증례: 선천성 근진장성 근이영양증과 연관된 재발성 양수과다증 1예

선천성 근긴장성 이영양증은 상염색체 우성으로 유전되는 근긴장성 이영양증의 드문 형태로, 발생율이 신생아 10만 명당 13명 정도이다. 이환된 신생아는 자궁내 발육지연, 조산, 가사, 호흡곤란이 있고, 전신적인 근긴장 저하는 항상 동반이 된다. 수유장애가 흔하여 단백질 손실 장염, 태아수종증(hydrops fetalis), 신생아 폐동맥 고혈압 지속증(persistent pulmonary hypertension of newborn)이 동반된다. 약 25%가 18개월 이내 사망하게 된다. 분자생물학적으로 Dystrophia myotonica-protein kinase(DMPK)에 관여하는 염색체 19q 13.3의 CTG 삼핵산의 불안정한 상염기 반복시의 변이에 의해 생긴다. 병의 중증도는 CTG 확장 길이와 연관있다. 양수과다증과 태동의 감소, 조기분만, 분만 후, 근긴장 저하 등의 소견과 같은 과거력이 있을 시에는 가족력과, 부모, 신생아의 근긴장성 이 영양증에 대한 PCR을 시행하여야 하겠다. 근긴장성 근이영양증의 산전진단은 태아의 융모막 생검이나 양수검사에 의한 PCR의 검사를 통하여 가능하다. 저자들은 두 번의 조산을 경험한 산모에 있어, 세 번째 임신에서 PCR을 통해 재발된 선천성 근긴장성 이영양증 1예를 경험하여 문헌고찰과 함께 보고하는 바이다. Congenital myotonic dystrophy is an autosomal dominantly inherited myotonic dystrophy, rare form, with an incidence estimated to be 13/100,000 liveborns. Affected newborns can present with intrauterine growth retardation, prematurity, birth asphyxia, respiratory distress, and always exhibit generalized muscular hypotonia. Feeding problems are common and an association with protein losing enteropathy, hydrops fetalis, and persistent pulmonary hypertension of the newborn has been described. Twenty-five percent of the affected infants die within the first 18 months of life. The molecular basis is an unstable DNA fragment consisting of a variable expansion of a CTG triplet, Dystrophia myotonica-protein kinase (DMPK) which is localized on chromosome 19q 13.3 The severity of the disease is directly correlated to the length of the CTG sequence. Women with idiopathic polyhydroamnios, decreased fetal movement, prematurity, hypotonia, should be counselled family, and mother, father and baby should be evaluated congenital myotonic dystrophy, as PCR (polymerase chain reaction). It is possible to diagnose congenital myotonic dystrophy, by PCR, antenatal test, such as CVS, amniocentensis. We experienced a case of recurrent congenital myotonic dystrophy, with neonatal death, twice, and report with a review of related literatures.

조선시대 서원 교육과 현대 대학 교양국어교육(國語敎育)의 융합 방향의 일 모색

본 논문은 대학 교양 국어교육(國語敎育)에서 미래사회를 이끌어 나갈 인재양성에 반드시 갖추어야 할 덕목으로 전통적인 서원의 교육적 의의를 살펴보고 이를 바탕으로 대학 교양 국어교육이 지향해야 할 발전적인 방향을 제안하고자 하였다. 첫째, ‘우주 삼라만상의 진리는 인간의 내부에 있다’는 서원교육의 이념은 ‘인성과 지성’과 같은 교양강좌에 적용하여 지식 습득과 함께 갖추어야 할 도덕적 덕성교육을 반드시 함께 교육해야 할 필요성이 있다. 둘째, 학습이란 마음속의 진리를 스스로 깨치는 것이라는 서원의 교육 목표는 다양한 도서의 다독을 바탕으로 ‘자기계발서’나 개별 ‘포트폴리오’ 작성 등을 통하여 창의적인 국어교육에 적용이 가능하다. 셋째, 서원 교육의 근본이념인 성리학의 인존정신(人尊精神)을 바탕으로 현대 사회 대학 교양 국어교육 중 ‘사고와 표현’과 같은 과목은 서원의 토론법, 교육방법을 활용하여 자신의 생각을 확장시키고 타인과의 원활한 소통을 위한 방법을 학습할 수 있다. 넷째, 깨달음은 돌발적이고 순간적인 학습의 방법이 아니라는 이념을 가지고 서원교육에서 중요하게 여겨진 독서활동이 현대 대학 교양 국어교육에서 ‘독서토론’을 통해 활용될 수 있다. 이는 민족의 언어예술에 대한 이해의 깊이와 관심의 폭을 넓히면서 문학을 향유하고 생활화하여 정서를 함양하며 앎의 깨달음을 통해 비로소 성장할 수 있다. 조선시대 서원의 교육이념을 바탕으로 고차원적 정신능력으로서의 문제해결력과 위기관리력, 상황 대처능력과 혁신주도력, 미래예견력과 환경변화 적응력, 강인한 정신력을 갖춘 미래 사회인을 양성할 수 있는 기회제공을 대학 교양 국어교육을 통해 제고해 보아야 한다. Educational goals of the Joseon dynasty and modern Korea attempt to reach the ideal human state and have in common learning Hongik Human Ideology. The purpose of this study was to research for the virtues based on the meaning for traditional Seowon`s Education that university students must have to lead future society through the liberal Korean education in university and suggested the direction of development for liberal Korean education. The direction of convergence can be summarized as follow:First, it should use the philosophy of Seowon`s education, which is ‘The truth of the universe is in the interior of the human’ by learning through the subject such as ‘Personality and Intelligence’ to improve knowledge and moral virtues for university students. Second, using the goal of Seowon`s education, which is ‘Learning is understood by themselves’, it can be used for variety of subjects, such as ‘Appellative Texts’ and ‘Developing a Portfolio’. Third, based on Neo-Confucianism`s ‘Injon’ spirit of Seowon`s education, it can be adapted to the subject of ‘Thinking and Expression’ to extend the skills of communication by developing discussion skills. Fourth, through the philosophy of ’The Enlightenment is not the sudden and instantaneous learning’ the subject of ‘Reading and Discussion’ should be employed in to improve understanding of national language culture and emotional cultivation. It should be considered of adapting the Seowon`s educational philosophy to liberal Korean education in university to encourage improvement of critical and creative thinking skills to be a capable person in future society.
