      • 가토에서의 삽입물 피막피판에 관한 실험적 연구

        Silicones are a group of inert and inorganic compounds that were introduced over three decades ago and can be prepared in many different forms, which vary from hard rubber to the liquid crystals. Use of the silicone is widely accepted in almost every medical and dental specialty, perhaps most commonly in reconstructive surgery. A silicone implant elicits a capsular response in surrounding soft tissue. This phenomenon is well documented since significant capsular contracture occurs commonly as a complication of breast implants. The capsules are built up of an extremely tough, tendon-like collagenous material, which is arranged in a layer and surrounds the prosthesis completely. Three to four weeks after insertion of the implant, the capsule is already formed and it is made of capillaries, larger blood vessels, active macrophages, fibroblasts and a small amount of collagen. Later the capsule becomes less vascularized, more and more collagen are laid down and macrophages become inactive. The aim of this study was to examine whether this side effect could be used as a new source of flaps since the capsule has a structure and texture similar to fascia. With this objective, capsule formation was induced in rabbits by implanting spherical tissue expanders under the skin vascularized by the inferior epigastric vessels. When expansion was completed and the capsule was formed, the expander was removed and the animals were categorized into three groups. In the first group, the roof of the capsule was simply everted as hinge flap. In the second group, a capsule island flap was raised and orthotopically placed. In the third group, a capsule flap was raised and transferred to the ipsilateral injured Achilles tendon area for evaluation of ability to provide gliding surface. Four months later, a tendon-capsule-skin unit was excised and microscopically studied for late changes of capsule flap. The inner side of the various flaps for the first and second experimental groups and the outer side of the island flaps for the third group were covered with autologous full thickness skin graft to find out whether a peri-implant capsule was an adequate recipient site and this might be a new source of flaps for use in reconstructive surgery and this might be a new source of flaps for use in reconstructive surgery. In the three experimental groups, there was complete 'take' of the skin grafts in 80% of the animals. In conclusion, island or free flaps of capsular tissue may be raised and transferred safely. And the capsular flap, which provides a good gliding surface without adhesion even in the late stage, can be very useful if it is used as a fascia-like tissue to cover tendons.

        성전환증 치험례

        The transsexualism is defined as "a distribution of gender identity in which the person manifests, with constant and persistent conviction, the desire to live as a member of the opposite sex and progressively takes steps to live in the opposite sex role full-time." Although its etiologty is unknown, the majority of modern psychiatrists and behavioral scientists agree that the transsexualism is a disease entity belonged to gender identity disorder, but not to be confused with the transvestite, homosexualism, or psychopaths. We experienced five cases of male-to-female transsexualism. The basic requirements for this type of operation were that the patient be more than 21 years old, a transsexual for at least 3 years, cross dressing for more than 1 year and taking female hormones for at least 9 months. The patient who approved himself a transsexualism by psychiatrists with psychological tests including MMPI, Rosarch test had recieved several preoperative studies including EEG to rule out temporal lobe epilepsy, excrtory urogram to rule out genitourinary abnormalities, sex chromation study with peripheral blood smear for diagnosis of genetically normal male person, and hormonal study(17-KS in urine) to rule out endocrine abnormalities. Once the patient was admitted to the hospital , he was placed on a 2-day mechanical bowel preparation with a clear liquid diet. The three male transsexuals underwent the vaginal reconstruction using one-stage penile skin flap inversion surgery after vaginal pocket dissection, and the other two transsexuals who had recieved penile amputation underwent modified McIndoe operation for vaginal reconstruction using full-thickness skin graft from inguinal area. The ancillary procedures for male transsexuals including double fold operation, augmentation rhinoplasty, augmentation mammoplasty, facial bone contouring surgery, laryngeal chondroplasty, and electrolysis were also done, before of simultaneously. Herein we describe the technique, mainly about vagnal reconstrution and briefly report the results

        피부피판의 생존을 증가시키는 약제의 실험적 연구

        There are several kinds of known pharmacological agents to increase the flap survival. Those are known to be sympatholytics, vasdilators, rheologic agents and drugs which increase the tolerance of ischemia. many experimental studies were reported, however, the results were conflicting. Therefore, we select three medicines which are more known to be effective for flap survival. Those are Pentoxylline (Trental), Chloropromazine (Sepamine) and Prednisolone. Experimental study was performed on rats to demonstrate the efficacy of them. We used 40 Sprague-Dawley rats which were divided into 4 groups; those were treated with Pentoxyfyline, with Chloropromazine, with Prednisolone and with saline as a control. Cranially based McFarlane flaps were used in the midline of the dorsums of rats. The surviving lengths were measured. From this experiment, we proved that those three medicines are very effective and almost samely effective to increase the flap survival.

        한 가족에서의 안인두근 이양증에 의한 안검하수증의 치험례

        Oculopharyngeal muscular dystrophy is a hereditary, autosomal dominant, slowly progressive disorder with middle age onset, major symptoms are ptosis and dysphagia mainly due to selectively involved levator palpebrae and the pharyngeal muscles. Progressive, usually symmetric blepharoptosis with or without dysphagia appears in middle age. Limb muscular weakness can be noted in some patients. This report describes 4 cases of oculopharyngeal muscular dystrophy in one family. All patients presented with slowly progressive bilateral ptosis and slight weakness of facial and bulbar muscles. The ptosis was severe in all cases at the time of surgery and levator resection was done via transcutaneous approach. The surgical result was satisfactory with all patients after 1 year follow-up.

        컨테이너 터미널 운영사의 경영성과 분석에 관한 연구 : 중국 항만 운영사와 부산 신항 운영사를 중심으로

        본 연구는 중국 항만 운영사와 부산 신항 운영사의 재무 및 운영 현황을 파악하여 주요 개선방안을 제시하고자 한다. 2013년부터 2017년까지 부산과 중국의 항만 운영사의 연간 재무보고서를 토대로 수익성, 안정성, 성장 및 활동을 부산 신항 5개 운영사와 중국의 6개 항만 운영사의 실제 운영을 바탕으로 8개의 지표를 분석하였다. 그 결과 첫째, 부산 신항과 중국 항만 운영사는 수익성, 안정성, 성장성, 안보 측면에서 나름의 장점을 가지고 있다. 둘째, 일반적으로 중국 6개 항만 운영사의 자산수익률과 총자산 등 재무상태가 부산 신항 운영사보다 더 좋은 것으로 나타났다. 그러나 중국 항만 운영사의 유동비율은 낮고 자산수익률은 높지 않다는 결과가 나왔다. 부산 신항 운영사는 유동비율과 자산수익율이 중국 6개 항만 운영사보다 양호하지만 매출이 매우 낮고 일부 순이익은 음(-)수이며 부채비율은 높은 것으로 나타났다. 따라서 중국 항만 운영사는 이익과 자산수익률의 증가와 재무위험을 줄이고, 정부의 항만에 대한 관리감독 강화로 수익성과 경쟁력을 향상시켜야 한다. 한편 부산 신항 운영사는 해외 비즈니스 유치로 매출액 증대, 부채 감소의 전략과 경영비용을 낮추어야 한다. 운영체계의 개선과 볼륨 인센티브의 개편으로 효율성을 높이는 방안이 필요할 것으로 분석되었다. Purpose : The purpose of this study tries to grasp the financial and operational status of the Chinese and the Busan new port operators, and to present improvement measures. Research design, data, methodology : Based on the annual financial reports of the port operators of Busan and China from 2013 to 2017, the profitability, stability, growth and activities were analyzed based on the actual operation of the 5 new port operators of Busan and 6 operators of China. Results : Analysis shows that the six Chinese port operators are somewhat better than the Busan New Port operators. It showed that the return on assets is stable, the total turnover rate is high but the current ratio is low and the return on assets is not high. The Busan new port operator has a higher liquidity ratio and some asset returns than the Chinese port operator, but sales are very low and some net profits are negative and debt ratios are high. Conclusions : Therefore, the Chinese port operator should improve profitability and competitiveness by increasing profit and asset return, reducing financial risk, and strengthening the government s supervision and supervision of ports. Busan s new operators are expected to need measures to increase sales, reduce debt and management costs by attracting overseas businesses and increase efficiency by improving its operating system and volume incentives.

        반복하중을 받는 고강도강 기둥을 적용한 WUF-W 접합부의 이력 거동

        This study's purpose is to evaluate the seismic performance of Welded Unreinforced Flange-Welded Web connections with the column composed of high-performance steel. The study involved the inelastic cyclic loading test of 4 full-scale exterior connection specimens to evaluate the effect of column strength and beam depth. All specimens are designed as strong column-weak beam and strong panel zone. They are observed at the peak deformation of each cycles with the unaided eye. 3 of 4 specimens including 2 specimens consisted of high-performance steel column are achieved total story drift of 0.04 rad or more prior to failure. The test results showed that ductility capacity of connections with high-performance steel column is sufficient in light of current AISC seismic design. Energy dissipation in specimens consisting of high-performance steel column is less than those of normal strength steel column. Also, K120TG, which is the developed weld metal for 800MPa grade steel, is estimated as good material in case of welding between normal strength steel and high-performance steel through this study.
