Aims: Tenofovir disoproxil fumarate (TDF) has been considered causing hypophosphatemia as well as proximal tubular toxicity and renal dysfunction. Clinically, many patients treated with TDF experienced various degree of hypophosphatemia, and some of them should be stopped due to severe hypophosphatemia. Therefore we investigated which factors induced moderate hypophosphatemia among patients treated with TDF. Methods: We conducted a retrospective study of chronic hepatitis B patients who were initially prescribed TDF at kosin university hospital, busan, korea from January 2012 to January 2017. Baseline Phosphorus and follow-up Phosphorus levels were compared. The definition of hypophosphatemia is lower than 3.0 mg/dL. Results: 206 patients were treated with TDF. 128 patients were excluded for the following reasons. 59 patients had malignancy (including HCC), 36 patients had baseline Phosphorus check days not within 2 months of the first day of TDF administration, 14 patients were co-treated with TDF and other anti-viral agents, 14 patients were not followed up, and 5 patients had other reasons. Consequently, 78 patients were analyzed in this study. Median follow up duration was 350.5days. Follow up duration is the period from the day of tenofovir initiation to lowest phosphorus level. A total of 50 (64.1%) patients developed hypophosphatemia. There were 28 patients (35.8%) with less than 3 mg/dL, 16 patients (20.5%) with less than 2.5 mg/ dL and 6 patients (7.6%) with less than 2.0 mg/dL. Using univariate analysis, we found that male(HR= 3.397, P=0.022) and use of diuretics(HR= 12, P=0.021) and liver cirrhosis(HR=3.375, P=0.041) were significantly associated with hypophosphatemia. Using multivariate analysis, we found that male(HR= 3.836, P=0.024) and liver cirrhosis(HR= 6.062, P=0.002) was significantly associated with hypophosphatemia. Hypophosphatemia treatment was performed with nuts, dairy, protein intake(34 patients) or IV phosphorus(3 patient), or recommended stop drinking(3patient) and 25 out of 50 patients with hypophosphatemia recovered to normal levels. Conclusions: When using TDF in HBV patients, hypophosphatemia may be more likely to occur in men or in patients with liver cirrhosis. Therefore, in these patients, phosphorus should be closely monitored when TDF is used and appropriate treatment should be performed in case of hypophosphatemia.

<P><B>Abstract</B></P><P>Retinoic acid-inducible gene I (RIG-I) recognizes double-stranded viral RNAs (dsRNAs) containing two or three 5′ phosphates. A few reports of 5′-PPP-independent RIG-I agonists have emerged, but little is known about the molecular principles underlying their recognition. We recently found that the bent duplex RNA from the influenza A panhandle promoter activates RIG-I even in the absence of a 5′-triphosphate moiety. Here, we report that non-canonical synthetic RNA oligonucleotides containing G-U wobble base pairs that form a bent helix can exert RIG-I-mediated antiviral and anti-tumor effects in a sequence- and site-dependent manner. We present synthetic RNAs that have been systematically modified to enhance their efficacy and we outline the basic principles for engineering RIG-I agonists applicable to immunotherapy.</P>

Objectives; This study aimed to analyze the prevalence data of South Korea conscript soldiers who were prematurely discharged from military services due to mental illness. Methods; All demographic data, distribution of primary psychiatric diagnoses for medical discharge were collected retrospectively from Republic of Korea (ROK) Army Headquarters. Our data included 7,902 disabled soldiers and 24,733 maladjusted soldiers who were prematurely discharged from ROK army between January 1, 13 and December 31, 2018. Results; Soldiers with psychiatrical disabilities tend to decrease, but the number of maladjusted soldiers has increased continuously since 2013. In both cases, the most frequent rank was in the private class. Soldiers with psychiatrical disabilities reported high frequency of developmental disorders, schizophrenia, bipolar disorder and maladjusted soldiers showed adjustment disorder, depression, personality disorder, and anxiety problems. Conclusion; The number of maladjusted soldiers is increasing, and most of them are reported diagnostic names that require long-term care. Systemized epidemiologic, long term cohort studies are crucial to providing military-related mental health assessment and treatment.

<P>We present 2.5-5.0 mu m spectra of 83 nearby (0.002 <z <0.48) and bright (K <14 mag) type-1 active galactic nuclei (AGNs) taken with the Infrared Camera on board AKARI. The 2.5-5.0 mu m spectral region contains emission lines such as Br alpha (2.63 mu m), Bra (4.05 mu m), and polycyclic aromatic hydrocarbons (3.3 mu m), which can be used for studying the black hole (BH) masses and star formation activity in the host galaxies of AGNs. The spectral region also suffers less dust extinction than in the ultra violet (UV) or optical wavelengths, which may provide an unobscured view of dusty AGNs. Our sample is selected from bright quasar surveys of Palomar-Green and SNUQSO, and AGNs with reverberation-mapped BH masses from Peterson et al. Using 11 AGNs with reliable detection of Brackett lines, we derive the Brackett-line-based BH mass estimators. We also find that the observed Brackett line ratios can be explained with the commonly adopted physical conditions of the broad line region. Moreover, we fit the hot and warm dust components of the dust torus by adding photometric data of SDSS, 2MASS, WISE, and ISO to the AKARI spectra, finding hot and warm dust temperatures of similar to 1100K and similar to 220 K, respectively, rather than the commonly cited hot dust temperature of 1500 K.</P>

Objectives : Suicide is one of the leading causes of deaths in South Korea, however little data has been published about the Korea military. This study aimed to examine the epidemiology and characteristics of suicide related behavior in the South Korea Military. Methods : This study investigated the characteristics of patients who admitted to emergency room of the 14 military hospitals from 2009 to 2011 by suicide related behavior. 816 patients were identified by retrospective chart review including demographic, rank, attempt place, method, timing and suspected psychiatric problems. Results : The most vulnerable rank associated with suicide related behavior was private first class. The main methods of suicide attempt were poisoning(37%) and knife cutting(30%). About 25% of the total suicide related behavior was assessed as high risk attempt including falling down, firearm, burning oneself. In addition, psychiatric disorders, particularly depression, adjustment disorder were suspected with these patients. Media diffusion of stories about suicide was shown to increase the suicide related behavior in military personnel. Conclusion : This study implies that suicide and related behavior is main health issues in Korea military. Systemized psychiatric evaluation and management of suicide behavior are crucial to prevent suicide during military services.

최근 이상기후로 인해 봄철 낮과 밤의 기온차가 커지면서 동해 피해를 입는 농장이 늘어나고 있다. 현재 동해 피해를 줄이기 위해 페인트 도포와 보온재 피복을 통한 방법을 사용하는데, 국내 농가의 평균 재배 주수인 375 그루에 달하는 재배면적에서 각각의 나무에 수작업으로 이루어지고 있어 과도한 노동력을 필요로 하고 있다. 본 연구는 수작업으로 이루어지는 페인트 도포 작업을 펌프를 이용한 노즐 분사로 자동화함으로써 농민들의 노동력과 경영비 절감 및 작업 효율을 증대시키기 위한 선행연구로서 페인트 자동 도포장치를 설계하였다. 개발품은 개폐 가능한 집게와 분사노즐이 존재하는 분사부, 상하이동이 가능한 상하제어부, 운행을 위한 주행부 세부분으로 나누어 설계 하였다. 먼저 분사부는 집게의 개폐를 위한 서보모터와 노즐로 구성하였다. 서보모터를 통한 기어 회전으로 집게개폐 동작이 구현되며 AutoCAD와 Inventor 프로그램을 이용하여 분석한 결과 무게 1.5kg이 적정한 것으로 나타났다. 또한 기어의 효율, 안전계수를 적용하여 토크는 36kgf·cm로 분석되었다. 노즐은 시뮬레이션을 통하여 최소 73도의 분사각이 필요한 것으로 분석되었으며 Sejin 타입의 노즐을 적용하였다. 유량은 이동거리 80cm, 페인트량 0.05L, 희석변환계수 1.1과 최대속도 5cm/s에 필요한 최대 유량이 0.2075L/min로 분석되었으며 이를 바탕으로 펌프를 선정하고, 노즐과 연결함으로서 페인트공급이 가능하게 하였다. 상하제어부는 액추에이터 모터의 회전속도와 방향을 제어함으로써 상하운동이 가능하도록 설계하였다. 간편한 조작을 위하여 볼스크류 방식의 액추에이터를 고려하였고, 도포작업범위 80cm를 최대 5cm/s의 속도로 움직이는 설정값에 따라 선정하였다. 마지막으로 주행부는 페인트통, 펌프, 배터리로 구성된 파트로 적재량과 배치 및 무게중심을 고려하여 설계하였다. 주행부와 상하제어부 연결을 위하여 원형 회전판을 적용하였고, 손잡이 부분에는 조작을 위한 스위치를 설치하였다. 다양한 설계요인을 고려하여 설계된 본 개발품을 동해방지를 위한 페인트 도포에 적용하면 작업 능률을 크게 향상시킬 수 있으며 노동력 절감 및 작업시간의 단축을 이룰 수 있을 것이다. 추후 설계된 장치의 액추에이터 허용 모멘트를 고려한 보완과정을 거쳐서 분사량과 자동제어 방식을 수정하여 시제품을 제작하고 현장실험을 통한 작업능률 분석 및 수정 보완을 진행할 계획이다.
