Experience with Ledipasvir/Sofosbuvir-Based Treatment for the Treatment of HCV Genotype 1a in Korea

Aims: In this multicenter retrospective cohort study, we aimed to evaluate the effectiveness of IFN-free direct acting anti-viral agents (DAA) for patients with HCV genotype 1a in a real life setting in Korea. Methods: We analyzed clinical data of all consecutive DAA-treated patients for chronic HCV genotype 1a infection from eight clinical centers in Korea. Between May 2016 to April 2017, 35 patients were included in this study. The primary efficacy endpoint was end of treatment response (ETOR) and sustained virological response 12 weeks (SVR12). Adverse events (AEs) and laboratory data were also collected for analysis. Results: A total of 35 patients with genotype 1a HCV infection, 8 patients (22.9%) had compensated liver cirrhosis and 14 patients (40%) were treatment-experienced. Ledipasvir/sofosbuvir (LDV/SOF) with ribavirin were administered to six of seven treatment-experienced patients with cirrhosis. Thirteen patients were over 65 years old (37.1%) and female patients were dominant (65.7%, 23/35 patients). The Child-Pugh scores of all patients were 5 or 6. The rate of SVR12 was 94.7% (18/19 patients) and the rate of ETOR was 96.6% (28/29 patients). SVR12 and ETOR were obtained in all patients completed treatment for 12 weeks. Five of 33 patients (15.2%) had detectable HCV RNA at week 4 of therapy, but all of them achieved SVR12. LDV/SOF without ribavirin was tolerated well. The dose of ribavirin was reduced in two patients because they complained fatigue and general weakness after the treatment of LDV/SOF with ribavirin. Conclusions: In patients with HCV genotype 1a infection, LDV/SOF based regimen was very effective and well tolerated.

Aims: The risk of hepatocellular carcinoma (HCC) in chronic hepatitis C (CHC) is decreased in patients achieved sustained viral response (SVR) after antiviral therapy based on pegylated interferon. However, the increasing risk of HCC development and recurrence in the patients who achieved SVR with all-oral DAA agents is controversy. The aim of this study was to evaluate the de-novo HCC development after antiviral therapy based on all-oral DAA agents in CHC patients without a history of previous HCC. Methods: A total of 963 CHC patients who were treated with interferon free regimen in the 8 affiliated hospitals of the Catholic University of Korea between March 2015 and March 2017 were retrospectively analyzed. De-novo HCC was defined as the newly development of HCC after DAA regimens but no prior HCC history before the DAA treatment. Results: Of the 963 patients, a total of 574 patients received DAA treatment who were followed up for at least 12 weeks with no history of previous HCC. The overall median follow-up period from the start of antiviral therapy was 10.4 months. SVR was achieved in 548 of the 574 (95.5%) patients. De-novo HCC was detected in 5 of 574 (0.87%) patients during the follow-up period. A univariate analysis showed that liver cirrhosis (p = 0.006) and a higher level of serum alpha fetoprotein (AFP) at the time of end treatment response (ETR) (p = 0.010) were significantly associated with HCC development. However, a multivariate analysis indicated a higher level of serum AFP at the time of ETR (p = 0.044, HR 11.80, 95% CI: 1.07-130.44) was the independent risk factor for HCC development. Conclusions: Our study demonstrates that there still exists a risk of newly HCC development in patients treated with all-oral-DAA agents for CHC. Also, serum AFP at the time of ETR was associated with HCC development and it may be a useful biomarker for the prediction of HCC development after all-oral-DAA agents for CHC.

본 연구는 장애학생 통합형 직업교육 거점학교 운영에 대한 현황을 검토하고 발전 방안을 모색할 수 있도록 성과지표를 개발하여 거점학교가 설립 목적에 부응한 운영의 효과성을 제고할 수 있도록 하는 데 목적을 두었다. 이를 위해, 87명의 전문가들을 대상으로 1차, 2차 델파이 조사를 실시하였다. 연구의 결과로 ‘인식’ 8문항, ‘체제’ 19문항, ‘운영’ 38문항, ‘성과’ 10문항으로 구성된 75개의 성과지표를 개발하였다. 이 성과지표 중 거점학교 운영에서 반드시 고려해야 할 30개를 핵심지표로 선정하였으며, 그 외 45개는 참고지표로 명명하였다. 또한 본 연구를 통해 개발된 성과지표를 활용하는 방법을 제시하였으며, 연구의 결론과 함께 이 성과지표를 활용함에서 있어서 고려해야 할 점 등을 논의하였다. The purpose of the study was to develop the performance indicator of Inclusive Schools Being Based in the Vocational Education. The developed Indicators will be used to support the efficiency of vocational education. For this study, 87 vocational education field professionals participated tp rate importance degree of the first and second version of Delphi survey. As the research result, 75 performance indicators were developed. Those are 8 items in perception, 19 items in system, 38 items in operation, 10 items in outcome. 75 items consist 30 core performance indicators and 45 referenced performance indicators. It was also suggested how to use this developed performance indicator in the result. In conclusion, prerequisites for usage for the developed performance indicators were discussed . In conclusion, it was suggested prerequisites for the usage of the developed performance indicators.

약용식물은 피부와 모발을 보호하는 잠재적 기능을 가지고 있다고 알려져 있다. 약용식물의 유용 성분들은 대부분 비수용성 약용성분들이므로 수용성 분자 담지체인 β-cyclodextrin을 이용하여 초분자 복합체를 형성시켜 기능성의 변화를 살펴보았다. 약용식물의 유용 성분이 함유된 크림, 샴푸, 바디워시, 그리고 헤어토닉은 계면활성제, 방향제, 약용식물 추출물과 β-cyclodextrin을 혼합하여 제조하였다. 복합체를 함유한 샴푸 및 바디워시에서 그람 양성균들과 비듬균으로 알려져 있는 효모에 대한 항균활성과 균 성장억제 효과를 확인하였다. HDF (human dermal fibroblast)에 대한 초분자체의 독성실험결과, 초분자체의 함량이 1 mg/mL 이하의 경우에서는 세포 독성이 나타나지 않았다. 본 연구결과는 약용식물 추출물과 약용성분 초분자 복합체에 대한 세포무독성을 확인하고 항균활성을 확인함으로써 향후 기능성 화장품 제조 시 안정성과 기능성의 과학적 근거를 확보하였다고 사료된다. In this report, we have investigated the functional properties change of phytochemicals by the encapsula-tion using water soluble host, β-cyclodextrin. The cream, shampoo, bodywash, and hair tonic containing phytochem-ical supramolecules were produced by mixing the surfactants, fragrances and the oriental herbal extracts encapsulated with β-cyclodextrin. Shampoo and bodywash including the encapsulated phytochemicals exhibited anti-growth ac-tivity against Gram (+) and Malassezia furfur which is known to cause dandruff. In cytotoxicity test against HDF (human dermal fibroblast), we could not detect any toxicity when the supramolecules content was less than 1 mg/mL. Our results suggest that the supramolecule of β-cyclodextrin with phytochemicals could be a safe anti-bac-terial agent for cosmeceuticals.

As manifested in various urban policies, the Seoul Metropolitan Government(SMG) has made great efforts to improve the pedestrian environment. The City of Seoul has managed “Car-Free Day” events several times, and has designated pedestrian-only streets in many areas. However, it is not always convenient to walk in Seoul. On the streets, citizens still experience narrow sidewalks, streets cluttered with street furniture, and slanted sidewalk angles which are not safe for the mobility disadvantaged. The SMG still has work remaining to enhance the quality of the walking environment. In pursuing this goal, public officials in the SMG have realized an urgent need to seek guidance from a street design manual. Herein, the existing manuals were reviewed to see if they could be of use in the redesign of Seoul streets. It was found that the manuals were simply not up to the job, presenting many problems. First, in most cases, the national government created the manuals. Thus, they did not reflect the unique characteristics of Seoul’s built environment. Second, the manuals provided only general direction for street design, without specific details to deal with the diverse types of streets found in Seoul. Third, they set the focus on a few elements of street design, while considering various factors in an integrated manner is a key element for desirable street design. For these reasons, the SMG plans to create a Seoul Street Design Manual. As preliminary research for such a manual, this study drew on lessons from other cities in the world. The manuals for various cities abroad all have a common theme: they aim to convert current “Incomplete Streets” that were designed for automobiles to “Complete Streets” that put pedestrians and bicycles on equal footing with cars. Nevertheless, their coverage of design elements, the level of details included in the guidelines, and the contents of the manuals still differ from each other. Throughout the 1980s and 1990s, Seoul experienced rapid growth with the construction of major urban highways such as the Olympic Freeway. It may be evident that Seoul has not treated pedestrians and bicycles with due respect compared to automobiles. Therefore, we suggested that the Seoul Street Design Manual should focus on the design of Complete Streets, for which the design principles are proposed herein. Moreover, for a city like Seoul with various street types, it is important to provide customized design strategies for each street pattern. Readers can also find them in this report. Since physical developments, including buildings and transport infrastructure, have almost been completed, it is crucial to implement the design manual step by step. Although a design scheme such as road diet is a desirable outcome for pedestrians, it would bring about chaos for auto users if executed instantly. Learning from cities abroad(such as the Parklet program in San Francisco), we proposed a method to carry out the design proposal through gradual and acceptable changes. Finally, the creation of a fine street design is a task that should involve a wide variety of fields including transportation, urban design, and environmental engineering. This report also suggested organizing a Task Force(TF) to collect useful ideas from many disciplines. That way, citizens walking on the streets in Seoul could benefit for years to come.
