재발하는 심실 빈맥으로 내원한 환자에서 Sheehan 증후군을 진단하였고, 적절한 호르몬 보충 치료 후 심전도 이상이 정상으로 회복된 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Torsades de Pointes is a form of polymorphic ventricular tachycardia that is associated with prolongation of QT intervals, characterized by QRS complexes of progressively changing ampulitude and cyclic length, giving the appearance of oscillations around the baseline. A 77 years-old woman was presented with hypotention and arrhythmia. Twenty four-hour ECG showed typical episodes of torsades de pointes with QT prolongation. This arrhythmia was controlled by isoproterenol and magnesium infusion, but it was not disappeared. She was diagnosed as Sheehan's syndrome by past history, anterior pituitary stimulation test and empty sella on CT scan. After hormone replacement treatment, Prolonged QT interval was normalized and polymorphic ventricular tachycardia has not recurred. We report a case of torsades de pointes associated with Sheehan's syndrome
조선후기 서울 포전(布廛)이 남긴 기록의 조각들 -"정승모 문서"의 소개와 분류-

이 글에서는 故 정승모 박사가 발굴한 조선후기 서울 布廛의 문서를 소개하고, 그 식별 및 분류에 관해 試論하였다. 향후 여러 가지 측면에서 심화 연구가 이루어질 수 있도록, 書誌的 특징을 포함하여 검토하였다. 40여 장의 斷片으로 구성된 ‘정승모 문서’는 조선후기 서울의 포전 조직에서 작성한 것으로서, 18세기 후반에서 19세기 후반 사이에 생산된 문서가 뒤섞여 있음을 밝혔다. 포전의 삼베 구매 및 분배와 관련된 문서 중에는 공동 구매에 참여하여 지분(깃)을 행사한 포전 구성원이 나열된 사례가 있는데, 그 기록의 순서는 牛耕式을 따르고 있었다. 인명의 기재 양식에 대한 파악은 조직의 인적 구성을 비롯한 다각도의 후속 연구를 위해 반드시 선행되어야 하는 문제인데, 이글에서의 실증 분석을 통해 해결된 것이다. 삼베의 상납 및 운반과 관련된 문서의 이해는 그리 쉽지 않으나, 대략 다음과 같은 다섯 가지의 경로를 상정해 볼 수 있었다: (1) 포전에 속한 개별 상인에 대한 공급, (2) 고위 관료에 대한 衣資布 상납,(3) 민간을 대상으로 판매한 포전 건물[房]에의 공급, (4) 중앙 관청에의 貢布 상납, (5) 貢人에 대한 공포 공급. 삼베를 공급한 다음의 대금 정산은 별도의 기록을 통해 이루어지고 있었음이 확인되었다. 이 글에서, 비록 충분한 정보가 없는 상황임에도 여러 가지 추론을 통해 ‘정승모 문서’의 분류안을 제시한 결과, 다음과 같은 두 가지 효과를 기대해 볼 수 있다. 첫째, ‘정승모 문서’ 및 각종의 시전 관련문서에 대한 심층적 이해 및 실증 분석이 가능할 것이다. 둘째, 앞으로 이와 유사한 문서가 발굴될 경우에 ‘정승모 문서’의 분류안을 일반적 지침으로 활용할 수 있을 것이다. Ten years ago, a set of documents and scraps of the hemp merchant guild (p``ojn) in Seoul was introduced by the late Dr. Chung, Seung-Mo, a historical folklorist. This study attempts to identify each document and scrap by classifying all the materials. Various aspects, including bibliographic items, were discussed. The documents and scraps are assumed to have been written between the late 18th and late 19th centuries. Empirical analysis in this study reveals that the entries of the p``ojon members who joined each joint venture were written in ``boustrophedon.`` Using this knowledge, it is possible to perform further research, including personnel setup. Documents and scraps related to the procurement and supply of hemp cloths enabled us to suggest the five paths of distribution, namely, (1) supply for the individual members of the p``ojon guilds, (2) procurement for bureaucrats`` clothing, (3) supply for retail shops, (4) procurement for government offices, and (5) supply for tribute merchants. Despite the limited information, a categorization candidate was proposed through inductive reasoning. This proposal can serve as a guideline for the classification of new documents or scraps in the future.

석청에 포함된 grayanotoxin에 의해 독성 작용이 나올 수 있으며, 이러한 독성 작용은 일반적으로 24시간 이내 저절로 회복된다. 최근에 본 저자들은 석청 복용 후 발생한 심실빈맥을 경험하였기에 보고하는 바이다. 49세 남자가 호흡곤란으로 내원하였으며, 내원당시 분당 40회 정도의 동성 서맥이 관찰되었고, 수액을 투여하던 중 심실 빈맥이 발생하였다. 항부정맥제를 투여 후 정상 동율동으로 전환되었으며, 특별한 이상 없이 4일 후 퇴원하였다. Mad-honey intoxication caused by the consumption of honey producted from the nectar of rhododendrons. The grayanotoxins cause the intoxication. The toxic effects of mad-honey poisoning are rarely fatal and generally last for no more than 24 hours. We experienced one case, a 49 years-old man who presented with dyspnea after ingestion of mad-honey. He showed marked sinus bradycardia with < 40 beats per minute on admission. The cardiac rhythm was changed to ventricular tachycardia immediately. These features resolved completely in 24 hours with continuous infusion of amiodarone(600 mg per day) and fluids. We report the case of intoxication of mad-honey as a presentation of fatal cardiac arrhythmia.

3-Monochloro-1,2-propanediol(3-MCPD) is a toxic compound, often present in different foods containing acid hydrolyzed(AH) protein, like seasonings and savory food products. The purpose of the present study was to investigate the effects of 3-MCPD on male fertility, sperm and testosterone secretion. In vivo male fertility test was performed for observing the adverse effects of 3-MCPD on the function of male reproductive system and pregnancy outcome. 0.01, 0.05, 0.25, 1 and 5 mg/kg b.w. of 3-MCPD was given daily by gavage to groups of 15 adult male SD rats for 4 weeks. At the end of pre-treatment period, males were mated overnight with normal females. Following morning, males demonstrating successful induction of pregnancy were sacrificed on that day to assess<br/> sperm parameters and histopathology of reproductive organs. The resulting pregnant females were sacrificed on day 20 of gestation to evaluate pregnancy outcome. As a result, four-week paternal<br/> administration with 3-MCPD resulted in adverse effects on male fertility and pregnancy outcome without remarkable histopathological changes in testes and epididymides; sperm motility, copulation index and fertility index were markedly decreased in the treated group and numbers of live fetuses showed<br/> steep dose-response curves. Also, spermatogenesis was investigated in this experiment. However, no effect was observed on production of sperm in testes treated with 3-MCPD for 4 weeks. Hormone assay was performed for observing the effects of 3-MCPD on testosterone and luteinizing hormone (LH) in blood and testes of male SD rats and cultured primary Leydig cell. In result, significant changes of related hormones did not observed by treatment of 3-MCPD. These results indicated that paternal treatment with 3-MCPD induced spermatotoxic effect, which caused an antifertility on male.
