편측 부신절제와 반대측 부분절제로 완치된양측 기능성 부신피질 선종에 의한 쿠싱증후군 1예

양측 부신 선종에 의한 쿠싱증후군은 보고가 드물다. 또한 치료에 있어서 양측 부신 종양 각각의 기능성 여부를 확인하는 것이 중요하다. 저자들은 당뇨병, 고혈압 및 비만을 새로 진단 받은 환자에서 선택적 부신 정맥 채혈을 통해 양측 기능성 부신 선종을 진단하고 우측 부신절제술과 좌측 부분절제술 후 스테로이드를 복약하지 않고 체중 감량과 함께 당뇨병, 고혈압이 완치된 예를 경험하였기에 이를 보고하는 바이다.

서론: 급성 신우 신염(acute pyelonephritis)이 의심되는 환자는 내원 시 임상 양상의 중증도와 환자의 기저 질환 및 항생제 치료 반응에 따라 초음파나 컴퓨터 단층 촬영을 통해 신농양이나 급성 세균성 신엽염이 합병되었는지 확인할 필요가 있다. 그러나 영상 검사를 시행하여야 하는 기준이 객관적이지 않아 치료 초기에 신농양과 같은 합병증을 발견하지 못하여 효과적인 치료를 지연 시키거나 반대로 영상 검사를 무분별하게 시행하여 환자가 불필요한 조영제와 방사선에 노출되는 경우가 흔하다. 급성 신우신염은 매우 흔한 감염 질환임에도 구체적인 영상 검사의 적응증을 세우기 위한 연구는 드물어 저자들은 지역 사회 획득 급성 신우신염 환자에서 신엽염과 신농양을 예측할 수 있는 위험 인자를 후향적인 연구를 통하여 알아보고자 하였다. 방법: 2004년 12월부터 2012년 12월까지 지역사회 획득성 급성 신우신염으로 진단된 환자 중 신장 초음파 또는 전산화 단층 촬영을 시행한 환자의 임상 자료를 수집하여 분석하였고 환자들의 영상 자료를 본원 영상의학과에서 1명의 전문의가 다시 검토를 하였다. 신실질이 정상이거나 단순 신비대 (swelling), 1개의 쐐기형 저음영 병변이 있는 경우는 group 1, 2개 이상의 쐐기형 저음영이 있는 경우는 group 2, 신농양이 형성 된 경우는 group 3으로 분류하였다. 결과: 지역 사회 획득성 급성 신우신염으로 진단된 환자 중 초음파 또는 전산화 단층 촬영을 실시한 환자는 총 157명이었다. 대상 환자들의 연령은 48.83±17.4세(평균±표준 편차)였고 여성이 89.8% (141명)이었으며 group 1, 2, 3에 속하는 환자는 각각 77, 61, 19명이었다. 원인균이 분리된 환자가 86명이었고 이중 E. coli가 76명으로 가장 많았으며 혈류 감염(bacteremia)은 33명이었다. 영상 검사에서 중증도가 심할수록 39도 이상의 발열(p=0.002)과 농뇨(p=0.017)가 통계적으로 유의하게 많았고 말초 혈액 검사에서는 백혈구(p=0.014), alanine aminotransferase (p=0.015), alkaline phosphatase (p=0.012), C-reactive protein (p<0.001)의 평균치가 통계적으로 유의하게 높은 경향을 보였다. 결론: 본 연구에서는 39도 이상의 고열과 농뇨 및 말초 혈액에서의 백혈구, alanine aminotransferase, alkaline phosphatase, C-reactive protein의 상승 정도가 영상 검사에서의 중증도와 상관관계가 있었다. 그러나 기존의 연구에서 공통적으로 중증도와 관련이 있다고 알려진 인자인 당뇨는 본 연구에서는 통계적으로 유의한 관련이 없었다. 이에 대해서는 HbA1c를 이용한 추가적인 분석이 요구된다.

에어로빅댄스가 중년여성의 사상체질별 신체변화 및 체력수준에 미치는 영향

The purpose of this study is to investigate the effect of an aerobic dance on body changes and physical fitness level in middle-aged women according to Sasang Constitution classification. Subjects(n=39) did the aerobic dance 1 hour/day, 3 days/week, for 12 weeks, with graded exercise intensity of 50-80 %HRmax. For the data analysis, we used SPSS 18.0, especially paired t-test for the comparison of body changes with physical fitness level and one-way ANOVA for the difference among groups with Scheffe post hoc method, with significance level of .05. As the results, weight, BMI(Body Mass Index), obesity, and waist hip ratio showed the falling tendency in the order of Taeumin, Soeumin, and Soyangin. There were the rising tendency for the muscle strength and muscle endurance in the order of Taeumin, Soeumin, and Soyangin, for the flexibility in the order of Taeumin, Soyangin, and Soeumin, for the cardiopulmonary endurance in the order of Soyangin, Taeumin, and Soeumin. In summary, we found that the aerobic dance positively influence body changes and physical fitness improvement. And in further studies, multilateral approaches, including a variety of the exercise intensity and duration, will be necessary.

노인 여성의 운동참여에 따른 기초체력의 비교

The purpose of this study was conducted to compare the basic physical fitness according to the exercise participation of elderly women. The subjects of this study have participated 50 elderly women who regularly participated in exercise and 50 elderly women who irregularly participated in exercise. The participants measured the Senior Fitness Test. And their results were compared. For data processing, the mean and standard deviation were calculated using the SPSS 26.0 program, an independent t-test between groups was performed, and the significance level was set to .05. The results are as follows: First, there was a significant difference in lower extremity muscle function between the exercise group and the control group, and the exercise group showing superiority(p<.001). Second, there was no significant difference in lower extremity flexibility between the exercise group and the control group(p>.05). Third, there was a significant difference in balance between the exercise group and the control group, and the exercise group showing superiority(p<.001). Fourth, there was a significant difference in cardiorespiratory endurance between the exercise group and the control group, and the exercise group showing superiority(p<.001). This study is expected to serve as a basis for the direction of exercise suitable for the elderly.

신자유주의 시대의 영웅, 부르주아의 형상과 윤리 -2000년대 자기계발서와 최인호의 『상도』를 중심으로

본고는 2000년대 대중의 감정 구조를 베스트셀러와 대중서사물에서 표상된 ‘인간형’을 통해 살펴보고, 신자유주의 시대 대표적인 ‘영웅’ 형상인 부르주아 윤리에 대해 고찰했다. 2000년대 베스트셀러 목록을 차지하는 다수의 재테크 판타지와 자기계발서는 돈에 대한 전통적인 인식과 가치관의 변화, 경제주체로의 변모라는 대중의 감정구조를 보여준다. 호모에코노미쿠스의 대중적 판타지는 ‘속물과 동물’ ‘생존자’에 머물지 않고 ‘영웅’으로서의 기업가와 부자의 형상을 탄생시킨다. 이들은 김우중의 『세계는 넓고 할 일은 많다』와 이명박의 『신화는 없다』와 같은 재벌 기업가 에세이 및 자서전을 통해 화려하게 등장하고 대중의 인기를 얻는다. 이 책들에 의해 기업가는 꿈과 야망, 불굴의 의지와 사명감으로 세상 혹은 운명에 맞서 싸워나가 공동체를 구한다는 지도자와 국가 영웅의 이미지로 거듭난다. 이들 신화는 재벌-국가 동맹의 자본주의, 독점 자본주의, 노동착취, 여성차별 등의 곤혹스러운 현실을 삭제하고 있으나, 이 문제적 지점과 무관하게 과거 통치자, 장군, 학자, 혁명가 그리고 식민시대의 독립투사, 계몽운동가 등의 위인의 목록에 이어 ‘자본가’를 근대적 영웅의 반열에 올려놓는 데 성공한다. 조선의 인삼무역왕 임상옥의 일대기를 다루고 있는 최인호의 『상도』는 2000년대 전후의 대중의 욕망을 재빠르게 읽어내어 새로운 유형의 영웅서사를 탄생시킨다. 이 소설에서 임상옥은 평민이라는 신분적 한계를 극복하고 부자와 군수가 되는 특출한 개인으로, 그의 무역활동은 마치 난세의 영웅 활약상처럼 그려진다. 『상도』를 통해 작가 최인호가 제시한 ‘경제의 신철학’이란 가포 임상옥이 남겼다는 “재상평여수(財上平如水) 인중직사형(人中直似衡)”으로 요약된다. ‘재의 자유로운 흐름과 올바른 상도’를 의미하는 이 경제 신철학은 곧 한국식 자본주의 정신으로 확대 해석할 수 있다. 재(財)의 자유로운 흐름은 곧 근대민주주의 평등 사상을, ‘사람 사이의 저울과 같은 바름’은 전통적인 유교 윤리라고 할 수 있는데 이 둘의 결합을 뜻하는 ‘상도’는 비록 그 현실성이라는 한계가 있으나 한국식 자본주의의 정신을 제시하고 부자의 윤리의 필요성을 대중들에게 설파하고 있다는 점에서 의의가 있다. This thesis studied the characters and the structure of feelings represented through the 2000s bestseller and the bourgeois ethics, the representative hero of the neoliberalism. Many of the financial technology and self-improvement books that make up the bestseller lists in the 2000s show the structure of feelings and traditional perceptions and changes in values about money. The popular fantasy of Homoeconomycus did not stay in the ‘slanders and animals’ and ‘survivors’ but created figures of entrepreneurs and rich people as ‘heroes’. They appear splendidly by entrepreneurs’ essays and autobiography, such as Kim Woo-chang’s ‘The World is Wide and There is a Lot to Do’ and Lee Myung-bak’s ‘There is No Myth’ and gains popularity from the public. These books succeed in placing ‘capitalist’ in the ranks of modern heroes following the list of great figures from ruler, general, scholar, revolutionist, resistance fighter and enlightenment activist. Choi In-ho's ‘Sangdo’, which deals with the life of Im Sang-ok, the king of ginseng trade in Joseon, quickly read the popular desire around the 2000s and created a new type of heroic epic. In the novel, Im Sang-ok is an outstanding individual who overcomes the status limits and becomes a rich man and a governor, and his trade activities are portrayed as a hero's performance. It is also summarized as "Wealth is equal to water, and human beings are like scales" which was presented by writer Choi In-ho as ‘New Philosophy of Economy’. This new economic philosophy, which means ‘free flow of wealth and the commercial ethics’ can be interpreted as a Korean-style capitalist spirit. The ‘free flow of wealth’ is the idea of equality in modern democracy, and the ‘rightness like a scale between people’ is a traditional Confucian ethic. Although ‘Sangdo’ has limitations, it presents a Korean-style capitalist spirit and preaches the rich ethics to the public.

Optimization of shoot cultures and bioactive compound accumulation in Rosa rugosa during acclimatization

Rosa rugosa is a medicinal, ornamental, and edible plant native to Eastern Asian countries, including Korea, Japan, and China. The aim of this study was to establish a system for biomass production and secondary metabolite accumulation during in vitro culture and acclimatization of Rosa rugosa. The highest rate of multiple shoot proliferation was achieved with 8.8 μM benzyladenine (BA) (83.3%). However, the number of shoots (14.4 per explant) at 4.4 μM BA was higher than that at 8.8 μM BA. Compared to BA, a combination of thidiazuron (TDZ) and indole butyric acid (IBA) exhibited significantly lower shoot induction, with only 50.0 ~ 79.2% and 4.2 ~ 16.7% relative shoot formation, respectively. During acclimatization, shoots were sampled every week and their total phenolic contents were analyzed. Among various growth factors, fresh weight showed the most dramatic increase from the 3rd week (88.0 mg/plant) to 4th week (132.7 mg/plant). Total phenolics and flavonoids contents were the highest at 1 st week of acclimatization. Depending on developmental stages, total phenolics and flavonoids contents were higher in 1-yr-old shoots grown ex vitro than in those of older field-grown or in vitro-grown plants. Amongst different ages of field grown plants, 6-year-old plants, the oldest in this study, showed the lowest content in total phenolics.
