      • 정지형 자전거 운동 훈련이 건강한 여성의 체구성,심폐기능 및 유연성에 미치는 효과

        The purpose of this study was to determine the changes in body composition, cardiopulmonary function, and flexibility following 8 weeks' bicycle ergometer exercise training. The subjects of this research consisted of healthy adult women between 30 and 40 years of age. Initial intensity of bicycle ergometer exercise was based on the target heart rate equivalent to 55% of maximal oxygen uptake, and intensity of the exercise increased by 5% every 2 weeks. Body composition, cardiopulmonary function at rest and during maximal exercise, as well as flexibility were determined before and after 8 weeks of bicycle ergometer exercise training. Masimal exercise was performed on the treadmill according to Bruce Protocol. The results obtained were as follows; 1. There were o changes in body weight, percent body fat, fat body weight, and lean body weight as well. The ventilation volume for 1 minute, respiratory quotient and expired CO₂ volume have not changed significantly after the training, either. 2. As a result of training, resting systolic and diastolic blood pressure decreased significantly(p<.05) 3. Maximal heart rate, maximal oxygen consumption and maximal running time increased significantly after the training(p<.05) 4. Pelvic flexibility increased slightly after the training, however, it did not reach the statistical significance. Trunk flexion forward increased significantly after the training(p<.05). From these results, it may be concluded that 8 weeks bicycle ergometr training improve the cardiopulmonary function and flexibility in healthy adult women between 30-40 years of age.

        기초간호자연과학의 병태생리학, 병원미생물, 약물의 기전과 효과 내용별 필요도에 대한 연구

        The purpose of this study was to define the content of the requisite knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for clinical knowledge for nursing practice. Contents of knowledge on pathological physiology, clinical microbiology, and mechanisms and effects of drugs were constructed from syllabus of basic nursing subjects in 4 colleges of nursing, and textbooks. The degree of need of 72 items was measured with a 4 point scale. The subjects of this study were college-graduated 136 nurses from seven university hospitals in Seoul and three in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, and hospice ward. The results were as follows : 1. The highest scored items of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice were side effects of drugs, anticoagulants, mechanisms of drugs, antihypertensive drugs, tolerance and addiction of drugs, interactions among drugs, hospital infection in the order of importance. The lowest scored item was structure of microrganisms. 2. The highest order of need according to unit was repair in tissue injury unit, definition·etiology·classification of inflammation in inflammation unit, trasplantation and immunologic response in alterations in immunity unit, thrombus and thrombosis in disorders of cardiovascular function unit, gene disorders in genetic disorders unit, hospital infection in infection unit, virus in microrganisms unit, side reactions of drugs in introduction unit, anticonvulsants in drugs for central nervous system unit, local anesthesia in anesthesia unit, anticoagulants in drugs for cardiovascular system unit, anti-inflammatory drugs in antibiotics unit, anti-ulcer drugs in drugs for digestive system unit, and bronchodilators in drugs for respiratory system unit. 3. The common content of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for all clinical areas in nursing were side effects of drugs, anticoagulants, interactions among drugs, and hospital infection. However, the degree of need of each pathological physiology, clinical microbiology, and mechanisms and effects of drugs was different depending on clinical areas. 4. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as tissue changes due to injurious stimuli, degenerative changes of tissue, alterations in metabolism of carbohydrates, ischemia, hyperemia and congestion, hospital infection, structure of microorganism, classification of microorganism, bacteria, virus, antidepressants, antipsychotic drugs, antiemetic drugs, antiparkinsonism drugs, antianxiety drugs, antibiotics, tuberculostatics, antiviral drugs, antifungal drugs, parasiticides, antiulcer drugs, antidiarrheais, and anti constipation drugs were shown according to the work area. 5. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as transplantation and immunologic response, alterations in the metabolism of uric acid, structure of microorganism, classification of microorganism, immunosuppresants, drugs for congestive heart failure were demonstrated according to the duration of work. Based on these findings, all the 72 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of the knowledge of pathophysiology, clinical microbiology. and mechanisms and effects of drugs

        기초간호자연과학의 인체구조와 기능 내용별 필요도에 대한 연구

        The purpose of this study was to define the content of requisite human structure and function knowledge needed for clinical knowledge of nursing practice. Subjects of human structure and function were divided into 10 units, and each unit was further divided into 21 subunits, resulting in a total of 90 items. Contents of knowledge of human structure and function were constructed from syllabus of basic nursing subjects in 4 college of nursing, and textbooks published by nurse scholars prepared with basic nursing sciences. The degree of need of 90 items was measured with a 4 point scale. The subjects of this study were college graduated 136 nurses from seven university hospitals in Seoul and three university hospitals located in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, hospice ward, and their working period was mostly under 5 years. The results were as follows : 1. The highest scored items of human structure and function knowledge necessary for nursing practice were electrolyte balance, blood clotting mechanism and anticoagulation mechanism, hematopoietic function, body fluid balance, function of plasma, and anatomical terminology in the order of importance. The lowest scored items of human structure and function knowledge necessary for nursing practice was sexual factors of genetic mutation. 2. The highest order of need according to unit was membrane transport in the living unit, anatomical terminology in movement and exercise unit, mechanism of hormone function in regulation and integration unit, component and function of blood in oxygenation function unit, structure and function of digestive system in digestive and energy metabolism unit, temperature regulation in temperature regulation unit electrolyte balance in body fluid and electrolyte unit, concept of immunity in body resistance unit, and genetics terminology in genetics unit. The highest order of importance according to subunit was membrane transportation in cell subunit, classification of tissues in tissue unit, function of skin and skin in skin subunit, anatomical derivatives of the skeleton subunit, classification of joints in joint subunit, an effect of exercise on muscles in muscle subunit, function of brain in nervous system subunit, special sense in sensory subunit mechanism of hormone function in endocrine subunit, structure and function of female reproductive system in reproductive system unit, structure and function of blood in blood unit, structure of heart, electrical and mechanical function in cardiovascular system unit, structure of respiratory system in respiratory system subunit, structure and function of digestive system in digestive system subunit, hormonal regulation of metabolism in nutrition and metabolism subunit, function of kidney in urologic system subunit, electolyte balance in body fluid, electolyte and acid-base balance subunit. 3. The common content of human structure and function knowledge need for all clinical areas in nursing was structure and function of blood, hematopoietic function, function of plasm, coagulation mechanism and anticoagulation mechanism, body fluid, electrolyte balance, and acid-base balance. However, the degree of need of each human structure and function knowledge was different depending on clinical areas. 4. Significant differences in human structure and function knowledge necessary for nursing practice such as skin and derivatives of the skin, growth and development of bone, classification of joint, classification of muscle, structure of muscle, function of muscle, function of spinal cord, peripheral nerve, structure and function of pancrease, component and function of blood, function of plasma, structure and function of blood, hemodynamics, respiratory dynamics, gas transport, regulation of respiration, chemical digestion of foods, absorption of foods, characteristics of nutrients, metabolism and hormonal regulation, body energy balance were demonstrated according to the duration of work. 5. Significant differences in human structure and function knowledge necessary for nursing practice such as classification of tissue, classification of muscles, function of muscles, muscle metabolism, classification of skeletal muscles, classification of nervous system, neurotransmitters, mechanism of hormone function, pituitary and pituitary hormone, structure and function of male reproductive organ, structure and function of female reproductive organ, component and function of blood, function of plasma, coagulation mechanism and anticoagulation mechanism, gas exchange, gas transport, regulation of respiration, characteristics of nutrients, energy balance, function of kidney, concept of immunity, classification and function of immunity were shown according to the work area. Based on these findings, all the 90 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of human structure and function knowledge.

      • Astromicin sulfate의 약동학 및 안전성 평가

        목적 : Astromicin은 aminoglycoside계 항생제와 유사한 화학구조와 특성을 지니고, 기존의 aminoglycoside와 유사하거나 향상된 임상효과 및 부작용을 지닌 것으로 알려져 있다. 저자들은 astromicin의 약동학적 특성을 분석하고, astromicin 투여 환자를 대상으로 한국인에서 그 임상적 및 세균학적 효과와 안전성을 평가하고자 하였다. 대상 및 방법 : 약동학적 지표는 정상성인 남자를 대상으로 astromicin 200㎎을 30분간 정주한 후 24시간까지 혈장 및 뇨 검체를 수집하여 one-compartment open model에 따라 분석하였다. 중증으 세균감염이 있는 18세 이상의 환자를 대상으로 병용 또는 단독 투여한 후 임상적 효과와 세균학적 효과를 판정하였고, 치료전과 후의 순음청력검사와 혈청 크레아티닌으로 이독성과 신독성을 평가하였다. 결과 : 정상 성인 남자 12명을 대상으로 약동학적 지표를 평가하였고, 최고혈장농도는 투여 종료시점(C_(0))에서 16.87±1.68㎍/mL였으며, 반감기는 1.86±0.43 시간 이었고, AUC_(0-12h)은 38.12±10.57㎍ㆍhr/mL, Vd는 0.18±0.02L/㎏, CL은 5.25±2.07L/hr였다. 이러한 약동학적 지표는 다른 aminoglycoside와 유사한 결과를 보였다. 59명의 환자를 대상으로 시행한 임상적 효과 분석에서는 단독요법을 시행한 50명 중 임상적 치유율 94%, 세균학적 치유율 100%를 보였고, 순음청력검사를 시행한 48례의 환자중 의미있는 청력감소 소견은 관찰되지 않았으며, 신독성 또한 전혀 나타나지 않았다. 결론 : Astromicin의 약동학적 지표는 다른 aminoglycoside의 약동학적 지표와 비슷한 양상을 보이므로 향후 astromicin의 약물농도 감시에 있어서 다른 aminoglycoside의 치료적 약물 농도 감시 방법과 유사한 방법을 적용할 수 있으리라 기대된다. 또한 임상적 치유율이 우수하면서 신독성이나 이독성이 적어 세균 감염 환자의 치료에 있어서 단독 또는 병합 요법으로 비교적 안전하게 사용할 수 있으리라 생각된다. Astromicin(Fortimicin®) has some characteristics in common with other aminioglycoside antibiotics, although it has a unique chemical structure, which is different from them. This study was performed to elucidate the pharmacokinetic (PK) parameters of astromicin following single-dose intravenous infusion of 200 mg and to evaluate clinical efficacy and safety of astromicin in Korean populations. PK parameters of astromicin were determined in 12 healthy volunteers (65.5±5.2㎏). The plasma and urine samples were collected up to 24hrs. PK variables were calculated by fitting individual concentration-time curves to a one-compartment open model. Plasma level at the end of infusion was 16.87?1.68 ㎍/mL and declined to 1.05±0.35 (㎍/mL 8hr later. The half-life was 1.86±0.43 hr. Apparent volume of distribution was 0.18±0.02 L/㎏g, and total body clearance was 5.25±2.07 L/hr. These values were similar to those of other aminioglycosides. Clinical efficacy and safety were eviuated in 59 patients with moderate to severe bacterial infections who needed parenteral antibiotics. Among 50 patients who recieved astromicin monotherapy, 49 (98%) had favorable clinical reponse and 28 (100%) had favorable bacteriological response. Serial audiograms revealed no change in all of 48 patients. No nephrotoxicity was observed in all patients. Conclusively, our data suggest that therapeutic drug monitoring of astromicin can be conducted in a similar fashion as other aminioglycosides and astromicin is a useful and safe antibiotic in the treatment of severe bacterial infections.

        성인간호학회지 논문(1989-2000)의 내용과 경향 분석

        "Journal of Korean Academy of Adult Nursing" has been published since 1989 and more than 330 articles have been published up 2000. We reviewed all the articles published in this journal to analyze the contents and trend of published articles and to find out the way of improvement both in quantity and quality of studies in this area. The results are as follows : 1) Ninety percent of articles were quantitative studies. Experimental design was used in 16.1% and quasi-experimental design was adapted in 73.6% of articles. Qualitative research were 10.3% of total articles reviewed. 2) Only 5.1% of articles described theoretical framework and 73.6% of experimentally designed articles included research hypotheses. 3) Among experimental studies, only one was true experimental studie design. Eighty nine point three persent had descriptive survey method among non-experimental studies. Amog the thirty four qualitative studies, phenomenologic(73.5%), grounded theory(14.7%), and ethnographic (11.8%) design were used. 4) Most of research subject were patients and few healthy people, healthcare worker, and patient family. 5) Psychosocial data collection method which used questionnaire were 53.7% and interview. physical measurement, observation were used in 22.8%. 8.2% 5.4% of articles respectively. 6) Total number of keywords were 692, and average number of key words per article was 2.2. Health-related keyword was the most frequently used according to four metaparadigm of nursing (human, health, nursing, and environment). Environment related keyword was rarely appeared. The number and quality of nursing research in "Journal of Korean Academy of Adult Nursing" have been improved continuously. It is difficult to find out special trend or characteristics which is unique in this journal because of relatively short history of adult nursing studies. This review study was performed to analyze adult nursing studies only in the view of quantitative aspect, but it is needed to analyse qualitatively to find philosophical or theoretical trend in nursing.

      • 운모 입자의 표면상에 대한 옥시염화비스무트의 흡착

        The laxurious Bismuth oxychloride is prepared form Bismuth nitrate solution dissolved in hydrochloric acid by hydrolyzing with sodium hydroxide solution at pH 2.0-2.5 and heating at 100±5℃ for 4hrs. In the presence of excess chloride ion to 1 : 1.5 (Bismuth:Chloride). Bismuth oxychloride is formed quantitatively in the range of pH 2.0-2.5. Under the same condition the hydroly3ed Bismuth oxychloride is adsorbed on the surface of mica particles which was cleaned preliminarily by treating which ultrasonicator. The adsorbed mica particles are shown the dull white and the adsorbed Bismuth oxychloride is octagonal flower type which was aggregated duplicatedly more then five plates and it is size is average 5 μm.

        Gastric perforation is a rare abdominal catastrophe which associated with high mortality in newborn infant. Mostly the causes are not known but those are asphyxia, congenital muscle defect, sepsis, gastric tube insertion, trauma, ulcer and intestinal obstruction, etc. It usually occurs within the first week of life. Roentgenograms of the abdomen are most helpful in making a positive diagnosis and surgical repair is the treatment of choice. We had two cases of spontaneous gastric rupture in neonate. One had perforation on the greater curvature of the fundus and the other on the anterior wall of body. Failure to feed and abdominal distension with radiographic demonstration of pneumoperitoneum were seen in them. Two cases underwent surgical repairs. So one was survived, but the other died in the early postoperative period.

        α -Ferric oxide was prepared from α-FeOOH by hydrolysis of ferric nitrate solution at pH 9-10 and the form was identified by infrared spectrophotometer and X-ray diffractometer. After mica treated with ultrasonicator for 1 hr., α -FeOOH was deposited on the surface of mica particle and transformed it to α-Fe2O3by heating for 1 hr. at 700 C. The maximum wavelength of scattered light by α- Fe2O3 adsorbed on the surface of mica particle was shifted from 436nm to 546nm as the oxide was increased from 1% to 50% by weight per-centage to mica.

        This study intends to analyze the frequent errors on English word order and also to present an effective way in which words are actually used in particular contexts, especially with regard to their meanings and structures. Compared with Korean sentences which are considered to haγe relatively free word orders, English sentences can be analyzed to have relatively strict word orders , It is suggested that the importance of English word orders should be emphasized with regard to structural constraints and semantic constraints in systematic learning and teaching steps of English Grammar. The following data are analyzed and discussed: articles, quantifiers, adjectives, adverbs , to-infinitives , -ing phrases, negative sentences, interrogative sentences, direct questions , indirect questions , and exclamatory sentences, etc.

        대학에서의 인문학 수업에 대한 교수와 학생의 인식과 우수 인문학 강의 사례 분석

        '인문학의 위기'는 이제 주요한 사회문제의 하나로 인식될 정도로 그 심각성을 더해가고 있다. 이 연구는 인문학 수업에 대한 교수와 학생의 인식을 분석하고 우수 인문학 강좌를 선정하여 분석함으로써 효과적인 인문학 강좌의 교수-학습 측면의 특성을 파악하고자 하였다. 효과적인 교수-학습 방법의 개발이 인문학 위기를 초래한 원인들을 해결하기 위한 근본적인 처치 방안은 되지 못할지라도, 현재 학생들의 인문학 기피 현상에 대한 인문학 내부의 적극적이고, 구체적인 대응 방안이 될 것으로 기대된다. 이를 위해, 인문학 담당 교수들과 수강 대학생을 대상으로 하는 질문지 조사 방법이 동원되었고, 우수한 인문학 강의 사례를 발굴하여 그들 강좌의 운영 방식과 사용된 교수-학습 방법을 연구하는 사례 연구가 병행되었다. 인문학 강좌의 교수-학습 방법에 대한 학생들과 인문학 전공 교수들의 인식, 인문학 강좌의 사용되는 교수법, 교수-학습 매체, 홈페이지 활용 실태 등이 분석되었고, 효과적인 교수-학습 방법 개발을 위한 교수자 역할, 의사소통, 동기 유발, 교수 매체 활용, 학습 평가 방법 등에 관한 시사점이 도출되었다. The "crisis"of the humanities is getting worse, that it has been recognized as an important social issue. The "crisis" exists not merely in the decreasing number of students majoring in the fields, but in the widespread avoidance of humanities courses among college students. As a solution to overcome the crisis of the humanities, this study focused on developing effective teaching and learning methods which would increase students' interests and motivation in the fields by reflecting recent social changes. Although it cannot be a fundamental solution, we expect it to be a specific and active answer to the problem within the humanities. For the study, both survey and case studies were conducted. The survey collected information from professors in the humanities department and students taking the humanities courses. The cases known for good courses were selected to examine the teaching methods and instructional activities in depth. The survey results described responses of professors and students on teaching and learning methods in the humanities and the utilization of instructional media and homepages, etc. Also, the case study analyzed teaching strategies, communication styles, motivation, instructional media, and testing and course evaluation for effective teaching and learning.
