      • 복합유기용제 노출로 인한 밀초신경장해의 조기진단에 있어 신경전도 검사가 유용한가?

        목적 : 복합유기용제에 저농도의 유기용제에 장기간 노출된 근로자들에서 말초신경병증을 조기진단 함에 있어서 신경전도 검사의 유용성을 알아보고자 하였다. 대상 및 방법 : 다섯 곳의 공장에 근무하는 근로자들 중 노출군 21명과 대조군 20명을 선정하고 두 그룹의 근로자들로부터 종사 기간, 현재 작업에 근무한 기간 그리고 흡연력 등을 포함하는 직업력 및 일반적인 병력을 설문지를 통해 조사하였으며 요중 대사산물인 톨루엔, 크실렌, 스틸렌을 분석하고 진동감각역치 검사, 신경전도 검사, 그리고 색도분별 검사를 시행하였다. 결과 : 두 그룹간에 연령, 흡연 습관, 종사기간 및 현재 작업에 근무한 기간은 중요한 차이를 보이지 않았다(p>0.05). 신경전도 검사, Lanthony desaturated D-15 검사, 진동감각역치 검사는 의미있는 차이를 보이지 않았다(p>0.05). 신경전도 검사와 색도분별 검사, 색도분별 검사와 진동감각역치 검사, 신경전도 검사와 진동감각역치 검사간에는 의미 있는 상관 관계를 보이지 않았다(p>0.05). 결론 : 이에 본 연구에서 신경전도 검사는 복합유기용제에 저농도로 노출된 근로자에게서 말초신경병증을 조기 진단하는데 유용하지 않다고 생각한다. Objectives: To evaluate the utility of nerve conduction study in screening of peripheral polyneuropathy among workers with a low level exposure to mixed organic solvents. Methods: An exposure group of 21 workers and reference group of 20 workers were selected from different sections of the five factories. Workers of both groups were examined with a questionnaire on the occupational and general history, including duration of employment, of present job, and smoking. Urinary metabolites of toluene, xylene, and styrene were analysed. The vibration perception threshold test, the nerve conduction study, and the color discrimination test were done. Results: There were no significant differences in age, smoking habits, and duration of employment and present job (p>0.05) between the groups. There were no significant difference in parameters of nerve conduction study, Lanthony desaturated D-l5 test, and vibration perception threshold examination (p>0.05). There were no significant correlations between the nerve conduction study with the color discrimination test, the color discrimination test with the vibration perception threshold test, and the nerve conduction study with the vibration perception threshold test (p>0.05). Conclusions: The result suggests that the nerve conduction study is not universally useful for the screening of the workers with a low level exposure to mixed solvents.

        Intraoperative formation and management of a thrombus in right atrium has been reported occasionally. Nevertheless, it is rare that a right atrial thrombus with unstable hemodynamic changes detected by transesophageal echocardiography is resolved spontaneously. We report upon the 44-year-old woman, who had a right atrial thrombus detected by transesophageal echocardiography during laparoscopic assisted vaginal hysterectomy and resolved during thromboembolectomy.

        <P>Dishevelled (Dvl/Dsh) is a key scaffold protein that propagates Wnt signaling essential for embryogenesis and homeostasis. However, whether the antagonism of Wnt signaling that is necessary for vertebrate head formation can be achieved through regulation of Dsh protein stability is unclear. Here, we show that membrane-associated RING-CH2 (March2), a RING-type E3 ubiquitin ligase, antagonizes Wnt signaling by regulating the turnover of Dsh protein via ubiquitin-mediated lysosomal degradation in the prospective head region of Xenopus. We further found that March2 acquires regional and functional specificities for head formation from the Dsh-interacting protein Dapper1 (Dpr1). Dpr1 stabilizes the interaction between March2 and Dsh in order to mediate ubiquitylation and the subsequent degradation of Dsh protein only in the dorso-animal region of Xenopus embryo. These results suggest that March2 restricts cytosolic pools of Dsh protein and reduces the need for Wnt signaling in precise vertebrate head development.</P>

        최근 엣지 컴퓨팅과 같은 임베디드 디바이스에서 CNN과 같은 딥러닝 모듈을 수행하기 위해서 하드웨어 설계 및 구현이 많이 진행되고 있다. 이러한 임베디드 시스템에 필요한 CNN모듈을 위한 하드웨어 설계를 위해서 먼저 모델링을 통해서 시뮬레이션이 필요하다. 본 논문에서는 오픈 라이센스를 이용한 RISC-V로 딥러닝 시뮬레이터를 제작하였다. SystemC로 구현된 RISC-V를 Virtual Platform로 시뮬레이터의 제작을 하여 시뮬레이팅을 하였고, SystemC의 특징인 모듈화와 모듈간 통신에 유의하여 시스템을 구성하였다. CNN 알고리즘을 참조하여 Convolution, Activation, Pooling 연산의 기능을 하는 시스템을 구성하였다.

        일개 자동차 부품공장 근로자에서 초음파로 측정한 수근관 형태와 수근관증후군 위험도

        목적 : 일개 사업장의 생산직 근로자를 대상으로 손목 계측치 및 초음파를 이용한 수근관 계측치의 상관성 분석을 통하여 수근관증후군 발생에 관여하는 개인의 감수성 및 위험요인을 파악하여 생산직 근로자들의 수근관증후군 예방에 활용하고자 본 연구를 수행하였다. 방법 : 경주시에 위치한 자동차 방음재를 생산하는 사업장에 생산직 근로자 남자 24명, 여자 11명 등 총 35명을 대상으로 2000녀 4월과 2000년 10월 반드시 각 설문조사, 이학적검사, 전기진단검사를 실시하였으며, 2000년 10월에는 초음파를 이용한 수근관계측도 실시하였다. 결과 : 수근관증후군의 신체계측 요인에 따른 분석에서는 수근관 전치/폭 비가 0.17 미만인 군에 비하여 0.17 이상인 군에서 통계적으로 유의하게 높았다(p<0.05). 총 수근관 면적(두께×폭)은 300 ㎟이상인 군에 비하여 300㎟ 미만인 군에서 통계적으로 유의하게 높았다(p<0.05). 성별을 통제변수로 처리한 신체 계측치간 상간 분석에서는 수근관폭은 전기진단 검사와 음의 상관성을 보였으며, 수근관 두께는 전기진단 검사와 양의 상관성을 보였다. 수근관 두께/폭 비는 전기진단 검사와 양이 상관성을 보였으며, 수근관비 전치/두께 비는 전기진단 검사 결과와 음의 상관성을 보였다. 하부 수근관 면적[폭×(두께-전치)]은 전기진단 검사와 양의 상관성을 보였으며, 상부 수근관 면적(폭×전치)은 전기진단 검사와 음의 상관성을 보였다. 또한 수근관 전치의 상대적 비가 클수록 수근관증후군의 회복이 유의하게 좋았다. 결론 : 수근관증후군은 수근관 폭이 좁을수록, 수근관 두께가 두꺼울수록, 수근관 전치가 작을수록 수근관증후군 위험도가 증가한다. 저자들은 초음파를 이용한 수근관 형태의 측정은 수근관증후군의 발생의 내적 요인에 대한 중요한 정보를 제공하면 수근관증후군 발생 및 회복의 개별적 위험요인을 예측하는데 중요한 근거를 제공하는 것으로 판단하였다. Objectives: This study was conducted to evaluate individual susceptibility to carpal tunnel syndrome (CTS) by ultrasonographic measurement of the carpal tunnel configuration in workers doing repetitive work. Methods: The study subjects consisted of 24 male and 11 female workers in a soundproof material manufacturing company in Gyeongju. We conducted a self-reported questionnaire survey, a physical examination and an electrodiagnostic study (EDS) in April 2000. After the examination, jobs were rearranged for workers with CTS. A follow up physical examination. EDS, and measurement of the carpal tunnel by ultrasonography was done six monsths later. Results: Of those studied. prevalence or CTS was 63.6/100 persons among women and 29.2/100 persons among men. Mean depth and width of wrist was shorter in those with CTS compareal to the controls (p<0.05). The risk of CTS was higher in workers whose carpal tunnel ratio (displacement/width) was 0.17 or higher (OR 7.13, 95% confidence interval 1.18-43.1), and in workers whose carpal tunnel area was less than 300 ㎟ (OR 8.00, 95% confidence interval 1.18-68.5). Carpal tunnel depth and depth/width ratio had a positive correlation with motor latency of the median nerve and median-ulnar sensory latency difference (p<0.05). whereas the carpal tunnel width. displacement/depth ratio, and area (width×displacement) had a negative correlation with median nerve latencies after adjusting for gender. Workers who showed an improvement in the clinical stage of CTS after job rearrangement had a significantly lower carpal tunnel displacement/width ratio and displacement/depth ratio. They also had smaller upper carpal tunnel area (width×displacement) and larger upper carpal tunner\l area [(depth-displacement)×width] than the controls (p<0.05). Conclusions:Ultrasonographic measurement of the carpal tunnel is a good predictor of susceptibility and prognosis of CTS in workers doing repetivive work.

        혈액투석 환자에서 하대정맥 직경 측정의 의의와 혈장 Alpha - human - Atrial Natriuretic Peptide 및 cyclic Guanosine 3`:5`-Monophosphate 농도와의 관계

        N/A Background: A correct estimation of volume status and dry weight in dialysis patients remains a difficult clinical problem Clinical status and chest X-ray are not sensitive enough, while invasively measured central venous pressures are not routinely available. Recently, the ultrasonographic determination of the diameter and collapse index of the inferior vena cava has been proposed as a noninvasive method for estimating intravascular volume. We tried to evaluate the clinical relevance of this method in dialysis patients by comparing it with alphahuman-atrial natriuretic peptide (alpha-h-ANP) and cyclic guanosine 3:5-monophosphate (cGMP) levels. Methods: Using echocardiography, the diameter of the inferior vena cava (VCD) and its decrease on deep inspiration (collapse index : CI) were evaluated in 27 hemodialysis patients. Echocardiography of the inferior vena cava (IVC) was performed in the supine position after 10 minutes rest. The transducer was placed in the subxiphoid region and long and short axis views of the IVC were obtained just below the diaphragm in the hepatic segment. VCD was measured before the P-wave on the electrocardiogram to avoid interference with A-wave and V-wave on the venous pressure curve, and corrected for body surface area. Pre- and post-hemodialysis levels of the plasma alpha-h-ANP and cGMP were measured by radioimmunoassay. The relationship between VCD, CI determined by echocardiography, and alpha-h-ANP and cGMP concentrations were studied. Results: The levels of alpha-h-ANP and cGMP were markedly elevated before hemodialysis and significantly lower values were found after hemodialysis (alpha-h-ANP: 162.7 102.6 pg/ml vs 90.6 61.0 pg/ml, cGMP: 35.3 8.8 pmol/ml vs 21.3 6.2 pmol/ml). A significant correlation was found between VCD and alpha-h-ANP before (r=0.81, p<0.05) and hemodialysis (r=0.65, p<0.05). No such significant correlation was observed between CI and alpha-h-ANP levels. Furthermore, a significant correlation was observed between VCD before hemodialysis and the change in alpha-h-ANP during hemodialysis (r=0.64, p<0.05). The relationship between VCD and cGMP before hemodialysis was not significant (r=0.26, p>0.05) and also no relation was observed between the decrease of cGMP during hemodialysis and VCD before hemodialysis (r=0.12, p>0.05). A significant correlation between the percent change in body weight and the percent change in VCD during hemodialysis (r=0.91, p<0.05) and also significant relation was observed between the pecent change in body weight and the percent change in alpha-h-ANP levels (r=0.40, p, 0.05). Conclusion: Echocardiography of the inferior vena cava allow an estimation of changes of intravascular volume in ESRD patients without cardiac filling impairment as shown by the correlation to other indices of intravascular volume, such as alpha-h-ANP. In this study, CI and cGMP levels were less informative. Inferior vena cava echocardiography is noninvasive and easily available and serial measurements of VCD and alpha-h-ANP levels allow an estimation of chages of intravascular volume in FARD patients on maintenance hemodialysis.
