한국인 남성 불임 환자에서 5,10-Methylenetetrahydrofolate Reductase (MTHFR) 유전자의 1298번의 다형성과의 관련성에 관한 연구

Objective: To investigate the association of genetic background between MTHFR A1298C genotype and male infertility. Materials and Methods: We compared 377 infertile males with 396 healthy fertile males with one or more offspring. Infertile males were classified into four subtypes (281 azoospermia, 26 oligoasthenoteratozoospermia (OAT), 59 severe OAT and 11 remnants) by World Health Organization (WHO). Pyrosequencing analysis for MTHFR (methylenetetrahydrofolatereductase) A1298C variation was performed on polymerase chain reaction (PCR) product of study group. To validate pyrosequencing data of A1298C variation for randomly selected 50 samples, we compared the pyrosequencing result with the PCR-RFLP (Restriction Fragment Length Polymorphism) result of MTHFR A1298C genotype. Results: We studied MTHFR A1298C variation by pyrosequencing. A1298C variation data (1298 AC; p=0.2166 and 1298 CC; p=0.5056) of MTHFR gene was no significant difference in between fertile and infertile males. Conclusion: The genetic analysis in MTHFR gene didn't appear genetic difference in Korean fertile and infertile males. We require further study for MTHFR gene in infertile males.

효율적인 멀티캐스트 서비스를 위한 인증 기법

멀티캐스트는 송신자가 많은 수신자들에게 동시에 데이터를 전송하므로 송신자의 자원을 절약하고 네트워크의 점유율을 낮춤으로 효율성을 제공하는 통신 기술이나 여러 수신자들이 같은 그룹 주소를 사용하므로 공격을 받을 위험이 크다. 따라서 멀티캐스트 스트림에 대한 인증 및 서명은 중요한 문제이다. 본 논문에서는 패킷을 전송한 송신자의 신원을 확인하고 데이터가 변조되지 않았음을 확인하는 소스 인증 방법으로 다중 체인 인증기범을 제안하였다. 제안한 기법은 부인 방지를 제공하고 여러 패킷에 대한 전자서명으로 인증함으로써 오버헤드론 줄이며 지연없이 이루어지므로 실시간 멀티미디어 서비스에 사용할 수 있다. Multicast communication is simultaneous transmission of data to multiple receivers and saves considerably sender resources and network bandwidth. It has high risk to attack using group address and inherent complexity of routing packets to a large group of receivers. It is therefore critical to provide source authentication, allowing a receiver to ensure that received data is authentic. In this paper, we propose the multiple chain authentication scheme for secure and efficient multicast stream. To evaluate the performance of our scheme, we compare our technique with two other previously proposed schemes using simulation results. Our scheme provides non-repudiation of origin, low overhead by amortizing the signature operation over multiple packets, and high packet loss resistance.

In the case of a fire in the city center where population and buildings are dense, the risk of a social disaster increases. In order to find concrete solutions to prepare for this, it is necessary to analyze the spatial scope. Accordingly, using GIS for Jung-gu, Seoul, which has a high population density, the population vulnerable to disasters, the ratio of old buildings, and the density of fires were derived, and the areas excluded from the fire service area through network analysis were analyzed for each administrative dong to determine the final fire-vulnerable area. Therefore, based on the analysis results, a plan for readjustment of areas that can prepare for disasters according to social, physical, and regional characteristics was studied.

텍스트 마이닝을 활용한 4차 산업혁명 관련 보도분석: 부산 지역일간지와 중앙일간지 비교를 중심으로

본 연구는 미래 먹거리라고 하는 4차 산업혁명에 대하여 중앙일간지와 부산 지역일간지에서 다루고 있는 기사를 분석하여 주요 의제와 내용의 차이를 살펴보았다. 특히 지방정부에서 4차 산업혁명을 지역특화산업 발전과 인구증가방안 등 도시발전을 위한 주요의제로 삼고 있는 상황에서, 부산 지역일간지에서 4차 산업혁명과 관련해 지역에 특화된 논의가 충분히 이루어지고 있는지 중앙일간지와의 비교를 통해 검토하고자 하였다. 이를 위해 중앙일간지 <중앙일보>, <조선일보>기사 4,770개, 부산 지역일간지 <부산일보>, <국제신문> 기사 2,204개를 바탕으로 텍스트 마이닝의 한 기법인 토픽모델링과 언어 네트워크 분석을 활용하여 중앙지와 지역지에서 언급되는 주요 주제와 그 관계성을 살펴보았다. 중앙일간지의 토픽을 살펴본 결과 4차 산업혁명과 관련하여 정부의 정책 기조와 경제적 영향과 대응, 교육 등 전반적이고 거시적인 수준에서 논의하고 있음을 확인하였으며, 네트워크 분석에서도 중심성이 높은 단어들을 중심으로 각 분야별 주제어들이 광범위하게 연결되어 있었다. 반면, 부산 지역일간지에서는 ‘해양·수산 인공지능 활용’이나 ‘교육프로그램 운영’ 등 중앙일간지보다 좀 더 지역적이고 목적 지향적인 주제들이 나타나긴 했지만, 주제어들끼리의 관계성이 약해 정책소개나 기술소개 등 단편적인 기사가 많은 것으로 나타났다. This study analyzed online news article on the 4<SUP>th</SUP> Industry Revolution covered by the national and local daily newspapers, and looked at the differences in topics covered by the national and local newspapers. Since the 4<SUP>th</SUP> Industry Revolution is one of the main agendas of local government‘s development plan, we also examined whether the local newspapers discussed enough local agendas and issues. For the study, we collected 4,770 news articles from JoongAng Ilbo, Chosun Ilbo and 2,204 news articles from Busan Ilbo, Kookje Daily News. We used topic modeling and semantic network analysis of text mining to examine news topics mentioned in the central and local newspapers. The result showed that national newspapers reported the 4th Industrial Revolution with more macro level issues compared to local newspapers, including the government"s policy stance and the economic impact of the 4th Industrial Revolution. Local newspapers mentioned more localized topics such as marin industry and specialized education, but the key words from the topics were mostly about introduction or needs of the 4<SUP>th</SUP> Industry Revolution in the fields, not about issues or details in the topics.

한국과 태국 군병원 간호장교의 환자안전 지식, 태도, 수행 비교

Purpose: The purpose of this study was to compare patient safety knowledge, attitude, and skills between the Republic of Korea and Thailand nursing officers. Methods: Data were collected from 104 Korean nursing officers and 106 Thai nursing officers using structured questionnaires, patient safety knowledge, attitude and skills. Results: The proportion of men in Korean nursing officers were higher than that of Thai nursing officers, and Thai nursing officers had longer work experience than Koreans. The ROK nursing officer's patient safety knowledge score was significantly higher than Thai's (t=5.38, p<.001). However there was no difference between them in patient safety attitude and skills scores. Both countries had no difference in patient safety knowledge, attitude, and skills according to general characteristics. Conclusion: Nursing officers of the two countries had similar levels of patient safety knowledge, attitudes, and skills. However, in order to understand the gaps in patient safety culture, policy, and education programs between them, the two parties need to exchange information and benchmark with each other to strengthen their nurses' competence in patient care.

ICNP 기반의 군병원 전자간호기록 시스템 개발

연구목적: 본 연구는 군병원 간호장교의 간호기록을 질적·양적으로 향상시키기 위하여 확장성과 표현성을 인정받고 있는 간호용어체계인 ICNP 기반의 전자간호기록 시스템을 개발하기 위해 수행되었다. 연구방법 :본 연구는 ICNP 기반의 군병원 전자간호기록 시스템을 개발하기 위한 서술적 연구이며, 시스템 수명주기 모델(System development life cycle model)을 근거로 하였다. 개발단계는 1단계 표준간호진술문의 pool 구성및 ICNP 매핑과, 2단계 사용자 화면 구성의 두 개 단계를 주요 과정으로 하였다. 1단계는 2011년 3월 시작되어 2012년 2월 현재 계속 진행 중이며, 2단계는 2011년 3월부터 2012년 1월 말까지 수행·완료되었다. 연구결과: 총 113개의 간호진단을 도출하였으며, 간호사정 진술문 총 22,471개와 간호중재 진술문 총11,805개로 표준간호진술문 pool을 구성하였으며, 간호평가의 지표가 되는 간호결과 진술문은 간호사정진술문에 포함되었다. ICNP 매핑은 간호사정 진술문의 56.1%, 간호중재 진술문의 31.2%가 완료되었고 프로그램을 시범 운영하면서 지속 시행할 계획이다. 사용자 화면은 사용자들이 간호과정을 준수하며 작성할 수 있도록 구성하였고, 입·퇴원, 검사 및 시술 등 절차 기술을 위한 간호기록은 프로토콜 메뉴를 이용하여 간략하게 작성할 수 있도록 구성하였다. 결론: 본 연구는 ICNP 기반의 군 병원 전자 간호기록 체계를 개발하였으며 이를 통해 간호장교들이 보다 효과적·효율적으로 간호기록을 할 수 있을 것으로 기대한다.

Rapid Prototyping Model을 적용한 간호사를 위한 재난윤리 교육 프로그램 개발

Purpose: The purpose of this study was to develop a disaster ethics education program for Korean nurses to improve their ethical competency in disaster response. Methods: This study adopted Rapid Prototyping Model with three phases. For the information gathering phase, a literature review and a focus group interview were carried out to identify education contents and nurses’ demand for disaster ethics education. During the 1st iterative loop, a prototype was developed, and a validity test was done. Next, the authors implemented the prototype for nurses as a pilot test, revised it based on their feedback, and finalized it. Results: The disaster ethics education program consisted of 5 sessions: Introduction Session, three Disaster Nursing Ethics Sessions, and Debriefing Session. Each Disaster Nursing Ethics Session included a pre-lecture, a case analysis and discussion, and a post-lecture. S-CVI of the prototype was .94 and the participants' satisfaction score in the pilot test was 4.68 out of 5.00. Conclusion: This study developed a disaster ethics education program for nurses, using Rapid Prototyping Model, for the first time in Korea. This program will contribute to enhancing the ability of nurses to respond to disasters by improving their ability to make ethical decisions.

Purpose : With the recognition of an imbalance in performance measurement, nursing organizations need to focus on efficiency as much as effectiveness. Although many studies have evaluated efficiency of nursing organizations with data envelopment analysis (DEA), they were usually confined to wards or nursing homes rather than a nursing department as a whole. This study aimed to measure technical efficiency of nursing departments of South Korean general hospitals using DEA and to identify its determinants. A Descriptive study design was used. 140 nursing departments from general hospitals across the nation were recruited as the decision making units (DMUs) for this study. Methods : Two input and three output variables were employed for the input-oriented VRS model. And seven determinants were chosen for the two-part model. For the analyses, the 140 DMUs were classified into three groups according to the number of beds available as a way to keep them homogeneous. Results : The efficiency frontier for each group was estimated, and each group had technically efficient DMUs of more than 30%. The technical efficiency score of the average DMU from each group was around 0.8. The ratio of FTE RNs to inpatients per day and length of stay were negative influencing factor of the level of technical efficiency of inefficient DMUs. Conclusion : Nurse managers should be careful not to achieve technical efficiency of their organization at the expense of quality of care. At the same time, they should not ignore efficiency in pursuit of quality of care.
