        Bronchial Schwannomas: Clinicopathologic Analysis of 7 Cases

        Background: It has long been recognized that bronchial schwannomas are extremely rare. As such, diagnosing tumors in this extraordinary location can sometimes be problematic. Methods: We reviewed seven cases of bronchoscopically or surgically resected endobronchial schwannomas and evaluated their clinical and pathologic features. Results: The present study included five female and two male patients, with ages ranging from 16 to 81 years (mean age, 44.9 years). The clinical presentation varied according to tumor size and location. Patients with more centrally (trachea or main bronchus) located tumors experienced respiratory symptoms (80%) more often than patients with more peripherally (lobar or segmental bronchus) located tumors (0%). Histologically, the tumors were composed of spindle cells that stained with S100 protein. Some of the tumors showed typical Antoni A areas with Verocay body formation. Five of six patients (83.3%) underwent complete tumor removal by rigid bronchoscopy. Conclusions: Pathologists should consider endobronchial schwannoma in the differential diagnosis of a spindle cell tumor involving the bronchus. Additionally, our results showed that rigid bronchoscopy is an effective tool for tumor removal in endobronchial schwannoma patients.

        Protective Effect of Phosphatidylcholine on Lipopolysaccharide-Induced Acute Inflammation in Multiple Organ Injury

        Soybean polyunsaturated phosphatidylcholine (PC) is thought to exert anti-inflammatory activities and has potent effects in attenuating acute renal failure and liver dysfunction. The aim of this study was to investigate the effects of PC in protecting multiple organ injury (MOI) from lipopolysaccharide (LPS). Six groups of rats (N=8) were used in this study. Three groups acted as controls and received only saline, hydrocortisone (HC, 6 mg/kg, i.v.) or PC (600 mg/kg, i.p.) without LPS (15 mg/kg, i.p.)injections. Other 3 groups, as the test groups, were administered saline, HC or PC in the presence of LPS. Six hours after the LPS injection, blood and organs (lung, liver and kidney) were collected from each group to measure inflammatory cytokines and perform histopathology and myeloperoxidase (MPO) assessment. Serum cytokines (TNF-α , IL-6 and IL-10) and MPO activities were significantly increased, and significant histopathological changes in the organs were observed by LPS challenge. These findings were significantly attenuated by PC or HC. The treatment with PC or HC resulted in a significant attenuation on the increase in serum levels of TNF-α and IL-6, pro-inflammatory cytokines, while neither PC nor HC significantly attenuated serum levels of IL-10, anti-inflammatory cytokine. In the organs, the enhanced infiltration of neutrophils and expression of ED2 positive macrophage were attenuated by PC or HC. Inductions of MPO activity were also significantly attenuated by PC or HC. From the findings, we suggest that PC may be a functional material for its use as an anti-inflammatory agent.

        Ultrasonographic Findings of a Chondrolipoma Arising from the Left Supraclavicular Region: A Case Report

        Chondrolipomas, which are lipomas with chondroid metaplasia, are rare benign soft tissue tumors with no relevant epidemiological reports or radiological information. A limited number of lipomas with osteo/chondroid differentiation have been reported in the literature between 1960 and 2008. Moreover, only few studies have described the radiologic findings of chondrolipomas. Herein, we present a case of chrondrolipoma arising from the left supraclavicular region in a 77-year-old female.

        Correlation of AR, EGFR, and HER2 Expression Levels in Prostate Cancer: Immunohistochemical Analysis and Chromogenic In Situ Hybridization

        Purpose The androgen receptor (AR) plays a central role in prostate cancer. Evidence from several groups indicates that epidermal growth factor receptor (EGFR) and human epidermal growth factor receptor 2 (HER2) may enhance AR activity in prostate cancer cell lines. This study was designed to investigate the protein expression of AR, EGFR, and HER2 and to determine whether the EGFR and HER2 genes are amplified in prostate cancer tissues. Materials and Methods The protein expression levels of AR, EGFR, and HER2 in a tissue microarray block of 66 prostate cancer samples were investigated by immunohistochemical analysis and chromogenic in situ hybridization was used to determine whether the EGFR and HER2 genes were amplified in these tissues. Results The AR and EGFR proteins were expressed in 59.1% and 40.9% of prostate cancers, respectively,but their expression levels were not significantly associated with clinicopathologic factors. Of the cases in which tissues were negative for EGFR protein expression, 69.2% were positive for AR protein expression; however, AR protein expression was significantly reduced (44.4%) in tissues in which EGFR protein was expressed. HER2 expression was detected in only 1 case (1.5%). No amplification of the EGFR or HER2 genes was found in prostate cancer specimens. Conclusion This study was limited by small number of subjects, but it can still be inferred that the expression levels of the AR and EGFR proteins are inversely correlated in prostate cancer patients. The potential utility of EGFR and HER2 as prognostic factors or therapeutic targets warrants further study.

        결막 유피지방종의 임상 양상

        Purpose: To evaluate the clinical features of conjunctival dermolipoma. Methods: We reviewed the ocular records of 18 consecutive patients diagnosed with dermolipoma in our hospital between March 2016 and March 2018. Results: The study population consisted of eight females and 10 males with a mean age of 61 months (range 3 months to 45 years old), with a pinkish conjunctival mass located at a mean of 4.7 mm (range 2-8 mm) from the temporal limbus and anterior to the lateral orbital rim. The mean visual acuity of eight patients in whom it was possible to measure visual acuity was 0.9 (range 0.6-1.0). Surgery was performed in two eyes. Pathological examination revealed conjunctival dermolipoma. Postoperatively, there was no evidence of visual changes or specific complications. Conclusions: Conjunctival dermolipoma may be suspected in cases with a congenital subconjunctival non-movable mass, which can be confirmed by computed tomography and pathological examination. 목적: 결막 유피지방종의 임상 양상에 관해 알아보고자 하였다. 대상과 방법: 2016년 3월부터 2018년 3월까지 본원 안과에 방문하여 결막 유피지방종으로 진단받은 18명의 임상 소견을 후향적으로 분석하였다. 결과: 내원 당시 평균 연령은 61개월(3개월-45세)이었으며, 남자가 10명, 여자가 8명이었고, 각막윤부에서 평균 4.7 mm (2-8 mm) 거리의 이측, 안와연 앞쪽 눈알결막에 분홍색의 종괴가 관찰되었다. 시력 측정이 가능했던 8명의 종괴가 있는 눈의 평균 시력은 0.9 (0.6-1.0)였다. 2안에서 제거 수술을 시행하였으며, 제거한 조직의 병리검사상 결막 유피지방종으로 진단되었고, 수술 후 시력 변화는 없었으며, 특별한 합병증도 관찰되지 않았다. 결론: 선천적으로 안구의 이측 결막하에 볼록한 종괴가 관찰되고 움직임이 없으면, 결막 유피지방종을 의심해 볼 수 있으며, 전산화단층촬영과 병리조직학적 검사를 통해 확진할 수 있겠다.

        Fibrolipomatous Hamartoma of Digital Branch of the Median Nerve without Macrodystrophy: Two Case Reports with Magnetic Resonance Imaging Findings

        Fibrolipomatous hamartoma (FLH) of the nerve is a rare, benign tumor that most commonly originates from the median nerve of the hand. Fibrofatty tissue proliferates around the nerve and infiltrates the epineurium and perineurium. We present two cases of pathologically proven FLH of a digital branch of the median nerve, without macrodystrophy with magnetic resonance imaging, surgical and pathologic findings. Magnetic resonance images of both cases show well-circumscribed mass with fat signal intensity around an enlarged digital branch of the median nerve and characteristic coaxial-cable-like appearance on axial images and spaghetti-like appearance on coronal images.

        추적관찰 중 형태 변화를 보인 유방종괴에서 발견된 아포크린 상피내암종: 증례 보고

        Apocrine carcinoma in situ of breast is a rare, unique, and morphologically distinct type of breast carcinoma. Low-grade apocrine ductal carcinoma in situ (DCIS) and apocrine metaplasia with atypia are the pathologic spectrum of apocrine breast lesions. Differentiating these two lesions is difficult due to partial microscopic overlap. We describe a case of apocrine DCIS which presented an asymptomatic hypoechoic mass with morphological change on a follow up ultrasonography. 유방의 아포크린 상피내암종은 드문 질환으로, 형태학적으로 독특한 유형의 유방암이다. 저등급 아포크린 상피내암종 (low grade apocrine ductal carcinoma in situ)과 비정형 아포크린 화생(apocrine metaplasia with atypia)은 유방 아포크린 병변의 병리적 스펙트럼이며, 두 병변은 현미경상에서 중첩되는 소견을 포함하고 있어 감별이 어렵다. 우리는 무증상 저에코 유방종괴에 대한 추적 초음파 검사에서 형태학적 변화가 관찰된 아포크린 상피내암종에 대해 보고하고자 한다.

        Intracranial Extra-Axial Ependymoma in the Ambient Cistern That Initially Presented as Calcification: A Report of Case

        Ependymomas usually occur in intraventricular or intraspinal locations. Intracranial extraaxial ependymomas (IEAEs) are very rare. Here, we present an unusual case of an IEAE in 25-year-old man. The patient experienced head trauma two and half years prior to presentation, at which time brain computed tomography (CT) showed only a dense calcification in the left ambient cistern. After two and a half years, follow up brain CT and magnetic resonance imaging depicted interval growth of a calcified mass with cystic change. The tumor was successfully treated surgically, and the pathologic examination confirmed ependymoma.
