Acute fulminant invasive fungal sinusitis in an immunocompromised host and bacterial rhinosinusitis with intracranial or orbital extension is challenging to manage. And it sometimes constitutes true otolaryngologic emergencies. In the absence of rapid diagnosis and treatment, these diseases can be fatal. A 57-year-old female was admitted for chills and headache, who received a deceased donor renal transplantation 3 months ago. Paranasal sinus CT showed enhanced soft tissue density and MRI showed low-signal with hyperintense signal of around paranasal sinus cavity. The histological investigation revealed invasive aspergillosis of paranasal sinuses. Clinical improvement occurred after endoscopic sinus surgery and post-operative systemic antifungal therapy with amphotericin B and voriconazole.

국가 주요 기반시설을 생애주기 관점에서 관리하고 소요재원의 확보근거를 마련하고자「지속가능한 기반시설 관리 기본법」이 2020년 시행되고 있으며, 관련법에 의거하여 15종의 인프라 기반시설에 대한 인프라 총조사사업(국토안전관리원)이 수행중에 있다. 본 연구에서는 기반시설중 하천시설에 대한 특성, 데이터 구축의 한계를 조사후 정보관리 표준화 방법 및 구축된 데이터에 대한 활용방안을 제안하고자 한다.

An analytic methodology was developed for free surface motions between liquid metal coolant and cover gas in order to calculate the phenomena of gas entrainment in hot pool surface through IHX EMF and reactor core. The methodology was setup by applying the first order VOF convection model to CFX4 general purpose fluid dynamics analysis code. The methodology was validated by applying it to an experimental apparatus designed for free surface motions of KALIMER reactor. The distributions of free surface calculated by the present methodology were almost coincident with the experimental data. The developed methodology was applied to the KALIMER reactor of full power operating condition. The shapes of the free surface were nearly uniform. From the results, it was found that the altitude of the free surface from the IHX inlet nozzle of KALIMER reactor is high enough not to affect to free surface motions of generating gas bubbles from the turbulent shear flows such as hydraulic jump and water falls.

This paper presents a design strategy of Concentrated Flux-type Synchronous machine(CFSM) with ferrite permanent magnets (PM) as a substitute for the rare-earth PM. The design strategy is to apply eccentricity or chamfer to the rotor. Applying eccentricity or chamfer to the rotor decreases the back electromotive force but reduce the torque ripple. Eccentricity or chamfer is applied to the same rotor area to compare the torque ripple reduction effect. The back electromotive force and torque of CFSM with eccentricity or chamfer are calculated by 2-D finite element analysis. As a result, the optimum shape model with eccentricity reduces the torque ripple by 8% compared to the chamfered model.
지속적 신대치요법 (Continuous Renal Replacement Therapy)으로 치료받은 급성 신부전증 환자들의 예후 인자

Purpose: Continuous renal replacement therapy (CRRT) has been used widely for treating critically ill patients with acute renal failure (ARF). We performed this study to identify predictors of mortality in critically ill ARF patients treated with CRRT. Methods: We analyzed the data of 128 patients who were treated with continuous veno-venous he-mofiltration (CVVH) or continuous veno-venous hemodiafiltration (CVVHDF) from May, 2002 to March, 2008. We compared the clinical data of survivors with non-survivors. Results: On univariate analyses of prognostic factors of patients treated with CVVHDF, APACHE II scores (p=0.004), prothrombin time (INR) (p=0.033) and the number of inotropics used (p=0.005) were significantly lower in survivors than those of non-survivors. MAP (p=0.027), diastolic BP (p=0.015) and fibrinogen level (p=0.007) were significantly higher in survivors than those of non-survivors. Multivariate analysis revealed that APACHE II scores and fibrinogen level were the independent factors for the prediction of mortality. And on univariate analyses of prognostic factors of patients treated with CVVH, APACHE II scores (p=0.002) and the number of inotropics used (p=0.006) were significantly lower in survivors than in non-survivors. MAP (p=0.03), systolic BP (p=0.02) and diastolic BP (p=0.03) were significantly higher in survivors than in non-survivors. Multivariate analysis also revealed that APACHE II scores was the only independent factor for the prediction of mortality. Conclusion: This study showed that the independent prognostic factor for mortality in ARF patients treated with CRRT was the APACHE II score.

염증성 장 질환에 일반적으로 사용되는 설파살라진은 고용량 및 장기 섭취 후 다양한 부작용이 있다. 본 연구에서는 TNBS로 유발된 마우스 대장염 모델에서 설파살라진, 육계와 시호 복합추출물의 항염증 및 세포 자멸 개선효과를 확인하고자 하였다. 실험은 정상군, TNBS 대조군, Sulfasalazine (30 mg/kg)군, Sulfasalazine (60 mg/kg)군, Sulfasalazine (30 mg/kg)+육계 및 시호 혼합 (30 mg/kg)군, 총 5개의 군으로 나누었으며, 7일간 경구투여 하였다. 염증 및 세포 자멸 단백질은 western blot을 통해 발현량을 확인하였다. SCB 투여는 염증 단백질 및 세포 자멸과 관련된 단백질의 억제에 유의한 효과를 나타냈다. 이러한 결과로 보아 설파살라진, 육계와 시호 복합 추출물은 염증과 세포 자멸의 억제를 통해 염증성 장 질환을 개선시킬 수 있으며, 염증성 장 질환의 치료 대안 가능 물질로 사료되는 바이다. Inflammatory bowel disease (IBD) is including Crohn`s disease and ulcerative colitis. Sulfasalazine commonly used in IBD, possibly has various side effects after high dosage and long term intake. The present study aimed to investigate the sulfasalazine and combination with herbal medicine on 2,4,6- trinitrobenzene sulfonic acid (TNBS)-induced in mice model. TNBS-induced mice were injected through a flexible catheter 4 cm in length 1.6 mg TNBS. Animals were divided into five groups (n=12): Normal group, TNBS control group, Sulfasalazine (30 mg/kg) group, Sulfasalazine (60 mg/kg) group, Sulfasalazine (30 mg/kg)+Cinnamomi cortex and Bupleuri radix mixture (30 mg/kg) (SCB) group. Administration groups were fed extract during 7 days. The inflammatory, and apoptotic protein levels were determined using western blotting. SCB treatment showed an outstanding effectiveness in counteracting the IBD, as assessed by reduction of body weight loss, down-regulation of proinflammatory proteins and cytokines, and by inhibition of proteins related to apoptosis. This is the first report that sulfasalazine and Cinnamomi cortex plus Bupleuri radix mixture improve the severity of experimental IBD through the inhibition of both inflammation and apoptosis. We confirm that the SCB treatment instead of sulfasalazine alone may be promising as an alternative therapeutic plan against IBD, without any evidence of adverse effects.

In order to improve the mileage of a hybrid vehicle, it is necessary to increase the power density and efficiency of the traction motor. Therefore, the traction motor requires a high fill factor. When the rectangular wire is applied to the motor, the fill factor can be increased, so that it has a high power density. However, in the high-speed region, the AC resistance increases due to the effect of the eddy currents in the conductor, and thus the loss increases. Therefore, when designing a motor using a rectangular wire, the effect of AC resistance should be considered. In this paper, AC resistance according to several shapes and arrangements of rectangular wire is compared. The AC resistance is calculated through the 2-dimensional finite element analysis (2-D FEA) at the max-speed operation point, and the current density distribution of rectangular wire is compared. For each model according to the shape and placement of rectangular wire, the ratio of DC resistance to AC resistance is calculated. Finally, the loss and efficiency of each model are compared and analyzed.

Recently, because of the instability of rare-earth resource supply and demand, research on rare-earth free in various fields is urgent. Accordingly, studies are being actively conducted to apply ferrite permanent magnet to IPMSM (Interior Permanent Magnet Synchronous Motor) used as electrical vehicle (EV) traction motors. Ferrite permanent magnet is cheap and easy to obtain. However, it has lower coercive force values than rare earth permanent magnets, and there is a large armature reaction occurs during EV motor operation, it is necessary to check for irreversible demagnetization. In addition, the limited space of the vehicle requires a high power density of the motor. In order to satisfy the high power density, the motor must be speeded up. Therefore, the problem of mechanical stiffness due to the generation of very large centrifugal force must also be considered. In this paper, the irreversible demagnetization characteristics and mechanical stiffness of IPMSM with ferrite permanent magnet. When determining the rotor shape, the sensitivity of each performance is analyzed through of variance of design factors. Sensitivity analysis is based on the maximum stress of the rotor and irreversible demagnetization characteristics. P-value less than 0.05 can be used as major design factors in motor design by determining sensitivity factors for each performance.

This paper presents the optimal design of a 15 kW permanent magnet synchronous motor for personal mobility using non-rare earth permanent magnets (PMs) as an alternative to neodymium PMs. The characteristics of neodymium permanent magnet motors are compared with those when replaced with ferrite permanent magnets, and methods for improvement are suggested. The electromagnetic performance according to the rotor outer diameter, the shape of the PM, and the number of layers of the PM are compared, and the initial design is carried out considering the irreversible reduction characteristics and mechanical stability. Considering that it is for personal mobility, the efficiency of the main operating points for urban driving is considered, and two-step optimization is performed to reduce the number of experimental points. Based on the optimal design results of this paper, the suitability of synchronous motors using rare earth permanent magnets for personal mobility is suggested.
