        Local administration is closely related with intention of residents. Especially, a plan item is closely related with life of residents. Therefore, it has to be logical and objective solution for opinion convergence. Decision of opinion has to be in a triangular position standard and stand in a trio of criteria in standard. But, opinion convergence of residents very difficult. Recently, the technique of GIS presents method for opinion convergence with logical and objective and scientifically solution. And, this study present method for decision of intention to a complex element with GRID and NETWORK techniques of GIS. This study present optimun site of constructure with the GIS technique in consideration of a side face of transportation, technical, social economy and environments.

        左則 上두骨 및 頰骨에 發生한 殲維性 骨異形成症의 治驗例

        Fibrous dysplasia is an idiopathic skeletal disorder in which medullary bone is replaced and disturbed by poorly organized, structually unsound fibroosseous tissue, which may produce cortical expansion. When facial bones are involed, considerable esthetic deformity may result. The term monostotic fibrous dysplasia has been applied when one bone is involved : when more than one bone is affected, the term polyostotic used. The polyostotic form may be accomplished by pigmented skin lesion (Jaffe type), or by pigmented skin lesions with endocrine disturbance (Albright syndrome). No general agreement exists on the cause of fibrous dysplasia. A few authors have suggested that fibrous dysplasia arises as a resujlt of trauma. It occurs predominantly in infant, adolescent females and runs a variable clinical course. When several bones are involed, it tends to be unilateral. Involements of alveolar bone may produce displacement of teeth with malocclusion, or loss of teeth, or both. Radiographycally, ti shows an indistinctly delimited osteolytic defect with a bubble - like pattern, but without a sclerotic rim. The preferred treatment is almost always surgery. If the lesion is extensive, surgical intervention with use of recontouring procedures aimed at the correction of esthetic or funtional disturbances is preferred treatment. Now, we present a case of fibrous dysplasia on the left maxilla and the zygoma treated by bony contourign via hemicoronal flap and intraoral approach with good results.

        고압 커패시터의 고장 분석을 통한 신뢰도 예측

        본 논문은 고압 커패시터의 고장분석과 신뢰성 예측 결과를 다루고 있다. 부품의 수명과 고장률을 예측하기 위해서 두 가지 방법으로 고장 모드와 고장 메커니즘을 연구하였다. 에폭시 수지로 성형된 고압 커패시터가 절연내압 시험 하에서 저항이 제로로 되는 고장에 대하여, 근본원인 고장분석 체계를 효과적으로 수립함으로써 고장 메커니즘의 원인을 분석하였다. 특히 세라믹-에폭시 계면에서의 절연파괴 고장 현상이 강조되었으며, 본 연구에서 얻어진 결과의 타당성은 마그네트론에 장착된 고압 커패시터의 열사이클 시험 수행에 의한 가속시험 결과로부터 입증되었다. 시험 결과들은 결함이 있는 로트를 신속히 규명하고 B10수명을 결정하는데 유용하게 사용할 수 있다. 또한, 유전체의 절연파괴에 대해서 부하-강도 간섭모델을 이용하여 고장률을 예측하였다. This paper presents a result of failure analysis and reliability evaluation for high voltage ceramic capacitors. The failure modes and failure mechanisms were studied in two ways in order to estimate component life and failure rate. The causes of failure mechanisms for zero resistance phenomena under withstanding voltage test in high voltage ceramic capacitors molded by epoxy resin were studied y establishing an effective root cause failure analysis. Particular emphasis was placed on breakdown phenomena at the ceramic-epoxy interface. The validity of the results in this study was confirmed by the results of accelerated testing. Thermal cycling test for high voltage ceramic capacitor mounted on a magnetron were implemented. Delamination between ceramic and epoxy, which might cause electrical short in underlying circuitry, can occur during curing or thermal cycle. The results can be conveniently used to quickly identify defective lots, determine B10 life estimation each lot at the level of inspection, and detect major changes in the vendors processes. Also, thd condition for dielectric breakdown was investigated for the estimation of failure rate with load-strength interference model.

        화학물질 누출에 의한 대량재해 : Methyl Isocyanate 누출을 중심으로

        Background: We are in the edge of some human made disasters such as hazardous materials and air pollution, for example, the world news reported that the city of Bhopal, India had serious victims related with a leaking out of the chemical materials, Methyl Isocyanate and many people in India were killed by. These situations many people who live in this world are world are worrying about are not others, but just ours and people consider about that kind of the disasters are the possible situation to happen to all the people. Therefore, we performed this basic study to recognize the risk of Methyl Isocyanate leak accident and to prepare local disaster plan with EMS system. Method : Trace 8.0, a simulation software made by the U.S. company Safer System was used as a tool to estimate the diffusion distance, area and its victims at the concentrations of 0.02ppm, 0.2ppm 5ppm respectively for an assumed B-city of 2 hundred thousands population count in which was presumed 500kg Methyl Isocyanate gas to leak out. Results : 1. During 1 hour, maximum diffusion distances of 0.02ppm 0.2ppm and 5ppm were 5.41km, 1.61km and 0.29km respectively on the plume impact. 2. Maximum population counts influenced by Methyl Isocyanate gas at the concentrations of 0.02ppm 0.2ppm and 5ppm were 40838, 4346 and 222 on the plume impact, while those were 138238, 17261 and 1588 on the vulnerability impact, respectively. 3. Therefore, 17261 persons must put on respiratory device and 138283 persons must be evacuated to safety place within 1 hour. Conclusions : Only small amount leak of Methyl Isocyanate may cause tremendous chemical disaster in urban area so its disaster plan must be prepared with an accident simulation program and Material Safety Data Sheets(MSDS). Especially, nearby emergency center of an industrial complex must have a strong position about preparation of chemical disaster plan and perform a disaster dill of hazardous material accident annually.

      • 대장암종에서 암관련 유전자들의 Promoter Methylation과 암종 발생 및 진행과의 관게에 대한 연구

        The colorectal corcinoma is one of the most common malignant tumor in Korea and its carcinogenic mechanism has not been elucidated exactly yet. So far, the most common carcinogenic mechanism has been known to be multiple genetic changes. But, the CpG island hypermethylation has been revealed to be one of the most important carcinogenic mechanism second to the genetic changes. The epigenetic changes characteristically imprint and transfer to the next generation without changes of nucleotide sequences. When the CpG island of promoter or 5'- exon 1 of a specific gene is methylated by DNA methyltrasferases (DNMTs), the gene expression is inhibited. Carcinogenic mechansisms by promoter methylation has been well known, but the progression of the cancer has not been elucidated yet. The molecular researches on the carcinogenic mechanisms largely depended on the tumor tissue containing various components of cell, due to difficulties in harvesting pure homogeneous tumor cells. Promoter methylatonof the cancer related genes as APc, hMLH2, p16, DAP-K and MGMT in 50 cases of sporadic colorectal carcinoma was studied. The pure normal colorectal epithelial cells, and cancer cells from superficial and deep invasive portion were harvested respectively by laser captured microdissection(LCM), and DNA was extracted from them. The promoter methylation was studied by methylation specific poymerase reaction (MSP) after bisulfite modification of the DNA and the expression of the genes by immunohistochemistry. The results suggests that promoter methylation of the cancer related genes are closely related with the colorectal carcinogenesis and it occurrs from early phase of carcinogenesis, progressing constantly to the late stage. And the promoter methylation of normal epithelial cells increased by age. The p15, known to be the genes related to hematologic malignancy, was closely related to the colorectal carcinoma as well.

      • 간세포암에 대한 화학 색전 및 주입 요법 후에 발생한 발열과 감염성 합병증에 관한 전향적 연구

        목적 : 간동맥 화학 색전 및 주입 요법을 시행 받은 환자에서 시술 후 나타나는 발열 및 감염성 합병증의 빈도와 특성을 관찰하고, 이들 중 감염성 합병증이 발생한 환자들을 미래 예측할 수 있는 임상적 지표들이 있는 지 알고자 하였다. 방법 : 1997년 6월부터 8월까지 간세포암으로 간동맥 화학 색전 및 주입 요법을 시행받은 환자들을 대상으로 전향적으로 조사하였다. 화학 색전 및 주입 요법 시행 후 발열이 발생하였으나 감염 및 간 회 색전증의 증거 없이 자연히 소실된 경우에 한해 색전술 후 증후군이라고 정의하였다. 결과 : 284명에서 303예의 시술(화학 색전 요법 200예, 주입 요법 103예)이 시행되었다. 이중 108예(35.6%)에서 시술 후 발열이 있었다. 발열의 원인으로 감염성 합병증이 7예 (2.3%), 허혈성 담낭염이 5예(1.7%)였고 1예는 분류가 힘들어 나머지 95예(31.4%)를 색전술 후 증후군으로 분류하였다. 감염성 합병증은 배양 음성 중성구성 복수(culture-negative neutrocytic ascites)가 3예, 원발성 세균성 복막염이 2예(이중 1예는 패혈증 동반), 패혈송 쇽이 1예, 요로 감염증이 1예이었다. 색전술 후 증후군은 발열이 없었던 군에 비해 복수가 있거나(odds ratio [OR], 2.74; 95% contidence interval [CI], 1.30∼5.75) 종양이 클수록 (OR, 1/15; 95% CI, 1.06∼1.24), 도한 좌우 감동맥을 모두 막은 경우 (OR, 1.81; 95% CI, 1.00∼3.28) 빈번하게 발생하였다. 감염성 합병증일 때에는 색전술 후 발열이 있으면서 systemic inflammatory response syndrome(SIRS)의 기준을 만족하거나 (p<0.05) 시술 후 72시간 이후에 복부 동통이 발생하는 경우가 많았다.(p<0.01). 그러나 발열 시 감염을 미리 구별할 수 있는 검사실 소견은 없었다. 결론 : 간세포암에 대한 화학 색전 및 주입 요법 후 색전술 후 증후군은 31.4%, 감염성 합병증은 2.3%에서 발생하였다. 색전술 후 발열이 있으면서 SIRS의 기준을 만족하거나 시술 후 72시간 이 후에 복부 동통이 발생할 때에는 감염성 합병증을 반드시 고려해야 하겠다. Background : Transarterial chemoembolization(TACE) and chemoinfusion (TACI) are the methods used increasingly in hepatocelluar carcinoma patients. In this study, the incidence of fever and infectious complications after TACE or TACI in pateints with hepatocellular cacinoma was investigated and clinical parameters predicting such complications were evaluated. Methods : Prospective surveys were done in hepatocelluar carcinoma patients who had undergone TACE or TACI from June August, 1997. Three hundred three treatment cases with TACE (200) or TACI (103) in 284 patients were analyzed. The postembolization syndrome was defined as fever following TACE or TACI that spontaneously subsided without the evidence of infections or extrahepatic emboilizations. Results : One hundread eight (35.6%) out of 303 cases showed fever after the treatment. Regarding the etiology of the fever, after the treatment. Regarding the etiology of the fever, infections occurred in 7 cases (2.3%), and ischemic cholecystitis existed in 5 cases (1.7%). In one case the cause was not clear and 95 cases (31.4%) were classified as the postembolization syndrome. Out of the infections, three had culture-negative neutrocytic scites, two spontaneous bacterial peritonitis (one was accompanied with sepsis), one septic shock, and one urinary tract infection. The postembolization syndrome was more frequently found in the cases with ascites (odd ratio [OR], 2.74; 95% confidence interval[CI], 1.30∼5.75), with larger tumor (OR, 1.15; 95% CI, 1.06∼1.24) and with embolization both hepatic arteries (OR, 1.81; 95% CI, 1.00∼3.28). In the cases with infections, many satisfied the criteria for systemic inflammatory response syndrome (SIRS, P<0.05) or had abdominal pain 72 hours later (P<0.01) after the treatment. However, no other laboratory data predicted infection among the cases with fever. Conclusion : After TACE or TACI in hepatocellular carcinoma patients, 31.4% of cases showed the postembolization syndrome and 2.3% of cases showed infection. Infection should be considered as a cause of fever in patients satisfying the criteria for SIRS or with abdominal pain 72 hours later after the treatment.

        유치의 치근단 병소에 의해 변위된 영구치의 자발적 맹출

        The infection of primary teeth occurs by caries or trauma. But, if it is not treated on time, the complication may occur such as enamel hypoplasia, discoloration, impaction, displacement and dilaceration of permanent successors. The periapical lesion on primary tooth could displace the permanent successors in any direction. Treatment options of displaced tooth are observation after extracting the primary teeth, surgical exposure, orthodontic traction, transplantation and extraction. In these cases, displacement of permanent central incisor caused by the periapical lesion of primary teeth was observed. The spontaneous eruption after extraction of primary teeth was expected considering degree of displacement, development of root and direction of eruption. This case report shows spontaneous eruption of displaced permanent tooth germ was achieved with periodic examination after extraction of primary teeth.

      • 최대하 운동부하검사가 노인 허혈성 뇌졸중환자의 혈관탄성·운동유발성 고혈압 및 허혈성 심장질환진단에 미치는 영향 : 사례연구

        Purpose: The purpose of this study was to observe the blood pressure response during exercise test and to investigate baPWV and ABI and potentiality of the cardiovascular disease of three ischemic stroke elderly in submaximal exercise test. Method: It measured the blood pressure, ECG during submaximal exercise test and baPWV, ABI before and after exercise test. Results: It discovered low blood vessel elasticity, exercise induced hypertension, ST segment depression ≥ 1 mm through exercise test. Conclusion: Exercise test have a diagnosis valuable to decide a cardiac disorders. Moreover, It increases a sensibility of baPWV and ABI test and heighten a reliability of potential diagnosis of high blood pressure by submaximal exercise test. 뇌졸중(stroke)은 가령(加齡)과 함께 증가하며 크게 허혈성과 출혈성 형태로 니누고 근래에는 허혈성 뇌졸중의 발생빈도가 증가하고 있는 추세로 뇌졸중 환자의 혈류역학에 대한 연구는 순환기학 분야에서 뿐만 아니라 운동이 고혈압의 주요 개선 중재방법의 하나라는 관점에서 운동처방 분야에서도 심도 있는 연구가 요구된다. 그러므로 이 연구에서는 60세 이상의 허혈성 뇌졸중환자 3명에게 최대하 운동부하검사를 실시하여 각각의 사례에서 안정시에는 발견되지 않았던 혈관폐색의 높은 가능성, 운동유발성 고혈압 및 허혈성 심장질환의 정후인 ST분절≥1.0 mm 하강을 발견하였다. 이러한 점에서 운동부하검사는 심장질환을 판단하는 진단적 가치뿐만 아니라 baPWV와 ABI검사의 민감성을 증가시키며 고혈압 잠재성 진단에 유용하게 활용될 수 있다.

      • 황색 포도상구균의 vancomycin 내성 원인 유전자의 동정

        임상 분리되는 Gram 양성균인 황색 포도상구균(Staphyhcoccus aureus)에 대한 각종 학생물질의 최소 발육저지농도 측정(Minimal Inhibitory Concentration: MIC test)과 내성 pattern을 조사하였으며, 이들 균주 중 특히 vancomycin에 대하여 높은 내성을 나타내는 균주(Vancomycin resistant Staphylococcus aureus: VRSA)에 대한 vancomycin 내성원인 유전자의 탐색 및 cloning을 실시하여 vancomycm 내성기구의 연구에 필요한 조사를 수행하였다. Vancomycm에 내성인 균주(VRSA)를 고온(43.5℃) 처리하여 계대 배양을 한 다음, replica plating method에 의 해 vancomycin 감수성 균주(Vancomycin-sensitive Staphylococcus aureus: VSSA)를 얻었으며, VRSA 균수와 VSSA 균주로부터 membrane protein을 준비한 다음 SDS-poly-acrylamide gel electrophoresis(SDS-PAGE)로 전기 영동하여 그 단백질 pattern을 비교한 결과, VRSA 균주에서는 분자량(Mr: relative molecular weight)이 약 127Kadl 그리고 30Kdal 정도 되는 부위에서 vancomycin 내성에 관계되는 듯한 특이적인 단백질을 관찰할 수 있었다. 그러나 VSSA 균주에서는 이러한 단백질을 관찰할 수 없었다. 한편 이들 균주로부터 plasmid DNA를 분리한 다음 agarose gel electrophoresis하여 그 pattern을 분석한 결과, VRSA 균주에서는 약50Kb부위에서 plasmid DNA밴드가 관찰되었으나 VSSA 균주에서는 아무런 밴드도 관찰되지 않았다. 이상의 결과로 볼때 vancomycin 내성유전자는 plasmid DNA상에 존재한다고 생각되어진다. This study was carried out to study the characterization of the gene that is responsible for the vancomycin-resistant Staphylococcus aureus (VRSA) In order to obtain vancomycin-sensitive S. aureus(VSSA)from VRSA stain, vancomycin-resistant S. aureus(VRSA) was subcultured twice at the high temp.(43.5℃)and then VSSA was obtained by replicaplating method after MIC test with vancomycin in high temp. treted VRSA strain. Membrane proteins were prepared from VRSA and VSSA strain and protein patterns of those were compared bny 7.5% SDS-polyacrylamide gel electrophoresis. Only in VRSA strain appeared novel protein bands at a position about 120 Kdal, 80 Kdal and 30Kda1, but it did not appear in VSSA strain. The patterns of plasmid DNA in VRSA and VSSA strain were clearly classified. Plasmid DNA band in VSSA strain appeared at a position about 50Kb, but it did not appear any band in VSSA strain. These results suggested that plasmid DNA may be encode the gene that is responsible for vancomycin-resistance.

        타타늄의 열처리 조건에 따른 동전위 양극분극특성 및 표면 미세구조 변화

        The corrosion resistance of metallic implants is essential for successful implant osseointegration. Moreover, the implant surface should have appropriate surface roughness for the attachment of the osteoblast and collagen fibrils. When titanium is exposed to air a thermodynamically very stable oxide film is formed at once. This oxide is very thin and yet it protects the metal from corrosion instantaneously. However, the oxide would not be ideal, and it would be supposed that the state of oxide film would be various by the different conditions when it is formed. The objective of this study was to evaluate the electrochemical properties and surface microstructural changes of titanium after passivation treatment or exposure to high heat (in vacuum or in air). ASTM grade 2 commercially pure titanium (Ti) disks of 10㎜ diameter were wet ground and polished with 240 and 600 grit SiC, and then ultrasonically cleaned with distilled water and ethanol. Cleansed as-received samples (As-R group) were passivated with 30% nitric acid solution at 50℃ for 15 minutes to make the Pas group. The Std-V samples were made by treating the Ti disks in vacuum quartz tube (10^-8 Torr) at 530℃ for 40 min for stress relieving, followed by annealing at 700℃ for 90 min, and then slow cooling to room temperature. The Std-A samples undergo identical procedure as the Std-V group except that it is done in air. For 530℃-V samples, Ti disks were heated at 530℃ for 40 minutes and slow cooled to room temperature. The 1000℃-V samples were produced by heating Std-V specimens again at 1000℃ for 2 hr, followed by cooling to room temperature. 530℃-A and 1000℃-A samples were also prepared. Potentiodynamic anodic polarization measurement, SEM, AFM and XRD analysis were performed to evaluate the effects of the various treatments. The difference in the mean values of E_corr between groups showed statistically significant difference (P<0.05), but that of I_corr did not show significance (P>0.05). When Ti surface was passivated there was a shift of corrosion potential (E_corr) in the noble direction and a lower corrosion current density (I_corr) value. Std-A samples showed most noble E_corr value among the tested groups. 530℃-V samples showed more noble E-corr than other tested groups except Std-A group. 1000℃-V samples showed more noble E-corr than those of Std-V sample. In the case of Std-, 530℃- and 1000℃-groups, -A sub-groups showed better corrosion resistant characteristics than -V sub-groups in regard to E-corr and I_corr values. In the SEM observations, the surfaces of the As-R samples showed only scratch line on smooth surface. In Pas samples, thick oxide layer covered the surface under high magnification observation. 530℃-A samples were covered by oxide granules, and Std-A samples had 25~50 nm-thick acicular oxides. The 1000℃-A samples were covered by coarse oxide crystals with a granule diameter of 0.1~0.6 μm having a gross appearance of milky white color, which was identified by XRD as a rutile. On the other hand, 1000℃-V samples showed an Widmansta¨tten structrue of which the orientation of the lattice is formed along crystallographic planes of the parent crystals. On the AFM observation, 530℃-A samples showed RMS roughness of 79.38 nm which is 5 times rougher than those of the As-R samples. Std-A sample had a roughness of 330.47 nm which is about 20-times rougher than that of the As-R sample. 1000℃-A samples were covered by coarse granular oxides and the roughness were about 0.56 μm. Vacuum sub-group samples showed a more smooth surface than air sub-group samples, and the oxide film was more dense. Due to variability of the surface treatment and heat treatment conditions, the process of the oxide formation seems to be affected thereby making the corrosion properties and surface micro-morphology different. However, more various electrochemical analysis together with surface analysis, and an investigation for the effects of those on the osteoblast cell adhesion and calcium phosphate apposition are needed for the improvement of the osseointegration rates of the titanium implants.
