Arthroscopic Treatment of Elbow Osteoarthritis and Arthroscopic Ulnar Nerve Decompression

Although arthroscopic surgery has been used conventionally, it has not been widely adopted yet due to the risks of complications, including nerve damage, technical difficulties, and limited indications. As shown in other joints, however, the use of an arthroscope will gradually increased in the elbow joint (‘Arthroscopy always wins’). Herein, arthroscopic treatments and arthroscopic ulnar nerve decompression will be discussed in cases of elbow osteoarthritis.

For a long time mankind has faced the challenge of survival and one way to meet it was to understand the nature and adapt to it. The first historical evidence of Chinese view of nature appears in Eun-sang(殷商) Dynasty. Therefore, understanding the view of nature of people in Eun-sang(殷商) Dynasty is important in understanding the Chinese 'view of nature'(自然觀) in the Old Chinese period. The more important 'view of nature'(自然觀), however, is that of Chu(周) Dynasty which follows Eun-sang(殷商) Dynasty. The 'view of nature'(自然觀) in Chu(周) Dynasty deeply influences philosophy, literature, arts, life, science as well as Chinese national characters of the modern period. Therefore, this study investigates into the Chinese view of nature in Eun-sang(殷商) Dynasty and Chu(周) Dynasty in order to understand how "Chondokwan" (the view of the way of Heaven.天道觀) has affected the 'Chinese national characters in the modern period.

동치미 유래 생균제로서 가능성이 있는 Lactobacillus sp. FF-3의 분리와 특성

생균제로서 가능성 있는 유산균 선별을 위해 발효식품인 동치미로부터 분리된 균주들의 내산성, 내담즙성, 내췌장액성 및 유해균 억제능을 조사하였다. 특히 분리된 균종 중에서 FF-3가 인공위액과 담즙산에 대한 높은 내성을 나타내었을 뿐만 아니라, Salmonella sp.과 Escherichia coli에 대하여 높은 항균 활성을 나타내었다. FF-3유래 박테리오신은 그람 양성 및 음성 세균에서 넓은 항균범위를 나타내었다. 본 연구에서 최종선별된 FF-3 균주들 16s rDNA sequence 동정한 결과 Lactobacillus sp.으로 확인되어 FF-3으로 명명하였다. For selection of lactic acid bacterial for probiotics, we have examined isolated strains from Korean Dongchimi to assess the acid, bile, and pancreatic tolerance and the growth inhibition on the pathogens, Expecially, a kind of isolated strains, FF-3 showed the highest resistancy to both of HCI and bile sale, as well as the highest inhibitory activities against Salmonella sp. and Escherichia coil. Further the bacteriocin of FF-3 showed relatively wide range of inhibition spectrum against gram positive and some gram negative bacterias. By using 16s rDNA sequencing method, FF-3 of the selected lactic acid bacteria were found to be identified as genus Lactobacillus.

Oxalate와 Enrofloxacin투여한 랫트신장에 대한 병리조직학적관찰

To investigate the renal effects of enrofloxacin administration on rats induced with dehydration or hyperoxaluria, male rats were treated with enrofloxacin of 50 mg to 500 mg/kg b.w.. The microscopical observations of kidney and urine sediment were carried out in the experimental groups. The result obtained were as follows; The male rats deprived of water for 72 hours and administered with enrofloxacin. As enrofloxacin administration dose was increased, clinical signs such as loss of appetite, depression, weakness, and loss of urine output became more severe. In the histopathological findings, there were hyperemia and hemorrhage in renal cortex, vacuolation and necrosis of renal tubular epithelia, proteinous casts within renal tubules. The male rats were orally administered with sodium oxalate and injected with enrofloxacin for 7days. As enrofloxacin administration dose was increased, clinical signs such as the loss of appetite and water consumption, and weakness became more severe. In the histopathological findings, there are hemorrhage of glomeruli and cortical hyperemia, vacuolation and necrosis of tubular epithelia, proteinous casts in renal tubules. In the microscopical findings of urine sediment, there are calcium oxalate crystal (diamond-like type) and magnesium ammonium phosphate crystals (rhomboid). The male rats were intraperitoneally injected with sodium oxalate and administered with enrofloxacin for 7days. As enrofloxacin administration dose was increased, clinical signs such as the loss of appetite and water consumption, weakness were more severe. In the histopathological findings, there were hyperemia and hemorrhage in both glomeruli and renal cortex. Severe necrosis of renal tubular epithelia, bluish materials within renal tubules were also found. In the microscopical findings of urine sediment, there were many calcium oxalate crystals. The present results suggest that enrofloxacin has some injurious effects in rats having dehydration or hyperoxaluria, and clinically, we should consider these renal injury effects when we use enrofloxacin in patients accompanied renal disease, dehydration and hyperoxaluria conditions.

Mycobacterium mageritense에 의한 당뇨병성 족부 감염증 1예

Diabetic foot infection is one of the important complications in patients with advanced diabetes mellitus. Limb threatening infections such as osteomyelitis, abscess, and necrotizing fasciitis are frequently accompanied by the disease. Non-tuberculous mycobacterium (NTM) is a rare causative organism of diabetic foot infection. Thus, if one is not suspicious or meticulous, infection due to NTM will be easily overlooked and this will result in delayed diagnose and treat. Therefore, it is necessary to consider NTM as the causative organism if the wound does not respond to the conventional antibiotic treatment and the culture from the adequately obtained specimen reveals atypical acid-fast bacilli. We present a case of diabetic foot infection with osteomyelitis and abscess due to Mycobacterium mageritense, one of the rapid growing mycobacteria, that was successfully treated with surgical debridement and appropriate antibiotic treatment.

지루피부염에 대한 ketoconazole 샴푸의 효과

목적 : Pityrosporum ovale는 지루피부염의 발생에 중요한 역할을 하며 이 친지성 진균은ketoconazole제재에 감수성이 있다고 알려져 있다. 이에 저자들은 두피에 사용하기 편리한 2% ketoconazole 샴푸로 지루피부염을 치료하여 그 효과를 확인하고자 본 조사를 실시하였다. 대상 및 방법 : 두피의 지루피부염 환자 52명에게 소양감의 정도를 설문조사하고 피부과 의사의 육안으로 인설의 양을 검사한 후 광학현미경 고배율하에서 진균학적 검사를 실시하였다. 결과 : 지루피부염 환자의 소양감과 인설의 양은 치료전에 비해 치료 2주와 4주 모두에서 유의하게 감소하였으며(p<0.05) 이들 환자에서 실시한 Pityrosporum ovale의 평균 포자수 역시 치료전에 비해 치료 2주와 4주 모두에서 유의하게 감소하였다(p<0.05). 결론 : ketoconazole 샴푸는 지루피부염의 임상 및 진균학적인 면 모두에서 치료효과가 있는 것으로 판단되며 간접적으로나마 Pityrosporum ovale가 지루피부염의 발생에 관여한다는 사실을 알 수 있었다. It is suggested that Pityrosporum ovale may play an important role in the pathogenesis of seborrheic dermatitis and this organism is sensitive to ketoconazole compound. So the efficacy of 2% ketoconazole shampoo in the treatment of seborrheic dermatitis was tested and was calculated by mean scores of scale, itching and yeast cells. We interviewed fifty two patients with seborrheic dermatitis about the degree of itching and examined with naked eyes the scale of scalp of them. In a group of the patients, mycological study of P. ovale was also performed by direct smear with 20% KOH solution containng 1 : 1 Parker-ink^R The results were as follows : As for itching and scale of the scalp, the scores were significantly decreased at week 2 and week 4 as the duration of treatment prolonged in comparison with that of week 0. The mean yeast cell score at the two points of time was also significantly decreased in comparison with that of week 0. There was no significant difference in the response from the view of age, sex and duration of seborrheic dermatitis. It is suggested that the shampooing with ketoconazole 2% shampoo every three other days gave a good result clinically and mycologically.

한국 성인에서 이하 두정 방사선 계측사진 분석

교정 치료계획을 세우기 전에 부정교합에 기여하는 안면 비대칭의 정도를 정확히 평가하는 것은 무엇보다도 중요하다. 안면 비대칭의 진단은 주로 정모, 이하 두정, 두정 이하 방사선 사진이 이용되고 있다. 이중 이하 두정 방사선 사진은 반복재현성이 떨어지고 연구자료와 적절한 분석방법이 없다는 이유로 많이 사용되지 않고 있다. 이에 이 연구의 목적은 교정환자나 악교정 수술을 요하는 환자의 치료를 위하여 임상적으로 이용할 수 있는 이하 두정 방사선 계측사진의 분석방법을 개발하고 한국 성인에서의 정상치를 얻는데 있다. 연구 대상은 조화를 이루고 균형 잡힌 안모를 가진 교정치료 경험이 없는 40명의 성인을 대상으로 하였으며, 이하 두정 방사선 사진을 촬영하여 2개의 각도계측 항목과 9개의 선 계측항목을 측정하였다. 이하 두정 방사선 사진 분석에서 정상인에서 하악체의 길이는 좌, 우측 차이를 발견할 수 없었으나 정모, 측모 방사선사진의 계측 값과는 큰 차이를 보여, 비대칭의 절대적 양의 측정에는 어느 정도 한계가 있다. 기준선에 대한 좌, 우측 비교는 기준선을 설정하는 방법에 따라 큰 차이를 보였다. 과두의 위치 평가는 f. spinosum을 연결한 선의 수직이등분선을 정중선으로 사용하는 것이 가장 작은 편차를 보였으며, 상, 하악의 skeletal midline과 dental midline의 일치 여부 등을 판단할 때는 crista gli와 anterior process of atlas와 같이 midsagittal line상에 위치하는 구조물을 이용하여 정중선을 설정하는 것이 가장 작은 편차를 보였다. 그리고 하악 과두 및 하악체의 형태이상을 관찰하는데는 매우 높은 진단적 가치를 가지고 있다. 그러므로 이하 두정방사선사진의 한계를 정확히 이해하고 정모, 측모 두부방사선사진과 함께 비대칭 분석에 이용한다면 보다 정확하게 비대칭 부위와 정도를 측정하는 데 도움이 될 것이다. It is important that the orthodontist accurately assess the degree to which facial asymmetry contributes to a given malocclusion before treatment planning. P-A, submentovertex and verticosubmental view have been used in the assessment of facial asymmetry. Among them, submentovertex view is rarely used because it has low reproducibility and is short of normal data and proper analysis method. The purpose of this study was to develop a submentovertex cephalometrics and obtain normal data in Korean adults. The subjects consisted of 40 normal adults (male : 22, female : 18) without the experience of orthodontic treatment. We find the 2 angular and 9 linear measurements. Though submentovertex cephalomentrics has the limitation in comparing the absolute length between right and left, it is useful to examine the relationship of skeletal and denial midline, the shape and location of condyle head and the shape of mandibular body in submentovertex view. Therefore, if we understand the limitation of submentovertex cephalomentrics and use lateral, P-A and submentovertex cephalomentrics together, we will measure the location and amount of skeletal disharmony more exactly.

實驗水路에서 數値解析法에 의한 跳水에 관한 硏究

In this study, Boussinesq equations describing on dimensional unsteady, rapidly varied flows are integrated numerically to simulate both the sub- and supercritical flows and the formation of a hydraulic jump in a rectangular channel having a small bottom slope. for this purpose MacCormack (second -order accurate in space and time) and two-four (second -order accurate in time and fourth-order in space)explicit finite-difference schemes are used to solve the governing equations subject to specified end conditions until a steady state reached. Complete test results for a range of Froude numbers are presented that may be used by other researchers for the verification of mathematical models. A comparison of the computed and measured results shows that it is satisfactory for the fourth-order finite-difference schemealthough the second-order scheme does not accurately predict the location of the jump. These simulations show that the Boussinesq terms have little effect in determining the location of the hydraulic jump.
