        해면상 혈관종을 동반한 경수부 신경초종 : 1례 보고 Report of One Case

        We report a rare case of cervical intradural extramedullary neurilemmoma associated with extradural cavernous angioma in adolescent male patient. This 19-year-old man complained of increasing weakness of left extremities with pain and numbness, starting upwards from the region of left leg since 2 months before, which has aggravated gradually and developed to right side. Neither the patient nor any family member displayed the stigmata of spinal tumor. Neurological examination showed spastic qudriparesis, more severe on left side, disclosed absent vibratory sense and diminution of pain and touch bleow level of cervico-thoracic junction. Myelogram demonstrated a large filling defect at the C4 -6 vertebral level, which was interpreted as an intradural extramedullary or extradural mass. Laminectomy was performed 4 days after admission. At surgery, reddish, diffuse and well demarcated friable mass was found extradural space at the level of C4 -6. But post -operative course was not improved for 2 weeks, so we are performed second operation after identified another intradural extramedullary mass by spinal CT. A total removal of tumor mass was accomplished at second procedure. Histological examination confirmed the diagnosis of cavernous angioma at the extradural mass, neurilemmoma at the intradural extramedullary mass. The post-operative course was satisfactory.

      • Adriamycin및 Epiadriamycin투여가 Mouse심장근의 Mg++-ATPase활성에 미치는 영향에 관한연구

        Adriamycin and epiadriamycin are anthracyclin antibiotics and epiadriamycin has a different configuration of the -OH group in in the C-4 positive (equatorial configuration) of the aminosugar moiety of the adriamycin. The antibiotics inhibit the DNA synthesis by intercalating between strands of the DNA double helix and they have been widely used as anticancer drugs in the treatment of sarcoma, leukemia, and solid tumor. Adriamycin and epiadriamycin have little specificity toward the normal and tumor cells, cytotoxicity of the normal cells being observed during the drug therapy. Therefore the author undertook the present study to pursue the effect of adriamycin and epiadriamycin on mitochondria of the cardiac muscle cell. Albino mice, ICR strain, weighing 20gm were used as an experimental animals. The experimental animals were killed 12, 24, 48 and 72 hours after administration of 30mg/kg of adriamycin and 36mg/kg of epiadriamycin, respectively. The specimens obtained from left ventricle of the heart were prepared and processed by Wachstein's method for detecting ??-ATPase activity in the mitochondria. The results obtained were as follows: 1. The ??-ATPase activity in left ventricular cardiac muscle cells was strongly positive in the 12-hour adriamycin treated group and decreased with time, exhibiting only trace activity in the 72-hour adriamycin treated group. 2. The ??-ATPase activity in left ventricular cardiac muscle cells was strongly positive in the 12-hour epiadriamycine treated group and weakly positive in the 72-hour epiadriamycin treated group. Consequently it is suggested that adriamycin and epiadriamycin inhibit ??-ATPase activity showing cardiotoxic effect of the drug.

        말초성 신경아세포종 : 두개골 발생 증례 보고 Involving Skull

        The peripheral neuroblastomas are malignant neoplasms that originate from the adrenal gland and sympathetic nervous tissues. They usually occur in childhood. They exhibit such malignant features that they metastasize early to lung, liver, bone, rarely skull and other structures. The patients with peripheral neuroblastoma have short duration of symptom and short survival period. They reveal the characteristic light microscopic features that resemble other small cell carcinomas. The establishment of final diagnosis of peripheral neuroblastoma needs various special stainings for small cell carcinomas, and the electron microscopic findings are the most reliable. The authers recently experienced a case of peripheral neuroblastoma in a 26-year old man which involved right frontoparietal skull vault. The only chief complaint was a local non-tender mass at right frontoparietal scalp. Neither headache nor any neurological deficits was detected. Simple skull X-ary revealed a punched out rediolucency at right frontoparietal bone and brain CT showed a superficial elliptical high density mass that enhanced strongly. The mass was totally removed by wide craniectomy. The tumor invaded and penetrated the dura but the arachnoid membrane. at the tumor bed was not invaded by the tumor. The tumor was confirmed as peripheral neuroblastoma by various special stainings for small cell carcinomas. Following surgical resection of the mass, post-operative radiotherpy was offered(4800 rads for about 5 weeks). The patient aggrevated progressively and showed numerous metastases to such bones as lumbar vertebrae, pelvis and humerus to became paraplegic. 8 months after the operation, the patient died.

        두개강내 뇌수막에 발생한 간엽성 연골육종 : 증례 보고 A Case Report

        Recently, the authors have experienced a case of intracranial meningeal mesenchymal chondrosarcoma. Mesenchymal chondrosarcoma is a rare tumor of the bone and soft tissue. It has been reported that the most common site of their extra-osseous origin is the central nervous system. Precise differential diagnosis should be done because of 1) its similarity to angioblastic meningioma or hemangiopericytoma in pathological aspect, 2) much more malignant tendency. We present one case of intracranial meningeal mesechymal chondrosarcoma with a brief review of the relevant literature.

        측두엽에 발생된 Gliosarcoma

        저자들은 근래 좌측 측두엽에 발생된 Gliosarcoma 1예를 수술적 부분적출 및 수술후 방사선 조사로 치료한 후 일시적 증상 호전을 보이다가 수개월 내에 급속한 임상적 악화 및 종양 재증식을 치험하고 희귀한 증례이기에 보고하고저 한다. Gliosarcoma는 악성 뇌교종과 섬유육종이 혼합된 뇌종양으로써, 문헌 고찰 결과 악성 뇌교종 내의 혈관내피세포 과증식이 어떠한 알려지지 않은 기전에 의하여 악성 변화를 일으켜 섬유육종이 발생되는 경우가 대부분으로 사료된다. A case of left temporal gliosarcoma is reported. The patient was a 40-year old man who had headache, behavior change, dysphasia, and mental confusion for about 2 months duration. On brain CT, a large partially cystic mass with homogeneous enhancement was located at left temporal lobe and associated with remarkable pentumoral edema and mass effect. Angiographic tumor vascularity was faintly visualized. Surgical subtotal resection of the tumor and post-operative radiotherapy offered transient symptomatic improvement, being followed within several months by clinical deterioration and regrowth of the tumor on brain CT. Light microscopic studies of the H & E and special stained specimens showed two different components within the tumor in a mixed fashion, i.e. intermingled glioblastoma multiforme and fibrosarcoma. Electron microscopic study of the sarcomatous cells disclosed intracytoplasmic Weibel-Palade-like bodies, a specific marker of endothelial cells. The fibrosarcomatous component of gliosarcoma is believed to be originated from malignant transformation of the hyperplastic endothelial cells within the anaplastic glioma. Literature concerned with gliosarcoma is reviewed.

        cis-Dichlorodiammineplatinum (II) 이 생쥐 폐포간중격의 미세구조에 미치는 영향

        cis-Dichlorodiammineplatinum (II) (cis-Platin), a metallic compound, has widely been used as an effective anticancer chemotherapeutic agent. The precise mechanism of action of this agent is still unknown, but it is postulated that cis-Platin may act on the cancer cell like bifunctional alkylating agents. Although this agent is very beneficial to the patients with cervical cancer, germinoma of testis, neuroblastoma and others, it may also damage to the normal cell so that many side effects; severe hemorrhagic enterocolitis, bone marrow depression, renal damage and liver damage will develope. This experiment has been undertaken to pursue the cytotoxic effects of the cis-Platin on the ultrastructures of the interalveolar septum in the mouse lung. A total of 55 healthy male mice of ICR strain were used as experimental animals and divided into 5 mice of normal control group and 50 mice of cis-Platin treated group. The mice of cis-Platin treated group were sacrificed by carotid exsanguination at 6, 12, 24 hours, 3 days and 7 days after intraperitoneal injection of 6.0 mg of cis-Platin ($Abiplatin^R$ Abic Co. Ltd.) per kg of mouse body weight. The specimen obtained from the lower lobe of left lung were sliced into $1mm^3$ and prefixed with 2% glutaraldehyde -2.5% paraformaldehyde solution prepared with Millonig's phosphatae buffer solution (pH 7.4) at $4^{\circ}C$ for 3-4 hours. After postfixation with 1% osmium tetroxide solution all specimens were embedded in Epon 812. Ultrathin sections about $600-800{\AA}$ in thickness were stained with uranyl acetate and lead citrate and observed with Hitachi-600 electron microscope. The results obtained were as follows: 1. Local swellings with increase of electron density and number of pinocytic vesicles in the cytoplasms of the type I pneumocyte and endothelial cell of the blood air barrier in interalveolar septum of cis-platin treated mice were observed. 2. Cisternae of rough endoplasmic reticulum were dilated and sacculated in association with detachment of membrane bound ribosomes of the type II pneumocyte in interalveolar septum of cis-Platin treated mice. 3. Swollon mitochondria with uneven electron density of their matrix were observed in the type II pneumocyte of interalveolar septum in the cis-Platin treated mice. 4. The lamellae of lammelar bodies in type II pneumocyte of interalveolar septum in cis-Platin treated mice were devoided or transformed into homogeneous electron dense material. It is consequently suggested that cis-Platin would induce the cellular edema of type I pneumocyte and endothelial cell, and degenerative changes of cytoplasmic organelles of the type II pneumocyte in the interalveolar septum of the mouse lung.
