통합 의학 치료로 5년 생존 및 완전 관해에 도달한 췌장암 증례 보고 1례

Objectives: This is a five-year survival and complete response (CR) report on pancreatic cancer treated with western medicine and Korean traditional medicine. Method: A 59-year-old woman diagnosed with pancreatic cancer visited ○○ Korean traditional medicine hospital after neoadjuvant chemotherapy and pylorus-preserving pancreaticoduodenectomy. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from March 2018 to September 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years and three months of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on October 26, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for pancreatic cancer. Further studies should be performed to establish the proper treatment protocol of integrative medicine for pancreatic cancer.
축열식 촉매 산화(RCO) 반응의 성능 최적화를 위한 전산 해석

본 논문에서는 RCO 반응기의 최적 조업 조건을 도출하고자 현재 조업 중인 RTO 반응기 촉매층의 압력분 포, 유속분포, 유선장, 체류시간, 온도분포 등에 대하여 분석하였고 최적의 조업 조건을 도출하기 위한 전산유체해석 을 수행하였다. 전산 해석 결과, 현재 조업중인 반응기 촉매층의 축열재로 인한 압력손실은 크지 않은 것으로 나타 났으며, 세라믹 필터 이후 유속은 1.8∼2.7 m/s로 특별한 편류없이 비교적 안정적 유속분포를 보이고 있었다. 기류분 포를 좀 더 개선하려고 한다면 설비 내부에 플래넘과 촉매층 접속 구간 연장 등의 방법을 고려할 수 있을 것으로 판단된다. 그러나 기류 안내판이나 다공판 부착 등의 방법은 큰 효과는 없을 것으로 나타났다. The computational fluid dynamics was analyzed for the pressure distribution, stream velocity distribution, stream line field, retention time and temperature distribution which are applied to the catalyst layer in the RCO reactor to derive the optimum operating condition of the heat condensing type catalytic oxidation (RCO) reactor. The results from the computational analysis revealed that the pressure loss due to the ceramic honeycomb in the catalytic bed of the reactor which is operating currently is not significant and the stream velocity (1.8∼2.7 m/s) after the ceramic filter is working in stability without big channeling. To improve the stream velocity distribution of the air stream, it is necessary to extension of the connecting range between the plenum and catalytic bed inside the facility. However, the method of attaching the air stream guide vane or the perforated plate inside the reactor was not so effective.
SCR 공정에서 반응기 내부의 유동 균일화를 위한 수치적 연구

SCR의 NOx 제거 성능은 촉매 요인(촉매 구성물질, 형태, 공간속도 등), 배가스의 온도, 유속 분포 등의 다 양한 인자에 의해 좌우되며, 이 중 촉매층으로 유입되는 유동의 균일도는 가장 중요한 요소가 된다. 본 연구에서는 3 차원 수치 해석 기법을 이용하여 설계 단계의 SCR 반응기 내의 유동 특성을 모사하여 기류 균일도 여부를 확인하였 다. 또한 SCR 반응기 내의 유동 균일도를 최적화시키기 위해 가이드 베인과 배플 및 다공판 등을 설치하였을 경우 반응기 내부 유동 및 촉매층의 기류 균일도에 미치는 영향에 대해 연구를 수행하였다. 유동 개선을 위해 인입 덕트 곡관부에 가이드 베인을 설치하여 처리가스를 적절하게 배분시키고, 반응기 상단에 배플을 설치한 결과 반응기 내부 유동의 편류 개선에 매우 효과적임을 알 수 있었다. 또한 다공판을 예비 촉매층 하단부에 추가로 설치함에 따라 유동 을 한번 더 완충시킬 수 있어 기류 균일도가 매우 양호해짐을 확인 할 수 있었다. Performance of NOx removal in SCR(Selective Catalytic Reduction) process depends on such various factors as catalyst factors (catalyst composition, catalyst form, space velocity, etc.), temperature of exhaust gas, and velocity distribution of exhaust gas. Especially the flow uniformity of gas stream flowing into the catalyst layer is believed to be the most important factor to influence the performance. In this research, the flow characteristics of a SCR process at design stage was simulated, using 3-dimensional numerical analysis method, to confirm the uniformity of the gas stream. In addition, the effects of guide vanes, baffles, and perforated plates on the flow uniformity for the inside and catalyst layer of the reactor were studied in order to optimize the flow uniformity inside the SCR reactor. It was found that the installation of a guide vane at the inlet duct L-tube part and the installation of a baffle at the upper part is very effective in avoiding chaneling inside the reactor. It was also found that additional installation of a perforated plate at the lower part of the potential catalyst layer buffers once more the flow for very uniform distribution of the gas stream
보편적 학습 설계에 기초한 자기결정 프로그램이 정신지체 학생과 비장애 학생의 자기결정력에 미치는 영향

이 연구는 보편적 학습 설계에 기초한 자기결정 프로그램이 정신지체 학생의 자기인식, 자기옹호, 목표설정, 문제해결과 비장애 학생의 자기결정력에 미치는 영향을 알아보는데 목적이 있다. 이를 위해 충남에 소재한 초등학교에 재학 중인 2학년 정신지체 학생 2명과 같은 학교, 학년의 비장애 학생 11명을 대상으로 6주 동안, 매주 2회기씩 보편적 학습 설계에 기초한 자기결정 프로그램을 실시하였다. 정신지체 학생의 자기결정력 변화 행동을 알아보기 위해 중재 활동을 촬영하고 분석하였으며, 비장애 학생들의 자기결정력은 학생용 자기결정 검사를 사용하여 중재 이전과 이후의 차이를 검증하였다. 연구 결과, 정신지체 학생들은 자기인식, 자기옹호, 목표설정, 문제해결 행동에 긍정적인 행동 변화를 보였고, 비장애 학생의 자기결정력도 통계적으로 유의한 향상을 나타냈다. 이 같은 결과는 장애 학생과 비장애 학생이 통합교육 장면에서 보편적 학습 설계라는 원리에 근거하여 함께 자기결정 프로그램에 참여하고 긍정적 성과를 얻었다는 점에서 의의가 있다. The purpose of this study was to examine the impacts of the self-determination program based on universal design for learning on self-determination of students with mental retardation and without special needs. This study was identify impacts of self-determination program based on universal design for learning on self-awareness, self-advocacy, goal setting, problem solving of students with mental retardation and without special needs. Subjects of this study were 2 elementary students with mental retardation and 11 elementary students without special needs. They took part of self-determination program based on universal design for learning twice a week for 6weeks. For analysing results, all the activities of subjects were recorded by videotape recorder and transcribed by 2 observers, and checklist of self-determination was implemented by students without special needs. The results of this study follow as. First, the self-determination program based on universal learning design had positive impacts on self-awareness of the students with mental retardation; self-advocacy, goal setting, and problem solving of the students with mental retardation. Second, the self-determination program based on universal learning design influenced the positive effect on self-determination of students without special needs.
중환자실에서 사용되는 의료장비의 경보음 발생과 관리 현황

Purpose: This study aimed to investigate the clinical alarm occurrence and management of nurses toward clinical alarms in the intensive care unit (ICU). Methods: This observational study was conducted with 40 patients and nurses cases in two ICUs of a university hospital. This study divided 24 hours into the unit of an hour and conducted two times of direct observation per unit hour for 48 hours targeting the medical devices applied to 40 patients. Data were analyzed using IBM SPSS Statistics 23. Results: On average, 3.8 units of medical devices were applied for each patient and the ranges of alarm settings were wide. During 48 hours, 184 cases of clinical alarm were occurred by four types of medical devices including physiological monitors, mechanical ventilators, infusion pumps, and continuous renal replacement therapy. Among them, false alarm was 110 cases (59.8%). As for the alarm management by ICU nurses, two-minute alarm mute took up most at 38.0% (70 cases), and no response was second most at 32.6% (60 cases). When valid alarm sounded, nurses showed no response at 43.2%. Conclusion: The findings suggest that a standard protocol for alarm management should be developed for Korean ICU settings. Based on the protocol, continuous training and education should be provided to nurses for appropriate alarm management.

Method: A 25-year-old woman diagnosed with rectal cancer visited ⃝⃝ Korean traditional medicine hospital after neoadjuvant concurrent chemoradiation therapy, chemotherapy, and low anterior resection with regional lymph node dissection. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from January 2018 to February 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on March 11, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for rectal cancer. This study was performed to identify the state of the exposure and characteristics of pollutants by each process at 4 casting sites located in Gyeongsang-do from April to November 2013. The concentrations of methanol, crystallized silica glass, formaldehyde and phenol were analyzed by different process - casting process, molding process, core process, and shakeout & finishing process. The highest concentration of methanol was found in casting and molding process, whereas the highest concentration of crystalline quartz(Silica) was observed in core process. The most oxidized steel dusts and the highest concentration of fume were found in shakeout & finishing process. As a result of this study, those labors working at the casting site were found to be constantly exposed to various forms of hazardous chemicals; therefore, it is considered that this is the time to manage and plan how to reduce them. In addition, it is required to thoroughly manage the local exhauster, and improve the process and working environment to reduce various forms of hazardous chemicals.
