      • 인후두부 이상감을 호소하는 환자와 갑상선 병변과의 임상적 연관성

        Background and Objectives : Globus pharyngeus is the feeling of a lump or foreign body in the throat. Otolaryngologists are often confronted with this condition, and some author reported that it represents 3 percent of all new clinic referral. The purpose of this study is to estimate the incidence of impalpable thyroid lesion in patients with globes pharyngeus and investigate possible association between globus pharyngeus and thyroid lesions. Material and Method : 146 patients with globus pharyngeus and 30 controls were prospectively examined by high resolution ultrasound. Results: Thyroid abnormalities were present in $75/146(51\%)$ patients with globus pharyngeus and in $7/30(23\%)$ controls. Of the patients with globus, 55 patients had a nodule or nodules, 11 patients had a cyst or cysts, 9 patients had diffuse abnormalities. Solitary nodule ranged from 4mm to 20mm. Conclusion : Impalpable, ultrasound detectable thyroid abnormailities are significantly more common in patients with globus pharyngeus than in controls. The thyroid abnormalities may be the cause of globus symptoms in some patients, and high resolution ultrasound is an important modality in patients presenting with globus.

        구리, 니켈, 코발트 혼합용액으로부터 침전법에 의한 구리의 분리

        구리, 니켈 그리고 코발트를 함유한 용액으로부터 구리를 선택적으로 분리하기 위하여 $Na_{2}S$를 첨가하여 구리를 CuS로 침전시키는 연구를 행하였다. 구리의 침전율은 용액의 pH가 높을수록, 첨가한 $Na_{2}S$의 양이 많을수록,농도가 낮을수록, 반응온도와 반응시간이 증가할수록 증가하였다. pH 1, $20\%\;Na_{2}S$ 첨가량이 구리의 3 당량비, 온도 $25^{\circ}C$ 그리고 반응시간 30분에서 구리의 침전율은 $94.1\%$이었으며 니켈과 코발트도 각각 $4.3\%$와 $4.5\%$가 침전되었다. 즉 일부 니켈과 코발트 이온이 같이 공침되기 때문에 정량적으로 구리만을 침전, 분리하는 것은 가능하지 않았다. The selective sulphide precipitation of copper from sulphate solution containing nickel and cobalt was studied with adding $Na_{2}S$ solution. Precipitation efficiency of copper increased with raising pH of solution and increasing the amount of $Na_{2}S$ added and lowing its concentration. The increase in reaction time and temperature also improved the precipitation of copper. However, attempts to selectively precipitate copper met with limited success because of co-precipitation of nickel and cobalt. With adding $20\%$ $Na_{2}S$, 3 times equivalent of Cu, at pH 1.0 of solution, $25^{\circ}C$ and 30 minutes of reaction time, precipitation efficiencies of copper, nickel and cobalt were $94.1\%$, $4.3\%$ and $4.5\%$ respectively.

        대장선암의 치료에 있어 수술후 방사선 치료의 역할

        대장선암의 근본적인 치료는 외과적 절제술로 알려져 있으나 이 경우 직장선암과 마찬가지고 수술 후 국소재발이 문제시되어 왔다. 직장선암의 경우 수술후 국소재발의 위험성이 높은 환자에 수술후 방사선 치료를 병용함으로써 국소제어율 및 나아가서 생존율까지도 상당히 개선된다고 알려져 있다. 그러나 대장선암의 경우 국소제어율 및 생존율의 개선이 보고되고 있기는 하나 아직 알려져 있지는 못하다. 이에 저자는 1970년 3월부터 1984년 12월까지 대장선암으로 진단받고 일차적으로 근치적 수술을 시행한 후 연세대학교 의과대학 치료방사선과에 내원한 21명의 환자를 후향성 분석하여 다음과 같은 결과를 얻었다. 1. 21명의 환자 중 수술후 방사선 치료를 병용하여 계획대로 무사히 마친 환자는 11명이었다. 2. 수술후 방사선 치료를 병용한 환자들의 전체적인 국소실패율은 $9\%(1/11)$이었고 병기별로는 B2+B3가 (0/4), Cl+C2+C3가 $14\%$ (1/7)였고 Cl이 0 (0/2) B2+C2가 $17\%$ (1/6), B3+C3가 0(O/3)였다. 3. 수술후 방사선 치료를 병용한 환자들의 5년 생존율은 $55\%$였다. From March 1970 to December 1984, treatment results of 21 patients treated initially with curative surgery for adenoarcinoma of the colon and referred to the Department of Radiation Oncology, College of Medicine, Yonsei University, were analyzed retrospectively. Thirteen of 21 patients who were considered to be at high risk (i.e, stage B2 or above), received postoperative adjuvant radiation therapy. However,2 of 13 patients did not complete their courses of radiotherapy as planned because of poor tolerance to radiotherapy or patient's refusal and were excluded from this study. Remaining 8 of 21 patients who did not receive postoperative radiotherapy, presented with recurrence at the time of referral and treated for palliation. In 11 patients who finished postoperative radiotherapy, overall local failure rate was $9\% (1/11)$ and the 5 year actuarial suwival rate was $55\%$. Local failure rates by stage were 0 (0/4), $14\%$ (1/7) for stage B2+B3, Cl+C2+C3 respectively and 0(0/2), $17\%$ (1/6), 0(0/3) for stage C1, B2+C2, B3+C3 respectively.

        고정형(固定形) Bed와 컬럼을 이용(利用)한 망간 단괴(團塊)에 의한 폐수(廢水) 중의 구리이온 제법(除去)

        망간단괴는 높은 비표면적과 주 구성성분이 ${\delta}$-MnO$_2$ 이기 때문에 폐수 중의 중금속 흡착제로서 적합하다. 본 연구에서는 구리 함유 폐수를 이용하여 고정형 컬럼과 bed형 흡착장치를 사용하여 현장에서 망간단괴의 중금속 흡착제로서 사용 가능성에 대하여 검토하였다. l kg의 망간단괴 (입자크기 1-3 cm)를 사용하여 구리이온을 0.97 g/L 함유한 폐수를 3시간 처리한 결과 고정형 칼럼과 고정형 bed에서 각각 4.0g과 2.3g의 구리가 흡착, 제거되었다. The typical properties of manganese nodules are its high porosity and high specific surface area and manganese in nodules is existed as ${\delta}$-MnO$_2$. These properties suggest that manganese nodules ran be used as an adsorbent for heavy metal ions. This study investigated the practical applicability for the removal of copper ions in the waste water by manganese nodules as an adsorbent using fixed column and fix bed systems. Manganese nodules of 1kg (size 1-3 cm) can absorb 4.0g Cu in fixed column system and 2.3g Cu in fixed bed system from waste water for 3 hours respectively.

        황(黃) 원료(原料)로서 pyrite와 chalcopyrite를 사용(使用)한 matte 상(相)의 제조(製造) 및 침출특성(浸出特性)

        Artificial mattes were prepared with adding pyrite or chalcopyrite as sulfur sources with Cu-Ni-Co-Fe alloy. The major phases identified by X-ray diffraction pattern were $(FeSi)_9S_8$, $CuFeS_2$, FeS, $Co_4S_3$, $Ni_3S_2$ and $Cu_2S$ for both mattes, and the matte prepared by adding chalcopyrite showed the higher peak of $Cu_2S$ due to high content of copper. Under optimum conditions, more than 95% copper, 90% nickel and 90% cobalt were extracted into leaching solution and sulfur concentration in the mattes did not much affect the leaching efficiency of the metals. The increase of the amount of pyrite or chalcopyrite added decreased pH in leaching solution and increased the concentration of iron ion dissolved in the leaching solution and the amount of residue. Cu-Ni-Co-Fe 함금상에 황의 원료로서 pyrite 또는 chalcopyrite를 첨가하여 매트상을 제조하는 방법에 대하여 검토하였다. 두 매트상 모두 주성분은 $(FeSi)_9S_8$, $CuFeS_2$, FeS, $Co_4S_3$, $Ni_3S_2$ 그리고 $Cu_2S$의 금속황화물 이었으며, chalcopyrite를 첨가한 매트의 경우 구리의 함량이 높아 $Cu_2S$의 피크가 더 높게 나타났다. 제조한 매트상의 침출 결과 모두 구리는 95%이상, 니켈과 코발트는 90% 이상의 침출율을 보였다. Pyrite와 chalcopyrite 첨가량이 증가할수록 침출액의 pH는 낮아지고, 용해되어 있는 철이온의 농도와 잔사량은 증가하였다. 한편 매트 중 황의 함량변화에 따른 유가금속의 침출율은 모든 시료에 대해 크게 변하지 않았으며, 다만 pyrite를 이용하여 제조한 매트의 경우에 Ni과 Co가 황 함량 증가에 따라 다소 증가하는 경향을 나타내었다.

        산 침출 시 실리카 광물의 용해 및 제거

        산 침출에 있어서 생성되는 가용성 실리카는 고액분리 시 여과를 어렵게 하고 목적금속의 순도를 저하시키는 등 습식제련공정에 있어서 큰 문제점으로 대두되고 있다. 따라서 본 고에서는 실리케이트 광물과 산과의 반응성, 가용성 실리카의 특성, 제거방법에 대하여 검토하였다. 가용성 실리카는 알카리 전처리에 의한 제거, 결정상태의 $SiO_2$로 변환, 응집 등의 방법을 통한 여과성 향상 등의 방법으로 처리할 수 있다. Soluble silica generated from acid leaching process is very difficult to filter and deceases the purity of products, and thus becomes one of hot issues in hydrometallurgy. This paper reviewed the dissolution and reactivities of silicates in acid solution, and the methods for treatment of soluble silica. Removal of silica with alkaline pre-treatment, crystallization to $SiO_2$ and precipitation behaviour of silica with coagulation under acid conditions were briefly described.

      • 과거민감도 스펙트럼을 포괄하는 공정 스케줄링 모델

        기존의 공정 스케줄링 방법들 중 GPS(generalized processor sharing)는 순간적 관점에서의 공정성을 추구하는 반면에, virtual clock은 장기적 관점에서의 공정성을 추구하는 특성을 지닌다. 이 논문에서는 이들의 차이가 과거의 서비스 정보를 추후의 스케줄링에 반영하는 정도에 있음에 주목하고, GPS와 virtual clock을 포괄하는 스펙트럼 형태의 스케줄링 모델을 제시한다. 이 모델에서 각 응용의 자원 획득 권한은 예치권한이라는 값으로 표현되는데, 예치권한은 각 응용별로 미리 정해진 고유한 비율로 계속 증가하며, 서비스를 받으면 소비된다. 소비되지 않고 누적된 예치권한은 과거에 서비스가 이루어지지 않은 정도를 표현하는 값이라고 볼 수 있으며, 이는 응용의 스케줄링 가능성을 높이므로 이후의 서비스 지연시간을 상대적으로 단축하는 효과를 낸다. 예치권한을 주기적으로 감쇄시키면 과거 정보의 반영 정도를 줄일 수 있으며, 이 때 그 감쇄 정도는 과거행태를 반영하는 정도를 의미한다. 과거의 정보를 전혀 반영하지 않을 경우 GPS의 특성을 나타내게 되며, 모두 반영할 경우 virtual clock의 특성을 보이게 된다. 이러한 스펙트럼 상에서는 평균지연시간과 장기적 공정성 사이에 절충 관계가 존재한다. 이 논문에서는 제시된 모델의 특성을 분석하고 실험을 통해 검증한다. GPS(generalized processor sharing) is a fair scheduling scheme that guarantees fair distribution of resources in an instantaneous manner, while virtual clock pursues fairness in the sense of long-term. In this paper, we notice that the degree of memorylessness is the key difference of the two schemes, and propose a unified scheduling model that covers the whole spectrum of history-sensitiveness. In this model, each application's resource right is represented in a value called deposit, which is accumulated at a predefined rate and is consumed for services. The unused deposit, representing non-usage history, gives the application more opportunity to be scheduled, hence relatively enhancing its response time. Decay of the deposit means partial erase of the history and, by adjusting the decaying rate, the degree of history-sensitiveness is controlled. In the spectrum, the memoryless end corresponds GPS and the other end with full history corresponds virtual clock. And there exists a tradeoff between average delay and long-term fairness. We examine the properties of the model by analysis and simulation.

        이프리플라본 서방정 제조 및 약동학적 평가

        Ipriflavone is non-hormonal antiosteoporotic drug which inhibits bone resorption by reducing recruitment and/or differentiation of osteoclasts, and stimulates proliferation and differentiation of osteoblast, and also enhances calcitonin secretion in the presence of estrogen. Although some kinds of immediately-released preparation of ipriflavone are available in commercial market, in present study, we tried to formulate sustained-release tablet using coating method with hydrophobic and hydrophilic coating materials. In vitro dissolution test was applied to evaluate sustained-release patterns of several test preparations (Test tablet A, B and C) designed using different preparation method or different compositions. From the results of dissolution test, test tablet A which showed suitable dissolution profile was selected as the candidate of new product. Pharmacokinetic evaluation of test drug, ipriflavone sustained-release tablets, was conducted in 6 beagle dogs weighing $11.5{\pm}0.5\;Kg$ compared with $Theobon^{\circledR}$ tablet, immediately-released tablet (Kukjae Pharm. Co.) as reference drug. Two products were randomly administered to 6 beagle dogs, and after 1 week, cross-over study was conducted. From the present study, AUC and $T_{max}$ of test product were significantly different from those of reference product (p<0.05), respectively$(AUC\;:\;3646.28{\pm}472.56\;vs\;3646.28{\pm}472.56\;ng{\cdot}hr/ml,\;T_{max}\;:\;4.33{\pm}1.03\;vs\;1.42{\pm}0.38\;hr)$. But $C_{max}$ was not significantly different between two products (p>0.05) $(\;512.52{\pm}48.18\;vs\;443.97{\pm}140.53\;ng/ml)$. From the results of pharmacokinetic evaluations, it was noted that absorption amount of test product was increased, but absorption rate was delayed and $C_{max}$ of two products were not changed. And it was concluded that redesign of the sustained-release preparation which has a lower content of iprifavone rather than test tablet A must be considered.

        정신분열증 환자에서의 Haloperidol 및 Reduced Haloperidol의 혈장농도와 임상반응과의 상관성

        The relationship between the plasma haloperidol (HP) concentration and clinical response, and the effects of its active metabolite, reduced haloperidol (RH) on clinical response of HP were investigated in schizophrenic patients. In clinical study I, with 17 schizophrenic patients (male 8, fermale 9) who were administered with three different fixed doses of HP (15, 30 and 50 mg/day) for 3 weeks, the concentrations of HP and RH in plasma and blood and clinical response had been checked before and every week during the study. The clinical response was evaluated by the method of brief psychiatric rating scale (BPRS), and relative improvement of clinical response based on baseline BPRS (before drug treatment) was calculated. The concentrations of HP and RH in plasma and blood were assayed by HPLC. In clinical study II, the plasma RH/HP concentration ratios were checked in 11 patients who were administered with high doses of HP, over 60 mg a day, because of the poor clinical response at usual doses of HP. Plasma HP concentration and relative improvement of BPRS at 3 week in schizophrenic patients showed a 'curvilinear' relationship, and the clinical response was improved relatively over 50% based on the baseline BPRS in the range of $5{\sim}57\;ng/ml $ of HP in plasma. Also, the plasma RH concentrations were increased nonlinearly as the plasma HP concentration increased, and in high plasma HP concentration, over 30 ng/mI, clinical response gradually decreased, while the plasma RH/HP concentration ratio increased nonlinearly. Blood partition coefficients of HP and RH were not changed according to daily HP dose and duration of drug therapy. From these results, it is noted that the higher plasma RH/HP concentration ratio, resulted from the accumulation of RH as HP concentration increased, might explain the 'curvilinear' decrease of HP clinical response.
