        Estradiol과 Medroxyprogesterone Acetate가 골아세포의 성장에 미치는 영향

        N/A This study was designed to clarify the hormonal effect on the growth of osteoblast. And the osteoblasts from neonatal mouse were cultured in the medium at various concentrations of 17β-estradiol(EST), medroxyprogesterone acetate(MPA) in addition 10nM of dexamethasone(DMS) and alcohol for 48 hours. And in order to detect the response of osteoblast on the those hormones, the alkaline phosphatase(ALP) activity also was measured. 1. At 48 hours after culture of osteoblasts in media at various concentrations of hormones, EST has an effect on increasing of the cell number in fashion of dose dependent, whereas MPA did not increase the cell number. 2. Dexametasone significantly promoted differentiation of osteoblasts does dependently with increasing of ALP activity. 3. In osteoblasts culture in media supplemented with alcohol in the absence of 10nM DMS, EST increased significantly ALP activity, while decreased ALP activity of osteoblast in the presence of DMS, 4. In culture of osteoblasts treated with alcohol in the absence of 10nM DMS, MPA did not show any change of ALP activity. But in the presence of DMS, MPA decreased ALP activity of osteoblast. 5. EST plus MPA stimulated the ALP activity in osteoblasts treated with alcohol in the absence of DMS. While, in the cells treated with DMS, they synergistically decreased ALP activity.

      • Sodium Nitroprusside(SNP)를 사용한 유도 저혈압 마취시 Renin 활성치, Aldosterone, Epinephrine, Norepinephrine의 변동

        악안면 성형재건술 환자 10명을 대상으로한 sodium nitroprusside(SNP) 유도 저혈압 마취에서 SNP를 주입한 후에 체내의 renin-angiotensin-aldosterone system과 sympathoadrenal system이 활성화되었음을 관찰할 수 있었다. Sodium Nitroprusside (SNP) is used during induced hypotension to decrease bleeding in operation site by direct relaxation of vascular smooth muscle. It is known that the infusion of SNP increases plasma renin activity and this activation of remain-angiotensin system is one physiologic mechanism opposing the hypotensive action of SNP. The purpose of this study was to determine plasma renin activity and activation of sympathoadrenal system following infusion of SNP for hypotensive anesthesia in 10 patients needed maxillofacial reconstructive surgery. Blood samples for analysis were drawn according the time sequence of SNP infusion ; Stage 1; After the induction and before SNP infusion, Stage 2 ; 30 min after when mean arterial pressure maintained 60-70 torr and within 30 min after SNP infusion, Stage 3;Before slopping of SNP, Stage4; 30 min after stopping of SNP. The results were as followings, 1) The duration of anesthesia and infusion of SNP were 197.7±131.3 Min and 100.2±40.3 min. 2) Total doses of 0.01% SNP solution were 115.2±36.4 ml through hypotensive anesthesia 3) PRA in stage 2,3 and 4 (25.3±7.6, 26.2±7.2 and 24.5±8.2 ng/dl/hr respectively) were significantly increased compared with the value of stage 1 (8.9±7.0ng/dl/hr) and the level of aldosterone in stage 2, 3, 4 (28.4±12.7, 33.6±20.0 and 32.9±18.0 mg/dl respectively) were significantly increased compared with the value of stage 1 (13.4±9.1 ng/dl). The increased values of PRA and aldosterone fowllowing infusion of SNP were continued eyen after the time of stopping SNP. 4) Norepinephrine in stage 2, 3(545±157.5, 347.7±115.0 pg/ml respectively) and epinephrine in stage 2,3 (178.4±58.7, 132±55.7 pg/ml respectively) were significantly increased compared with the values of stage 1(norepinephrine ; 236.2±107.3, epinephrine ; 111.8±73.9 pg/ml) and they were returned to the control value after slopping of SNP infusion 5) Sodium potassium and chloride were not changed significantly during SNP induced hypotensive anesthesia. In summary, the activity of renin-angiotesin-aldostprone system and sympathoadrenal system were increased following infusion of SNP during SNP induced hypotensive anesthesia.

        비세그라드그룹 시장에서 한국의 무역전략과 유망수출품목

        연구목적: 본 연구에서는 우리나라와 중동부 유럽 4개국(이하 비세그라드그룹, V4) 간 무역 투자 관계와 비교우위 구조를 분석하고, 우리나라의 V4에 대한 무역전략과 유망수출상품을 도출하였다. 논문구성/논리: 본 연구의 구성은 다음과 같다. 먼저, 비세그라드그룹(V4) 4개국의 거시경제 개황과 주요 산업 및 교역 현황을 간단히 분석한다. 그리고 우리나라와 V4 간 무역 투자 관계와 비교우위 구조를 분석한다. V4 시장에 대한 SWOT 분석을 바탕으로 무역 확대방안을 도출하고, 우리나라의 對V4 유망수출상품을 선정하고자 한다. 결과: 우리나라의 對V4 무역 확대전략을 도출하면, 첫째, 전기전자, 자동차 및 자동차부품, 전기차배터리, 에너지 등 제조업 및 건설업 전반에 걸친 對V4 산업협력 다각화 지원이 필요하다. 둘째, EU기금을 활용하여 공공 조달과 인프라 건설, 지역개발 프로젝트, 그리고 전자정부 및 정보화 시스템사업에 대한 수주 지원이 요구된다. 셋째, 對V4 원자력발전 산업기술협력 확대 및 신규 원전 수주지원이 필요하다. 넷째, 對V4 시장에 대한 수출 유망품목 및 거래선 다변화, 그리고 수출마케팅 지원 등을 고려해 볼 수 있다. 우리나라의 對V4 유망수출품목으로 對세계 비교우위(RCA)를 갖는 상품 중 V4 시장에서의 점유율과 성장률이 높고, 수입 비중이 높은 품목을 선정하면, 차량 부분품과 부속품, TV 및 송수신기기 부분품, 기타(고유 기능의) 기계류, 엔진 부분품, 혈액 및 백신 용품, 펌프 압축기와 팬 등, 전자직접회로, 철 비합금강의 평판 압연제품 등 8개 품목이다. 독창성/가치: V4에 대한 기존 연구는 주로 EU 내의 중동부 유럽 2-3개 국가 또는 개별 국가, 또는 특정 국가의 특정 산업에 대해 조사가 이루어졌으나, 비세그라드그룹 차원의 무역 투자 전반에 대한 포괄적인 연구는 많이 이루어지지는 않았다. 따라서 본 연구에서는 우리나라와 V4 국가 간의 무역 투자 현황과 구조를 분석한 이후, 우리나라가 추진하여야 하는 V4에 대한 무역 확대방안을 도출하고 對V4 유망수출상품을 선정하였다. Purpose: This paper investigates the economic conditions of the four countries in the Middle and Eastern Europe, known as the Visegrád Group(hereinafter, V4), analyzes the trade and investment relations and the comparative advantage structure between Korea and the V4 and then attempts to derive Korea’s trade strategy and promising export products toward the V4 markets. Composition/Logic: First, we briefly analyze the macroeconomic and trade trends in the V4. Second, we analyze trade and investment relations and comparative advantage structure between Korea and the V4. Third, based on the SWOT analysis, we derive Korea’s trade strategy and promising export products toward the V4 markets. Findings: Korea’s trade strategy toward V4 is as follows: First, Korean firms should participate in public procurement, infrastructure construction and regional development projects, utilizing the EU fund. Second, Korea should establish industrial technology cooperation with the V4 countries in nuclear power generation. Finally, Korea must diversify promising export items and trading lines in the V4 markets. The following eight items are among Korea s promising export products in the V4 markets: parts and accessories of the motor vehicles, parts for transmission, radar, and reception apparatus, other machinery and mechanical appliances, engine parts, blood products and vaccine supplies, air or vacuum pumps, air or other gas compressors and fans, electronic integrated circuits, flat-rolled products of iron or non-alloy steel, etc. Originality/Value: While the previous studies have focused on the particular country, two or three countries, or on a particular industry in the Middle-Eastern Europe, the present study comprehensively deals with the overall trade-investment relations with Korea at the level of the V4.

        한국의 대(對)발트 3국 시장진출전략과 유망수출품목 선정에 관한 연구

        발트 3국은 발트해 남동 해안에 위치해 있는 에스토니아, 라트비아, 리투아니아를 가리킨다. 기존 문헌자료나 보고서에서 발트 3국에 대한 연구는 거의 없다. 본 연구에서는 발트 3국의 거시경제 현황과 산업 및 교역 동향을 살펴보고, 한국과 발트 3국간 무역관계를 분석하였다. 그리고 발트 3국 시장에 대한 SWOT 분석을 이용하여 한국의 발트 3국에 대한 시장진출전략을 도출하고, 현시비교우위지수(RCA), 무역특화지수(TSI) 및 무역통계를 활용하여 한국의 발트 3국에 대한 유망수출품목을 선정하였다. 한국은 발트 3국의 지리적 위치와 우수한 IT 기술과 적응 능력, 우수한 기술인력 공급, 정부의 적극적인 외국인투자 유치 정책 등 기회 요인들을 이용하여, 중ㆍ장기적으로는 발트 3국을 중동부 유럽 및 CIS 지역을 연결하는 물류/유통 허브로 구축해야 한다. 또한 한-EU FTA를 활용하여 발트 3국에 대한 수출 증대와 수출 품목 및 거래선 다변화를 추진해 나가야 하며, 발트 3국에 대한 내수시장 개척을 위해 ‘한국산’ 이미지 제고, 발트 3국 대상의 한국상품전시상담회 등을 추진해 나가야 한다. 한국의 발트 3국에 대한 유망수출품목은 8개 품목(MTI 3단위 기준)인데, 자동차부품, 무선통신기기, 합성수지, 철강판(아연도강판), 플라스틱 제품, 고무제품(타이어), 금속공작기계(금속절삭가공기계, 농업용 트랙터), 인조섬유(폴리에스테르 섬유) 등이다. Baltic nations located in southeastern coast of the Baltic Sea include Estonia, Latvia and Lithuania. So far, there is no rigorous research on the Baltic nations in the prior literature. This paper intends to examine macroeconomic and industrial status of the Baltic nations as well as the trends in trade and investment, and analyze trade relations between Korea and the Baltic nations. Using SWOT analysis and revealed comparative advantage(RCA) and trade specialization index(TSI) for the Baltic nations, this paper derives Korea’s market entry strategy and promising export items toward the three Baltic nations. Korea should take advantage of the geographical location of the three Baltic nations, excellent IT technology and adaptability, skilled technological manpower, and their governments’ aggressive policy of attracting foreign investment to establish logistics/wholesale-retail services bridgehead, connecting the three Baltic nations to Middle Eastern Europe and CIS regions. In addition, Korea must make use of FTA with EU to increase its exports to the three Baltic nations, diversify export items and trading lines to these markets, and enhance the image of Korean products to expand the Baltic market. Among the potential exports toward the three baltic nations, the promising export items in the MTI 3 unit code are the following eight items such as automobile parts, wireless communication devices, synthetic resins, steel plates, plastic products, rubber products(tires), metal-working machinery, and synthetic fibers.

        넙치 (Parlichthys olivaceus) 혈액중 Insulin-Like growth factor-1의 함유수준

        양식 넙치, Paralichthys olivaceus를 실험어로 하여 동물 성장인자중의 하나인 insulin-like growth factor-I (IGE-I)의 함량을 검토하였다. 넙치의 혈액중에 존재하는 IGF-I의 양은 47ng/ml이고, 절식을 할 경우 식이를 섭취한 것에 비해 약 15% 감소하였으며, 운동에 의해서는 약 13%의 감소를 보였다. 혈액중 IGF-I의 농도는 어체의 크기에 따라서도 영향이 있음을 알수 있었다. Insulin-like growth factor-I (IGF-I) is a mitogenic peptide with molecular mass of 7kDa. It is produced mainly in the liver and has important functions in the regulation of development and somatic growth. Recently, several investigations were undertaken to examine the biological actions and structures of IGF-I in fish. In this study, the serum levels of IGF-I were estimated from flounder, Parlichthys olivaceus, before, during and after fasting, and the levels were accounted for 47 ng/ml, 40 ng/ml and 45 ng/ml, respectively. These results suggest that food deprivation primarily reduces IGF-I level in the blood.

        고혈압 환자에서 위험도 층별화에 따른 진료 형태 및 혈압변화 분석

        Background/Aims: The aims of this study were to identify real world treatment patterns of hypertension according to cardiovascular risk stratification and to evaluate blood pressure changes with anti-hypertensive treatment in each risk group. Methods: This study included patients who were newly-diagnosed with hypertension or known hypertensive patients with uncontrolled blood pressure (BP) at seven tertiary hospitals in Busan and Ulsan. World Health Organization/International Society of Hypertension (WHO/ISH) cardiovascular risk stratification was performed through retrospective chart review. Results: Angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers were the most frequently prescribed drugs. The higher WHO/ISH risk group received a greater number of drugs at the initial treatment, and one year after treatment. Target BP was achieved less frequently in the higher risk group (68.2% vs. 85.2% vs. 89.0%, p < 0.001). The rate of attaining target BP was lower (50.7% vs. 81.6%, p < 0.001), and the time to attaining target BP was longer (106.5 ± 79.2 days vs. 82.1 ± 75.3, p = 0.001), in patients with renal disease or diabetes. Initial systolic BP above 160 mmHg (OR: 4.91, 95% CI: 2.27~10.65), renal disease (3.42, 1.60~7.32), medium or high risk group status (2.27, 1.23~4.20), initial diastolic BP above 100 mmHg (2.11, 1.11~4.04), and diabetes (2.06, 1.29~3.25) were independent factors that predicted failure of attaining the target BP. Conclusions: BP control was relatively unsatisfactory in patients with higher initial BP, renal disease, higher WHO/ISH risk group status, and diabetes. Individualized approaches for such patients are needed to improve BP control in routine clinical practice. (Korean J Med 2011;80:298-307)

        Biomechanical advantage of lengthening of the femur with an external fixator over an intramedullary nail

        The present experimental study investigated biomechanical differences in methods of femoral lengthening using a monolateral external fixator only and using an external fixator over an intramedullary nail. Three materials, namely polyvinyl chloride rods, composite synthetic bone, and cadaver-bone, were tested using the MTS machine. We evaluated the differences of axial stiffness according to the presence of a nail or the numbers of half-pins (two or three half-pins) that were fixed at each side of osteotomy. The addition of a nail may increase the axial stiffness of the frame of monolateral external fixator for limb lengthening. Additionally, it is enough to distract the femur with fixing two half-pins at each side, when using the technique of lengthening over a nail.

        40세 이상 재발성 견관절 탈구 환자에서 관절내 병변에 대한 관절경적 연구

        목적: 40대 이상의 환자의 외상성 견관절 재발성 전방 탈구에서 관절 내 병변 구조물을 조사하고, 그 발병 기전 및 향후 치료 방침을 정하는데 임상적 의의를 제공하고자 한다. 대상 및 방법: 2001년 1월부터 2009년 5월까지 재발성 견관절 전방 탈구로 본원에서 수술적 치료를 받은 환자 중 수술 당시 나이가 40세 이상인 16명의 환자 (평균 나이 52.7세)를 조사하였다. 모든 환자에서 관절경을 이용하여, 관절와 순, 관절막, 회전근 개, 골병변 등의 관절 내 구조물에 대한 관절경적 소견을 조사하였다. 결과: 조사한 16례의 모든 환자들에서 상완 골두의 Hill-Sachs 병변이 보였으며, 골성 Bankart 병변 3례 (19%), 관절와 순 파열 6례 (38%), 관절막 파열은 15례 (94%)에서 발견 되었다. 12례 (75%)의 환자에서 극상건 파열이 발견되었고, 견갑하근 파열은 5례 (31%)에서 발견되었다. SLAP 병변은 1례 (6%)에서 관찰되었다. 결론: 40세 이상 견관절 재발성 탈구 환자에서는 관절막 및 회전근 개 파열의 빈도가 상대적으로 많이 나타났다. 따라서 청소년기와 달리, 장년층 이상의 환자에서는 관절막 및 회전근 개 손상에 대한 수술 전 대비가 요구된다. Purpose: This study is to analyze the prevalence of various intra-articular lesions in patients with traumatic recurrent anterior dislocation of shoulder over 40 years and suggest clinical implications for treatment. Materials and Methods: We retrospectively studied 16 cases that underwent surgical treatment for recurrent anterior dislocation of shoulder from January 2001 to May 2009. There were 9 males and 7 females, and the mean age was 52.7years. We carried out arthroscopic exam for all patients based on standard protocol, which included labrum, capular lesion, cuff, bony lesions. Results: All 16 cases showed Hill-Sachs lesion, 3 patients (19%) had bony Bankart lesion, 6 patients (38%) had labral tear. Capsular tear were found 15 patients (94%). Twelve (75%) had ruptured supraspinatus and 5 (31%) had subscapularis tear. Only one (6%) had SLAP lesion. Conclusion: There was relatively higher incidence of capsular and rotator cuff tears in patients over age 40 years. Preoperative planning to address these lesions is highly recommended.
