        Matched Comparison of Fusion Rates between Hydroxyapatite Demineralized Bone Matrix and Autograft in Lumbar Interbody Fusion

        Objective : To compare the fusion rate of a hydroxyapatite demineralized bone matrix (DBM) with post-laminectomy acquired autograft in lumbar interbody fusion surgery and to evaluate the correlation between fusion rate and clinical outcome. Methods : From January 2013 to April 2014, 98 patients underwent lumbar interbody fusion surgery with hydroxyapatite DBM (HA-DBM group) in our institute. Of those patients, 65 received complete CT scans for 12 months postoperatively in order to evaluate fusion status. For comparison with autograft, we selected another 65 patients who underwent lumbar interbody fusion surgery with post-laminectomy acquired autograft (Autograft group) during the same period. Both fusion material groups were matched in terms of age, sex, body mass index (BMI), and bone mineral density (BMD). To evaluate the clinical outcomes, we analyzed the results of visual analogue scale (VAS), Oswestry Disability Index (ODI), and Short Form Health Survey (SF-36). Results : We reviewed the CT scans of 149 fusion levels in 130 patients (HA-DBM group, 75 levels/65 patients; Autograft group, 74 levels/65 patients). Age, sex, BMI, and BMD were not significantly different between the groups (p=0.528, p=0.848, p=0.527, and p=0.610, respectively). The HA-DBM group showed 39 of 75 fused levels (52%), and the Autograft group showed 46 of 74 fused levels (62.2%). This difference was not statistically significant (p=0.21). In the HA-DBM group, older age and low BMD were significantly associated with non-fusion (61.24 vs. 66.68, p=0.027; -1.63 vs. -2.29, p=0.015, respectively). VAS and ODI showed significant improvement after surgery when fusion was successfully achieved in both groups (p=0.004, p=0.002, HA-DBM group; p=0.012, p=0.03, Autograft group). Conclusion : The fusion rates of the hydroxyapatite DBM and Autograft groups were not significantly different. In addition, clinical outcomes were similar between the groups. However, older age and low BMD are risk factors that might induce non-union after surgery with hydroxyapatite DBM.

        3차원 재구성법을 이용한 수복물의 정량적 미세누출도 측정

        Established microleakage tests have their own disadvantages. In this study, 3D reconstruction method was tried to overcome these disadvantages. Four types of microleakage tests were used and relationships among them were estimated: penetrated dye volume: marginal adaptability: degree of dye penetration and relative penetrated length to cavity wall. Twenty-four Class V cavities were bulk filled with composite (Esthet X) following surface treatments: N group (no treatment): E group (etching only): T group (etching + Prime & Bond NT). 50% silver nitrate was used as a dye solution after thermocycling (5℃ & 55℃, 1.000 times). Teeth were serially ground with a thickness of 0.2 mm. Volume of dye penetration was estimated from a three-dimensionally reconstructed image with a software (3D-DOCTOR). Percentage of margin without gap was estimated from SEM and degree of dye penetration and the relative length of dye penetration to overall cavity wall were also estimated. ANOVA and Scheffe test for dye volume, Kruskal-Wallis and Mann-Whitney test for marginal quality, Spearman's rho test for checking of relationships among methods were used. The results were as follows: 1. Dye penetration could be seen from several directions, furthermore, its volumetric estimation was possible. 2. Reverse relationship was found between dye volume and marginal quality (r = -0.881/ p = 0.004). 3. Very low relationship was seen between dye volume and two-dimensional tests (degree of dye penetration and relative length). However, 2D evaluation methods showed high relationship (p = 0.002-0.054) each other. 4. Three times vertical section could be recommended as a 2D test.

      • 천마의 GABA-benzodiazepine 수용체 복합체에 대한 조절작용

        Methanol extract of G. elata inhibited the binding of [³H]Ro15-1788, a selective benzodiazepine receptor antagonist, to benzodiazepine receptor of rat cortices. Saturation experiments followed by Scatchard analysis of the results showed that the inhibition of [³H]Ro15-1788 binding by G. elata. appeared to be competitive. These competitive inhibiton of the butanol fraction was observed to be higher than the methanol extract. Methanol extract of G. elata inhibited a [³H]flunitrazepam, a selective benzodiazepine receptor agonist, binding to benzodiazepine receptor. GABA significantly enhanced the inhibition of [³H]flunitrazepam binding by G. elata, and these "positive GABA shift" supported the strong possibility of agonistic activity to benzodiazepine receptor. Butanol fraction was observed to be higher than crude extract by methanol in an agonistic activity to benzodiazepine receptor, furthermore enhanced the binding of [³H]SR95531 to GABA_(A) receptor. Butanol fraction of G. elata significantly diminished the pentylenetetrazole-induced lethality of mice. From these results, it can be concluded that substance or substances with neurochemical properties characteristic of a benzodiazepine receptor agonist may be important components, and contribute to the anticonvulsant property of G. elata.

        실내사무환경의 환경성담배연기(ETS)의 지표물질에 관한 연구

        The purpose of this research is to evaluate indoor office environment using the concentrations of nicotine, 3-enthenyl-pyridine(3-EP), and respirable suspended particulate(RSP), which are indicators for environmental tobacco smoke(ETS) and the correlations between indicators and environmental conditions(smoking density, smoking index). The mean air charge per hour (ACH) in smoking rooms was 10.4 and most of the smoking rooms showed non-compliance withe ASHRAE standard value except only one smoking rooms. The concentrations of RSP, 3-EP, nicotine showed log=normal distributions, and became different statistically depending on smoking condition(p〈0.01). The geometric mean concentration of RSP in smoking room was 441.7 ug/㎡ that is far exceeded environmental standard(150 ug/㎡). This implies that fine particulate in smoking room should be carefully controlled considering smoking density and ventilation fate. The mean concentrations of nicotine and 3-EP were 93.4 ug/㎥, respectively. The correlation coefficients between RSP and SI, 3-EP and SI, and Nicotine and SI were 0.67, 0.84 and 0.74, respectively. The correlation coefficient between nicotine and 3-EP, nicotine and RSP, and RSP and 3-EP were 0.76,0.78 and 0.57 respectively.

      • 『金匱要略』과 『傷寒論』의 相似條文에 대한 分析

        『傷寒論』과 『金匱要略』은 仲景의 著述로서 醫方之祖 인정되어 왔지만, 두 책의 관계에 대해서는 오랫동안 논란이 있어 왔다. 그러나 『金匱要略』과 『傷寒論』의 相似條文은 각 책의 108%, 11%를 차지할 만큼 많이 있으며 각 조문을 분석한 결과 높은 상동성을 가지는 조문이 전체의 63.9%를 차지하고 있는데, 이것은 두 책이 서로 동일한 근원에서 나왔음을 보여준다. 따라서 『傷寒論』과 『금궤요략』의 관계를 이해함에 있어서 이들 相似條文에 대한 인식이 반드시 선행되어야 할 것으로 생각된다. Shanghanlun and Jinguiyaolue has the analogous text by the rate of 10.8% and 11.3% respectively. And We have found that 63.9% of them are very similar by the analysis on the analogous text. It is supposed that the Books are originated by same source. So we suggest that the analogous text is very important to understand the relationship of the Books.

        부정교합의 유형과 성별에 따른 자연두부위치의 재현성에 관한 연구

        교정치료의 진단 및 치료계획의 수립시 이용되는 측모두부방사선 사진을 이용하여 골격성 부정교합의 형태와 성별에 따른 두부위치의 재현성을 파악하고자 성인남녀 90명을 대상으로 자세변수 6항목을 설정하여 연구한 결과 다음과 같은 결과를 얻었다. 1. 자연두부위치의 재현성에 있어서 남,녀 각각의 자세변수는 통계학적인 유의성이 없었다 (P〉0.05). 2. 자연두부위치의 재현성에 있어서 각 부정교합군과 전체 표본 각각의 자세변수는 통계학적인 유의성이 없었다(P〉0.05). 3. method error를 이용한 자연두부위치의 재현성은 모든군에서 우수하였다. 결론적으로 자연두부위치의 재현성은 골격성 부정교합의 양상과 성별에 관계없이 우수하였는데, 이는 자연두부위치를 이용한 많은 연구에 도움이 될 뿐 아니라 교정적 문제와 관련된 기능과 형태의 관계를 파악하고자 할 때 그 가치를 높이는데 기여할 수 있으리라 생각된다. This study was performed to evaluate the reproducibility of natural head position according to skeletal malocclusion types and sex using cephalometric radiographs for establishing orthodontic diagnosis and treatment planning. The sample consisted of 90 young adults (male 45, female 45) who had the skeletal malocclusion. Cephalometric radiographs were taken in natural head position, and statistical analysis was performed and method error of 6 postural variables were estimated to evaluate the reproducibility of the natural head position. The following results were obtained : 1.In the reproducibility of the natural head position, postural variables had no statistical significance in male and female (P〉0.05). 2.In the reproducibility of the natural head position, postural variables had no statistical significance in Class I, II, III, and total group (P〉0.05). 3.The reproducibility of natural head position using method error was excellent in all groups.

        레이저 박피술의 피부면적 감소효과와 레이저 punch out 병변의 치유에 대한 실험적 연구

        An experimental study was designed to analyze the surface reduction effect of ultrapulse carbon dioxide laser on the skin of guinea pigs receiving laser resurfacing. Two squares of skin measuring 3x3 ㎠ were designed on each side of the abdomen of guinea pigs(n=10). Five animals received laser resurfacing on their abdomen at energy of 250mJ/5 watt, and five other animals received 500 mJ/5 watt. On each animal, the left side square was treated with one pass of a CO2 laser, and the right side q=square was treated with two passes using a C.P.G. scanner. The size of the resurfacing area was determined by celluloid overlays at 1,2,3,4 and 6 weeks. Immediately following laser resurfacing, the area was decreased to 69%(left side) and 56%(right side) of the designed area in the 250 mJ/5 watt group, and 59% and 40% in the 500mJ/5 watt group. After 6 weeks, the resurfaced area had been reduced to 70% and 56% of the designed area in the 250mJ/5 watt group, and 60% and 40% in the 500mJ/5 watt group. As a result, the effect of surface reduction by CO2 laser resurfacing increased at a higher power and when more passes of the ultrapulse carbon dioxide laser were applied. The laser treated skin continued to maintain its contracted dimensions. These findings suggest that ultrapulse carbon dioxide laser may be an effective treatment method for rhytides. Another experiment was performed on multiple punch-out lesions on guinea pig skin(n=2) with 1,2,3 mm collimated hand pieces for the purpose of determining a new laser treatment method. After 4 weeks, on gross and histological examination, there were no differences in the healing of multiple punch-out lesions and the healing of punch-out lesions was complete without any scarring. These findings suggest that the laser punch-out method can be used for the treatment of acne scars.

      • 소중합 디엔체인 1,3-dithiole-2,4,5-trithione의 Diels-Alder 형태의 [2+4] 고리화 첨가 반응에 관한 연구

        A Diels-Alder type [2+4] cycloaddition reaction of oligomeric diene (1,3-dithiole-2,4,5-trithione : (C3S5)n) with dienophiles containing warious functional groups was studied. Extended multisulfur system was achieved via [2+4] cycloaddition of TTF(1). Quinone(4), dienophiles containing cyanyl group(5-7), triple bond(14-17) and conjugated double bonds (25,26), however, failed to form cycloadducts with (C3S5)n. the reaction of (C3S5)n with dienophiles containing electron withdrawing groups such as -Co2CH3(18), phenyl(19-24), ferrocenyl(27,28), pyridyl(29,32) group was readily achieved. In case of asymmetrical dienophiles such as 19, 27, 29 and 30, the electron withdrawing ability was more efficient than symmetric dienophiles. Dienophiles of 4-vinyl pyridine group(30,32) were more reactive than that of 2-vinyl pyridine group(29,31). This reaction also could be carried out by photoreaction as well as direct thermal reaction. Eventhaugh we observed no reaction between 6 and (C3S5)n by direct thermal method, it yielded some cycloadduct via photoreaction even in very low yield. The cycloadducts, EDT-DTT derivatives, were converted to EDT-DTO derivatives using Hg(OAc)2 in CHCI3. The EDT-DTT(O) DERIVATIVES can be good can-didates as precursors to organic electronic materials.
