Spontaneous coronary artery dissection (SCAD) is an uncommon cause of acute coronary syndrome which may be related to lethal condition. Although several modalities including medical therapy have been suggested, agreement on optimal treatment has not yet been determined. We describe a case of SCAD which was presented as ST-segment elevation myocardial infarction, and treated successfully with medical treatment. Coronary angiography, intravascular ultrasound and multi-detector computed tomography showed the serial changes of this disease entity.

Background and Objectives:Nitric oxide (NO) is thought to have antiatherosclerotic properties. On the otherhand, NO activity is reduced in patients with metabolic syndrome, and endothelial dysfunction is an important earlysign of atherosclerosis in patients with metabolic syndrome. The aim of this study was to investigate the effect ofpioglitazone on the endothelial function in terms of the plasma NOx (combined nitrate/nitrite), the circulatinginflammatory markers and the autonomic nervous system. Subjects and Methods:We randomized 40 subjects withmetabolic syndrome, and they were assigned to receive 15 mg of pioglitazone per day (the PIO group, n=21) during12 weeks or they were placed in the placebo group (the PLA group, n=19). We estimate the endothelial function byperforming vascular ultrasound. The plasma NOx levels, the levels of the inflammatory markers and the GRK2 levelswere measured. Results:After 12 weeks of therapy, flow mediated dilation (FMD) was improved in the PIO group(from 6.7±6% to 11.7±5%, respectively: p<0.05), but not in the PLA group. The level of plasma NOx was increasedin the PIO group (from 67.7±30 nmol/dL to 92.9±41 nmol/dL, respectively: p<0.001), but not in the PLAgroup. The plasma levels of hsCRP and IL-6 dropped significantly (from 2.6±2.3 mg/L to 1.2±1.3 mg/L and1.7±2.1 pg/mL to 0.7±0.5 pg/mL, respectively: p<0.05) in the PIO group, but not in the PLA group. The levelsof GRK2 (the PLA group from 0.0061±0.0023 ng to 0.0075±0.0031 ng, and the PIO group from 0.0024±0.002ng to 0.0015±0.001 ng, p=ns) didn’t dropped significantly. Conclusion:Administration of PPAR-γ agonist inpatients suffering with metabolic syndrome improves their endothelial function, enhances the production of NOxand reduces the proinflammatory markers, but this is not related to sympathetic regulation. PPAR-γ agonist maybe able to modulate the progression of atherosclerosis. (Korean Circulation J 2007;37:221-229) 배경 및 목적:Nitric Oxide(NO)는 강력한 혈관확장인자로서 항동맥경화효과가 있다. 대사증후군환자는 이런 NO 활성도가 저하되어있으며 동반된 내피세포기능장애는 동맥경화의 중요한 조기징후이다. 본 연구의 목적은 pioglitazone이 혈관내피세포기능에 미치는 영향을 혈장 NOx, 염증인자와 교감신경계활성화측면에서 밝혀보고자 하는데 있다.방 법:40명의 대사증후군환자를 대상으로 12주 동안 하루 1회 15mg의 pioglitazone(PIO군, 21명) 또는 위약(PLA군, 19명)을투여하였다. 내피세포기능을 평가하기 위해 혈관 초음파로 상완동맥에서 FMD를 측정하였다. 염증표식자로 ESR, interleukin-6, high-sensitivity C-reactive protein(hsCRP)를측정하였으며 real-time reverse transcriptase-polymerasechain reaction(RT-PCR)로 임파구내 GRK2를 측정하였다. 결 과: 12주 동안 치료 후 내피세포기능을 반영하는 충혈에 의한 유량-매개성 확장 백분율은 PIO군에서만 통계학적으로 유의 하게 개선되었다(6.7±6%에서 11.7±5%: p<0.01). 혈장 NOx 는 PIO군에서만 유의하게 증가하였고(67.7±30 nmol/dL에 서 92.9±41 nmol/dL: p<0.001). 혈장내 hsCRP와 IL-6는 PIO군에서만 유의하게 감소하였다(2.6±2.3 mg/L에서 1.2± 1.3 mg/L, 1.7±2.1 pg/mL에서 0.7±0.5 pg/mL: 각각 p< 0.05). 그러나 임파구내 GRK2는 두 군 모두에서 유의한 차 이가 없었다. 결 론: 대사증후군환자에서 PPAR-γ 작용제투여는 혈관내피세포 에서 NO 생성을 촉진하고 전염증성인자를 감소시키는 기전을 통해 혈관내피세포기능을 개선시킨다. PPAR-γ 작용제는 죽 상동맥경화의 진행을 억제하는 효과가 생각된다.
통계적 Correlation을 이용한 다중센서 영상 정합

현재 많이 연구되는 영상융합(Image fusion)에서는 필히 두 영상의 정합(alignment)이 이루어져야만 수행 된다. 각기 다른 특징을 갖는 센서(EO,IR,Radar등)로부터 얻는 영상에서는 각각 다른 특징점 정보를 가지므로, 특징점을 이용한 영상 정합 구현에는 전처리 과정이 매우 복잡하고 까다롭게 이루어져야 한다. 본 논문에서는 Correlation에 대한 통계적 상관 관계를 이용하여, 전처리 과정을 단순하게 수행 하여도 매우 강건한 영상 정합이 이루어지도록 구현 하였다. 또한, 통계적 기법에 적합하도록, 효율적인 전처리 과정을 통해 계산량이 적어 지는 방법을 제안 한다.

Isolated left ventricular noncompaction (LVNC) is a rare disorder caused by embryonic arrest of compaction. LVNC is sometimes associated with other congenital cardiac disorders; however, there have been few reports of its coexistence with a left ventricular aneurysm. A 40-year-old woman was admitted to our hospital for renal infarction. She had a history of embolic cerebral infarction 10 years ago. Transthoracic echocardiography showed prominent trabeculae and deep intertrabecular recesses which are filled with blood from the left ventricular (LV) cavity. A thrombus in the akinetic apical wall was confirmed by contrast echocardiography. Using cardiac computed tomography and magnetic resonance imaging, we rejected a possible diagnosis of suspicion of coronary artery disease. She was diagnosed LVNC with a thrombus in apical aneurysm. Here, we report the first patient in Korea known to have LVNC accompanying LV congenital aneurysm presenting with recurrent embolism.
맵핑용 3차원 영상 레이저 레이다의 시스템 설계 및 성능 분석

The system design and the system performance analysis of 3D imaging laser radar system for the mapping purpose is addressed in this article. For the mapping, a push-bloom scanning method is utilized. The pulsed fiber laser with high pulse energy and high pulse repetition rate is used for the light source of laser radar system. The high sensitive linear mode InGaAs avalanche photo-diode is used for the laser receiver module. The time-of-flight of laser pulse from the laser to the receiver is calculated by using high speed FPGA based signal processing board. To reduce the walk error of laser pulse regardless of the intensity differences between pulses, the time of flight is measured from peak to peak of laser pulses. To get 3D image with a single pixel detector, Risley scanner which stirs the laser beam in an ellipsoidal pattern is used. The system laser energy budget characteristics is modeled using LADAR equation, from which the system performances such as the pulse detection probability, false alarm and etc. are analyzed and predicted. The test results of the system performances are acquired and compared with the predicted system performance. According to test results, all the system requirements are satisfied. The 3D image which was acquired by using the laser radar system is also presented in this article.
A Case of Thymic Cyst In The Middle Mediastinum Mimicking Pericardial Cyst

A thymic cyst in the middle mediastinum adjacent to the right pericardium is extremely rare. We report a case of a large thymic cyst in the right cardiophrenic angle, compressing the right atrium, which was presented as a cardiomegaly on the chest radiograph and a pericardial cyst on the echocardiography. The definitive diagnosis was confirmed using surgical resection and biopsy.
