μg/mg, respectively. FRAP values in AT extracts were 1.67~1.75 μM/μg. AT extracts were capable of directly scavenging DPPH and ABTS free radicals, with higher inhibitory activities for TBA. The hepatoprotective effect of hot water extracts from AT against ethanol-induced oxidative damage was investigated. Ethanol-induced damage on HepG2 liver cells were protected by hot water extracts from AT. Administration of hot water extracts from AT (200 mg/kg) had reduced serum alcohol levels in acute alcohol-treated rats. These results indicate that AT extracts can be protective against alcohol-induced toxicity, potentially through its antioxidant properties.

      • 소아 일반촬영 검사 시 보호자 참여의사와방사선에 대한 인식도 조사

        Purpose:This study was conducted to improve the quality of the examination and facilitate the communication be-tween the radiologist and the guardians by investigating participants and their perception of radiation about general radiography of children. Materials and Methods:From June 1, 2017 to December 31, 2017, the questionnaires were surveyed for the guardians of children who visited the P Hospital in Yangsan, Kyung-nam Province. The number of subjects was calculated using statistical power analysis using G * power 3.1 analysis. Estimated sample size was calculated as 93 when it was the moderate effect size of 0.15 for the simple regression analysis, 95% for the power of test, the significance level of 0.05, and the awareness of radiation exposure prevention of 1, which is related to the degree of awareness of the guardian. A total of 210 people were selected considering the recovery rate and insufficient number of responses. All items were evaluated on a 5-point Likert-type scale, except for questions related to general and participant intention. In this study, data were analyzed using SPSS 21.0 for windows statistical package program and variables were evaluated based on Chronbach s Alpha coefficient. Descriptive statistics were used to analyze the general characteristics and participation intention of the sample. T-test, One-way ANOVA, Pearson correlation, and Simple regression analysis were used to comply with the purpose of the study. Result: The results of the reliability analysis of the variables were more than 0.6 in Chronbach’s Alpha, 0.645 for Psychological status at general radiography of children, 0.667 for radiation recognition, and 0.666 for radiation exposure prevention recognition. In the recognition survey of guardian participant intention, to this question “Have you ever been involved in the general radiography of children in the past?” Yes (66.2%) and No (33.8%) were answered. The reason for participating in the general radiography of children were request of radiologist (46.0%) own volition and by child’s request (20.9%). To the question of “Do you think it is necessary for parents to participate in the gen-eral radiography of children?” yes (84.3%) and no (15.7%) were responded. The question “If you are willing to participate, why?” showed 80.8% for the stability of the child and 18.1% for the confirmation of the exact examination according to the site. The question “What if there is no intention to participate?” were 54.8% , 19.4%, and 12.9% due to radiologist’s duty, negative health effects and vague fear of radiation exposure respectively. There was a statistically significant difference according to gender and age of children (p<0.05) in the analysis of the psychological status of the guardians in the general radiography of children. There was statistically significant difference according to gender and educational background (P<0.05). There was a statistically significant difference according to age, gender, childhood age and educational background in the perception of prevention of radiation exposure of guardians in general radiography of children (p<0.05). Conclusion:From the results of the study, the guardians at the general radiography recognized the need for accompanying examination with them due to the stable and accurate examination of the children. However, it was not considered as distrust of radiologists or dangerous to the physical hazards of radiation exposure. Therefore, it is desirable to eliminate the psychological anxiety of the children and to induce the guardians to accompany in the pediatric examination. In addition it is considered that the plans to explain the examination method and the necessity of wear protective equipment are needed. 목 적:소아 일반촬영 검사 시 보호자들의 참여 여부와 방사선에 대한 인식도를 파악함으로써 검사자인 방사선사와 보호자의 상호 의사소통을 원활히하고 검사의 질을 향상시키고자 본 연구를 시행하였다. 대상 및 방법: 경남 양산 소재 P대학병원에 내원한, 소아 아동이 있는 보호자를 대상으로 2017년 6월 1일부터 2017년 12월 31일까지 설문조사하였으며 연구 대상의 수는 Statistical power analysis using G * power 3.1 analysis를 이용하여 산출하였고, 예상 표본수는 단순 회귀분석을 위한 중간 정도의 효과크기 0.15, 검정력 95%, 유의수준 0.05, 보호자의 인식도와 관련성이 있는 변인인 방사선 피폭방지인식도를 1로 하였을 때 93명으로 계산 되었고, 회수율과 불충분한 응답 수를 고려하여 총 210명을 선정 일반적 사항 및 참여의사 여부에 관련된 문항을 제외한 모든 문항을 Likert 방식의 5점 척도로 평가하였다. 본 연구에서는 SPSS 21.0 for windows 통계 패키지 프로그램을 사용 하여 자료 분석하였고 Chronbach s Alpha계수를 근거로 변수를 평가하였다. 표본의 일반적 특성 및 참여의사 여부를 분석하기 위해 기술통계를 하였으며 연구의 목적에 부합 되도록 평균차 분석 (T-test) 및 일원배치분산분석(One-way ANOVA), 상관관계분석(Pearson correlation), 단순 회귀분석(Simple regression analysis)등을 시행하였다. 결 과:변수의 신뢰도 분석결과는 소아 일반촬영 시 심리 상태 0.645, 방사선 인식도 0.667, 방사선 피폭 방지 인식도 0.666으로 Chronbach s Alpha 0.6이상이었다. 소아일반촬영 시 보호자 참여의사 여부 인식도조사에서 “ 과거에 소아 일반촬영에 참여해 본적이 있는가?”라는 질문에 그렇다(66.2%), 아니다(33.8%)로 나타났다. 소아 일반촬영에 참여해 본 경우, 참여한 이유는 방사선사의 요청 (46.0%), 본인의 의지(28.8%), 아이의 요청 (20.9%)로 나타났다. “소아 일반촬영 시 보호자의 참여가 필요하다고 생각하는가?”라는 질문에 그렇다(84.3%), 아니다(15.7%)로 나타났다. “참여할 생각이 있다면 그 이유는 무엇인가?”라는 질문에는 아이의 안정을 위해(80.8%), 부위에 따른 정확한 검사확인을 위해(18.1%)라고 나타났으며, “참여할 생각이 없다면 그 이유는 무엇인가?”라는 질문에는 방사선사의 업무이기 때문에(54.8%), 건강에 나쁜 영향을 미치기 때문에(19.4%), 방사선 피폭에 대한 막연한 두려움 때문에(12.9%)로 나타났다. 소아 일반촬영 시 보호자의 심리 상태 인식도 분석에서는 성별, 소아 연령에 따라 통계적으로 유의한 차이가 있었고(p<0.05), 소아 일반촬영 시 보호자의 방사선 인식도 분석에서는 성별, 학력에 따라 통계적으로 유의한 차이가 나타났으며(p<0.05), 소아 일반촬영 시 보호자의 방사선 피폭 방지에 대한 인식도 분석에서는 연령, 성별, 소아 연령, 학력에 따라 통계적으로 유의한 차이가 있었다(p<0.05). 결 론:연구 결과에서 볼 때 소아 일반촬영 시 보호자들은 우선적으로 아이의 안정과 정확한 검사를 위해 방사선사와 동반검사가 필요하다고 인식하고 있었으며, 방사선사의 불신이나 방사선의 피폭에 대한 신체적 위해요소에는 그다지 위험하다고 인식하고 있지 않았다. 그러므로 소아 일반촬영 검사 시 그들의 심리적 불안감을 해소하고 정확한 검사를 위해 보호자의 동반을 유도하는 것이 바람직하며 검사방법과 보호 장구 착용의 필요성을 설명할 수 있는 방안이 필요한 것으로 사료되어진다.
