Low-FODMAP Formula Improves Diarrhea and Nutritional Status in Patients Receiving Enteral Nutrition

Background & Aims: Fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAPs) are poorly absorbed, short-chain carbohydrates that play an important role in inducing functional gut symptoms. A low-FODMAP diet improves abdominal symptoms in patients with inflammatory bowel disease and irritable bowel syndrome. However, there is no clinical intervention study for the effect of FODMAP content on gastrointestinal intolerance (GI) and nutritional status in patients receiving enteral nutrition (EN). Therefore, we investigated the effect of EN formulas containing different FODMAP contents on GI and nutritional status in patients receiving EN through the randomized, multicenter, double-blind, 14-day clinical trial. Methods: Eligible hospitalized patients receiving EN (n=95) from 10 hospitals in Busan and its environs were randomly assigned to three groups; 84 patients completed the trial (low-FODMAP EN, n=30; moderate-FODMAP EN, n=28; high-FODMAP EN, n=26). Anthropometric and biochemical parameters were measured; stool assessment was performed using the King’s Stool Chart and clinical definitions. Results: Baseline values were not significantly different among the three groups. After the 14-day intervention, diarrhea was significantly improved in the low-FODMAP group (73.3%) than in the moderate- (33.3%) and high-FODMAP (30.8%) groups (P=0.046). King’s Stool scores were significantly reduced in the low-FODMAP group compared with the other two groups (P<0.05). BMI increased significantly in the low- and high-FODMAP groups over the course of the intervention (P<0.05 for both), and showed a trend toward increasing in the moderate-FODMAP group (P<0.10). Serum prealbumin increased significantly in all groups by day-14; by day-3, it had increased to the levels at day-14 in the low-FODMAP group. At day-14, serum transferrin had increased significantly in the moderate-FODMAP group. When subjects were classified by condition (unimproved, no-symptom, diarrhea-improved, and constipation-improved), BMI increased significantly in all groups except the unimproved. Prealbumin levels significantly increased in the diarrhea-improved group at day-3 and day-14, and in the constipation-improved group at day-14. Transferrin levels significantly increased in the diarrhea-improved group at day-14. Conclusion: Low-FODMAP EN may improve diarrhea, leading to improved nutritional status and facilitating prompt recovery from illness.
S-539 : A Case of Oral Allergy Syndrome caused by Crown Daisy and Sesame

Background: Oral allergy syndrome (OAS) is a subtype of food allergy, and has been commonly associated with pollen allergy. Crown daisy and sesame leaf are popular vegetables in our country, but food allergy related with these two vegetables has been rarely reported. Here, we experienced a rare case of OAS induced by both crown daisy and sesame leaf, and evaluated their pathogenic mechanisms. Case history: A 33-year-old female visited our clinic to evaluate numbness of tongue and gingiva after eating fresh crown daisy and sesame leaf for 2 years. The patient had suffered from allergic rhinitis and atopic dermatitis for 20 years and took medications intermittently. She had also a history of food allergy to crab and shrimp. Results: White blood cell counts were 8,600/μL (eosinophil 6.5%) and other routine laboratory tests were unremarkable. Total serum IgE level was 404 KU/L with 68.50KU/L of D.pteronyssinus specific IgE and 69.40 KU/L of D. farinae specific IgE, measured by immunoCAP®. Skin prick test showed strong positive reactions to tree and weed pollens-Hop Japanese (8+), pine (5+), plane tree (3+), sh and willow (3+). Serum specific IgE to crude extracts of crown daisy and sesame leaf were not detected, while high serum specific IgG4 antibodies to two extracts wered detected by ELISA. Basophil activation test (BAT) was performed with additions of two extracts, together with anti-IgG4, anti-IgE antibody and, Ca2+. The BAT showed significant upregulations of both CD63 and CD203c expressions with additions of both extracts in dose dependent manners. Conclusion: We confirmed a case of OAS induced by both crown daisy and sesame leaf, in which non-IgE mediated mechanisms were suggested based on detection of high serum specific IgG4 antibodies and positive results on BAT to each extract.

Oral allergy syndrome (OAS) is a subtype of food allergy composing of itching sense and edema in the oral cavity, lips, throat, pharynx, and larynx following ingestion of some fresh fruits or vegetables. Although the major pathogenic mechanism of OAS is known to be IgE-mediated response, here we experienced a case of OAS due to crown daisy (CD) and sesame leaf (SL) mediated by a non-IgE antibody mediated mechanism. A 33-year-old female visited our clinic to evaluate numbness of the tongue and gingiva after eating fresh CD and SL for 2 years. The patient had suffered from allergic rhinitis and atopic dermatitis for 20 years and took medications intermittently. There had been a history of food allergy to crab and shrimp. The serum total IgE level was elevated (404 kU/mL). The skin prick test showed strong positive reactions to tree and weed pollens, but not to CD and SL extracts. Enzyme-linked immunosorbant assay for detecting serum specific IgE to crude extracts of CD and SL showed negative results. The basophil activation test performed with crude extracts of CD or SL showed significant up-regulation of CD63-positive basophils by both CD and SL. In conclusion, we report a case of OAS due to CD and SL, not associated with pollen allergy, which is shown to be mediated by a non-IgE mediated mechanism.

Purpose: The clinical interpretation of children sensitized to allergens is challenging, particularly in children with food allergies. We aimed to examine clinical differences between children with monosensitization and those with polysensitization to common food allergens and to determine risk factors for polysensitization in young children <10 years of age with immediate-type food allergies. Methods: The study included children <10 years of age with signs and symptoms indicative of immediate-type food allergies. Serum total IgE level was measured, and ImmunoCAP analysis for food allergens was performed. Results: The mean age of the study subjects was $1.6{\pm}1.6$ years (75 boys and 51 girls). Thirty-eight children (30.2%) were monosensitized and 88 children (69.8%) were polysensitized. Multivariate logistic regression analysis showed that the development of polysensitization to common food allergens was positively associated with a parental history of allergic rhinitis (adjusted odds ratio [aOR], 6.28; 95% confidence interval [CI], 1.78-22.13; P=0.004), season of birth (summer/fall) (aOR, 3.10; 95% CI, 1.10-8.79; P=0.033), and exclusive breastfeeding in the first 6 months of age (aOR, 3.51; 95% CI, 1.20-10.25; P=0.022). Conclusion: We found significant clinical differences between children with monosensitization and those with polysensitization to common food allergens and identified risk factors for the development of polysensitization in young children with immediate-type food allergies. Clinicians should consider these clinical risk factors when evaluating, counseling, treating, and monitoring young children with food allergies.

연구배경 수직적 관계를 형성해오던 기존 오프라인 서점의 가치 변화로 점차 수평적 상호관계를 추구하기 시작했다. 이에 따라 제안한다는 의미를 지닌 큐레이션 개념을 도서공간에 대입하여 나타난 북 큐레이션을 적용하기 시작했다. 북 큐레이션 서점은 서점과 사용자 간의 수평적 상호관계 형성을 통해 지속적인 발전을 보여, 개념이 연결되는 리좀을 적용하여 본 연구는 북 큐레이션 서점 공간에 있어 리좀적 사유가 그 배경이 되는 기초 자료로 활용되고자 하였다. 연구방법 북 큐레이션 서점과 리좀에 대한 이론적 고찰과 선행연구를 통해 공간과 특성을 알아보고, 두 개념의 상관성을 고찰해 연구 가치를 살펴본다. 앞서 북 큐레이션 공간구성과 리좀적 공간 특성을 결합해 분석틀을 추출하고, 사례분석을 통해 구체적인 리좀적 공간 특성을 분석해 정리하였다. 연구결과 북 큐레이션 서점과 리좀은 개념적 맥락을 같이 하고 있으며, 여러 방면에서 상관관계를 가진다. 이에 따른 북 큐레이션 공간분석 분류과 리좀적 공간 특성 선행연구를 통해 도출하여 결합한 것을 분석틀로 삼았다. 프로그램은 다중심성, 비위계성, 비선형성을 중심으로 나타나고, 공간구성 또한 유사한 결과를 나타냈다. 동선은 이질성, 비위계성, 비선형성이 강하게 나타났다. 결론 북 큐레이션 서점의 프로그램, 동선, 공간구성에서 영역별 구성은 높아 사용자의 다양한 마주침을 계획하여 리좀적 공간 특성을 나타내지만, 경계가 짙어 다른 공간으로의 호기심 유발은 약하다. 본 연구는 리좀적 공간구성에 대한 분석과 그 결과에 대한 고찰에 그쳐 한계를 지니는바 추후 북 큐레이션 서점 영역 간의 연결성을 높일 방안에 관한 연구가 필요하다고 사료된다. Background Offline Bookstores have begun to pursue changes in value and horizontal interrelationships. Curation containing the meaning of the proposal was applied to the book space and began to be used as a Book Curation. Bookstore Curation bookstores show continuous development through the formation of horizontal interrelationships between bookstores and users, It wanted to apply the Rhizome linked to the concept of Book Curation to the bookstore space and use it as the basis for the background. Methods Through theoretical considerations and prior research on Book Curation bookstores and Rhizome, we explore space and characteristics and examine the correlation between the two concepts to examine the value of the study. The analysis framework was extracted by combining the spatial composition and characteristics, and the specific spatial characteristics of Rhizome were analyzed and summarized through case analysis. Result Book curation bookstores and Rhizome share conceptual context and are interrelated in many ways. Accordingly, the results of the case analysis using a combination of the analysis framework and characteristics of spatial analysis are as follows. The Program focuses on multicentric, non-hierarchical and non-linearity, and has similar results in Spatial Composition. Circulation showed heterogeneity, non-hierarchical and non-linearity. Conclusion Various user interfaces are planned to reveal spatial characteristics due to the high composition of areas such as Programs, Circulation, and bookstore Space Composition. This research is limited to spatial composition and analysis of research results, and it is necessary to study ways to enhance linkages between bookstores in the future.
