고관절 치환술후 무균성 해리와 골용해 조직에서 세포사멸

목적 : 인공 고관절 치환술 후 실패의 가장 중요한 원인이 되는 무균성 해리및 골용해와 nitric oxide(NO)에 의한 염증세포 사멸의 연관성을 규명하기 위해 본 연구를 시행하였다. 대상 및 방법 : 무균성 해리와 골용해로 인해 재치환술을 받은 18명의 환자의 조직에서 세포들의 사멸을 Terminal Deoxyribonucleotidyl Transferase-Mediated dUTP Nick End Labeing (TUNEL) 방법을 이용하여 검사하였으며, inducible nitric oxide synthase(iNOS)와 미토콘드리아 관련 세포사멸 경로를 p53, Bax, Bcl2, Ki67 항체를 이용해서 면역 염색하였다. 대조군으로 대퇴 경부 골절 환자 6명의 관절낭을 이용하였다. 결과 : 16명(89%)에서 iNOS, p53, Bax 항체에 양성 반응을 보였으며, 12명(67%)에서 Bcl2, Ki-67 항체에 양성반응을, 전례에서 TUNEL에 양성반응을 보였다. 대조군에서는 1명(17%)에서 iNOS와 Bax에, 3명(50%)에서 TUNEL에 양성반응을 보였다. 무균성 해리와 골용해 조직의 세포사멸에 대한 양성반응의 발현빈도가 대조 조직에서 보다 유의하게 높았다. 결론 : Nitric oxide에 의한 염증세포의 사멸이 인공고관절 치환술 후 생기는 무균성 해리와 골용해에 작용함을 알수 있었다. 이 결과는 해리 및 골용해의 예방과 치료방법을 개발하는데 도움을 줄 수 있다. Purpose : To evaluate the relationships between the apoptosis induced by nitric oxide(NO), aseptic loosening and osteolysis, which are the most serious causes of failure after total hip arthroplasty. Materials and Methods : Apoptosis of the inflammatory cells of interface tissues from 18 patients who underwent revision hip arthroplasty was identified by Teminal Deoxyribonucleotidyl Transferase-Mediated dUTP Nick End Labeling(TUNEL). The reaction to immunostaining of inducible nitric oxide synthase(iNOS), p59, Bax, Bcl2, and Ki-67 were evaluated. Six joint capsules obtained from six patients of femoral neck fracture were studied as controls. Results : Sixteen(89%) of 18 interface tissues were positive for iNOS, p53, and Bax monoclonal antibody and twelve(67%) for Bcl2 and Ki-67 monoclonal antibody. All tissues were positive for TUNEL. In the control group of six joint capsules, only one(17%) was positive for iNOS and Bax, and three(50%) were positive for TUNEL. The incidences of apoptiotic signals at the interfacial tissue of aseptic loosening and osteolysis were significantly greater than those of the control. Conclusion : The current study suggested that the apoptosis of inflammatory cells, due to oxidative stress by NO, might be involved in the development of implant loosening and osteolysis after total hip arthroplasty. This information might be crucial for the treatment and prevention of periprosthetic osteolysis and subsequent loosening.

저체중 또는 과체중 청소년에서 캠프 프로그램을 이용한 영양 교육효과에 관한 연구

This study was performed to evaluate the effectiveness of a nutrition education program developed for nutritionally imbalanced adolescents. A summer nutrition camp was held for 23 overweight and 16 underweight subjects. Its effectiveness with regard to was evaluated at the end of the camp and 6 months later. Nutrition knowledge, nutritional attitude, food behavior, nutrient intake, exercise habit and ideal body figures desired by the subjects. The results showed significantly higher nutrition knowledge scores at the end of the camp as compared to those obtained prior to the camp program, and these scores were maintained for at least six months. Nutrition attitude scores also improved after the education program, and these improved scores also lasted for 6 months. However, the food behavior scores measured 6 months after the education program were not significantly different from those obtained prior to the camp. Also, the exercise habit, the ideal body figures and the body figures desired by the subjects remained unchanged. When nutrient intakes of subjects were assessed before the program and 6 months later, the mean daily vitamin C intake was significantly increased after the education program. Also, the intake of iron from plant food sources increased in the overweight subjects, while less iron from animal source were consumed by the underweight subjects. Both groups tended to consume more vegetables and fruits 6 months after the education program which may have contribute to the higher vitamin C and plant-based iron intakes. These results indicate that a 4-day nutrition education camp program sustained changes in nutrition knowledge and nutrition attitude for 6 months. The increased intake of vegetables and fruits was also achieved through this education program. However, changes in dietary behavior in adolescents may require repeated education.

수지(手指)에 발생한 Glomus Tumor - 치험 2례 보고 -

Glomus tbmor of the hand has been well documented lesion that usually presents as painful dermal nodule in the finger. Glomus tumor may be difficult in diagnostic and therapeutic problems because of relatively rare and not easily experienced cases. Normal glomus body presents in the skin and subcutis of the extremity, but the normal body is not more than 1 mm3) in size. Most evidence indicates that the main function of the normal glomus body is to aid in regulation of the skin circulation. The glomus tumor is thought to be an abnormal proliferation of the glomus body and accompained by the classic triad of symptoms : pain, tenderness and temperature sensitivity. This paper reports two cases of glomus tumor occurring in the nail bed and finger pulp. One case was found 30 years later & another case 15 years after the onset in spite of srvere pain, tenderness and lancinating pain when exposed to the cold tempera ture but the excision of the tumor resulted in complete recovery.

타액반 피검물에서 개인식별을 위한 DNA의 유전자형 검사

Cigarette butts from 5 smokers were gathered and then, place in room temperature for 1, 3, 5, 7, 15 days. The possible use of the cigarette butts for individual identification was evaluated in sex determination, amplification of D1S80 locus, polymorphisms of HLA-DQA1 gene from the extracted DNA. 1. DNA extraction was possible in cigarette butts left in room temperature for 15 days, so it can be applicable to individual identification by polymerase chain reaction (PCR). 2. Amplification of X-Y homologous amelogenin gene by PCR made it possible to identify the sex in saliva stains (cigarette butts). 3. Amplification of D1S80 locus can be acquired from adding the bovine serum albimine and hot start PCR procedures from forensic samples such as saliva stains (cigarette butts), so the AMP-FLPs examining is possible. 4. Genotype could be determined simply and rapidly using Amplitype?? HLA-DQα forensic kit in examining the HLA-DQA1 gene. From the investigation, DNA extraction, sex determination, amplification of D1S80 locus, polymorphisms of HLA-DQA1 gene were successfully done even though the cigarette butts were left for 15 days at room temperature. Therefore cigarette butts are highly reliable and applicable as molecular biological samples for individual identification.

The purpose of this study is to establish an evaluation method of fracture of light-weight framed walls by human impact load. In this paper, part (I), we analyzed human impact load applied to framed walls. At first, we made original measuring equipment of impact load, and carried out the experiment on human impact load in various action. After the experiment, we extracted the essential characteristics. And grasped the property of human impact load systematically based on the relation between maximum load and load velocity.

비만을 동반한 위암 환자에서 두 병을 같이 수술 치료한 경험(증례)

Obesity is growing problem in Korea. We had a case of bariatric surgery during gastric cancer operation. Patient was 29 year old Korean lady with early gastric cancer located in the lesser curvature side of the middle 1/3 of the stomach. Her body weight was 89 kg, height 163 cm, and thus body mass index was 33.5 kg/m2. Preoperative blood pressure was 130/90 mmHg, hemoglobin 12.9 g/dL, total lymphocyte count 3,280/mL, serum albumin 4.3 g/dL, CEA 1.1 ng/mL, CA 19-9 9.1 ng/mL, and alpha fetoprotein 2 ng/mL. Another associated disease was right thyroid follicular neoplasm. The chest X-ray was normal. After IM injection of 2,500 units of heparin 30 minutes before the induction of general anesthesia, she received curative subtotal gastrectomy and Roux en Y gastrojejunostomy when a malabsorption loop of 120 cm jejunum was incorporated between the Treitz ligament and the end to side jejuno-jejunostmy site at May 29th 2006. Her postoperative course was uneventful except a minor wound seroma and the elevations of serum amylase(up to 4 folds) and lipase(up to 2 folds). She lost her body weight 9 kg in 5th, 21 kg in 7th, and 27 kg in 10th postoperative months to became 61.5 kg. On follow up exams in July 2008, she gained 4 kg to overcome her weakness and fatigue. From the above result, the incorporation of a malabsorption loop during reconstruction phase of gastric cancer operation was a good option for obese gastric cancer patients especially in cases of early gastric cancer.

We examine the evolution of market structure by technological innovation. There are two kinds of phenomena in the evolution. One is the 'increasing dominance' where the efficient firm continues to be a leading firm. The other is the 'action-reaction' where technological superiority of firms changes over time. We derive the conditions under which these two phenomena occur. Also, we look at the effect of network externality on the pattern of the innovation incentive and market evolution.

This study examined the differences between South Korea and the United States of America’s interior designer's contract duty, standard documents, and reward. The research methodology analyzed many documents including AIA, ASID, and KOSID professional standards. The research process was as follows: First, find out the evolution of the two countries interior design professions and how each acknowledged contract documents. Second, identify the document guidelines established by professional organizations to determine suitable materials. Third, compare the two countries contract duty, standard documents, and reward. The results are as follow: Duty of Interior Designer: Little difference was found between South Korea and US interior design duties. Owner and Designer Contract Documents: In South Korea KOSID published contract documents in 1982 with first general revisions occurring in 1997. Designers work by using a variety of contracts including verbal, personal, office, and KOSID. In the United States the cooperation of National Association of Builder first published documents in 1888 with revisions occurring from 1911-1987. Today over 100 contract documents are published by the AIA and 7 residence documents are published by ASID. Reward of Interior Designer: In South Korea the table of duty rewards sets the base price with owner and designer making the final decision. Different rates are set according to total construction price and type of building. The highest billable rate comes from a museum, while an education facility has a lower rate; rates range between 5.0-25.5. In the United States firms use different strategies dependent upon project constraints and demand. Choices include fixed fee, hourly rate, percentage of cost, area fee, and retail method.

[1] This article is written to bring light on the problems of Korean elites with special references to their recruitment, characteristics, roles and substitute. The weight of elites which is a product of modern political ideology is heavier than any other sects in developing countries. Whether a new state can maintain both stability and development hinges, to a large extent, on the integration of its political elites. Especially during the transition from traditional polities to a more modern political system, elites play a crucial role. Elite activities determine the speed and effectness with which the polity can move toward national integration and political modernity. [2] The recruitment of elites in developing countries can not be considered apart from the mass. Whatever their fear of the masses, the governmental elites in new nations can not do anything without them. While the elite may be unsymphathetic to mass effort to excercise influence, they do want to mobilize the mass for its goals. In Korean case, history of formation of elites can not be deemed as a long one. It did not date back to before the liberation from Japanese rule, because it was after the liberation that the Korean intellectuals approached to the political problems with ideology. In this respect, early elites were recruited from the returning groups from the foreign refuge. But they had many difficult problems, because that they had not any political foundation in their native country; that they were lack of political training : that it was inevitable for them to divide themselves unwillingly on account of territorial difference for their independent fight ; and that their Western political thought was unfamiliar to masses. Those "heroes of renunciation" ex-pected in vain the closeness between mass and themselves in a minute, therefore they felt another alienation. They also have a desperate fight with weakness of economic foundation. [3] The roles what Korean elites should play can be explained as following : First, they should complete the political integration by solving the conflicts around democracy. From the political view, in Korea where is lack of civilian political tradition, our democratic constitution seems to he a superficial graft. But we have had some deomocratic heritages in our history. The thirst task of the Korean elites is to systematize those heritages as a real ideology. Especially, the fact that we have faced to the communist challenge makers it inevitable. And our democratic education must be accomplished with critique on the Communism. Secondly, it is another task of Korean elites to solve the convicts and to integrate politically through educational activities. National patriotism would he in direct proportion to the ratio of literacy. Therefore it is impossible to impose the educational activities to the educators only, as it were, elites should take a part in educational problems. And it musts be remembered that all intellectual activities should be shaded within context of tradition. Thirdly, Korean elites must have regard to the nationalism. Political, economic and ideological unit of today is nation. The nature of nation is not the objective elements as territory or blood but the subjective one as a "state of mind." Therefore in nationalist movement, the leading elites are indispensible more than in any other gorups. Because national sentiment has high suggestibility. [4] In discussing the problems of elites, we can hardly turn our faces from the military as their substitute. As recently as 1930's military government was identified as the ultimate type of totalitarianism. But military industrialism during and after the WW II promoted their states. As Amos Perlmutter suggested, the army's presence in civilian affairs indicates that the existence of corruption is not expected to disappear in the near future ; that material improvements and ideological perspectives do not match; that traditional institutions are unable to bring about material improvement ; that modernized elites are incapable of establishing political institutions and structures which will sustain the momentum of social mobilization and modernization. The roles which military must play in Korea can be classified to four points : First, technical improvement and constructive business, secondly, application of personnel management and staff function, which they obtained in army services, to civilian affairs after the discharging, thirdly, getting training in citizenship, and lastly, the fact that the military has been the traditional bulwark of anti-communist crusades. But military intervention into the civilian affairs may encounter with dilemmas because that religious groups to not often agree on the military activities : that immoderate investment to the military affairs may make national economy lose its equiliibrium: that military intervention in one country encourages intervention by the armed forces of other countries in their own political systems; and the propensity for a military intervention is likely to decrease with increased social mobilization. [5] The main task of elites in Korea is, in conclusion, to accomplish the political integrity and political development by solving the conflicts remaining under the mass consciousness. For these aims, first, elites should stress the importance of consensus and maximize uniformity, secondly, they should emphasize the interplay of individual and group interests by maximizing the integrative values, which are main coals of elites' role.
