      • 안구운동이 성인 편마비 환자의 마비측에 따른 균형증진에 미치는 영향

        The purpose of this study is to identify influence that eye movement have an effect on postural control and balance performance by plegia side of the impaired patients of central nervous system. Subjects are patients with the impairment of central nervous system and experimental and control groups are forty people and thirty people seperatively, Total subjects were selected to seventy persons, but twelve persons of experimental group quit during the experimental periods and eight persons of control group did not measure after exercise. Therefore, twenty eight persons of experimental group and twenty two persons of control group were selected in this study and experimental group peformed eye movement for eight weeks according to the exercise program of this study. The measurement of dependent variable is right static balance, left static balance, bilateral static balance, and bilateral dynamic balance before exercise and after eighth week of exercise, examiner again measured and analyzed the results. The results were as follows: 1. As the result of comparing balance performance ability after test of the experimental and control group with right hemiplegia and control group, all of static balance performance ability of right side (p < 0.01), static balance performance ability of left side (p < 0.01), static balance performance ability of bilateral side(p < 0.01), dynamic balance performance ability of bilateral side(p < 0.05) of experimental group were statistically greater improved than those of the control group. 2. As the result of comparing balance performance ability after test of the experimental and control group with left hemiplegia and control group, all of static balance performance ability of right side (p < 0.01), static balance performance ability of left side (p < 0.01), static balance performance ability of bilateral side(p < 0.05), dynamic balance performance ability of bilateral side(p < 0.05) of experimental group were statistically greater improved than those of the control group.

        진단검사의학을 위한 세균동정 쿼리시스템의 구현

        세균검사실의 검사업무는 균종 및 검사종류가 다양하고 최소시간에 검사결과를 도출해야 하므로 업무내용이 복잡하다. 이러한 문제를 해결하기 위하여 에이전트를 이용하여 세균검사에 스케쥴링 시스템을 구현하였고, 스케쥴링을 관리하는 워크플로우 관리 시스템을 도입하였다. 스케쥴링 체크를 위한 쿼리들은 조회가 가능하도록 쿼리 시스템을 구현하였다. 그리고 세균검사의 중간 리포트나 결과리포트를 자동으로 보고할 수 있게 하였다. 구현된 스케쥴링 시스템은 자원의 낭비나 부족을 파악하고 효율적인 자원할당 및 관리를 할 수 있었다. 쿼리 시스템은 스케쥴링 체크와 세균검사의 진척상황을 짧은 응답시간에 조회할 수 있었고, 리포트의 자동보고 및 근거리나 원거리에서의 조회가 가능하고, 확인과정으로 인한 검사업무의 지연이나 방해요소를 줄일 수 있었다. 또한 검사기기의 오류나 배지, 시약의 부족 등 예측치 못한 예외사항에 대해 검사의뢰자가 확인할 수 있기 때문에 예외사항으로 발생할 수 있는 인간관계의 갈등 요소를 해결할 수 있었고, 작업진행의 효율화를 기대할 수 있었다. The work of investigation in the laboratory medicine includes various kinds of investigations and microbes and it is too complicated to draw needed results in time. So, we aim to improve work performance of the laboratory medicine. For this study, we implemented the scheduling system in microbe investigation using agent environment and the workflow management system to manage the schedule of investigation, and the query system to check the schedule. And preliminary report and final report of microbe investigation can be announced automatically using agent. The scheduling system implemented could identify the lack or waste of resources and thus enable efficient management and distribution of resources. The query system could check the schedule and retrieve the processing status in short time, enabled the automated report, and reduced possible interrupts and the delay of work that can be occurred in confirming process. It also enables users to access from local and remote sites. Also, this system can reduce the conflicts among people that may occur in unexpected situations because it enables doctors to confirm those situations such as the malfunction of instrument and the lack of agar or reagent, and the efficiency of work process can be expected.

        The patients with a central nervous impediment and a sequela caused by a Stroke are continuously increasing, and the burden of family and society for Stroke patients are also increasing. Since Stroke patients are required to receive a long time medical treatment and care. It adds a economical burden as well as a mental and physical burden of patient's family. In addition, it is a primary factor which deepens the disparity of medical resources in social aspect. But the social concern for these patients are very insufficient and are entirely left to the patient's family The mediation of Skilled Nursing Facility for Stroke patients enable to receive more special and proper medical treatment and recuperation service in the middle of time when they return to home from hospital. It also contributes to the settlement of the charge of patient's family and the serious disparity of medical resources. Therefore, this study will summon the social understanding of the necessity of Skilled Nursing Facilities through the above debate.

      • 견갑대 운동 기능장애에 대한 치료 접근

        Functional stability is dependent on integrated local and global muscle function. Movement dysfunction can present as a local and global problem, though both frequently occur together. To good understand how movement induces pain syndrome, the optimal actions and interaction of the multiple anatomic and functional systems involved in motion must be considered. Minor alterations in the precision of movement cause microtrauma and, if allowed to continue, will cause macrotrauma and pain. These alteration of the movement result in the development of compensatory movement and movement impairment. Muscle that become tight tend to pull the body segment to which they are attached, creating postural deviation. The antagonistic muscles may become weak and allow postural deviations due to lack of balanced support. Both hypertonic and inhibited muscles will cause an alteration of the distribution of pressure over the joint(s) that they cross and, thus, may not only result from muscle dysfunction, but produce joint dysfunction as well. Alteration of the shoulder posture and movement dysfunction may sometimes result in compression of neurovascular structures in the shoulder and arm. There is a clear link between reduced proprioceptive input, altered motor unit recruitment and the neurovascular compression. This report start with understanding of the impaired alignment, movement patterns and neuromuscular compression of the shoulder girdle by movement impairment to approach method of the movement dysfunction.

      • 경추의 전신조정술 관절치료가 관절가동범위에 미치는 영향

        Purpose: The purpose of this study is to search effect that GCM joint treatment gets to range of motion of cervical, lumbar, trunk and anke. And cervical gets in ankle joint. Methods: Estimated body deformity using GCM body type assesment chart then measured range of motion of each region. After control group did as act freely after do experiment pre measurement control group did post measurement. After control group did as act freely after do experiment premeasurement, control group did postmeasurement. Each region was measured by measurer who each subject person differs. Experimental group did GCM joint treatment and all measurements each region by measurer who each subject person differs three times measure postmeasurement after premeasurement. When measure with each measurement, measured after leave and walk time interval for 10 minutes. Result: For the analysis of the result of experiment, the results is change amount comparison increased to keep in mind except cervical flexion and both ankle joint's dorsiflexion after experiment of experimental group. In experimental group, cervical, lumber and ankle joint of range of motion was significantly increased(p<.05).

        3-AP로 인한 소뇌병변 흰쥐에서 운동과 저출력 레이저가 행동변화에 미치는 영향

        The purposes of this study were to test the effects of the low power laser and exercise on the recovery in the cerebellar injured rats by 3-Acetylpyridine. Cerebellar injury was induced by 3-Acetylpyridine chemoablation of the inferior olive. Thirty Sprague-Dawley rats were assigned to the normal control and 3AP control and 3 experimental groups. Each experimental group was treated from 5 days after being injured for the 5 min(laser group), 10 min(exercise group) and 15min(exercise with laser) everyday during the 2 weeks. The Hindlimb splay test, Vestibular drop test, Hindlimb stride width test, Maximal Height Vertical Jump test were examined at pre-treatment on 1st day and 5th, 10th, 14th days after treatment on the cerebellar injured rats by 3AP. The results of this experiment were as follows; There were significantly increased exercise on the 3 experimental groups comparied with the 3AP control group, in the Hindlimb splay test, Vestibular drop test, Hindlimb stride width test, and Maximal Height Jump test(P<.05).

        정신훈련이 편마비환자의 상지기능 향상에 미치는 영향

        This study was performed to examine the influences of the mental practice to the hemiplegic upper limb motor function improvement. 20 minute neurologic treatment based on the neurophysiological theory, 10 minute activities of daily living training, and 10 minute mental practice 5 times a week were given in turn to the experimental group(N=11). On the other hand 20 minute neurologic treatment, and 10 minute activities of daily living training 5 times a week were given in turn to the control group(N=11). Both Fugl-Meyer Assessment Scale and Manual Function Test were used to evaluate upper limb motor recovery, upper limb motor function and movement ability. And the Motor Activity Log; Amount of Use and Motor Activity Log; Quality of Movement before training, 2 weeks after training, and 4 weeks after training were measured to assess the upper limb motor quantitatively and qualitatively each. The results are as follows. 1) Considering the interactions of the rate of change on the upper limb motor recovery, motor function, movement ability improvement, and qualitative motor improvement in ADL of experimental group and control group, the change rates of experimental group were found to be greater than those of the control group. 2) In experimental group, the higher the achievements were, the better upper motor recovery was.

        지식 데이타베이스를 적용한 효율적인 세균 의료영상 검색 시스템의 구현

        본 연구는 신규 임상병리사로 하여금 보다 정확한 의사결정과 효율적인 교육에 이용할 수 있는 지식 및 내용기반 의료 세균화상 검색 시스템을 설계 및 구현하는 것이다. 이를 위해, 먼저 알고리듬방식의 검색 이전에 경험적 지식을 바탕으로 세균동정단계 중 가장 빠른 경로를 탐색하여 원인균 동정에 소요되는 시간을 줄일 수 있도록 룰 베이스를 근거로 유연성 있는 탐색경로를 설정하여 전체적인 추론을 수행한다. 다음으로, 색상 모델 중에서 HSV 컬러 모델을 이용하여 세균화상 중에서도 특히 세균화상으로부터 시각정보의 색상 특징 벡터를 추출할 수 있는 색상 특징 추출방법을 제안한다. 아울러 대용량 세균화상 데이터베이스를 기반으로 보다 빠른 검색 성능을 위해, 배지, 검체, 부서, 세균명과 같은 단순속성들에 대해서는 B+-트리, 세균화상에 대한 부가적인 설명정보로부터 추출한 키워드들에 대해서는 역화일기법, 그리고 화상으로부터 추출한 고차원 색상 특징벡터에 대해서는 스캔-기반 필터링(Scan-Based Filtering:SBF) 기법을 결합한 통합 색인기법을 기술한다. 마지막으로 구현된 시스템은 시각적인 내용 자체의 정보와 지식을 이용하여 효과적으로 복잡한 세균화상을 검색 및 관리할 수 있는 가능성을 보인다. 아울러 구현한 지식 및 내용-기반 세균화상 검색 시스템을 통해 임상분야의 지식을 잘 구조화함으로써 초보적인 임상병리사의 학습기간을 현저히 단축시킬 수 있을 것으로 기대된다. This study is to design and implement an efficient microbial medical image retrieval system based on knowledge and content of them which can make use of more accurate decision on colony as well as efficient education for new technicians. For this, we first address overall inference to set up flexible search path using rule-base in order to reduce time required original microbial identification by searching the fastest path of microbial identification phase based on heuristics knowledge. Next, we propose a color feature extraction method, which is able to extract color feature vectors of visual contents from a given microbial image based on especially bacteria image using HSV color model. In addition, for better retrieval performance based on large microbial databases, we present an integrated indexing technique that combines with B+-tree for indexing simple attributes, inverted file structure for text medical keywords list, and scan-based filtering method for high dimensional color feature vectors. Finally, the implemented system shows the possibility to manage and retrieve the complex microbial images using knowledge and visual contents itself effectively. We expect to decrease rapidly learning time for elementary technicians by well organizing knowledge of clinical fields through proposed system.

        익상견갑에 대한 전거근 운동 : 증례 보고 1 Case Study

        Objective: The purpose of this study is to describe the effect of the serratus anterior muscle exercise in a patient with winging of the scapular. Background and Purpose: The patient was a 33-year-old man with an 3 years history of the left deltoid region. He had not responded to previous treatments of rest, TENS and medicine. Materials and Methods: After Surgical operation of the shoulder, the patient was treated for 6 months in 144 visits. treatment consisted of push-up with plus in the standing position with his hands against the wall and unique push-up with quadruped position at mat. Also, Sling exercise therapy was utilized to control scapular stabilization and reduce stress on the scapulohumeral joint. Result: Following 104 physical therapy visits, the patient was able to full range of motion of the shoulder without shrug motion Conclusion and Discussion : Scapular winging and serratus anterior muscle function are important consider in the rehabilitation of the shoulder dysfunction. Therefore, specific exercise program for people with the winging scapular should be considered.
