        기저세포암의 모즈 미세도식 수술시 동결절편에서의 광학현미경 판독에 관한 연구

        N/A Background : Mohs micrographic surgery (MMS) achieves higher cure rates for cutaneous squamous cell and basal cell carcunomas than any other therapeutic modality. For a unifocal tumor, a 100% cure rate after MMS should theoretically be possible, however for primary basal cell carcinoma, 98-99% 5-year disease-free rates have been achieved. Objective : Our purpose was to investigate the pitfalls in microsscopic Interpretation of frozen sections in Mohs micrographic surgery for basal cell carcinoma which decrease the cure rate after surgery. Methods : From March 1991 to February 1998, fifty-nine patients were diagnosed with basal cell carcinoma at our department. All the tumors were removed with Mohs micrographic surgery and frozen section specimens were stained with hematoxylin-eosin. The microscopic evaluation was done by Mohs surgeon and pathologist. Results : We can summarize the matters that demand special attention during microscopic Interpretation of frozen sections in Mohs surgery as two groups. First, as false negative interpretation, there are 1) small nests of tumor scattered within areas of heavy inflammation 2) tumor present along the hair follicle, 3) tumor present along a cut edge, 4) empty space in a tissue section, 5) hair follicle-like structure of the tumor, 6) gland-like structure of the tumor, and 7) infiltrative BCC-like inflammatory cells. Second, as a false positive interpretation, there are 1) foreign body reactions or scar containing trapped pilosevaceous structures, 2) horizontal and tangential cuts through the pilosevaceous apparatus, 3) some epidermal neoplasms including solar lentigines, seborrheic leratoses, and acantholytic actinic keratoses, and 4) contamination of the tumor tissue. Conclusion : With careful attention to the examples which can affect the interpretation as mentioned above, it may be possible to detect complete removal of tumor mass and achieve higher cure rate. We could achieve a 100% of cure rate for primary basal cell carcinoma and 95% of cure rate for recurrent basal cell carcinoma after Mohs Micrigraphic surgery in our hospital from March 1991 to February 1998.         DEA모형을 활용한 국내 태양광발전소의 효율성 평가에 관한 연구

        The purpose of this paper is to provide the methodology of evaluation for photovoltaic power plants in Korea and to maximize operational efficiency of business by conducting the overall efficiency analysis on regional photovoltaic power plants. As a research methods for the analysis of efficiency, this paper applies the DEA (Data Envelope Analysis), a kind of non-parametric approach. Five input variables selected by a group of experts were ‘otal project cost’ ‘onstruction unit cost’ ‘lant area’ ‘ersonnel’ operation cost, and ‘ower capacity’ and an output variables : ‘mount of power generation‘ For verification of the analysis model developed in the study, 70 photovoltaic power plants in the area of Jeo-nam province were selected as DMUs, and multiple regression analysis and multi-collinearity analysis were conducted. As result of the analysis, the analysis model are verified as proper. This study shows that the proposed method of evaluation will be useful and practical for the complement of the existing quantitative assessment methods by its quality oriented characteristic, and it will let the evaluation be more objective and comprehensive.

        조현병 환자의 인지기능 및 심리사회적 요인과 직업재활과의 연관성

        Objectives:This study was performed to assess the effect of cognitive function on occupational rehabilitation and to review the clinical and psychosocial factors that may affect occupational rehabilitation in patients with schizophrenia. Methods:A total of 58 patients were enrolled and divided into three groups : group of occupational rehabilitation (n=18), day-care(n=19), and out-patient care(n=21). For the cognitive function assessment, the Korean version of the MATRICS Consensus Cognitive Battery(MCCB) and the Korean version of the UCSD Performance-based Skills Assessment (UPSA) were performed. To assess clinical symptoms, the Positive and Negative Syndrome Scale, the Korean version of the Personal and Social Performance Scale(PSP), and the Clinical Global Impression–Schizophrenia scale(CGISCH) were performed. For the psychosocial factors assessment, the Korean Insight Scale for Psychosis and the Korean Version of 4th Revision of Schizophrenia Quality of Life Scale were performed. Results:On the MCCB, the occupational rehabilitation group showed higher scores than the day-care group in the verbal learning and social cognition domains. On the UPSA, the occupational rehabilitation group showed higher scores than the day-care group in the communication domain. No significant difference was found between the groups in the total scores of these cognitive function tests. On the CGI-SCH and PSP, the occupational rehabilitation group revealed better outcomes than the other groups. Conclusion:The schizophrenia patients with occupational rehabilitation had higher cognitive functions on some subscales, which may reflect social cognitive functions, and higher CGI-SCH and PSP scores.

        Solid pseudopapillary tumor of pancreas in children is a tumor with low malignant potentiality. and is rarely associated with distant metastasis. A 13-year-old girl was hospitalized because of abdominal pain of one week duration. Abdominal CT revealed not only a $12{\times}6cm$ sized mass at the pancreatic body and tail but also a 1cm sized mass in left lobe of the liver. The patient underwent a near-total pancreatectomy and tumorectomy of the liver. A solid pseudopapillary tumor with liver metastasis was confirmed by pathology. She has undergone 13 courses of chemotherapy and has been well for 13 monthswithout any sign of recurrence.

        인공신경망을 이용한 전단보강근이 없는 철근콘크리트 보의 전단강도에 대한 예측

        철근콘크리트 부재의 전단거동에 대한 오랜 연구에 의하여 이에 대한 다양한 이론모델들과 제안식들이 존재한다. 그러나 전판거동의 메커니즘이 복잡하고 영향을 미치는 요소들이 많아서 이론모델들은 대부분 매우 복잡한 경향이 있고, 실험에 의한 제안식들은 제한된 범위내의 실험변수에 대해서만 유효한 경우가 많다. 이러한 문제점을 해결할 수 있는 대안의 하나로써 인공신경망이 여러 연구자들에 의하여 제안되어 왔으며, 본 논문에서는 인공신경망을 이용하여 전단보강근이 없는 철근콘크리트 보의 전단강토를 예측하였다 특히, 기존의 전단실험결과를 광범위하게 모아 구축한 데이타베이스를 활용함으로써 넓은 범위의 구조변수들을 포함한 다양한 부재들을 인공신경망의 훈련자료로 이용하였고, 인공신경망에 의한 전단강토 예측 결과를 ACI의 규준식, Zsutty, Okamura의 제안식들과도 비교 분석하였다. ACI의 규준식은 전단보강근이 없는 철근콘크리트 부재에 대해서 매우 부정확한 전단강도를 제공하였으며, Zsutty의 제안식은 ACI의 규준식에 비해 향상된 예측 결과를 보였으나 부재의 크기효과를 반영하지 못하였다. Okamura의 제안식은 주요 변수들의 영향을 비교적 잘 반영하여 상당히 정확하면서도 안정적인 전단강토를 제공하였다 이에 비해 인공신경망은 실험 결과에 가장 근접한 부재의 전단강도를 제공함으로써, 다양한 변수들의 영향을 매우 정확하게 반영할 수 있는 것으로 나타나서 인공신경망이 전단강도와 같이 메커니즘이 복잡하고 영향을 끼치는 변수들이 많은 다른 구조적 거동이나 강도를 예측하는데 매우 적절한 수단을 제공할 수 있음을 보여주었다. Due to the complex mechanism and various parameters that affect shear behavior of reinforced concrete (RC) members, models on shear tend to be complex and difficult to utilize for design of structural members, and empirical relationships formulated with limited test data often work lot members having a specific range of influencing parameters on shear. As an alternative approach tot solving this problem, artificial neural networks have been suggested by some researchers. In this paper, artificial neural networks were used to predict shear strengths of RC beams without shear reinforcement. Especially, a large database that consists of shear test results of 398 RC members without shear reinforcement was used for artificial neural network analysis. Three well known approaches for shear strength of RC members, ACI 318-02 shear provision, Zsutiy's equation, and Okamura's relationship, are also evaluated with test results in the shear database and compared with neural network approach. While ACI 318-02 provided inaccurate predictions for RC members without shear reinforcement, the empirical equations by Zsutty and Okamura provided more improved prediction of Shear strength than ACI 318-02. The artificial neural networks, however provided the best prediction of shear strengths of RC beams without shear reinforcement that was closest to test results.

        자궁 내막에서의 저 용량 아스피린과 클로미펜이 신경세포유착물질 ( NCAM )의 발현에 미치는 영향

        N/A Objective : The treatment with low-dose aspirin in the patients with unexplained infertility has been reported to improve the pregnancy rate and implantation rate via increasing the blood flow in the endometrium. But there are little known about the relationships between low-dose aspirin and cell adhesion molecules, NCAM. The aim of this study was to evaluate the effect of low-dose aspirin and clomiphene citrate treatment on the expression of NCAM in the endometrium. Methods : The patients with unexplained infertility (N=37) were grouped into 3 groups: clomiphene citrate and low-dose aspirin treated group (N=8), clomiphene citrate treated group (N=10), and natural cycle group (N=10, no treatment). As control group, the proliferative and menopausal endometrium was used. Each endometium was obtained by endometrial biopsy performed in late luteal phase and immunohistochemical staining with NCAM was performed. Results : In the stromal cells, the staining intensity of NCAM expression and the number of vessels were significantly increased in the endomterium treated with clomiphene citrate and low-dose aspirin compared with other groups (p<0.05). And the expression of NCAM in the prolifertive and menopausal endometrium showed very weak staining. Conclusion : The expression of NCAM in the stromal cells and the number of vessels were increased in the endometrium of unexplained infertility patients treated with clomiphene citrate and low-dose aspirin. These findings may suggest low-dose aspirin has an important role during the secretory phase of endometrium to improve the implantation via increasing the expression of cell adhesion molecules, especially NCAM and increasing the number of vessels.

        실험연구 : Propofol 미세유탁액을 지속정주하여 진정된 비글견에서 집단 약동력학적 모형화

        Background: Aquafol(R) is a microemulsion formulation of propofol. This study was designed to investigate the population pharmacokinetic and pharmacodynamic modeling in beagle dogs sedated by Aquafol(R). Methods: Electroencephalogram was recorded and venous blood was sampled at preset times in 15 beagle dogs during 3 hours of infusion of Aquafol(R) and subsequently during 3 hours of recovery. Venous blood was sampled at 0, 2, 10, 30, 60, 120, 180, 190, 240 and 360 minutes after infusion. We evaluated the effect of propofol on electroencephalogram by calculating SEF90. In the preliminary analysis, two compartment model best described all data from all subjects. The pharmacodynamics were best described using an effect compartment model and ke0, a first-order elimination rate constant characterizing the effect-site equivalent to estimate the apparent effect-site concentrations. The relationship between propofol effect-site concentration and SEF90 was analyzed using an inhibitory sigmoid Emax model. Results: The final pharmacokinetic model was best described with the followings: V1 = 18.5e0.114*BWT, k10 = 1.86 min-1, k12 = 0.6 min-1, k21 = 0.684 min-1. The final pharmacodynamic model was best described with the followings: t1/2ke0 = 0.62 min, Ce50 = 32.2 ng/ml, Eo = 31.3 Hz, Emax = 20.9 Hz, γ = 1.28. Conclusions: The propofol microemulsion shows different pharmacokinetics and pharmacodynamics compared with the propofol lipid emulsion. (Korean J Anesthesiol 2006; 50: 689~97)

        증례 : 만성 설사와 저칼륨혈증으로 발현된 교원성 대장염 1예

        저자들은 만성 수양성 설사와 전신 쇠약감을 주소로 내원한 75세 남자 환자에서 교원성 대장염으로 진단하였고, 보존적 치료와 로페라미드를 하루 3 mg 투여로 임상 증상의 호전을 보인 증례를 경험하였기에 보고하는 바이다. Collagenous colitis is recognized as one of the causes of chronic diarrhea accompanied with autoimmune diseases. It is a disease associated with chronic watery diarrhea and typical histologic findings of a thick subepithelial collagenous deposit on biopsy. We experienced a 75-year-old man patient with chronic watery diarrhea and mild abdominal pain for 7 years. Physical examination, laboratory and radiologic studies were unremarkable except hypokalemia. Colonoscopy disclosed mucosal atrophy, loss of submucosal vessel and altered vascularity of terminal ileum, ascending colon. Colonoscopic biopsy revealed homogenous hyaline layered collagen deposition beneath the surface epithelium and Masson`s trichrome stain showed collagen deposition. We report the case of collagenous colitis with a review of literature.(Korean J Med 71:75-79, 2006)
