        Gingko biloba is one of the oldest trees on earth. It can be traced 300 million years or more into past. In recent years, this tree has become increasingly popular because of its longevity. Amount of comsumption of Gingko leaves in Korea is increased to produce drugs for blood circulating system's disorder. The workers who work at the company of product drug with Gingko leaves complain pruritus at belt position of west, however toxic substance from extracts of Gingko biloba was carried out to determine what kind of the toxic substance from extract could induce a contact dermatitis by means of histopathological study. Dried leaves of Korean G. biloba were exhaustively extracted with chloroform. The aqueous chloroform residue was extracted with hexane. After evaporation, the hexane extract was passed through a silica gel column chromatography, it was separated into 9 fractions by means of gradient solvent (benzene: acetone/ 98~93 : 2~7%). The 4th and 7th fraction were idendified as bilibol and gingkolic acid by silica gel thin layer chromatography and NMR spectrophotometer. The 4th and 7th fractions of Korean Gingko leaves have an irritant substances which can be induced ear swelling and increased thickness of stratum cornium of rabbit ear exposed with the fractions (3 time/3 day, 100μl of 50mg/ml ethyl alcohol). In the 4th fraction, the substance have induced the surface of ear to swell and mass of unknown cell like inflammatory cell were infiltrated within the stratum corneum, and the 7th fraction have the surface of ear to loss the hairs, and sebaceous gland and follicle of hair were infiltrated with inflammatory cells, and other fractions at all did not induced a certain contact dermatitis and also Deutschen drug of Gingko (named as Gerigeron) have induced the contact dermatitis such as dematitis of Korean Gingko extract.

        우리나라 1900년대 문헌에 나타난 주식류 변화에 관한 고찰

        This study reviewed the main-meals in 1900's Korean cook books. Boiled rice, gruel and thin gruel were mainly made of rice. As the additional ingredients, miscellaneous cereals, vegetables, potatoes, seafoods, meats and eggs were used to substitute for rice and to improve of preference. we observed some sort of main-meats were diminished and/or disappeared, changed its side ingredients to obtain foods easily, and also its cooking style has been gradually gelling to simplify.

      • 신생검증례에 의한 사구체신질환의 분석

        To elucidate the incidences and the clinical characteristics of glomerular diseases in our hospital, we reviewed the clinical records and histological findings of the 117 cases, which were diagnosed as one of the glomerular disease in renal biospy for about 6 years since 1991. In these 117 cases, 74 cases (63.2 %) fell into primary glomerulonephritis(GN), 18 cases (15.4%) into secondary GN, 2 cases (1.7 %) into end stage kidney, 4 cases (3.4%) into tubulointerstitial disease and the biopsy samples were inadequate for diagnosis in 19 cases. In primary glomerular diseases, most common pathologic diagnosis was IgA nephropathy (36.5 %}, followed by minimal change nephrotic syndrome (29.7%). The incidence of IgA nephropathy would be increased if we had biopsied every asymptomatic urinary abnormality (AUA) patients. Systemic lupus erythematosus (SLE) . hepatitis B accounted for most of secondary GN in our cases. One case of progressive scleroderma was included in this secondary GN. Minimal change nephrotic syndrome (22 cases) was the most common cause of nephrotic syndrome, followed by membranous GN (9 cases) and IgA nephropathy (8 cases). 2 cases of primary amyloidosis were presented with nephrotic syndrome. Nephrotic syndrome as presenting symptom in our IgA nephropathy patients was higher than in other studies, which may be due to indication bias for renal biopsy. We suggest that renal biopsy should be done in adult nephrotic syndrome and in SLE, because various kind of glomerular diseases can develop nephrotic syndrome in adult, renal biopsy can affect the treatment and prognosis in each patients and renal biopsy also determines the extent of renal involvement in SLE on which the treatment depends. In AUA urinary patients, renal biopsy could be reserved for the patients who show declining of GFR or rising of proteinuria. Because IgA nephropathy account for most of AUA. and there is no specific treatment for IgA nephropathy, and renal biopsy in those cases is helpful in ruling out the patients who need no treatment.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

        누두흉환자에서 Nuss 방법에 의한 수술 중 환기방법에 따른 호기 일회호흡량, 흡기 최대압력 및 활력징후의 변화

        Changes of Vital Signs, Expired Volume and Maximal Pressure by Mode of Ventilation during a Nuss Operation in Patients with Pectus Excavatum Soo Dal Kwak, M.D., Nam Soon Baek, M.D., Su Hyun Cho, M.D. Ki Ryang Ahn, M.D., Sun Hak Lee, M.D., Chun Suk Kim, M.D. Wook Park, M.D., and Hyung Joo Park, M.D. Departments of Anesthesiology, ??Chest Surgery, College of Medicine, Soonchunhyang University, Cheonan, Korea Background: This study was undertaken in order to investigate any cardiopulmonary changes caused by the surgical procedure during a Nuss operation in patients with pectus excavatum. Methods: Thirty patients undergoing the Nuss operation were divided into two groups. The patients were randomly assigned to either Group-V (n = 15) which volume controlled ventilation was applied, or to Group-p (n = 15) where pressure controlled ventilation was applied. Mechanical ventilation provided a tidal volume to be set at approximately 10 ml/kg and a frequency set to keep ETCO_2 in range, between 32 mmHg and 38 mmHg. During the surgical procedure, we recorded expired volume (V_E) and inspired maximal pressure (Pmax) changes according to the ventilator mode, and then also recorded changes in vital signs. Results: Heart rate and blood pressure had no significant changes in either groups. In Group-V, during rotation of the metal bar, there was a significant decrease in expired volume (P < 0.001), changes of inspired maximal pressure gradually increased with the operation (P < 0.001). In Group-P, rotation of the metal bar caused a significant decrease in expired volume (P < 0.001). Conclusions: During the Nuss operation, there should be careful observation of changes in the vital signs, expired volume and maximal pressure during insertion and rotation of the metal bar.         Methanol Extract of Goat's-beard (Aruncus dioicus) Reduces Renal Injury by Inhibiting Apoptosis in a Rat Model of Ischemia-Reperfusion

        Interruption or prolonged reduction and subsequent restoration of blood flow into the kidney triggers the generation of a burst of reactive oxygen species (ROS), leading to injury in the tubular epithelial cells. In this study, we determined whether methanol extract of goat's-beard (Aruncus dioicus) (extract) could prevent this ischemia/reperfusion injury. When in vitro radical scavenging activity of the extract was measured using a DPPH radical quenching assay, the extract displayed slightly lower activity than ascorbic acid. One hour after administration of the extract (400 mg/kg) by intraperitoneal injection in rats, renal ischemia/reperfusion injury was generated by clamping the left renal artery for forty minutes, followed by 24 hr restoration of blood circulation. Prior to clamping the left renal artery, the right renal artery was removed. Compared with the vehicle-treated group, pretreatment with the extract significantly reduced the tubular epithelial cell injury by 37% in the outer medulla region, and consequently reduced serum creatinine concentration by 39%. Reduction in the cell injury was mediated by attenuation of Bax/Bcl-2 ratio, inhibition of caspase-3 activation from procaspase-3, and subsequent reduction in the number of apoptotic cells. Thus, goat's-beard (Aruncus dioicus) might be developed as a prophylactic agent to prevent acute kidney injury.

      • A Novel Hyaluronate-Atelocollagen/β-TCP-Hydroxyapatite Biphasic Scaffold for the Repair of Osteochondral Defects in Rabbits

        <P>The authors devised a novel biphasic scaffold combining hyaluronic acid and atelocollagen for the chondral phase and combining hydroxyapatite (HA) and beta-tricalcium phosphate (beta-TCP) for the osseous phase. The biphasic scaffold was fabricated by placing the freeze-dried chondral phase over the HA/beta-TCP scaffold prewetted with hyaluronate/atelocollagen solution. Chondrocytes were isolated in 28 rabbits, expanded, injected inside the chondral phase of the biphasic scaffold, and then cultured in chondrogenic medium. After 2 weeks of in vitro culture, chondrocytes had evenly infiltrated inside the chondral phase and produced extracellular matrix. For in vivo study, a large osteochondral defect was made on the patellar groove of the right distal femur and managed using one of the following methods: filling with cell-biphasic scaffold composite (group I); implanting only biphasic scaffold (group II); placing the removed osteochondral fragments back into the defect (group III, positive control); leaving empty (group IV, negative control). Seven rabbits were allocated to each group. After 12 weeks, the International Cartilage Repair Society Macroscopic Score was highest in group III, followed by group I, group II, and lastly group IV. Depression of the defect was greatest in group IV. There were three rabbits (two in group I and one in group II) that were completely denuded of the chondral phase. The junction to adjacent native cartilage was distinct in rabbits of all groups. The International Cartilage Repair Society Visual Histological Score was highest in group III, followed by groups II and I, and lastly group IV. In conclusion, our results suggest that a biphasic osteochondral composite using a chondral phase consisting of hyaluronate and atelocollagen and an osseous phase consisting of HA and beta-TCP holds the promise for repair of osteochondral defects.</P>

      • Detection of copy number variations using array-based comparative genomic hybridization analysis in rice responding to different types of ionizing radiations

        Copy number variations (CNVs) are considered major sources of genetic variation, and CNVs may influence phenotypic variation and gene expression. To detect CNVs, rice seeds were exposed with 100~400 Gy of gamma-ray (GA, 60Co), cosmic-ray (CR) by Russia ISS, and 30 and 40 Gy of ion beam (IB, 220 Mev carbon ion). After the exposed rice seeds were cultured in 1/2 MS medium for 14 days, they were used for array-based Comparative genomic hybridization (CGH) analysis using Agilent’s RICE CGH array. As a results, the highest number of CNVs (Gain 808 and Loss 24,080) were detected in the CR treatment, whereas GA100 (100 Gy of GA) was identified the least CNVs. Compared individual chromosome, the chromosome 8 and 11 were identified the highest CNVs, the chromosome 3 had the least CNVs. Most of identified CNVs existed in the range of 10~500kb. In particular, the same CNV locations among different types of ionizing radiation were observed in chromosome 12, and these CNVs contained the commonly 5 amplified genes, containing retrotransposon protein, NADH-ubiquinone oxidoreductase chain 3, heavy metal transport/detoxification protein domain containing protein, and 2 unknown proteins. Other studies were reported that Ty1 (Long Terminal Repeat-retrotransposon family 1) transcription and retrotransposition were induced by different environmental stresses such as ionizing radiation, UV-light exposure, DNA damage and nutrient starvation in Saccharomyces cerevisiae. Our results also show that retrotransposon protein (LOC_Os12g34016) was specifically amplified by different types of ionizing radiation.
