        Trans-sphenoidal approach에 의한 pituitary microadenoma 수술 후의 임신 1례

        We treated a patient with pituitary microadenoma having hyperprolactinemia and irregular menstruation infertility by transsphenoidal microsurgery. Her menstrual cycle become irregular after marriage but previous menstrual cycle was regular. When she visited our hospital due to infertility, initial serum prolactine level was slightly elevated 35.68ng/ml, During bromecriptine therapy, abrupt increase of serum prolactine level up to 126.52 ng/ml and headache and facial flush and galactorrhea developed. Pituitary microadenoma was confirmed by CT and MRI. The patient underwent transsphenoidal microsurgery. Patient was pregnant after surgery, and it was confirmed by USG and she is following up, OPD now.

        Toxicological Characterization of Phthalic Acid

        There has been growing concern about the toxicity of phthalate esters. Phthalate esters are being used widely for the production of perfume, nail varnish, hairsprays and other personal/cosmetic uses. Recently, exposure to phthalates has been assessed by analyzing urine for their metabolites. The parent phthalate is rapidly metabolized to its monoester (the active metabolite) and also glucuronidated, then excreted. The objective of this study is to evaluate the toxicity of phthalic acid (PA), which is the final common metabolic form of phthalic acid esters (PAEs). The individual PA isomers are extensively employed in the synthesis of synthetic agents, for example isophthalic acid (IPA), and terephthalic acid (TPA), which have very broad applications in the preparation of phthalate ester plasticizers and components of polyester fiber, film and fabricated items. There is a broad potential for exposure by industrial workers during the manufacturing process and by the general public (via vehicle exhausts, consumer products, etc). This review suggests that PA shows in vitro and in vivo toxicity (mutagenicity, developmental toxicity, reproductive toxicity, etc.). In addition, PA seems to be a useful biomarker for multiple exposure to PAEs in humans.

      • Decreased Expression of FADS1 Predicts a Poor Prognosis in Patients with Esophageal Squamous Cell Carcinoma

        FADS1 (fatty acid desaturase 1) plays a crucial role in fatty acid metabolism, and it was recently reported to be involved in tumorigenesis. However, the role of FADS1 expression in esophageal squamous cell carcinoma (ESCC) remains unknown. In the current study, we investigated the expression and clinical pathologic and prognostic significance of FADS1 in ESCC. Immunohistochemical analyses revealed that 58.2% (146/251) of the ESCC tissues had low levels of FADS1 expression, whereas 41.8% (105/251) exhibited high levels of FADS1 expression. In positive cases, FADS1 expression was detected in the cytoplasm of cells. Correlation analyses demonstrated that FADS1 expression was significantly correlated with tumor location (p=0.025) but not with age, gender, histological grade, tumor status, nodal status or TNM staging. Furthermore, patients with tumors expressing high levels of FADS1had a longer disease-free survival time (p<0.001) and overall survival time (p <0.001). Univariate and multivariate analyses revealed that, along with nodal status, FADS1 expression was an independent and significant predictive factor (p<0.001). In conclusion, our study suggested that FADS1 might be a valuable biomarker and potential therapeutic target for ESCC.

      • WWW기반에서 테스트 및 진단을 위한 WBI 시스템의 설계 및 구현

        웹의 개방성과 융통성은 웹의 교육적 활용을 높였으며, 다양한 기술을 이용한 웹기반 교수 시스템이 많이 개발되고 있다. 그러나 이들 WBI(Web Based Instruction)는 교육 효율성면에서 아직 부족한 점이 많다. 특히 현재까지 개발된 웹기반의 테스트 시스템들은 단지 학습자의 반응에 대하여 정오와 점수 제시에 그치고 있는 실정이다. 따라서 학습자들은 면대면의 수업에서처럼 자신의 부족한 능력에 대해 적절한 조언을 받기가 어렵다. WBI가 학습자에게 단순한 테스트 결과만을 제시하지 않고, 학습 영역별 이해상태를 세밀히 진단하여 학습 정보를 제공하고, 특히 학습자의 오류에 대하여 이의 원인을 진단하여 제공한다면 이는 학습자에게 유익한 정보가 될 것이다. 본 연구에서는 학습자가 시간과 장소에 제한을 받지 않고 인터넷의 브라우저를 통해 본 교수 시스템에 접속하여 학습하면, 학습자의 반응에 대해 단순한 점수만을 제공하는 것이 아니라, 영역별 학습 성취도를 제시하고 부족한 점에 대해 조언하는 학습자 테스트 및 진단 시스템을 설계 개발하고, 그 방법을 기술하였다. A web support open environment in which flexibility that allows it to be applied in the education field has gradually evolved, but the WBI(Web Based Instruction) which compose it have many limitations and problems, as far as learning efficiency is concerned. In particular, existing web-based estimation systems just give information on whether learners' replies are 'correct' or 'incorrect' and offer the learners evaluations of results in terms of scores. Therefore, it is difficult for the learners to get more detailed information about their shortcomings and errors. What is needed for the learners is that web based instruction systems diagnose learners' comprehension status, providing causes: Why did the learners make the errors? In this paper, we propose the development of a web-based instruction system that learners can access with their browsers at any time and no matter where they are. Our system has a facility that analyses learners' weak points and diagnoses error cause, giving advice to learners and more detailed error information than existing systems. By accumulating user behaviors, relevant individualized information on the learners can be given.

        The differentiation of human multipotent adult progenitor cells into hepatocyte-like cells induced by coculture with human hepatocyte line L02

        Purpose: The aim of this study was to establish an in vitro method to purify human multipotent adult progenitor cells (hMAPCs) and assess their possible differentiation into hepatocytes by coculture with human hepatocyte line L02. Methods: hMAPCs were isolated by magnetic activated cell sorting (MACS) depletion selection using CD45 and GlyA microbeads. After indirect or direct coculture of hMAPCs and human hepatocyte line L02, the expression of albumin (ALB), alpha-fetoprotein (AFP), cytokeratin (CK) 18, and CK19 by hMAPCs was detected by immunocytochemistry. Results: With the MACS method, (5?10) × 10⁴/mL hMAPCs could be separated from 1 × 10?/mL bone marrow mononuclear cells. The purity of CD45-/GlyA- cells separated from bone marrow adherent cells was more than 98%, as determined by flow cytometry. In the coculture without cell-to-cell contact, hMAPCs expressed high AFP on day 1, and then tapered daily to low expression on day 7; ALB expression reached its peak on day 5, and remained high on day 7; CK18 was initially expressed on day 5 and was higher on day 7; CK19 was negative in all assays. In the coculture with cell-to-cell contact, ALB and CK18 were expressed by most cells while AFP appeared in only a few on day 5. Conclusion: hMAPCs were induced to differentiate into mature hepatocyte-like cells by coculture with a hepatocyte cell line, either with or without cell-to-cell contact, but the former seemed more effective.

        In recent years, all inorganic bismuth lead-halide perovskite nanocrystals [CsPbX3 (X=Cl, Br, I)] have received extensive attention due to their high performance in fluorescence quantum yield, narrow emission spectrum, and adjustable emission range. However, the disadvantages of high cost and poor stability have greatly limited the development prospects of the material. Here, in order to develop a perovskite quantum dot lasing cavity with high chemical stability, high quality factor and low fabrication cost, we have successfully fabricated a 3D random cavity device based on porous silicon/TiO2 nanowires. A TiO2 nanowire is grown on the porous silicon to form a 3D resonant cavity, and a perovskite quantum dot is spin-coated on the surface of the 3D resonant cavity to form a novel 3D complex film. The novel structure enhances the chemical stability and lasing quality factor of the resonant cavity while the fluorescence generated by the large quantum dots in the spatial interference structure constitutes the feedback loop, which will provide favorable support for the development of information optics.

        Backgrounds:?Tenofovir (TDF) is nucleoside analogue with high potency for profound and durable viral suppression and genetic barriers against resistance; TDF is recommended for the first-line treatment of chronic hepatitis B (CHB) in current guidelines, but data in Korean patients from community studies are lacking. The aim of this study was to evaluate the efficacy of TDF, and to determine patient-dependent or laboratory variables that predict virological response. Methods:?We retrospectively investigated the efficacy of TDF treatment for more than 6 months in 456 nucleos(t)ide-naive CHB patients. The primary endpoint was a virological response (VR), defined as an HBV DNA level of <12 IU/mL. Secondary endpoints were rates of ALT normalization (ALT <upper limit of normal), HBeAg seroconversion, partial virologic response (PVR), virologic breakthrough (VBT), and safety.?Results:?The median duration of TDF therapy was 22.0 (range, 6-40) months. Two hundreds sixty-five (58.1%) patients were HBeAg positive. The mean pre-treatment HBV DNA level was 6.34±1.41 log10 IU/mL. Among the 380 patients with elevated ALT levels at baseline, 343 patients (90.3%) had ALT normalization. VR was achieved in 379 patients (83.1%) during treatment. The cumulative rates of VR at 6, 12, 24 and 36 months were 53.4%, 73.2%, 90.6%, and 95.5%, respectively. PVR was evident in 208 (45.6%) patients. VBT was observed in 25 patients (5.5%). Multivariate logistic regression analysis using selected baseline factors identified absolute HBV-DNA levels at baseline (p<0.001; OR, 0.512; 95% CI, 0.397-0.661) and HBeAg positivity (p<0.001; OR, 0.781; 95% CI, 0.717-0.852) as factors showing significant association with VR. Among 265 HBeAg-positive patients, 43 (9.4%) achieved HBeAg seroconversion. Most adverse events were mild in severity.?Conclusions:?TDF is effective and well tolerated in Korean CHB patients in real life practice, consistent with larger registration trials. The lower HBV DNA levels at baseline and HBeAg-negative CHB patients were significantly associated with VR.

        CRITICAL FUJITA EXPONENT FOR A FAST DIFFUSIVE EQUATION WITH VARIABLE COEFFICIENTS

        In this paper, we consider the positive solution to a Cauchy problem in $\mathbb{B}^N$ of the fast diffusive equation: ${\mid}x{\mid}^mu_t={div}(\mid{\nabla}u{\mid}^{p-2}{\nabla}u)+{\mid}x{\mid}^nu^q$, with nontrivial, nonnegative initial data. Here $\frac{2N+m}{N+m+1}$ < $p$ < 2, $q$ > 1 and 0 < $m{\leq}n$ < $qm+N(q-1)$. We prove that $q_c=p-1{\frac{p+n}{N+m}}$ is the critical Fujita exponent. That is, if 1 < $q{\leq}q_c$, then every positive solution blows up in finite time, but for $q$ > $q_c$, there exist both global and non-global solutions to the problem.
