한·중·일 노인들의 신체활동 참여가 고독감 및 우울에 미치는 영향

본 연구는 한국, 중국, 일본 여성 노인의 신체활동 참가형태에 따라 노인의 부정적 정서인 고독감, 우울 등에 미치는 영향을 파악하고 기초자료를 확보함으로써 노인들의 심리적, 정신적, 안정과 건강한 노후를 맞을 수 있도록 도움을 주고자 하는데 목적이 있다. 이러한 연구를 위해서 한국, 중국, 일분에 거주하는 60세 이상 여성 노인을 모집단으로 설정하여 유층 집락 무선 표집법에 의해 최종적으로 한국 192명, 중국 178명, 일본 213명 총 583명을 표집하였다. 본 연구의 조사 도구는 원형중(1994)에 의하여 5점 Likert 방식으로 수정 번안된 UCLA 고독감 척도(10문항)와 Zung(1965)이 개발한 척도를 육영숙(1994)이 번안하여 표준화시킨 자기보고 우울 척도(20문항)을 평균, 표준편차, 신뢰도 분석, 빈도 분석, 이원변량분석, 사후검증 등의 통계기법을 사용하여 검증하였다. 이와 같은 자료분석 및 절차를 통하여 다음과 같은 결론을 얻었다. 첫째, 한국, 중국, 일본 3국의 노인이 가장 선호하는 신체활동은 맨손체조(25.0%), 댄스스포츠(19.1%), 걷기(18.3%), 등산(9.7%)의 순으로 나타났으며, 국가별 노인들이 선호하는 신체활동은 약간씩 차이가 있는 것으로 나타났다. 둘째, 고독감에 있어서 신체활동에 참가하는 노인들의 주당빈도 평균은 한국 5-6일, 중국 1-2일, 일본 3-4일이 가장 높은 것으로 나타났으며, 우울에 있어서 신체활동에 참가하는 노인들의 주당빈도 평균은 한국 1-2일, 중국 1-2일, 일본 매일이 가장 높은 것으로 나타났다. 셋째, 고독감에 있어서 신체활동에 참가하는 노인들의 지속기간 평균은 한국 2-3년, 중국 1년 이하, 일본 1-2년이 가장 높은 것으로 나타났으며, 우울에 있어서 신체활동에 참가하는 노인들의 지속기간 평균은 한국 1년 미만, 중국 2-3년, 일본 2-3년이 가장 높은 것으로 나타났다. This study grasp the effect of physical activity on the negative emotion such as loneliness, depression to Korean, Chinese and Japanese female elderly to help their psychological, mental safe and healthy old age. A participant was female over 60 ages and live in Japan is selected by stratified cluster random sampling, 192 Korean, 178 Chinese, 213 Japanese, total 583. The survey methods of this research are the Self-rating Depression Scale(20 item) by Zung(1965) translated standardized by Yook, Young-Sook(1994) and the 5 Likert Scale modified the UCLA Loneliness Scale(10 item) by Won, Hyung-Joong(1994). The used statistical methods for the data analysis were Mean(M), Standard deviation(SD), Reality Analysis, Frequency Analysis, 2-way ANOVA, Post-hoc. The results obtained by the appropriate analytic processes were followed. First, from the result Gymnastic(25.0%), Dance Sports(19.1%), Walking(18.3%), Climbing(9.7%), and we figured out that they are different from cach others. Second, the mean of loneliness by physical activity weekly frequency for elderly people is 5~6 days(Korea), 1~2 days(China), and 3~4 days(Japan). And, the mean of depression by physical activity weekly frequency is 1~2days(Korea), 1~2days(China), and every day(Japan). Third, the mean of loneliness physical activity continuing duration for old person is 2~3 years(Korea), less than 1 year(China), and 1~2years(Japan). And, the mean of depression by physical activity continuing duration for elderly people is less than 1 year(Korea), 2~3 years(China), and 2~3 years(Japan).

C++클래스 프로그래밍 지원 도구

The C++ language is object-oriented programming language which is derived from C language. Although C++ language is based on conventional programming language, it has few drawbacks in use. Particularly, the class programming which enables the reuse of the program is very hard without a supporting tool. When we program the software package using C++ ckasses, we need an information about the relating classes. To accomplish this, we have implemented a system which aids the C++ class programming. The main works of this system are that information about classes in acquired automatically and programmed classes can be added to the class library with very little effort. The system extracts information form classes by parsing the C++ source code and manages class library based on class inheritance hierarchy. And then this information about classes and the class source code are provided when the class library is browsed. So we can do the C++ class programming easily by using the system.

수부의 외상성 중수지 관절 탈구의 치료

목적 : 수부의 중수지 관절 탈구의 관혈적 정복이 필요했던 예에서는 정복을 방해하는 구조물에 대해 분석하고 도수 정복이 가능했던 예에서는 성별 및 탈구 형태에 따른 특성에 대해 알아보고자 하였다. 대상 및 방법 : 총 27예의 수부 중수지 관절 탁구 환자 (무지 11예, 이지 14예, 소지 2예)를 대상으로 연구하였다. 탈구된 중수지 관절의 정복을 방해하는 해부학적 구조는 수술시에 확인을 하였으며, 도수 정복이 가능하였던 예들에서도 연령과 손상된 구조물, 그리고 최종적인 임상 결과를 평균 7개월간 추시 관찰하였다. 결과 : 무지 중수지 관절의 탈구 11예 중 파열된 전방 관절낭, 수장판과 단무지 굴건의 파열을 동반한 복합 탈구가 8예 있어 관혈 정복을 시행하였으며, 2예에서만 도수 정복이 가능하였다. 관절염이 있던 1예는 관절 유합술을 시행하였다. 다른 수지의 중수지 관절의 탈구는 Kaplan의 개념에 따라 탈구를 수술 정복하였고 파열된 수장판을 복원하였으며, 1예에서 수술시 표재 중수 횡인대만의 절개와 이완으로 정복할 수 있었다. 소지 중수지 관절 탈구의 2예에서 파열된 요측 측부인대가 수술시 관찰되었고 이를 복원하여 수지의 불안정성을 예방하였다. 이들의 최종 결과는 특별한 운동 제한이나 불안정성 없이 좋은 결과를 보였다. 결론 : 적절한 국소 마취하에 도수 정복을 1~2회 시행할 수 있지만 단순 정복이 가능한 탈구가 부적절한 견인으로 정복이 불가능한 복합 탈구로 진행할 수 있다. 그리고 복합 탈구는 대부분 관혈적 정복이 필요하기 때문에, 정복 방해 구조물에 대한 충분한 이해와 적절한 조치가 필요하다. Purpose : To analyse the obstacles to prevent the reduction for dislocation of the metacarpophalangeal (MP) joints of the hand and evaluate the relationship between the sex and dislocation type in closed reduction case. Materials and Methods : Total 27 cases of MP joint dislocation of the hand (11 cases of thumb, and 14 index and 2 little finger) were reviewed retrospectively. The anatomical structures to prevent the reduction were confirmed at operation and the cases which were reduced immediately were also analysed for their ages and differences of damaged structures, and finally their complications or outcome were reviewed for average 7 months after reduction. Results : In 11 cases of thumb MP joint dislocations, the complex dorsal dislocations in which the protruded metacarpal neck was caught by buttonhole of torn anterior joint capsule, volar plate and FPB were reduced by open method in 8 cases, and closed reduction was done in 2 cases but one old case required arthrodesis. In other finger MP joint dislocations, the Kaplan's concept to prevent the reduction was confirmed. But reduction of torn volar plate and incision of transverse metacarpal ligament were sufficient to reduce the dislocation with gentle longitudinal traction during the operation. In two cases of little finger MP joint dislocation, the ruptured radial collateral ligaments were noted after open reduction and it must the repaired to prevent the finger instability later. Their overall end results were good without any significant restriction of MP joints motions and finger instabilities. Conclusion : One or two times of closed reduction with proper local anesthesia could be tried, but simple reducible dislocation can be converted to complex irreducible ones by the inappropriate traction method, and so proper reduction technique by closed or even in open way is important with the knowledge of anatomical obstacles to prevent the reduction of the metacarpophalangeal joints.

상부요로계감염 환자에서 Gallium-67 Citrate 스캔 및 Tc-99m Dimercaptosuccinic Acid(DMSA)스캔의 임상적 의의

It is often not certain whether the focal defect of renal cortex of Tc-99m Dimercaptosuccinic acid(DMSA) renal scan in patients with acute pyelonephritis means the cortical scar by previous inflammation or recent active inflammatory lesion. Gallium-67 citrate(Ga-67) also has been known to be accumulated at the active inflammatory lesion. In order to evaluate the usefulness of the Ga67 and Tc-99m DMSA renal scan in patients with acute pyelonephritis, we reviewed the results of both study and clinical findings. Nintecn patients with typical clinical findings of acute pyelonephritis and admitted to our institute from Jan. 1998 to Nov. 1998 were included in this study. Tc-99m DMSA and Ga-67 scan were performed mean 3.3 and 7.3 days after admission. Eleven of 19 patients (57.9%) showed abnormally increased focal renal uptake on the Gallium-67 citrate scan and 14 of 19 patients(73.7%) had a cortical defect on the kidneys in Tc-99m DMSA scan. Seven(36.8%) patients had an abnormality in both study. Seven(36.8%) patients had an abnormality only in Tc-99m DMSA scan and 4 (21.1%) patients had an abnormality only in Ga-67 scan. Of 7 patients who had an abnormality on both scan, 5 patients had. an abnormality at same region in both scan and 2 patients had an abnormality at the different sites on the kidneys. With the above results, we speculate that Ga-67 scan and Tc-99m DMSA scan play a complementary role in the patients with upper urinary tract infection.

Microsatellite Instability Is Associated with the Clinicopathologic Features of Gastric Cancer in Sporadic Gastric Cancer Patients

Purpose: Replication error is an important mechanism in carcinogenesis. The microsatellite instability (MSI-H) of colorectal cancers is associated with the development of multiple cancers. The influence of MSI-H on the development of multiple gastric cancers in sporadic gastric cancer patients has not been defined. This study was performed to reveal the association between the clinicopathologic features and MSI in sporadic gastric cancers. Materials and Methods: Between July 2004 and March 2009, the clinicopathologic characteristics, including MSI status, were evaluated in 128 consecutive patients with sporadic gastric cancers. None of the patients had hereditary non-polyposis colorectal cancer of familial gastric cancer. The markers that were recommended by the NCI to determine the MSI status for colorectal cancers were used. Results: MSI-H cancers were found in 10.9% of the patients (14/128). Synchronous gastric cancers were shown in 4 patients (3.1%). Synchronous cancers were found in 2 of 14 patients with MSI-H gastric cancer (14.3%) and 2 of 114 patients with MSS gastric cancer (1.8%; P=0.059, Fisher's exact test). Among the patients with synchronous cancer 50% (2/4) had MSI-H cancer, but 9.7% of the patients (12/124) without synchronous cancer had MSI-H cancer. MSI-H (RR, 24.7; 95% CI, 1.5~398.9; P=0.024) was related with to synchronous gastric cancer, but age, gender, family history, histologic type, location, gross morphology, size, and stage were not related to synchronous gastric cancer. Conclusions: MSI is associated with the intestinal-type gastric cancer and the presence of multiple gastric cancers in patients with sporadic gastric cancer. Special attention to the presence of synchronous and the development of metachronous multiple cancer in patients with MSI-H gastric cancer is needed.

Opuntia humifusa Ameliorated Cerulein-Induced Acute Pancreatitis

OBJECTIVE: The aim of this study was to evaluate the effects of Opuntia humifusa (OH) on cerulein-induced acute pancreatitis (AP). METHODS: Acute pancreatitis was induced via intraperitoneal injection of cholecystokinin analog cerulein (50 μg/kg). In the OH pretreatment group, OH was administered intraperitoneally (100, 250, or 500 mg/kg) 1 hour before first cerulein injection. In the posttreatment group, OH was administered intraperitoneally (500 mg/kg) 1 hour after the first cerulein injection. Furthermore, we isolated the pancreatic acinar cells using collagenase method, then investigated the acinar cell viability, cytokine productions, and the regulating mechanisms. RESULTS: The both pretreatment and posttreatment of OH treatment attenuated the severity of AP, as shown by the histology of the pancreas and lung, and inhibited neutrophil infiltration; serum amylase and lipase activities; proinflammatory cytokine expression such as interleukin 1, interleukin 6, and tumor necrosis factor α; and cell death including apoptosis and necrosis. Furthermore, OH inhibited the activation of c-Jun N-terminal kinases. CONCLUSIONS: These results suggest that OH reduces the severity of AP by inhibiting acinar cell death through c-Jun N-terminal kinases.

Acute cell-mediated rejection in orthotopic pig-to-mouse corneal xenotransplantation

<P>Abstract: Background: </P><P>To investigate the role of T cells and natural killer (NK) cells in mediating corneal xenograft rejection in a pig-to-mouse model.</P><P>Methods: </P><P>Pig corneas were orthotopically transplanted into BALB/c, C57BL/6, nude, severe combined immunodeficiency (SCID), and NOD/SCID/&ggr;c<SUP>null</SUP> (NOG) mice. Graft survival was clinically assessed by slit-lamp biomicroscopy and median survival times (MST) were calculated. The rejected grafts were histologically evaluated using antibodies against CD4, CD8, NK1.1, and F4/80.</P><P>Results: </P><P>The pig corneal xenografts were acutely rejected by BALB/c and C57BL/6 mice (MST 9.0 days), while nude, SCID and NOG mice rejected pig corneas in a more delayed fashion (MST 16.0, 16.4, and 16.9 days, respectively). The majority of infiltrating cells found in rejected grafts in C57BL/6 mice were macrophages and CD4<SUP>+</SUP> T cells, while CD8<SUP>+</SUP> T cells and NK cells were rarely found. The grafts in nude mice had markedly decreased inflammatory infiltration with small numbers of macrophages and CD4<SUP>+</SUP> T cells. Infiltration was even more modest in grafts in SCID and NOG mice.</P><P>Conclusions: </P><P>T cells play an important role in acute rejection of pig corneal xenografts in mice, although acute rejection is not solely the result of T-cell-mediated immunity. NK cells are less likely to be involved in the rejection process.</P>
