        한국인에서 류마티스 관절염과 비타민 D 수용체 유전자 다형성과의 연관성에 관한 연구

        Background : The purpose of this study was to determine whether the distribution of vitamin D receptor (VDR) polymorphisms in rheumatoid arthritis (RA) patients group is different from that of the control group and whether there is a relationship between VDR genotype and radiographic severity or bone mineral density (BMD) of RA patients. Methods : The VDR genotype was determined by polymerase chain reaction and digestion with three restriction enzymes Taq I, Apa I and Bsm I in 78 healthy women and 97 female RA patients. Hand X-rays were scored by Larsen's method and bone mineral density was measured by dual energy X-ray absorptiometry (DEXA) in RA patients. Results : VDR genotypic distributions in Koreans were much different from Caucasians in each control and RA patient groups. Especially, “tt” allele and “BB” allele were very rare, prominently differentiating from Caucasians. But within Koreans, no significant difference of VDR genotypic frequency was observed between control group and RA patients group. And there was no significant relationship of VDR genotype with radiographic scores, nor with BMD scores in RA patients. Conclusion : The distribution of VDR polymorphisms in Korean RA patients was not different from controls. The VDR polymorphisms correlate neither with the radiographic severity of RA, nor with BMD scores in Korean RA patients.(Korean J Med 62:209-217, 2002) 목적 : 한국인 류마티스 관절염 환자에서 비타민 D 수용체 유전자 다형성의 분포가 정상인들과 차이가 있는지를 비교하고 비타민 D 수용체 유전자형과 류마티스 관절염의 방사선학적 중증도 혹은 골밀도와 상관관계가 있는지를 알아보고자 하였다. 방법 : 대구 가톨릭 대학병원 류마티스내과에서 내원 치료 중인 여성 류마티스 관절염 환자 97명 (평균연령: 51.8세)을 대상으로 하였고 대조 군으로는 건강검진을 위해 내원한 건강한 여성 78명 (평균연령: 57.2세)을 대상으로 하였다. 비타민 D 유전자형은 polymerase chain reactioin (PCR) 증폭과 제한분절길이다형성 (Restriction Fragment Length Polymorphism, RFLP)에 의해 결정되었으며 환자 군과 대조 군 각각에 대해 비타민 D 유전자형의 분포를 비교하였다. 또한 류마티스 관절염 환자들의 손과 손목을 촬영하고 그 방사선사진을 골근육계 방사선과 전문의에 의해 Larsen 채점법에 따라서 판독, 채점하였고, 골밀도를 조사하였다. 그리고 비타민 D 수용체 유전자형과 방사선학적 중증도 및 골밀도와의 상관관계를 조사하였다. 결과 : 한국인에서의 비타민 D 수용체 다형성의 분포는 류마티스 관절염 환자 군 및 대조 군 모두에서 Caucasian과는 매우 달랐으며 특히 "tt"형과 "BB"형은 매우 드물었다. 그러나 한국인에서 환자 군과 대조 군간의 비타민 D 수용체 다형성의 분포는 의미있는 차이가 없었다. 그리고 류마티스 관절염 환자에서 비타민 D 수용체 유전자형은 방사선학적 중증도와 의미있는 상관관계가 없었으며 골밀도와도 상관관계가 없었다.

        벼 재배 농가의 농작물재해보험 중요도-만족도 분석- 전북지역을 중심으로 -

        This study surveyed and analyzed the importance and performance of 5 major categories (attributes) and 23 questions (factors) for crop insurance improvement plans for insured farmers. The survey targeted 106 rice farm households in Jeollabukdo province that had experience with crop insurance loss adjustment. According to the survey, farmers intend to continue using crop insurance in the future in 95.3% of the cases; however, satisfaction was low at 23.6%. According to the Importance and Performance Analysis (IPA), the following factors should be maintained (quadrant 1) with high importance and performance: the expertise of the person in charge of contracting insurance, government support for insurance premiums, convenience in claiming insurance premiums, guidance and appropriateness of adjusting the schedule, and expertise and fairness of loss adjusters. In addition, the conveniences of checking for insurance coverage, loss adjustment results, and appealing against loss adjustment results, as well as notification of premium payment timing, were factors requiring intensive improvement (quadrant 2) with high importance but low performance. In the future, the sections dealing with insurance accident reporting, loss adjustment, and indemnity payment will need to be continuously improved and supplemented to reflect the opinions of farm households, the main policyholders. 이 연구는 벼 농작물재해보험의 개선방안을 도출하고자 가입 농가를 대상으로 5개 대분류(속성), 23개 문항(요인)의 중요도와 만족도를 조사·분석하였다. 이를 위해 농작물재해보험 손해평가를 받은 경험이 있는 전북의 벼 재배 106 농가를 대상으로 설문조사를 실시하였다. 설문조사 결과 향후에도 농작물재해보험을 지속 이용하겠다는 농업인은 95.3%로 수요는 매우 높았으나 전반적인 만족도는 23.6% 수준으로 매우 낮았다. 농작물재해보험의 중요도와 만족도 분석(IPA) 결과 중요도와 만족도가 높아 지속 유지해야 하는 요인(1사분면)은 보험가입 담당자의 전문성, 보험료 국고지원, 보험사고 접수처리 편리성, 평가시기 안내와 적절성, 손해평가 인력의 전문성과 공정성 등이었다. 또한, 중요도는 높으나 만족도가 낮아 집중 개선이 필요한 요인(2사분면)은 보상하는 사고여부 확인, 손해평가 결과 확인의 편리성, 보험료 지급시기 안내, 이의신청의 편리성 등 이었다. 향후 보험사고 접수와 손해평가 및 보험금 수령과 관련된 부분은 주요 수요자인 농업인의 의견을 반영하여 지속 개선보완 해야 할 것이다.

        메트로놈을 이용한 트레드밀 보행훈련이 보행패턴에 미치는 영향

        PURPOSE: The purpose of this study was to investigate the effect of treadmill walking training using the metronome on the gait pattern. METHODS: A total of 33 healthy persons were studied consisting of 17 female and 16 male in the 20-30 age group. A gait analysis program was installed on a treadmill with a built - in gait analysis sensor and laptop. After 9 minutes of treadmill walking, gait analysis was performed for 1 minute. The mean values of the differences in the step length, angle of COP, separation line standard deviation and step force of the lower legs affecting walking symmetry were calculated for treadmill walking and treadmill walking using the metronome. The Shapiro-Wilk test was used to test the normality of the collected data and a paired t-test was performed to analyze the difference in walking before and after using the metronome. RESULTS: As a result of the analysis, the mean of difference between the measured values of the bilateral lower extremity for step length, angle of COP, separation line standard deviation and step force were statistically significant before and after treadmill walking using the metronome. CONCLUSION: Therefore, the treadmill walking training using the metronome is effective in decreasing the difference in the foot width, gait angle, gait distribution, and foot pressure. Because of this, the treadmill walking training using the metronome has a significant effect on walking symmetry among the elements for correct walking, which is a means for enabling efficient and continuous walking.

        류머티스 관절염과 골관절염 환자에서 Transforming growth factor β의 발현 양상

        The transforming growth $factor-{\beta}$ ($TGF-{\beta}$) is a multifunctional cytokine modulating the onset and course of autoimmune disease as shown in experimental models. In synovial inflammation, there is a potential role for $TGF-{\beta}$ in repairment, the inhibition of cartilage and bone destruction, and the down-regulation of immune response. The biologic effects of $TGF-{\beta}$ depend on the cell type, the isoform and the availability of active $TGF-{\beta}$. We investigated $TGF-{\beta}$ expression in patients with rheumatoid arthritis (RA) and compared to those of osteoarthritis (OA). And we determined a correlation between $TGF-{\beta}1$ and $TGF-{\beta}2$, and also the relationships between each $TGF-{\beta}$ isoform and the parameters for disease activity of RA. Methods: The study population consisted of 20 patients with RA and 20 patients with OA. The commercial ELISA kit was used to study $TGF-{\beta}1$ and $TGF-{\beta}2$ levels in peripheral blood (PB) and synovial fluids (SF). Results: 1) While PB $TGF-{\beta}1$ level was of no difference between RA and OA patient groups, SF $TGF-{\beta}1$ level was higher in RA group than OA group. Similarly, PB $TGF-{\beta}2$ levels of RA and OA groups was not different, but SF $TGF-{\beta}2$ levels was higher in RA group than OA group. 2) In patients with RA, the $TGF-{\beta}1$ levels were higher than $TGF-{\beta}2$ in both the PB and SF, while in patients with OA, there showed higher readings for $TGF-{\beta}1$ than $TGF-{\beta}2$ in SF but no difference between $TGF-{\beta}1$ and $TGF-{\beta}2$ levels in PB. 3) In patients with RA, there were no correlations between PB $TGF-{\beta}1$ and PB $TGF-{\beta}2$ levels, nor between SF $TGF-{\beta}1$ and SF $TGF-{\beta}2$ levels. At the same way, there was no correlation between PB $TGF-{\beta}1$ and SF $TGF-{\beta}1$ levels, nor between each levels of $TGF-{\beta}2$ in patients with RA. 4) There was also no correlation between each $TGF-{\beta}$ isoform and the parameters for disease activity such as ESR, CRP, tender joint count, swollen joint count, rheumatoid factor, and the duration of morning stiffness except between in PB $TGF-{\beta}1$ and disease duration of RA (r=0.637, p<0.01). Conclusion: Each $TGF-{\beta}$ isoforms were higher in synovial fluid of patients with RA than that of patients with OA. The data from the RA patients demonstrated different patterns of expressions of the isoforms depending on which compartment (PB or SF) was investigated. The quantification of different $TGF-{\beta}$ isoform is thought to be important when $TGF-{\beta}$ is measured under disease conditions of RA.
