        Purpose : With the recognition of an imbalance in performance measurement, nursing organizations need to focus on efficiency as much as effectiveness. Although many studies have evaluated efficiency of nursing organizations with data envelopment analysis (DEA), they were usually confined to wards or nursing homes rather than a nursing department as a whole. This study aimed to measure technical efficiency of nursing departments of South Korean general hospitals using DEA and to identify its determinants. A Descriptive study design was used. 140 nursing departments from general hospitals across the nation were recruited as the decision making units (DMUs) for this study. Methods : Two input and three output variables were employed for the input-oriented VRS model. And seven determinants were chosen for the two-part model. For the analyses, the 140 DMUs were classified into three groups according to the number of beds available as a way to keep them homogeneous. Results : The efficiency frontier for each group was estimated, and each group had technically efficient DMUs of more than 30%. The technical efficiency score of the average DMU from each group was around 0.8. The ratio of FTE RNs to inpatients per day and length of stay were negative influencing factor of the level of technical efficiency of inefficient DMUs. Conclusion : Nurse managers should be careful not to achieve technical efficiency of their organization at the expense of quality of care. At the same time, they should not ignore efficiency in pursuit of quality of care.

        ICNP 기반의 군병원 전자간호기록 시스템 개발

        연구목적: 본 연구는 군병원 간호장교의 간호기록을 질적·양적으로 향상시키기 위하여 확장성과 표현성을 인정받고 있는 간호용어체계인 ICNP 기반의 전자간호기록 시스템을 개발하기 위해 수행되었다. 연구방법 :본 연구는 ICNP 기반의 군병원 전자간호기록 시스템을 개발하기 위한 서술적 연구이며, 시스템 수명주기 모델(System development life cycle model)을 근거로 하였다. 개발단계는 1단계 표준간호진술문의 pool 구성및 ICNP 매핑과, 2단계 사용자 화면 구성의 두 개 단계를 주요 과정으로 하였다. 1단계는 2011년 3월 시작되어 2012년 2월 현재 계속 진행 중이며, 2단계는 2011년 3월부터 2012년 1월 말까지 수행·완료되었다. 연구결과: 총 113개의 간호진단을 도출하였으며, 간호사정 진술문 총 22,471개와 간호중재 진술문 총11,805개로 표준간호진술문 pool을 구성하였으며, 간호평가의 지표가 되는 간호결과 진술문은 간호사정진술문에 포함되었다. ICNP 매핑은 간호사정 진술문의 56.1%, 간호중재 진술문의 31.2%가 완료되었고 프로그램을 시범 운영하면서 지속 시행할 계획이다. 사용자 화면은 사용자들이 간호과정을 준수하며 작성할 수 있도록 구성하였고, 입·퇴원, 검사 및 시술 등 절차 기술을 위한 간호기록은 프로토콜 메뉴를 이용하여 간략하게 작성할 수 있도록 구성하였다. 결론: 본 연구는 ICNP 기반의 군 병원 전자 간호기록 체계를 개발하였으며 이를 통해 간호장교들이 보다 효과적·효율적으로 간호기록을 할 수 있을 것으로 기대한다.

        간호사의 억압집단행위(OppressedGroupBehavior)에 관한 개념분석

        The purpose of this study was to identify the concept of oppressed group behavior shown among nurses, and thus, to give an operational definition. Methods : The concept analysis method, which comprises of eight stages, developed by Walker and Avant was used. Results : The defining attributes of nurses` oppressed group behavior identified in this study were 1) gregariousness and conformity, 2) self-depreciation, and 3) submissive aggressiveness including horizontal violence. The antecedents to nurses` oppressed group behavior were 1) identification with the oppressor, 2) marginalization, and 3) low self-esteem. The consequences of nurses` oppressed group behavior included 1) lack of empowerment, 2) burnout, and 3) intension of turnover. Conclusion : Considering the results, with continuous examination, nurses` oppressed group behavior should not be tolerated so that nurses are able to improve their job satisfaction and quality of nursing care.

        제대 간호장교의 재취업 경험에 대한 탐구

        Purpose: This study was designed to explore the re-employment process of retired nurse officers and to develop a grounded theory about their experiences. Methods: The participants were 9 retired nurse officers holding jobs. Data was collected through interviews using MP3 recordings. The data were analyzed using the Strauss and Corbin(1998) method. Results: The process of ``seeking new dream by breaking down the barrier of reality`` was categorized into four stages: ``destiny: wrapping up the military service``, ``anxiety: looking for a job``, ``adaptation to the new job: giving up, meagerly holding on, satisfaction``, and ``dreaming: planning for the future``. There were four steps of emotional change in this chronological order: ``lack of concern``, ``anxiety: low self-esteem``, ``confidence: stepping up to the challenge``, ``missing the military``. Conclusion: The findings of this study will help better understand the retired nurse officers`` re-employment experience.

        군병원 인수인계 실태 및 간호사의 인수인계에 대한 인식

        Purpose : This study was conducted to identify the status of nursing handover in the ROK military hospitals and nurses` perception, experience level and causes of nursing handover errors. Methods : The subjects were 137 nurse officers and civilian nurses who were working in general wards of two military hospitals. In total, 110 data were used in the statistical analysis. Results : Only 8.2% of the respondents reported that they covered all the patients during their nursing handover process. The average handover time was 19.2±12.0 minutes. 59.8% of the respondents were found to use no written guideline or checklists during their nursing handover in the wards. The nurses perceived that their nursing handover was efficient (5.21±0.68), informative (5.13±0.69), and interactive and supportive (4.86±0.86). On the other hand, they expressed that their patients were rarely involved in the handover process (3.61±0.87). The experience of making a phone call to the preceding nurse for unclear or omitted information demonstrated the highest score (1.76±0.50) in experience related to nursing handover errors. The nurses indicated ``heavy workloads (3.54±0.96)``, ``many interruptions of handover (3.31±1.00)``, ``stress and fatigue (3.24±0.89)``, ``personal disposition (3.15±0.99)`` as the main causes of handover errors. Conclusion : The results of this study suggest that it is necessary to develop a standardized nursing handover protocol to promote patient safety by preventing errors in nursing handover.

        Purpose : The purpose of this study is to analyze the healthcare system of North Korea and compare it with that of South Korea and ultimately provide suggestions in the preparation for unification. Methods : The study started from literature review and in-depth interview, followed by specialist consultation. Results : 1) Healthcare system of North Korea is organized, but the service is provided depending on the status and ranks of an individual. 2) North Korean military medical service emphasizes three following principles : providing primary care, ensuring the continuity, and offering rapidity of medical support. 3) Medical personnel are military physicians, military nurses, and medics. 4) Major health issues are infectious and chronic degenerative disease, to which people have become more susceptible due to poor infrastructure. 5) Periodic training for nuclear war is conducted. Conclusion : This study analyzed North Korea`s healthcare system and tried to find challenges in military medical nursing field in the preparation for unification. These findings will offer valuable information to military medical readiness.
