        We experienced a case of organic mental disorder following Zinolta(Zipeprol 2HCI) abuse for about a year. he showed neuropsychiatric symptoms such as vivid auditory and visual hallucinations, idea of reference, aggressive behaviors, and drowsy mental states. During the hospitalization, he was treated with benzodiazepines without any other specific treatment. He was improved and discharged with clear sensorium without any psychotic symptoms. Zipeprol Hydrochloride is a centrally acting cough suppressant. There have been reports of abuse and overdosage producing neurological symptoms. Although lacking in the comprehensive study explaining the pttersn of abuse, pharmacological effects and incidence of psychotic symptoms, this case suggests the need for evaluation of the potentially dangerous orgainc psychotic symptoms among the populations who are exposed.

      • The rates in developmental dysplasia of the hip in breech presentation in twins and triplets

        Objective: To determine the incidence of developmental dysplasia of the hip (DDH) in breech presentation among multifetal pregnancies. Methods: This retrospective cohort study included 1256 consecutive live births who were born through cesarean section with at least one baby had been presented as non-cephalic presentation in each pregnancy from April 2013 to October 2018. Only births after 23+0 weeks of gestation were included. During analysis, neonates who had been presented as transverse lie in twins and triplets were excluded (n=39). In out institution, every baby is physically examined to assess the possibility of DDH by pediatricians and for the babies confirmed to be breech presentation during antenatal period, consultation to the orthopedists is routinely performed. The primary outcome was the diagnosis of DDH through ultrasonography. Results: The incidences of DDH diagnosed in neonates who had been presented as breech presentation antenatally were 5.1% (19/371) in singletons, 4.3% (20/463) in twins, and 8.1% (3/37) in triplets. In those three groups, the incidence of DDH was not significantly different. In twin pregnancy, the rate of DDH was significantly higher in breech presentations than cephalic presentations (4.3% vs. 0.3%, p<0.001). In the multivariate logistic regression analysis for the association between the incidence of DDH and various factors, breech presentation was the strongest and independent risk factor for the diagnosis of DDH (Odds ratio (OR) 8.9, 95% Confidence interval (CI) 2.1-38.3, p=0.003). Oligohydramnios and twin pregnancy were not statistically significant risk factors. however triplet pregnancy was found to be associated with DDH (OR 6.7, 95% CI 1.7-26.5, p=0.007). Conclusion: Pos Breech presentation is the strongest risk factor for the diagnosis of DDH even in the multifetal pregnancies.

        Changes in Meat Quality Characteristics of the Sous-vide Cooked Chicken Breast during Refrigerated Storage

        This study was performed to investigate the changes in meat quality characteristics of the sous vide cooked chicken breast during refrigerated storage at 4°C for 14 d between before and after sous-vide cooking. Cooking loss and shear force were significantly increased, whereas expressible drip was significantly decreased along with reduction in the water holding capacity in both of two groups. Redness of meat juice was significantly (p<0.05) increased during storage, and considerably increased in the refrigerated samples after sous-vide cooked at the 7 to 10 d. The thiobarbituric acid reactive substances (TBARS) was significantly increased and was higher in the refrigerator stored chicken breast samples after sous-vide cooking. The volatile basic nitrogen (VBN) value was significantly increased in both groups, but the VBN value of the stored raw meat sample before sous-vide cooking was increased at an early storage, while the VBN value of the stored sample after sous-vide cooking was increased gradually in this study. Total viable counts and coliform counts were significantly decreased during storage, and coliforms were not detected after 7 d of storage in both groups. Salmonella spp. was not detected during the whole studied period. The outcome of this research can provide preliminary data that could be used to apply for further study of chicken breast using sous-vide cooking method that could be attractive to consumers.

      • Hepatoprotective Effect of Gallotannin-enriched Extract Isolated from Gall on Hydrogen Peroxide-induced Cytotoxicity in HepG2 Cells

        <P><B>Background:</B></P><P>Gall (Galla Rhois [GR]) is known to have antibacterial, anti-inflammatory, antimetastatic, and anti-invasion activities and exert hepatoprotective effects. However, the hepatoprotective effects of gallotannin-enriched GR (GEGR) and their mechanisms have not yet been investigated.</P><P><B>Objective:</B></P><P>The potential protective effect of GEGR against hepatotoxicity induced by hydrogen peroxide (H<SUB>2</SUB>O<SUB>2</SUB>) was investigated.</P><P><B>Materials and Methods:</B></P><P>Changes in cell viability, apoptosis protein expression, and reactive oxygen species (ROS) generation were determined in HepG2 cells that were pretreated with four different concentrations of GEGR (6.25–50 μg/ml) for 24 h before H<SUB>2</SUB>O<SUB>2</SUB> exposure.</P><P><B>Results:</B></P><P>GEGR consisted of gallotannin (69.2%), gallic acid (26.6%), and methyl gallate (4.2%) and showed remarkable 2,2-diphenyl-1-picrylhydrazyl scavenging activity (inhibitory concentration 50% = 0.212 μg/ml). The lethal dose 50% and effective dose 50% values for the response of HepG2 cells to GEGR were determined to be 178 and 6.85 μg/ml, respectively. Significant reductions in the immunofluorescence intensity indicating apoptosis were also detected in the nuclei of HepG2 cells stained with 4’,6-diamidino-2-phenylindole and Annexin V after GEGR treatment. The Bax/Bcl-2 ratio and active caspase-3 level were higher in H<SUB>2</SUB>O<SUB>2</SUB> + vehicle-treated cells. However, these levels gradually decreased to those of the No-treated group in the GEGR pretreated group even though little or no decrease was observed in response to low GEGR concentrations.         Neuroprotective Effects of a Butanol Fraction of Rosa hybrida Petals in a Middle Cerebral Artery Occlusion Model

        The neuroprotective effects of a butanol fraction of white rose petal extract (WRPE-BF) were investigated in a middle cerebral artery occlusion (MCAO) model. Seven week-old male rats were orally administered WRPE-BF for 2 weeks and subjected to MCAO for 2 h, followed by reperfusion. Twenty-four h later, MCAO-induced behavioral dysfunctions were markedly improved in a dose-dependent manner by pretreatment with WRPE-BF. Moreover, higher dose of WRPE-BF not only decreased infarction area but also effectively reduced astrogliosis. The expression of inducible nitric oxide synthase, cyclooxygenase-2, and glial fi brillary acidic protein in MCAO model were markedly inhibited by WRPE-BF treatment. Notably, WRPE-BF decreased nitric oxide and malondialdehyde levels in the striatum and subventricular zone of stroke-challenged brains. These data suggested that WRPE-BF may exert its neuroprotective effects via anti-oxidative and anti-infl ammatory activities against ischemia-reperfusion brain injury and could be a good candidate as a therapeutic target for ischemic stroke.

      • Effect of Body Weight and Lifestyle Changes on Long-Term Course of Nonalcoholic Fatty Liver Disease in Koreans

        <P>BACKGROUND: Although the prevalence of nonalcoholic fatty liver disease (NAFLD) has been increasing, there have been few studies of long-term changes in NAFLD in large numbers of subjects. This study was performed to assess the long-term changes in severity of fatty liver and to analyze the association with changes in body weight and lifestyle factors. METHODS: We analyzed lifestyle factors, biochemical data, and abdominal ultrasonography in subjects who underwent voluntary medical checkups at a 5-year interval. Fatty liver on ultrasonography was graded as normal, mild, moderate, or severe by the degree of echogenicity. RESULTS: The analyzed population consisted of 2895 subjects of age 47+/-8.7 years (range, 20-79 years). Among the 1938 subjects who had normal livers at baseline, 374 subjects (19%) developed fatty liver 5 years later. Those who developed fatty liver showed more weight gain, increase of aspartate aminotransferase (AST), alanine aminotransferase (ALT), gamma glutamyl transferase (gamma-GT), and glucose levels during the 5-year period. Among the 930 subjects who had fatty liver at baseline, 263 (28%) subjects showed a decrease in grade and 209 (22%) progressed to a higher grade of fatty liver after 5 years. The group with a lower grade lost -2.2+/-4.3 kg of body weight from baseline, and reduction in hepatic steatosis grade was associated with decreased serum AST, ALT, triglycerides, gamma-GT, and fasting glucose levels. CONCLUSIONS: Even a small weight reduction was associated with improvements in NAFLD and related metabolic abnormalities such as dyslipidemia and hyperglycemia. Subjects with fatty liver should be advised to lose weight through lifestyle modifications.</P>
