        수술실 간호사의 경력개발시스템 개발

        Purpose: This study was a methodological research conducted to develop a clinical ladder system for operating nurses. Methods: Participants were 20 OR nurses, working in C Hospital, who had a mean tenure of 6 years and 10 months. Data collection consisted of 4 focus group interviews during May and June 2009. The content analysis method of Kim and Lee (1986) was used to analyze the data. Two clinical expert groups consisting of 16 nurses verified the content validity of the preliminary system from September 16 to 26, 2009 using Kim's tool (1999). Results: The final clinical ladder system consisted of goals, core values, and 4 domains of practice related to core values, which were defined as professional value, perioperative nursing practice, education/research, and collaboration/leadership. Eleven nursing competencies and 44 behavior indicators were included in accordance with the 4-step ladder. The 4 operation systems for the clinical ladder system were the promotion system, continuous learning system, reward system, and support system. Conclusion: The results indicate that nursing managers need to pay more attention to developing a clinical ladder system for nurses.

      • High metabolic tumor volume and total lesion glycolysis are associated with lateral lymph node metastasis in patients with incidentally detected thyroid carcinoma

        <P><B>Objective</B></P><P>The objective of this study was to investigate whether total lesion glycolysis (TLG) and metabolic tumor volume (MTV) measured by <SUP>18</SUP>F-fluorodeoxyglucose (<SUP>18</SUP>F-FDG) positron emission tomography/computed tomography (PET/CT) could predict the aggressiveness and lymph node metastasis (LNM) in patients with incidentally detected differentiated thyroid carcinoma.</P><P><B>Methods</B></P><P>A total 358 patients with focal FDG-avid thyroid incidentaloma during cancer evaluation were enrolled. Among 235 patients in whom fine-needle aspiration biopsy was performed, 51 patients underwent total thyroidectomy with LN dissection. We analyzed the relationship between volume-based parameters and clinicopathologic characteristics.</P><P><B>Results</B></P><P>The mean age and tumor size were 57.1 ± 11.3 years and 1.15 ± 0.81 cm, respectively. The prevalence of malignancy was 21.7 % (51/235). When SUV<SUB>max</SUB> > 5.91, MTV2.5 > 2.05 cm<SUP>3</SUP>, and TLG2.5 > 9.09 were used as cutoff points, sensitivity, specificity, and area under curve (AUC) for prediction of lateral LNM were 77.9, 69.1 %, 0.716 (<I>P</I> = 0.047), 77.8, 88.1 %, 0.839 (<I>P</I> < 0.001), 77.8, 85.1 %, and 0.815 (<I>P</I> = 0.002), respectively. However, MTV and TLG had no value in prediction of central LNM, extrathyroidal extension, and multifocality. On comparison ROC curve analysis, the MTV and TLG showed the statistical differences for the prediction of lateral LNM compared with SUV<SUB>max</SUB> (all <I>P</I>’s < 0.05).</P><P><B>Conclusions</B></P><P>This study has shown for the first time that volume-based PET functional parameters had a significant value for the prediction of lateral LNM in incidentally detected PTC. These results suggest that higher MTV and TLG can be potential new risk factors for preoperative risk stratification. The usefulness of TLG and MTV in preoperative risk stratification in patients with PTC needs to be confirmed in further large studies.</P>

        Analysis of Socio-Cultural Attitude towards Appearance and Related of MZ Generation Using Potential Profile Analysis

        Purpose: The purpose of this study is to examine and understand the types of consumers in their teens through their 30s who care for their appearance by analyzing the socio-cultural attitude types and related factors towards appearance using the potential profile analysis, and provide a practical assistance for marketing in line with the consumer type. Method: In this study, 520 online questionnaires were collected from among those in their teens and their 30s, and using SPSS 25ver and M-plus 7.0 programs, the basic analyses including frequency analysis, descriptive sta-tistical analysis, etc., as well as appearance using potential profile analysis were performed while cross-analysis and variance analysis were performed to identify differences in major variables according to the socio-cultural attitude type related classification and derived types. Results: First, as a result of the potential profile analysis, 4 groups were found to be the most appropriate, and the graphs were named 'overall low group', 'media awareness centric group', 'overall high group', and 'other perspective centric group'. As the main results of the difference test, the older the age group and the more office workers, the more likely they were to belong to the group that emphasized the point of view of others, while the interest in skin beauty and appearance were highly likely to belong to the general high-class and media aware-ness-centric group. That is, in the Korean society, there is a culture that emphasizes how you appear to others rather than yourself as you get older and enter social life, but this tendency does not have a positive effect on your interest in skin care or appearance. Whereas, such tendency can be interpreted as not having a positive effect on interest in skin beauty or appearance. Conclusion: According to the results of this study, the age group which entered society and occupation, etc. are highly likely to belong to the other-centric group because appearance is important, but the interest in ap-pearance and skin beauty tends to be higher in the media recognition-centric group. It can be seen that when it comes to taking care of one's appearance, it is more important for one to recognize the necessity through the media than to emphasize the interest of others. Hence, through this study, it is intended to be used as a promo-tional marketing strategy and provide services desired by consumers to increase their satisfaction and assistance lead to their reuse.

        Prognostic value of volumetric parameters measured by F-18 FDG PET/CT in surgically resected non-small-cell lung cancer

        OBJECTIVES: The aim of this study was to evaluate the usefulness of the tumor burden as characterized by the metabolic tumor volume (MTV) and total lesion glycolysis (TLG) measured by F-18 fluoro-2-deoxyglucose (F-18 FDG) PET-computed tomography (CT) in predicting recurrence-free survival (RFS) and overall survival (OS) in surgically resected non-small-cell lung cancer (NSCLC) patients. METHODS: We retrospectively reviewed 91 patients with pathologically documented stages I–IIIA NSCLC. MTV and TLG were obtained according to various thresholds of the standard uptake value (SUV) of primary tumor using preoperative F-18 FDG PET-CT. We used comparison receiver-operating characteristic curve analysis to test the statistical significance of the differences among the multiple volumetric parameters calculated by various SUV cutoff values. RFS and OS were evaluated with the Kaplan–Meier method and Cox regression analysis. RESULTS: On comparison receiver-operating characteristic curve analysis, no significant difference was found among the volumetric parameters calculated using various thresholds of SUV. Regardless of the thresholds, patients with smaller MTV and lower TLG showed longer RFS and OS. MTV and TLG measured by F-18 FDG PET-CT were found to have better predictive performance than SUVmax for recurrence and death. According to multivariate analyses, MTV2.5 was revealed as a significant prognostic factor for RFS. Tumor size over 3 cm was selected as a significant prognostic indicator of OS. CONCLUSION: Volume-based parameters of F-18 FDG PET-CT may have a role in providing prognostic information in NSCLC patients who have received surgical treatment.

        The Mediating Effect of Socio-Cultural Attitude towards Appearance on the Appearance Interest in the Relationship between Acne Perception and Appearance Satisfaction

        Purpose: The purpose of this study was to examine and understand the mediating effect of socio-cultural attitude towards appearance on the appearance interest in the relationship between acne perception and appearance satisfaction, and provide a practical assistance for the programs for the acne skin in the beauty industry and market. Method: In this study, the statistical analysis was conducted using the SPSS 25.0 and AMOS 22.0 programs of 282 online questionnaires for the subjects with acne skin. Results: As for the results of this study, the socio-cultural attitude towards appearance has had a positively significant impact on the appearance interest(β=.453, p<.001), acne perception(β=.149, p<.05), and appearance satisfaction(β=.535, p<. 001), and the direct effect of socio-cultural attitude towards appearance on the appearance satisfaction was negatively significant(β=-.362, p<.001), while having a positively significant effect on the acne perception(β=.149, p<.05). Furthermore, in the relationship between the socio-cultural attitude towards appearance and appearance satisfaction, the interest in appearance demonstrated a positive mediating effect. This is a result demonstrating that if a person's socio-cultural attitude towards appearance is large, he or she can directly cause dissatisfaction with one's appearance, yet if one's interest in the appearance is raised before then, the appearance satisfaction might also be increased as a result. Conclusion: Based on the results of this study, it was confirmed that the larger the socio-cultural attitude towards the appearance, the larger the appearance interest and acne perception scores, and the larger the appearance interest, the larger the appearance satisfaction. Meanwhile, the socio-cultural attitude towards appearance negatively and significantly impacts the appearance satisfaction. Hence, it is apparent that the overall appearance is important as it impacts the appearance interest, not the skin type, and when consulting with the customers with acne skin, it is necessary to present the direction of interest in the appearance and suggest that it is also important to accurately perceive one's own skin condition. Through which, it is intended that the direction of social and cultural attitude towards appearance and interest in the appearance are presented to provide the services desired by consumers to increase their satisfaction with their appearance, and furthermore, provide assistance for the practical on-site management programs and counseling skills for acne skin in the medical skin care beauty industry and market.

        The Effect of Macrolide Therapy on Bronchopulmonary Dysplasia in Ureaplasma-Positive Very Low Birth Weight Infants

        Objective: Ureaplasma has been demonstrated the cause of short and long-term morbidities of preterm infants, especially such as bronchopulmonary dysplasia (BPD). This study aims to evaluate the influence of Ureaplasma on neonatal morbidities and the effect of macrolide treatment in very low birth weight (VLBW) infants. Methods: We performed a retrospective review of clinical records of VLBW infants born between 2015 and 2016. Their endotracheal aspirate and gastric juice were obtained immediately after birth and tested for Ureaplasma. Therapeutic macrolides were administered according to the clinical judgment, not routinely, in Ureaplasma-positive infants. Neonatal morbidities were compared using individual matching analysis between Ureaplasma-positive and negative groups, and with macrolides administration. Results: A total of 144 infants with the mean (±standard deviation) gestational age of 28<sup>+3</sup> (±3<sup>+3</sup>) weeks and birth weight of 1,051.9 (±290.0) g were included, and 30 (20.8%) were Ureaplasma-positive. Ureaplasma-positive group was associated with higher incidence of respiratory distress syndrome (RDS, P=0.039) and severe neurologic injury (SNI; intraventricular hemorrhage ≥grade 3 or periventricular leukomalacia, P=0.013). However, Ureaplasma-positive group was not associated with the risk of BPD (P=0.706). In the subgroup analysis for Ureaplasma-positive infants, there was no difference in the incidence of morbidities according to the macrolides administration. Conclusion: Although Ureaplasma has no independent role in the development of BPD, Ureaplasma- positive VLBW infants were more likely to have RDS and SNI. The macrolides administered in Ureaplasma-positive was not effective to reduce neonatal morbidities.

        Design and Methodology for the Korean Observational and Escitalopram Treatment Studies of Depression in Acute Coronary Syndrome: K-DEPACS and EsDEPACS

        Depression is common after acute coronary syndrome (ACS), adversely affecting cardiac course and prognosis. There have been only a few evidence-based treatment options for depression in ACS. Accordingly, we planned the Korean Depression in ACS (K-DEPACS) study, which investigated depressive disorders in patients with ACS using a naturalistic prospective design, and the Escitalopram for DEPACS (EsDEPACS) trial, which assessed the efficacy and safety of escitalopram for treating major or minor depression in patients with ACS. Participants in the K-DEPACS study were consecutively recruited from patients with ACS who were recently hospitalized at Chonnam National University Hospital, Gwangju, South Korea. Diagnoses were confirmed by coronary angiography from 2005. Data on depressive and cardiovascular characteristics were obtained at 2 weeks, 3 months, 12 months, and every 6 months thereafter following the index ACS admission. The K-DEPACS participants who met the DSM-IV criteria for major or minor depressive disorder were randomly assigned to groups in the 24-week, double-blind, placebo-controlled EsDEPACS trial beginning in 2007. The outcome of treatments for depressive and other psychiatric symptoms, issues related to safety, including general adversity, and cardiovascular factors were assessed. The K-DEPACS study can significantly contribute to research on the complex relationships between depression and ACS. The results of the EsDEPACS trial provide an additional treatment option for clinicians treating these patients.

        A report of 38 unrecorded bacterial species in Korea within the classes Bacilli and Deinococci isolated from various sources

        A total of 38 bacterial strains within the classes Bacilli and Deinococci were isolated from various sources in Korea. Samples were collected from animal intestine, urine, soil, tidal flat mud, and kimchi. In the sequence comparison and phylogenetic analysis of 16S rRNA sequences, the 38 isolates were assigned to the classes Bacilli and Deinococci with sequence similarities more than 98.7%. Twenty-four strains and 13 strains were classified the order Bacillales and Lactobacillales in the class Bacilli, respectively. In the order Bacillales, there were nine species in the genus Bacillus, seven species in the genus Paenibacillus, and the remaining eight species in the genera Domibacillus, Halobacillus, Virgibacillus, Lysinibacillus, Paenisporosarcina, Planococcus, Savagea, and Staphylococcus. In the order Lactobacillales, there were four species in the genus Lactobacillus, three species in the genus Leuconostoc, three species in the genus Lactococcus, and the remaining three species in the genera Aerococcus, Enterococcus, and Streptococcus. One species was related to the genus Deinococcus of the order Deinococcales. Most of the isolated strains were Gram-stain-positive, but some were Gram-stain-variable or Gram-stain-negative. Cells were rod or cocci-shaped. Based on the results of 16S rRNA analysis, we report 38 strains as previously unrecorded species to Korea, and the basic characteristics of strains are described herein.
