        임신부의 인플루엔자 백신 접종률 및 관련요인

        Background : During seasonal influenza epidemics and previous pandemics, pregnant women have been at increased risk for complications related to influenza infection. Although influenza vaccination has been widely recommended to pregnant women and immunocompromised hosts, the vaccination rate is presumed to be low in pregnant women. This study was aimed to evaluate the vaccination rate and factors associated with Influenza vaccination among pregnant women, who are at high risk for developing complications from the influenza. Materials and Methods : This study was carried out by telephone survey from April, 2007 to August, 2007. Women who were in their second trimester of pregnancy or above,- during October, 2006 and February, 2007, the recommended vaccination period, and had given birth at Inha University Hospital were selected as the study population. Immunization status, general understanding, and factors associated with vaccination were evaluated. Results : Among a total of 506 eligible pregnant women, 227 (44.8%) responded to the questionnaires. The influenza vaccination rate among the pregnant women was only 4% (9/227). Major reasons for not receiving vaccination were first, not awaring the necessity of immunization during pregnancy (48.5%, 110/227) and second, misunderstanding that it is prohibited during pregnancy (36.1%, 82/227). The major factors that influenced the compliance of vaccination were correct understanding and doctor's recommendation on the influenza immunization during pregnancy. Conclusions : The influenza vaccine coverage on pregnant women was very low and the awareness of influenza vaccination during pregnancy in general was inaccurate. Shift in perception is required and this can be achieved by educating the pregnant women on the safety and efficacy of vaccination. In addition, doctors' active encouragement to vaccination is highly recommended.

      • Rocuronium의 애별투여방법이 모지내전근의 이완에 미치는 영향

        Objective : There are controversies in pnmmg technique of rocuronium whether it can or can't shorten the onset time. We want to compare the effects of two different priming doses and intervals of rocuronium. Materials and Methods : After giving propofol, 50 patients were randomly assigned to 5 groups. Group I received a placebo, followed 3 min later by rocuronium 0.6 mg/kg. Priming doses and intervals of Group II, Ill, N, V were 0.06 rng/kg and 2 min, 0.12 mg/kg and 2 min, 0.06 rug/kg and 3 min, 0.12 mg/kg and 3 min, respectively. Total dose of rocuronium was 0.6mg/kg. Train of four (TOF) count or ratio and 1st twitch height of TOF (T1) were monitored with 15 secondsintervals. TOF ratio of each groups just before bolus injection, duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were compared. Results : Duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were decreased in priming groups compared to Group 1 (P < 0.01) In group V, significant depression of TOF ratio just before bolus injection was seen (P < 0.05). Conclusion : Priming technique with rocuronium is effective method to shorten the onset of neuromuscular block. But priming dose of rocuronium itself could cause hypoxia and aspiration in awake patients and should be used carefully. Priming dose of rocuronium 0.06 mg/kg and 2 minutes interval maybe relatively safe method.

        A Case of Erythema Multiforme A ssociated with Malignant Lymphoma Un Cheol Yeo, M.D., Kwang Hyun Cho, M.D., Yoo Shin Lee, M.D., Dae Seog Heo*, M.D., Seoung Chul Lee**, M.D Department of Dermatology and Internal Medicine*, College of Medicine, Seoul National University, Seoul, Korea, Department of Dermatology**, College of Medicine, Inha University, Kyung Gi Do, Korea We report a case of erythema multiforme associated with malignant lymphoma. Fifty-six-year-old male patient visited our clinic complaining generalized skin lesions which were compatible with erythema multiforme clinically and histopathologically. The skin lesions were treatment-resistant, recurrent and persistent for 3 years. In consideration of old age, atypical features of erythema multiforme and no other demonstrable etiology of erythema multiforme, we studied for internal malignancy. Malignant lymphoma at the duodenum was revealed by UGI series, abdominal CT, and endoscopic biopsy. After chemotherapy, the tumor mass and active skin lesions were cleared simultaneously. Erythema multiforme may be regarded as one of the paraneoplastic syndromes.

      • 인증서 검증서버의 인증경로 생성 메커니즘에 관한 연구

        PKI core basic architecture for securing informations have to minimize client's burden to generations of authentication paths. Also, It must be solved the lack of timing and managements in the CRL, inter-operations among PKI domains according to its environment. To remove problems in the existing PKI, the validation server solves problems of timing for validating authentications and CRL using OCSP, supports inter-operation of PKIs, arid manages the authentication strategy of inter-domain. This paper proposes fast and effective the generation method of authentication path in the validation server. Because it generates previously and saves the authentication path, it can give the authentication path fast on requiring a validation. With saving the used path, it provides effective the authentication path by reusing the saved path.

        액체-고체 순환유동층의 상승관에서 액상의 점도가 고체체류량 및 열전달계수에 미치는 영향

        직경이 0.102 m이고 높이가 3.5 m인 액체-고체 순환유동층에서 액체의 점도가 고체입자의 체류량과 열전달계수에 미치는 영향을 검토하였다. 밀도가 2,500 kg/㎥인 유리구슬(d_(p)=1.0, 1.7, 2.1 또는 3.0 mm)과 CMC 용액(μ_(L)=0.96-38 mPas)을 각각 고체상과 액체상으로 사용하였다. 액체-고체 순환유동층에서 고체입자의 체류량은 액체의 유속이나 점도가 증가함에 따라 감소하였고 유동입자의 크기나 입자의 순환속도가 증가함에 따라 증가하였다. 액체-고체 순환유동층에서 열전달계수는 액체의 점도가 증가함에 따라 감소하였고 유동입자의 크기나 순환속도가 증가함에 따라 증가하였으나, 액체의 유속이 증가함에 따라서는 큰 변화가 없었다. 액체-고체 순환유동층에서 고체입자의 체류량과 열전달계수는 다음과 같이 실험변수와 무차원군에 의해 상관식으로 얻을 수 있었다. E_(S) = 0.783U^(-0.100)_(L)G^(0.164)_(S)d^(0.302)_(p)μ^(-0.036)_(L) h = 4349.3U^(0.010)_(L)G^(0.099)_(S)d^(0.151)_(p)μ^(-0.015)_(L) ◁그림삽입▷ Effects of liquid viscosity on the solid holdup and heat transfer coefficient have been investigated in the riser of a liquid-solid circulating fluidized beds, whose diameter is 0.102 m and 3.5 m in height. Glass beads (d_(p)=1.0, 1.7, 2.1 or 3.0 ㎜) whose density is 2,500 ㎏/㎥ and aqueous solutions of carboxymethy cellulose (CMC)(μ_(L)=0.96-38 mPas) have been used as the solid and liquid phase, respectively. It has been found that the solid holdup decreases with increasing liquid velocity or viscosity, but it increases with increasing solid particle size or solid circulation rate. The heat transfer coefficient decreases with increasing liquid viscosity but it increases with increasing particle size or solid circulation rate, however, it does not change considerablely with increasing liquid velocity.       • 다구찌 방법과 실험계획법을 이용한 농기계용 소음기의 최적화에 관한 연구

        본 연구에서는 배기계의 소음 조감을 위한 소음기의 성능향상에 대한 연구를 수행하였다. 기존의 파동방정식에 의한 전달 매트릭스법(transfer matrix method)과 유한요소법(FEM) 대신 다구찌 방법과 일부실시법을 이용하여 오버올 레벨을 최대로 하는 최적조건을 찾아내었다. 기존설계안을 바탕으로 성능에 영향을 미친다고 생각되는 각 부품의 길이와 반경등을 인자로 선정하였다. L18, L27 직교배열표를 사용하여 인자를 배치하였으며 1/3 일부실시법을 통하여 어떤 교호작용이 유의한지를 찾아내었다. 소음·진동제어연구실에서 개발한 LAEXS라는 시뮬레이터를 이용하여 시뮬레이션을 수행하였다. 이를 통하여 찾아낸 최적조건에서 기존의 설계안보다 오버올 레벨은 26.9 dB, SN비는 1.39dB 증가하는 효과를 가져왔다. In this study, we accomplished research of improving the muffler's capacity for reduction noise of exhaust system. Instead of using transfer matrix method by wave equation and finite element methods, we find optimal condition that maximizes overall level by using Taguchi methods and fractional factorial design. On the basis of existing design, we selected factors such as length and radius of each component that was thought to effect to capacity of muffler, factors are arranged by using L18, L27 table of orthogonal array, also, we found some significant interaction effects by using 1/3 fractional factorial design. For this simulation, we use the LAEXS which was developed in the laboratory of noise and vibration control. From the simulation results, compared with the existing design, overall level increased 26.9 dB and SN ratio increased 1.39 dB.

      • Styryl diphenyl phosphine oxide의 가수분해 반응메카니즘에 관한 연구

        The rate constants for the hydrolysis of p-substituted styryl diphenyl phosphine oxide(p-H, p-Cl, p-CH_3, p-OCH_3) were determined at various pH and rate equations which can be applied over wide pH range were obtained; below pH 5.0 the rate of hydrolysis of p-substituted styryl diphenyl phosphineoxide were catalyzed by hydronium ion and above pH 8.0, the specific base catalysis were observed. However, in the range of pH 5.0-8.0 the rate of hydrolysis were independent on the pH change andno general base catalysis were observed at pH 4.75. The substituent effects on the hydrolysis of styryl diphenyl phosphine oxide derivatives were also studied. On the basis of these results, the hydrolysis mechanism of p-substituted styryl diphenyl phosphine oxide were proposed.

        As it is well known the SILHAK(實學) was new academic wave of the latter period of the Chosun Dynasty. Soonam studied under the scholarship of SUNGHO(星湖:Yi ICH 李瀷 1681~1763,A.D.) who was the root(origin)of 'GENKi school'(近幾學派) within the 'KYONGSEeSHiYONG' school'(經世致用學派)^1) & 'YiYONGHOOSEANG' school'(利用淳生學派)^2) He was of the great conservative right wingers(守舊右派→左派:權哲身) and he was an extensive reader whose reading ranged over all the available books of his times. According to the catalog of his books: "SOONAM CHUNSEO HAEJE"(順 全書 解題: THE SOONAM BIBLIOGRAPHY DAEDONG CULTURALI(NSTITUTE, 1970, YUKANG PUBUSHING C0.,1984), he al1mored over 48 kinds in Books. That was the kind of scholar he was. From the depth and broad vision of his scholarship came the great achievement in philosophy of law. He cherished more than anything else 'HOKYUN'(好均:Equality by sharing) and 'KYESUN'(繼善:continuous in goodness)in 'SANGSUNYAKSOO'(上善若水: The best of goodness〈in the world〉is like water). The essentials of law(要法) of local autonomy deals with six areas. They are; ① Public welfare, ② Educational Code, ③ Legal Service, ④ Crime Prevention, ⑤ National Defense, ⑥ Kingship, etc. Somam authored a book named 'IMKWANJUNGYO' (臨官政要:Commissioned officers essentials in administration) The book was inspired by 'BOKAP LAW'(保甲法: Y00 10NG J00 劉宗周) and his insight of the latter period of the Chosun dynasty. TO the judicial and administrative officers (居官;KU KWAN) of those times, the book was an important guideline of administration. In the book he said, first(I) officers should work with wisdom, temperance and humility for ① Diligence in crime provention ② Prudence in punishment ③ Legal service, meaning eradicate all the hassels of legal action. secondly(Ⅱ). Superior officers' attitudes in leadership should be ① Think of the public before expressing the emotion(公意識:Public Consciousness) ② Work for public interest and Purification of society(淸濂) ③ Study all the law and its principles and practice with justice(明) ④ Face the pecoeple and subordinates with dignity and courtesy(威). In his other works, Soonam expanded 'HYANG YAK'(鄕約:village Discipline), to 'HYANG SA BUP'(鄕社法:community law) First(I). The political articles of 'HYAKGSA BUP are; ① Mutual watch of fire(火燭相戒) ② Mutual sharing of accidents(憂愚相恤) ③ Mutual sharing of congratulation(喜慶相賀) ④ Mutual watch against crime(盜賊相禦) ⑤ Mutual fear of law·(法令相畏) ⑥ Mutual prudence in Tax collecting and Payment(租賦相護) Secondly(Ⅱ). The Educational codes of 'HYANGSA BUP' are; ① Being dutiful to one's parents(孝順父母) ② Being respeclful to the elders(尊敬長上) ③ Being instructive to one's youngsters(敎訓子弟), ④ Being harmonious in local village(和陸賦里) ⑤ Being stable into one's living(各安生理〈計)) ⑥ Being without any falsehood(母作非僞). The educational codes reminds us of the phrase 'KYONG NO HYO CHIN'(敬老孝親) of 1985. Of course the Soonam's thought in this matter is much more clear and deep. They both could be used as basis for practice in law and ethics. In practice of political articles and educational codes Soonam believed in natural effective application rather than unnatural artificial system. And his self-regulating method was highly successful in; ① Judicial administration(形政) ② General administration (行政) ③ Tax collecting administration(租政) ④ National defense administration(國防) etc. It is my earnest hope that the Soonam's Philos9phy of law and his thought in law would be applied as the "Guide1ine for moral nature"(心性指針) as well as the promotions and further research for his spirit of law in the recent history of Korean thought in law.
