      • 자가 면역성 간염에서 자가항체의 발현 양상과 스테로이드 치료의 효과

        목적 : 최근 8년간 본원에서 경험한 자기면역성 간염의 혈액학적 특징과 스테로이드의 치료효과를 알아보고자 본 연구를 시행하였다. 방법 : 1996년 이후부터 2002년까지 충남대학교 병원에서 1999년 International Autoimmune Hepatitis Group(LAHG)에서 제시한 수정 진단 기준안에 의거하여 총 진단점수가 probable 이상에 해당하는 환자들을 대상으로 임상적 특징, 검사실 및 조직 소견과 스테로이드와 azathioprine의 치료 효과를 후향적으로 조사하였다. 결과 : 9예의 환자들은 여성이 78%, 40대가 많았고, 급성 발병이 33.3%, 피로감, 식욕부진 및 황달 등이 나타났으며, 항핵항체가 55.6%에서 그리고 항평활근 항체가 77.8%에서 양성이었고, 동반 질환은 류마치스 관절염이 2예, 특발성 혈소판 감소증 1예, Henoch-Sch nlein 자반증 및 그레브스병이 각각 1예 씩이었다. 7예에 대해서 스테로이드와 azathioprine으로 치료하여 생화학적 임상적 완해가 이루어져서 유효한 치료 결과를 얻었다. 결론 : 자가면역성 간염은 우리 나라에서 원인을 찾을 수 없는 만성 간질환의 경우, 특히 중년여성에서 다른 자가면역성 질환이 동반되었을 때 자가면역성 간염을 의심하여 자가항체 선별검사를 시행해 보아야 할 것으로 생각된다. 향후 다기관 연구를 통하여 좀 더 많은 증례를 모으고 전향적인 연구를 시행하여야 할 필요가 있다. Although autoimmune hepatitis(AIH) is common in western contries, some studties have been reported in Korea. The aim of the study was to analyze the clinical and histological features and expression pattern of the autoantibody and to examine clinical course of AIH in Korea. For 9 patients diagnosed as having AIH in our hospital since 1996, we reviewed medical records and analyzed clinical and laboratory findings retrospectively. They had a probable or definite AIH according to the revised criteria of International AIH group. The mean age was 42.7(21∼68)years and 7(77.8%) were female. Three of them(33.3%) showed acute hepatitis and five of them showed cirrhosis. Fatigue and anorexia were the most frequently complained symptoms(55.6%). Four patients(44.4%) were accompanied with other autoimmune diseases. Antinuclear antibody was detected in 5(55.6%) and anti-smooth muscle antibody was detected in 7(77.8%). Three patients showed focal picemeal necrosis and four showed periportal fibrosis. Among 7 patients who received immunosuppressive therapy, six patients(85.7%) showed initial response. and biochemical and serologic remission were achived in 5(71.4%) and 3(42.8%) patients, AIH seems to be partly responsible for chronic liver disease of unknown etiology in Korea. AIH should be suspected in female patients with unknown chronic liver disease of unknown etiolgy, especially accompanied with other autoimmune disease. The clinical characteristics of AIH may not be quite different from those of Western countries.

      • Gastric Calcifying Fibrous Tumor : A Rare Cause of Gastric Subepithelial Lesion

        석회화 섬유종양은 원인 미상의 흔하지 않은 양성 중앙이다. 이 종양은 복강이나 복막에서 기원하는 경우가 많으나 위에서 기원한 석회화 섬유종양은 매우 드물 다. 건강검진시 위내시경에서 우연히 발견된 위 점막하 종양 평가를 위해 28세 여성이 본원으로 전원되었다. 본원에서 시행한 위 내시경에서 위 중간체부에 1.2cm의 평탄한 표면의 위 점막하 종양이 관찰되었다. 초음파 내시경을 시행하였고 점막하층에 위치한 불균질한 저에코성의 병변과 함께 종양 내부에는 음향 음영을 동반한 다수의 고 에코성의 부위가 관찰되었다. 위 점막하 중앙은 악성종양을 완전히 배제할 수 없기 때문에 내시경적 점막하 절제술을 시행하였다. 조직 검사결과 종양은 석회화률 동반하는 호산구성 콜라겐 섬유가 풍부한 위 석회화 섬유 종양으로 진단되었다. 이에 위 점막하 중앙에 관한 다른 문헌을 고찰하면서 본 증례를 보고하는 바이다.

        Induction of Angiogenesis by Matrigel Coating of VEGF-Loaded PEG/PCL-Based Hydrogel Scaffolds for hBMSC Transplantation

        hBMSCs are multipotent cells that are useful for tissue regeneration to treat degenerative diseases and others for their differentiation ability into chondrocytes, osteoblasts, adipocytes, hepatocytes and neuronal cells. In this study, biodegradable elastic hydrogels consisting of hydrophilic poly(ethylene glycol) (PEG) and hydrophobic poly(${\varepsilon}$-caprolactone) (PCL) scaffolds were evaluated for tissue engineering because of its biocompatibility and the ability to control the release of bioactive peptides. The primary cultured cells from human bone marrow are confirmed as hBMSC by immunohistochemical analysis. Mesenchymal stem cell markers (collagen type I, fibronectin, CD54, $integrin1{\beta}$, and Hu protein) were shown to be positive, while hematopoietic stem cell markers (CD14 and CD45) were shown to be negative. Three different hydrogel scaffolds with different block compositions (PEG:PCL=6:14 and 14:6 by weight) were fabricated using the salt leaching method. The hBMSCs were expanded, seeded on the scaffolds, and cultured up to 8 days under static conditions in Iscove's Modified Dulbecco's Media (IMDM). The growth of MSCs cultured on the hydrogel with PEG/PCL= 6/14 was faster than that of the others. In addition, the morphology of MSCs seemed to be normal and no cytotoxicity was found. The coating of the vascular endothelial growth factor (VEGF) containing scaffold with Matrigel slowed down the release of VEGF in vitro and promoted the angiogenesis when transplanted into BALB/c nude mice. These results suggest that hBMSCs can be supported by a biode gradable hydrogel scaffold for effective cell growth, and enhance the angiogenesis by Matrigel coating.

        Comparison of Quality Traits of Thigh Meat from Korean Native Chickens and Broilers

        The quality attributes of thigh meat from a certified meat-type commercial Korean native chicken (WoorimatdagTM, WM) and a commercial broiler (Ross, CB) raised under the same environmental conditions, were compared. One-d-old, mixedsex WM (200), and 200 broilers (Ross, CB) each were randomly assigned to floor pens (20 chickens per pen). The production stages (I, II, and III) were selected based on similar live weight. The crude fat contents of WM thigh meat were lower than those of CB. WM thigh meat contained higher stearic and arachidonic acid contents but lower palmitoleic and oleic acid contents compared to those of CB. The thigh meat color CIE $L^*$- and CIE $b^*$-values of WM were lower, but inosine-5'-monophosphate content was higher in WM than CB in whole production stages. WM thigh meat showed higher collagen content, hardness, springiness, gumminess, and chewiness than CB. In sensory evaluation, odor, taste, and overall acceptance were higher while color was lower in WM. Based on these results, it can be concluded that even if commercial broilers are raised under the same environmental conditions as WM, the unique quality attributes of WM thigh meat are distinctively unique.

        Vaccination Policy in Korean Armed Forces: Current Status and Future Challenge

        <P>Infectious diseases have historically resulted in suspended or cancelled military operations. Vaccination for disease prevention is a critical component of the military's force readiness doctrine. Until recently, Korea had not recognized the importance of vaccinating military personnel. However, a 2011 meningococcal disease outbreak at an army recruit training center led to dramatic changes in the paradigm of traditional medical practice in the Korean armed forces. A new vaccination policy was formed by a 2012 Military Healthcare Service Act. Since then, <I>Neisseria meningitidis</I>, hepatitis A, and measles-mumps-rubella vaccines have been routinely administered to all new recruits early in basic training to ensure protection against these diseases. All active-duty soldiers also receive seasonal influenza vaccination annually. Despite quantitative improvements in vaccination policies, several instances of major infectious diseases and adverse vaccine reactions have threatened soldier health. In the future, vaccination policies in the Korean armed forces should be based on epidemiologic data and military medical research for vaccine use and safety management.</P>

        Background: Monovalent meningococcal conjugate vaccines against serogroup A or C had resulted in significant reduction of pharyngeal carriage rates against respective vaccine serogroup. However, the effect of quadrivalent meningococcal conjugate vaccine on meningococcal carriage has not been evaluated. We assessed on changing pharyngeal carriage rate of Neisseria meningitidis after the introduction of quadrivalent meningococcal conjugate vaccine in Korean military trainees. Methods: Twice oropharyngeal swabs were conducted at the beginning and end of army training period in Korean military trainees. First oropharyngeal swabs were undertaken prior to vaccination. Second round of swabbing was done 5 weeks later. Results: In 2013, overall carriage rate of N. meningitidis was higher at the end of training period (13.8%, 60 of 434 participants) than on entry of military service (8.9%, 39 of 434 participants). Although carriage rate of nongroupable and serogroup X isolates were increased over 5-week interval, carriage strains of vaccine serogroup, which were included serogroups in quadrivalent meningococcal vaccine, were relatively reduced from 7 to 2 isolates following vaccination. However, in 2014, overall carriage rate of N. meningitidis was significantly lower at the end of training period (5.2%, 23 of 443 participants) than on entry of military service (12.6%, 56 of 443 participants) (p=0.028). Carriage isolates of vaccine serogroups were significantly decreased with that of non-vaccine serogroup B over 5-week interval (p=0.006). Conclusion: Meningococcal vaccination in Korean military trainees resulted in reduction of pharyngeal carriage rates of not only vaccine serogroup meningococci but non-vaccine serogroup B. Quadrivalent meningococcal conjugate vaccine is likely to preventing carriage acquisition of meningococci in a closed population.
