        Acute Kidney Injury Accompanied by Acute Pyelonephritis and Renal Abscess in a Child with Vesicoureteral Reflux

        Acute pyelonephritis (APN) is a relatively common bacterial infection in children. In previously healthy children, acute kidney injury (AKI) is very rare in the course of APN without urinary tract obstruction, renal hypoperfusion due to hypotension or exposure to nephrotoxic agents. We describe a case of AKI secondary to APN and renal abscess in a child with vesicoureteral reflux. With antibiotic treatment and adequate hydration, the patient was improved. APN should be included in the differential diagnosis of AKI and adequate treatment by proper antibiotics is crucial for full recovery of renal function. 소아에서 급성 신우신염은 상대적으로 흔한 세균 감염 질환이다. 이전에 건강했던 소아에서 요로계의 폐쇄, 저혈압에 의한 신장 허혈, 신장 독성물질에의 노출 등이 없이는 급성 신우신염의 결과로 급성 신부전이 생기기는 매우 드물다. 저자들은 이전에 건강했던 소아에서 방광요관 역류가 동반된 급성 신우신염과 신장 농양에 따른 급성 신부전이 발생한 예를 보고하는 바이다. 환자는 적절한 수액 치료와 항생제 치료를 통해 호전되었다. 증례를 통해 저자들은 급성 신우신염이 급성 신부전의 감별진단에포함되어야 하며, 신장기능의 완전한 회복을 위해서는 적절한 항생제 치료가 수반되어야 함을 제시하는 바이다.

        IT 서비스의 안정적인 공급과 IT 서비스 품질 개선을 이루고자 하는 SLA (Service Level Agreement)에 대해 많은 연구와 시도가 지금까지 진행되어 왔다. SLA 시행에 있어서 측정 지표와 그 평가 기준 선정은 매우 중요하나, 대부분 SLA 측정 지표들은 기술관점의 항목들로 구성되어 있기 때문에, SLA 시행 결과가 사용자 중심이 아닌 서비스 제공자 중심으로 나타나고 있다. 이는 사용자의 불만족을 초래하고 서비스 제공자와의 의사 소통을 어렵게 만들고, 결국 SLA 도입 실패로 이어지는 원인이 되고 있다. 본 연구에서는 기존의 SLA 측정 지표들에서, 사용자 중심의 SLA 측정 지표로 활용할 수 있는 항목들을 BSC 의 인과관계모델을 통해 추출하고, AHP 기법을 통해 SLA 평가 기준들과 측정 지표들에 대한 가중치를 도출하였다. 이를 통하여 사용자 관점의 SLA 지표 선정 방법에 대한 명확한 기준을 제시하였다.

        병역법 제88조 제1항 위반에 따른 형사처벌 타당성에 관한 연구

        오늘날 대한민국에서는 다시금 양심적 병역거부자 처벌이 사회적 이슈로 부각되고 있다. 대법원과 헌법재판소는 종교적 신념을 이유로 입영을 거부한 입영대상자를 처벌하는 병역법 제88조 제1항에 관하여 합헌적 판단이 견고한 가운데 최근 지방법원을 중심으로 하급심에서 양심적 병역거부자에게 무죄를 선고한 판결이 2015년 6건, 2016년 7건 그리고 2017년 상반기에만 16건 등이 나오고 있는 것이다. 또한 종교적 양심에 따른 병역거부자의 처벌에 관하여 제주지방법원 내에서 유죄와 무죄의 판결이 엇갈리면서 논쟁은 더욱 가중되고 있는 실정이다. 이러한 논란가운데 양심적 병역거부자를 처벌하는 근거규정인 병역법 제88조 제1항에 관하여 대법원, 헌법재판소 그리고 최근 하급심의 판단을 살펴보고 특히 병역법 제88조 제1항의 정당한 사유의 인정범위에 양심적 병역거부가 포함될 수 있는지 그리고 자유권규약 제18조 해석 적용에 관하여 검토하고자 한다. In the Republic of Korea, punishment of conscientious objectors is rising as a social issue once again. Large number of news articles on the punishment of conscientious objectors are updated on portal sites several times a day. The background for this phenomena is based on the disagreement between the Supreme Court and the Constitutional Court. The Constitutional Court has ruled that it is constitutional for the government to punish conscientious objectors who denied their service for religious belief according to the Clause 1 of the Article 88 of the Military Service Law. However, district courts have taken different stance in the lower instance. 6 rulings in 2015, 7 rulings in 2016, and 16 rulings in the first half of 2017 has declared the conscientious objectors as not guilty. At the same time, the issue is becoming more controversial as the Jeju District Court has made two different rulings on the punishment of conscientious objectors who denied their military service for religious belief. Therefore, the study aims to find out whether conscientious objection can be included as a reasonable cause under the Clause 1 of the Article 88 of the Military Service Law, and take thorough review on the interpretation and the application of Article 18 of the Human Rights Covenant by examining the rulings at the Supreme Court, Constitutional Court, and District Courts.

        A Case of Atrial Septal Defect (ASD) in a Miniature Schunauzer Dog

        An atrial septal defect (ASD) is congenital heart disease with a communication between the atria, which allows blood to shoot from the atrium with pressure. A 3-month-old female Miniature Schunauzer was referred to the Veterinary Teaching Hospital of Konkuk University for the evaluation of systolic heart murmur. At presentation, the mucous membrane was cyanotic. On physical examination, an ejection-type systolic murmur was auscultated at the pulmonic area. In addition, thoracic radiography showed enlargement of main pulmonary artery and right atrial/ventricular enlargement. Echocardiography revealed dilated right atrium and atrial septal defect. However, mitral and tricuspid valve were still intact and well tolerating. The presence of an ASD was confirmed by identifying flow across the defect with color Doppler imaging. Doppler echocardiography provides a means of non-invasive documentation and quantification of ASD. Complete blood count and serum chemistry were not remarkable. Although large defect was confirmed between the two atrium, the patient did not show any obvious clinical signs of heart failure at this time.

        COVID-19로 인한 장애 아동의 가정운동 프로그램에 관한 부모들의 인식 조사

        Purpose: Since coronavirus disease 2019 (COVID-19) spread around the world, non-face-to-face services have emerged as the new normal. The functioning of children with developmental disabilities and cerebral palsy improves when parents, therapists, and institutions cooperate. We aimed to investigate the difficult environment of pediatric physical therapy caused by COVID-19, and grasp parents’ perceptions of home exercise programs. We intend to design home exercise programs for pediatric treatment in the future and create a non-face-to-face pediatric rehabilitation service system based on our findings. Methods: The first survey was conducted on 30 parents who had children with cerebral palsy and developmental disabilities. It was produced after consultation with relevant experts and discussion with the research team, based on a survey conducted in a previous study. The second survey was produced by revising and supplementing the opinions of parents who completed the first survey and consulting experts. Thereafter, an online survey was conducted on 118 parents who had children with cerebral palsy and developmental disabilities. Results: 61.01 % of the parents reported that they were anxious or afraid for their children to be treated for infectious diseases. 83.90 % of the parents reported that the role of parents had increased. 50.00 % of the parents complained of a lack of skill. 85.59 % of the parents reported that they needed a home exercise program provided by the organization. As a non-face-to-face exercise management method, 35.59 % of the parents wanted a real-time video recording electronically mailed to them, and 34.75 % wanted real-time education using video conferencing programs such as zoom. Conclusion: The threat of COVID-19 has further emphasized the importance of social cooperation and management, leading organizations to enter a new era of non-face-to-face rehabilitation services. It is necessary to collect and systematize related studies to reflect more diverse opinions and improve the perceptions of therapists and parents.

        Al 6061-T6 단조 성형품의 기계적 특성

        The effect of forging parameters, including different forging stock, strain rate and strain, on the mechanical properties of hot-forged Al 6061-T6 was investigated. The forging was conducted using either hydraulic press, crank press or hammer press, respectively, at a forging temperature of $400^{\circ}C$. When using an extruded bar as a forging stock, the tensile strength was lower for the specimens prepared by hammer forging than those by crank press forging. It was found that the coarsening of recrystallized grain was responsible for the decrease in tensile strength with hammer forging. Systematic studies on the effects of strain and strain rate on the tensile properties of hot-forged Al 6061-T6 products using extruded bar as a forging stock further suggested that the coarsening of recrystallized grains and$ Mg_2$Si precipitates complexed the observed trends in the tensile behavior. In case of hot forging with continuous cast bar as a forging stock, on the other hand, the mechanical properties of the specimen were largely improved with hammer press compared to those with crank press, which appeared to be due to the homogenization of microstructure.

        Derivation of a Verification Formula for the Dose Rate Contributing to the Maze Door of the 6 MV Treatment Room

        이 연구의 목적은 6 MV X선 빔의 미로 도어 설계 시 미로 도어 바깥 측정지점에서 계산되는 성분별 선량률의 정확성을 검증하는 식을 유도하는데 있다. NCRP 보고서 151과 IAEA 안전 보고서 시리즈 47에 기술된 미로 도어 바깥 측정지점에 대한 성분별 선량률 계산식 기반으로, 도면 기반 파라미터들의 값 적용시 성분별 선량률 계산식 및 보수적 파라미터들의 값 적용 시 성분별 선량률 계산식을 유도하였다. 각각의 성분에 대한 두 개의 선량률 계산식들로부터, 미로 도어 바깥 측정지점에서 성분별 선량률 검증식은 유도되었다. 결과로서 얻어진 미로 도어 바깥 측정지점에서 성분별 선량률 검증식은 설계자가 계산한 미로 도어 바깥 측정지점에서 성분별 선량률이 유도된 성분별 선량률 검증식에서 얻어진 선량률 범위 안에 포함 되는지 비교분석할 수 있다. 이 검증식은 설계자가 계산한 성분별 선량률의 정확성을 검증하는데 실무적으로 유용하게 사용될 수 있을 것으로 사료된다. The purpose of this study is to derive an equation to verify the accuracy of the dose rate for each component calculated at the measurement point outside the maze door when designing the maze door of 6 MV X-ray beam. Based on the component-specific dose rate calculation formula for the measurement point outside the maze door described in NCRP Report 151 and IAEA Safety Report Series 47, the dose rate calculation formula for each component when applying the values of the drawing-based parameters and the dose rate calculation formula for each component when applying the values of conservative parameters are derived. From the two dose rate calculation formulas for each component, the dose rate verification formula for each component at the measurement point outside the maze door was derived. The resulting dose rate verification formula for each component at the measurement point outside the maze door can be compared and analyzed whether the dose rate for each component at the measurement point outside the maze door calculated by the designer falls within the range of the dose rate obtained from the derived dose rate verification formula for each component. This verification formula is considered to be practically useful in verifying the accuracy of the dose rate for each component calculated by the designer.

        아산만 요각류 Calanus sinicus 의 알 생산

        Seasonality in biomass and egg production was investigated for Calanus sinicus, one of the major copepods in Asan Bay, Korea. Biomass of this species in this restricted embayment showed only one peak in spring, but egg production showed two peaks, spring (April and May) and fall (September). Average egg productions during the spawning seasons were 16.3 eggs/female/day (spring) and 7.6 eggs/female/day (fall) with maximum egg production of 39.0 eggs/female/day. Food concentrations in this bay measured in terms of particulate organic carbon (POC) were relatively high around the year, and correlation between egg production and POC was not significant. With the food provided sufficiently animals produced a lot of fecal pellets, but egg production was ceased after several days of experiment. From these results it was believed that egg production in this bay was influenced by food quality and feeding history rather than food concentrations. Habitat temperatures also seem to have influence on egg production from the facts that no egg was produced at extreme habitat temperatures, although the correlation between the two was not significant. In this bay, two generations with different time periods seem to progress yearly. Distribution was coupled with life cycle only at certain time of the year. During the other periods predation pressure by the higher trophic levels and shift of the centers of the distributions toward deeper outer bay seemed to be responsible for the observed distribution and egg production of this species in this bay. Comparing with the previous reports, migration of fairly long distance and continual reproduction seemed possible for this species.
