        In vitro fertilization outcomes in women with surgery induced diminished ovarian reserve after endometrioma operation: Comparison with diminished ovarian reserve without ovarian surgery

        Objective To compare the in vitro fertilization (IVF) outcomes between women with diminished ovarian reserve (DOR) after endometrioma operation and women with DOR without ovarian surgery. Methods This retrospective case-control study included 124 women aged under 40 and had DOR (serum anti-Mullerian hormone level <1.1 ng/mL or antral follicle count ≤6). They participated in fresh first and/or second IVF cycles between March in 2010 and December in 2015. Basal characteristics and IVF outcomes were compared between 47 cycles (32 women) with surgery-induced DOR and 119 cycles (92 women) with DOR without ovarian surgery. Results Basal characteristics were similar in both groups except that the median ages were lower in the surgery-induced DOR group compared to the DOR group without ovarian surgery. The data regarding the controlled ovarian stimulation and IVF cycle outcomes showed similar result in both groups. Also, clinical pregnancy and live birth rate were not different significantly between two groups. Conclusion In the same condition of DOR, clinical pregnancy and live birth rate were not different significantly between two groups regarding etiology of DOR.

      • The rates in developmental dysplasia of the hip in breech presentation in twins and triplets

        Objective: To determine the incidence of developmental dysplasia of the hip (DDH) in breech presentation among multifetal pregnancies. Methods: This retrospective cohort study included 1256 consecutive live births who were born through cesarean section with at least one baby had been presented as non-cephalic presentation in each pregnancy from April 2013 to October 2018. Only births after 23+0 weeks of gestation were included. During analysis, neonates who had been presented as transverse lie in twins and triplets were excluded (n=39). In out institution, every baby is physically examined to assess the possibility of DDH by pediatricians and for the babies confirmed to be breech presentation during antenatal period, consultation to the orthopedists is routinely performed. The primary outcome was the diagnosis of DDH through ultrasonography. Results: The incidences of DDH diagnosed in neonates who had been presented as breech presentation antenatally were 5.1% (19/371) in singletons, 4.3% (20/463) in twins, and 8.1% (3/37) in triplets. In those three groups, the incidence of DDH was not significantly different. In twin pregnancy, the rate of DDH was significantly higher in breech presentations than cephalic presentations (4.3% vs. 0.3%, p<0.001). In the multivariate logistic regression analysis for the association between the incidence of DDH and various factors, breech presentation was the strongest and independent risk factor for the diagnosis of DDH (Odds ratio (OR) 8.9, 95% Confidence interval (CI) 2.1-38.3, p=0.003). Oligohydramnios and twin pregnancy were not statistically significant risk factors. however triplet pregnancy was found to be associated with DDH (OR 6.7, 95% CI 1.7-26.5, p=0.007). Conclusion: Pos Breech presentation is the strongest risk factor for the diagnosis of DDH even in the multifetal pregnancies.

        태양열 하이브리드 공정을 위한 유동층 입자들의 마모 및 열전달 특성 연구

        전기와 천연가스와 같이 안정적이며 신뢰할 수 있는 에너지를 현대 사회가 요구하기 때문에 재생에너지와 화석연료의 장점들을 모두 보유하고 있는 다양한 방식의 태양열 하이브리드 공정들이 세계 각국에서 개발되고 있다. 특히 고체 입자에 태양열을 저장하는 유동층 기반의 태양열 하이브리드 공정은 기존의 유동층 연소 및 가스화에 적용할 수 있을 것으로 기대받고 있다. 이에 본 연구에서는 ASTM D5757 반응기와 0.14 m의 직경과 2 m 높이의 유동층 반응기를 이용하여 태양열 하이브리드 공정의 유동층물질로서 검토되고 있는 실리콘 카바이드, 알루미나 입자들의 마모 및 열전달 특성을 고찰하였다. 특히 다양한 상업 유동층 반응기에서 유동층물질로 이용되는 모래와 비교하였다. 실리콘카바이드와 알루미나의 내마모성은 모래보다 우수하였으며 평균 열전달 계수도 125 ~ 152 W m<SUP>-2</SUP><SUP></SUP>K<SUP>-1</SUP> 범위를 가지는 것으로 고찰되었다. Various solar hybrid energy conversion processes, which have both the advantages of renewable energy sources and fossil energy sources, have been developed in the world because stable and predictable energy supplies, such as electricity and natural gas, are necessary for modern societies. In particular, a solar hybrid energy conversion process based on a dual fluidized bed process concept has been expected as the promising solution for sustainable energy supply via thermochemical conversions, such as pyrolysis, combustion, gasification, and so on, because solar thermal energy could be captured and stored in fluidized bed materials. Therefore, the attrition and heat transfer characteristics of silicon carbide and alumina particles used for fluidized bed materials for the solar hybrid energy conversion process were studied in an ASTM D5757 reactor and a bubbling fluidized bed reactor with 0.14m diameter and 2m height. These characteristics of novel fluidized bed materials were compared with those of sand particles which have widely been used as a fluidized bed material in various commercial fluidized bed reactors. The attrition resistances of silicon carbide and alumina particles were higher than those of sand particles while the average values of heat transfer coefficient in the bubbling fluidized bed reactor were in the range of 125 ~ 152 W m<SUP>-2</SUP>K<SUP>-1</SUP>.

        Changes in Meat Quality Characteristics of the Sous-vide Cooked Chicken Breast during Refrigerated Storage

        This study was performed to investigate the changes in meat quality characteristics of the sous vide cooked chicken breast during refrigerated storage at 4℃ for 14 d between before and after sous-vide cooking. Cooking loss and shear force were significantly increased, whereas expressible drip was significantly decreased along with reduction in the water holding capacity in both of two groups. Redness of meat juice was significantly (p<0.05) increased during storage, and considerably increased in the refrigerated samples after sous-vide cooked at the 7 to 10 d. The thiobarbituric acid reactive substances (TBARS) was significantly increased and was higher in the refrigerator stored chicken breast samples after sous-vide cooking. The volatile basic nitrogen (VBN) value was significantly increased in both groups, but the VBN value of the stored raw meat sample before sous-vide cooking was increased at an early storage, while the VBN value of the stored sample after sous-vide cooking was increased gradually in this study. Total viable counts and coliform counts were significantly decreased during storage, and coliforms were not detected after 7 d of storage in both groups. Salmonella spp. was not detected during the whole studied period. The outcome of this research can provide preliminary data that could be used to apply for further study of chicken breast using sous-vide cooking method that could be attractive to consumers.

        Experience of Penetrating Gunshot Wound on Head in Korea

        Craniocerebral gunshot injuries (CGIs) are extremely seldom happened in Korea because possession of individual firearm is illegal. So, CGIs are rarely encountered by Korean neurosurgeons or Korean trauma surgeons, though in other developing countries or Unites states of America their cases are indefatigably increasing. Management goal should focus on early aggressive, vigorous resuscitation. The treatments consist of immediate life salvage through correction of coagulopathy, intracranial decompression, prevention of infection and preservation of nervous tissue. There have been few studies involving penetrating CGIs in Korea. Here we present a case of penetrating gunshot wound in Korea. We present a 58-year-old man who was unintentionally shot by his colleague with a shotgun. The patients underwent computed tomography (CT) for assessment of intracranial injury. The bullet passed through the left parietal bone and right lateral ventricle and exited through the posterior auricular right temporal bone. After CT scan, he arrested and the cardiopulmonary resuscitation was conducted immediately. But we were unable to resuscitate him. This case report underscores the importance of the initial clinical exam and CT studies along with adequate resuscitation to make the appropriate management decision. Physicians should be familiar with the various injury patterns and imaging findings which are poor prognostic indicators.

        Physical and Sensory Properties of Ice Cream Containing Fermented Pepper Powder

        This study was conducted to investigate the physical and sensory properties of ice cream containing fermented pepper powder. Three ice cream formulas were manufactured: 1, control; 2, supplemented with 0.1% fermented pepper powder; and 3, supplemented with 0.2% fermented pepper powder. Formulas 2 and 3 had significantly higher viscosity and lower overrun than formula 1 (p<0.05). Additionally, ice creams supplemented with fermented pepper powder were harder and maintained their forms longer than the controls. 0.2% fermented pepper powder added ice cream had no pungency as much as that of control and overall sensory attribute was not significantly different from control. Therefore, ice cream containing fermented pepper powder maintained physical and sensory properties similar to the controls, and maintenance was better. It means fermented pepper powder ice cream can be utilized as the material of functional food (dessert).
