Background: Several cases of pediatric acute hepatitis of unknown etiology related to adenoviral infections have been reported in Europe since January 2022. The aim of this study was to compare the incidence, severity, possible etiology, and prognosis of the disease with those in the past in Korea. Methods: The surveillance group collected data between May and November 2022 using a surveillance system. Acute hepatitis of unknown etiology was defined in patients aged < 16 years with a serum transaminase level > 500 IU/L, not due to hepatitis A-E or other underlying causes. For comparison, data from 18 university hospitals were retrospectively collected as a control group between January 2021 and April 2022. Results: We enrolled 270 patients (mean age, 5 years). The most common symptom was fever. However, the incidence was similar between 2021 and 2022. Liver function test results, number of patients with acute liver failure (ALF), liver transplantation (LT), death, and adenovirus detection rates did not differ between the two groups. None of the adenoviruspositive patients in either group experienced ALF, LT, or death. In the surveillance group, adenovirus-associated virus-2 was detected in four patients, one of whom underwent LT. Patients with an unknown etiology showed significantly higher bilirubin levels, a lower platelet count, and a higher LT rate than patients with a possible etiology. Conclusion: The incidence of pediatric acute hepatitis of unknown etiology and adenovirus detection rate have not increased in Korea.
Atypical Dysphagia in an Anti-Hu Positive Non-Small-Cell Lung Cancer Patient

Anti-Hu antibodies usually present in small cell lung cancer are associated with encephalomyelitis, encephalitis, cer-ebellitis, and neuropathies. However, to our knowledge, non-small-cell lung cancer (NSCLC) related anti-Hu syn-drome presenting acute exacerbation of dysphagia has not been reported. Herein, we report a rare case of atypical dysphagia in an anti-Hu positive NSCLC patient resulting in a fatal outcome. A 73-year-old Korean male went through surgical interventions for recurrent tongue cancer and NSCLC. After the surgery, he resumed full oral feed-ing with a regular diet. However, four weeks later, he developed a sudden difficulty in swallowing. Neck computed tomography angiography, brain magnetic resonance imaging, brain positron emission tomography, computed tomog-raphy and electromyography all showed nonspecific findings. A paraneoplastic antibody test was positive for an-ti-Hu antibodies. His dysphagia worsened and eventually, he expired due to malignant pleural effusion. Our case shows that anti-Hu antibodies can be expressed in NSCLC and when associated with dysphagia, the outcome can be fatal.

난소의 과립막세포종은 성기삭-간질 종양의 3/4을 차지하고 있지만 모든 난소암의 약 2-5% 밖에 되지 않는 드문 종양이다. 유년형 과립막세포종은 성인형 과립막세포종과는 임상적, 병리학적, 생물학적 양상이 서로 다르게 나타나는 특징을 가진다. 초경전의 여아에서 유년형 과립막세포종은 성조숙증을 보이는 난소 종양 중에서는 가장 흔한 원인으로 약 80% 이상에 이르는 것으로 보고되고 있다. 종양의 90% 이상이 조기병기에서 발견되며 예후가 비교적 양호하지만 진행된 병기에서는 불량한 예후를 나타낸다. 저자 등은 최근 유년형 난소 과립막세포종1예를 경험하였기에 간단한 문헌 고찰과 함께 보고하는 바이다. Granulosa cell tumors of the ovary account for 2-5% of all ovarian malignancies but, nearly three-quarters of all sex-cord stromal tumors. Juvenile granulosa cell tumors differ from the adult type in regard to their clinical and pathological features and biological behavior. In premenarcheal girls, these tumors are associated with sexual precocity. About 90% are diagnosed in early stage (FIGO I) with a favorable prognosis. More advanced stages (FIGO II-IV) have a poor clinical outcome. We experienced a case of juvenile granulosa cell tumor and report it with a brief review of literatures.
S-597 Basal Cell Carcinoma of Prostate with Liver Metastasis

Background: Basal cell carcinoma (BCC) of the prostate is a rare tumor. Diagnosis is made difficult, because serum prostate specific antigen (PSA) of BCC is usually within normal limit. Diagnosis is made relying on pathologic and immunohistochemistry findings. Author reports a case of a 67-year-old male with right upper quadrant mass came to clinic, which turned out to be Basal cell carcinoma of prostate with liver metastasis. CASE A 67-year-old male with right upper quadrant mass came to our clinic. Computed tomography (CT) of the abdomen and pelvis revealed multiple liver masses and another mass (3.5 cm diameter) in prostate gland. Serum PSA level was 0.669 ng/ml. Examination of a needle biopsy of liver showed metastatic carcinoma. We performed another biopsy of prostate and it was identified as same pathology of liver. Also, the immunohistochemistry (positive for BCL-2, p63, 34βE12) was consistent with BCC of prostate. The patient died after several months despite of systemic chemotherapy. LABORATORY FINDINGS TUMOR MARKER AFP: 3.46 ng/ml, PIVKA-II: 15 mAU/ml, PSA: 0.669 ng/ml DISCUSSION We present a rare case of basal cell carcinoma of prostate with hepatic metastasis, which is the first Korean case reported up to date. High level of Ki67 is related to metastasis. (Patient ranged 70%). This case shows BCC of the prostate should be suspected, if the patient has a prostate mass and metastatic lesion with normal PSA level. As the prognosis of disease is poor and the treatmentis not established, further studies are fundamental.

이 연구는 가상현실(VR) 환경에서 사용자 경험을 개선하는 것을 궁극적인 목표로 삼고 있다. 연구의 주요 초점은 VR 게임을 통해 발생하는 VR 멀미의 정도를 측정하고, 가상 세계에서의 사용자 행동과의 연관성을 분석하는 데 있다. 이를 위해, 시뮬레이터 멀미 질문지(SSQ)를 활용하여 사용자가 경험하는 멀미를 정량적으로 평가하였고, VR 게임 사용 중 수집된 사용자 활동 데이터를 분석함으로써 VR 멀미와 사용자 행동 간의 상호작용을 깊이 있게 이해하려 하였다. 이 연구는 VR 멀미에 따른 사용자 행동에 대한 통찰을 제공한다.
Whole-exome sequencing identifies recurrent <i>AKT1</i> mutations in sclerosing hemangioma of lung

<P>Pulmonary sclerosing hemangioma (PSH) is a benign tumor with two cell populations (epithelial and stromal cells), for which genomic profiles remain unknown. We conducted exome sequencing of 44 PSHs and identified recurrent somatic mutations of AKT1 (43.2%) and beta-catenin (4.5%). We used a second subset of 24 PSHs to confirm the high frequency of AKT1 mutations (overall 31/68, 45.6%; p.E17K, 33.8%) and recurrent beta-catenin mutations (overall 3 of 68, 4.4%). Of the PSHs without AKT1 mutations, two exhibited AKT1 copy gain. AKT1 mutations existed in both epithelial and stromal cells. In two separate PSHs from one patient, we observed two different AKT1 mutations, indicating they were not disseminated but independent arising tumors. Because the AKT1 mutations were not found to co-occur with beta-catenin mutations (or any other known driver alterations) in any of the PSHs studied, we speculate that this may be the single-most common driver alteration to develop PSHs. Our study revealed genomic differences between PSHs and lung adenocarcinomas, including a high rate of AKT1 mutation in PSHs. These genomic features of PSH identified in the present study provide clues to understanding the biology of PSH and for differential genomic diagnosis of lung tumors.</P>

The Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition newly developed the first Korean Guideline on the Diagnosis and Treatment of Obesity in Children and Adolescents to deliver an evidence-based systematic approach to childhood obesity in South Korea. The following areas were systematically reviewed, especially on the basis of all available references published in South Korea and worldwide, and new guidelines were established in each area with the strength of recommendations based on the levels of evidence: 1) definition and diagnosis of overweight and obesity in children and adolescents; 2) principles of treatment of pediatric obesity; 3) behavioral interventions for children and adolescents with obesity, including diet, exercise, lifestyle, and mental health; 4) pharmacotherapy; and 5) bariatric surgery.
