Carcinoma ex pleomorphic adenoma is transformed at the incidence of 1-20% in pleomorphic adenoma and frequently recurred. It accounts for 10% of all malignant salivary tumors and its average age of occurrence is 60s. It will present in a large, long-standing pleomorphic adenoma or in one that was previously treated but has recurred. According to cell composition in malignant cell carcinoma, and clear cell adenocarcinoma. Most (75%) occur in parotid gland, while about 20% occur in the minor gland of the oral mucosa. The metastasis rate to regional lymph node is about 25%, and to distant organs about 33% and the 5-year survival rates are 40%. Though the treatment of the carcinoma ex pleomorphic adenoma is not established, it is treated ideally with and extensive resection, neck dissection, postoperative radiotherapy, and chemotherapy. When occurred in parotid gland, facial paralysis is reported. With a review of literatures, we report a case of carcinoma ex pleomorphic adenoma which operated with total parotidectomy and supraomohyoid neck dissection.

A 51-year-old woman presented with sudden severe headache, vomiting, and right hemiparesis at first admission. Computed tomography(CT) scans revealed an hemorrhagic density at left basal ganglia. Preoperative cerebral angiography showed no vascular lesion. Under the diagnosis of hypertensive intracerebral hemorrhage(ICH), total extirpation of hematoma was done. The postoperative neurological condition improved gradually and discharged without any neurological sequelae. Two months later, she revisited with headache, vomiting and progressive right hemiparesis. CT scans at second admission showed an irregular rim enhanced mass with central low density with surrounding edema at the initial bleeding area. Repeated craniotomy was performed and the mass was partially removed. The histopathological diagnosis of the specimen was confirmed as glioblastoma. The authors report a glioblastoma, which occurred at initial ICH site and regarded as a brain abscess with literature review.

This study deals with the dynamic response of reinforced concrete beams strengthened with carbon fiber sheets. Test beams are subjected to static loading and repeated loading. Based on the static test results of the RC beams strengthened with carbon fiber sheets, repeated loading tests are performed. The variables of repeated loading test are composed of the number of carbon fiber sheets, the existence of U-shaped band at the end for anchoring, and loading rate of repeated loading, etc. Test results show the flexural behavior, the characteristics of ductility, the change of flexural rigidity, and the amount of energy loss of RC beams under monotonic incremental loading and repeated loading. The failure strain of carbon fiber sheets is also estimated under repeated loading. From the experimental results, this work presents a basis of the data needed to analyze and design the static and dynamic flexural response of RC beams strengthened with carbon fiber sheets.
