Nalbuphine 전처지후 Propofol을 이용한 전정맥마취시 Doxapram이 호흡에 미치는 영향

Background: Total intravenous anesthesia with propofol can cause respiratory depression and apnea especially during induction of anesthesia. To study the possibility of reversal of respiratory depression during anesthesia with propofol, pretreated with nabuphine or not, the respiratory effects of doxapram to spontaneously ventilating patients were investigated. Methods: Patients were divided into 4 groups - saline-propofol-saline group(SPS), saline-propofol- doxapram group(SPD), nalbuphine-propofol-saline group(NPS), and nalbuphine-propofol-doxapram group (NPD). After saline or nalbuphine pretreatment, anesthesia was induced with propofol and then doxapram or saline was intravenously injected. Apneic time interval, blood pressure, heart rate, respiratory rate, minute ventilation, end tidal CO2 partial pressure and oxygen saturation were measured in every minutes during induction of anesthesia. Percent changes of each values were compared. Results: There is no differences in apneic time intervals in each groups. The percent change of first minute ventilation in SPD group after doxapram injection unchanged significantly compared with those depressions of SPS, NPS and NPD group(p<0.05). Respiratory rates increased in SPD and SPS groups after laryngeal mask insertion. There is no differences in minute ventilation, respiratory rate and end-tidal CO2 concentration between nalbuphine pretreated groups regardless of doxapram injection. Conclusions: Doxapram has effect in increasing minute ventilation after propofol induction within first few minutes, but it cannot reverse respiratory depression during propofol induction pretreated with nalbuphine. Purpose: The most accepted method for the reanimation of a paralyzed face is the two-stage method that combines cross-face nerve grafting with free-muscle transfer. Although the results of reconstruction with this method are satisfactory, there is an excessive delay between the stages, which prolongs the period of rehabilitation. In order to overcome this drawback, a one- stage, neurovascular free-flap reconstruction method using free neurovascular muscle flaps is introduced. Methods: From 1994 to 2004, 35 patients with longstanding facial palsy were treated. Fifteen patients underwent the single-stage reconstruction with the latissimus dorsi muscle, and 20 patients underwent the two-stage reconstruction method with the gracilis muscle. We compared the long-term results of the two methods of reconstruction. The mean follow-up period was 28.7 months for one-stage reconstruction, and 35.2 months for the two-stage, respectively.Results: In the patient group of the single stage reconstruction, both mouth corner excursion and animation grade were markedly improved at the final postoperative visit. Moreover, the first muscle contraction occurred earlier in this group, than in the two-stage reconstruction group. However, four patients in the single stage group never achieved a first muscle contraction or mouth corner excursion.Conclusion: Facial palsy is a very challenging condition for cosmetic surgeons to deal with. Traditional methods for treatment of chronic facial palsy use a two- stage muscle flap which is time-consuming and burdensome to patients, many of whom are averse to waiting 8 to 12 months to complete the two stages. The one- stage reconstruction method described herein uses a latissimus dorsi free-flap and has demonstrated consistent positive outcomes in clinical assessments.
복강경 담낭절제술 후 정맥자가통증조절 시 fentanyl과 morphine의 비교

Background: Intravenous patient controlled analgesia (IV-PCA) is a most common used delivery system for intravenous administration of opioids during acute post operative pain management. The objective of this study is to compare the rate of opioid induced adverse reactions and effectiveness among two IV-PCA opioids,morphine, fentanyl, in laparoscopic cholecystectomy pain management. Methods: Sixty patients in ASA physical status 1 and 2 scheduled for laparoscopic cholecystectomy were allocated randomly to either morphine IV-PCA used (n = 30, Group M) group or fentanyl IV-PCA used (n = 30, Group F) group. In each group, numerical rating scale (NRS) score, sedation score and incidence of side effect were checked. Results: NRS score of Group F showed lower than that of Group M during PACU and 3 hrs after the recovery room (P < 0.05). There were no significant differences in sedation score among two groups. The incidences of adverse reactions were similar in the two groups, though the incidence of nausea and pruritus were higher in the Group M. Conclusions: Fentanyl IV-PCA is more advantageous than morphine IV-PCA for laparoscopic cholecystectomy in view of early pain control and adverse reaction incidences.

The goal in facial paralysis treatment is to achieve the normal appearance of the face as well as to reconstruct the natural symmetrical smile. In cases of facial paralysis, a widely accepted procedure is the two stage method, which combines neurovascular free muscle transfer with cross face nerve grafting. Although the results are promising, the two operations of this method, which are about 1 year apart, impose an economic burden on the patients and require a lengthy period before results are obtained. In order to overcome these drawbacks, one stage method, using latissimus dorsi neurovascuular free muscle flap was introduced. Between January 2000 and January 2004, fifteen patients with long standing facial paralysis were treated in the Korea University Anam Hospital. The segmental latissimus dorsi with long nerve and pedicle was transferred to the paralyzed side of the face. The first postoperative movement of the transferred muscle was reported at 8.9 months, faster than that of the two stage method. During the next 24 months, a constant increase in the power of muscle contraction was observed. The fifteen cases were evaluated within an average of 31.7 months following the surgery and satisfactory results including muscle contraction were obtained in eleven of the cases but muscle contraction was not found in three cases.

Metachromatic leukodystrophy (MLD; MIM 250100), a severe neurodegenerative disorder inherited as an autosomal recessive trait, is caused by mutations in the arylsulfatase A (ARSA) gene. Although several germ line ARSA mutations have been identified in patients with MLD of various ethnic backgrounds elsewhere in the world, no genetically confirmed cases of MLD have been reported in Korea. Recently, we identified a mutation in the ARSA gene of a Korean male with MLD. A male infant with late-infantile form of MLD had been admitted to our hospital for further examination. His neuromuscular symptoms, which included inability to walk at the age of 12 months, gradually worsened, even after allograft bone marrow transplantation; he died at the age of 9 yr. His elder brother had also been diagnosed with MLD. To confirm the presence of a genetic abnormality, all the coding exons of the ARSA gene and the flanking introns were amplified by PCR. A molecular analysis of the ARSA gene revealed both a novel heterozygous splicing mutation (c.1101+1G>T) in intron 6 and a heterozygous missense mutation in exon 2 (c.296G>A; Gly99Asp). The patient’s elder brother who had MLD is believed to have had the same mutation, which may be correlated with a rapidly deteriorating clinical course. This study identified a novel mutation in the ARSA gene, related to a late-infantile form of MLD with a lethal clinical course and suggested that molecular diagnosis of patients may be useful in early diagnosis and for deciding intervention measures for their family members. The Internet of Things (IoT), combined with various technologies, is rapidly becoming an integral part of our daily life. While it is rapidly taking root in society, security considerations are relatively insufficient, making it a major target for cyber attacks. Since all devices in the IoT environment are connected to the Internet and are closely used in daily life, the damage caused by cyber attacks is also serious. Therefore, encryption communication using a network security protocol must be considered for a service in a more secure IoT environment. A representative network security protocol includes TLS (Transport Layer Protocol) defined by the IETF. This paper analyzes the performance measurement results for TLS version 1.2 and version 1.3 in an IoT device open platform environment to predict the load of TLS, a representative network security protocol, in IoT devices with limited resource characteristics. In addition, by analyzing the performance of each major cryptographic algorithm in version 1.3, we intend to present a standard for setting appropriate network security protocol properties according to IoT device specifications. 사물인터넷(Internet of Things, IoT)은 다양한 기술과 결합하여 일상생활의 전반으로 빠르게 자리 잡았다. 빠른 속도로 사회 전반에 자리 잡은 데 반해 보안에 대한 고려는 상대적으로 미흡하여 사이버 공격의 주요 대상이 되고 있다. IoT 환경의 모든 기기는 인터넷에 연결되어 일상생활과 밀접하게 활용되고 있기에 사이버 공격으로 인한 피해도 심각하다. 따라서 보다 안전한 IoT 환경에서의 서비스를 위해 네트워크 보안 프로토콜을 이용한 암호화 통신이 반드시 고려되어야 한다. 대표적인 네트워크 보안 프로토콜에는 IETF에서 정의한 TLS(Transport Layer Protocol)가 있다. 본 논문은 제한된 자원 특성을 갖는 IoT 기기에서 대표적인 네트워크 보안 프로토콜인 TLS의 부하를 예측하기 위하여 IoT 기기 오픈 플랫폼 환경에서 TLS 버전 1.2와 버전 1.3에 대한 성능 측정 결과를 분석한다. 또한 버전 1.3에서 지원하는 주요 암호화 알고리즘의 성능을 분석하여 IoT 기기 사양에 따라 적합한 네트워크 보안 프로토콜 속성을 설정할 수 있는 기준을 제시하고자 한다.

This paper investigates the current state of Russian Cyrillic orthography of Korean names. With the rise of Hallyu or the Korean wave, an accurate translation and orthography of the names of Korean actors, singers and literary writers is an important issue. Different Cyrillization of the same name only exacerbates confusion. There are two types of Russian Cyrillic orthography to transcribe Korean words – the Holodovich System and the Kontsevich system. There exists controversy on some of the issues between the two systems. Among the differences in opinions among researchers, this paper categorizes the most urgent issues in Cyrillization of Korean proper nouns into four groups. Examples where such issues arise are collected and analyzed. Although there are various arguements around each of the types of Cyrillization, this paper attempts to narrow the differences in arguements and proposes alternatives that are believed to be more appropriate. Findings in this paper may contribute to developing a standard for Cyrillization of the Korean language.
