        인슐린비의존형 당뇨병 환자에서 Lipoprotein(a)농도와 혈관합병증

        연구배경: 당뇨병 환자에서 동맥경화증의 발생위험이 정상잉인에 비해 높은 원인의 하나로써 혈액응고와 섬유소용해작용의 이상을 초래하는 Lipoprotein(a) [Lp(a)]가 논의되고 있다. 당뇨병 환자의 동맥경화증과 당뇨병성 합병증 발생위험 증가에 Lp(a)가 어느 정도 관여하는지에 대해서도 아직 확실히 알려져 있지 않다. 저자등은 인슐린비의존형 당뇨병 환자의 혈관합병증(대혈관 및 미세혈관합병증) 발생과 Lp(a)농도와의 관계 및 혈관합병증을 일으키는 다른 위험인자와 Lp(a)농도와의 관련성에 대하여 알아보고자 본 연구를 시행하였다. 방법: 인슐린비의존형 당뇨병 환자 152명(여자:남자=72:80)을 대상으로 공복상태에서 혈중 Lp(a), ApoA1, ApoB, 총 콜레스테롤, 중성지방, 고밀도지단백질-콜레스테롤 등을 측정하였다. 대혈관합병증(허혈성 심질환, 뇌혈관질환, 말초혈관폐쇄성 동맥경화증)의 진단을 위해 증상이 있는 환자들에서 심전도, 관상동맥조영술, 뇌전산호단층촬영, Doppler velocimetry, 말초혈관조영술 등을 시행하였고, 미세혈관합병증은 안저활영, 신경전달속도, BBV(beat to beat variation), VPT(vibration perception threshold), 24시간 요단백량 등을 측정하여 확인하였다. 결과: 허혈성 심질환, 뇌혈관질환등 대혈관질환 및 당뇨병성 망막증 등 미세혈관질환을 갖는 환자에서 Lp(a)농도가 대조군보다 통계적으로 유의하게 증가되어 있었다. Lp(a)농도가 30㎎/dL이상인 군에서 ApoB/ApoA1 및 저밀도지단백질-콜레스테롤이 Lp(a)농도가 30㎎/dL 이하인 군보다 높았으며, 혈관질환을 갖는 군에서 Lp(a)는 높은 ApoB/ApoA1와 저밀도지단백질-콜레스테롤과 통계적으로 의의있는 양의 상관관계를 보였다. 결론: Lp(a)는 인슐린비의존형 당뇨병 환자의 대혈관합병증과 미세혈관합병증(특히 당뇨병성 망막증)에 관련이 있는 것으로 보이며, 특히 저밀도지단백질-콜레스테롤 및 ApoB/ApoA1이 높게 나타난 인슐린비의존형 당뇨병 환자에서 Lp(a)농도 측정이 필요하리라 생각된다.

      • Coacervation-상분리법으로 만든 Ethyl Cellulose 마이크로캅셀의 Glibenclamide 방출특성 및 수용성 첨가제의 영향

        Glibenclamide (GBC) is a well-known antidiabetic agent, which stimulates the secretion of endogenous insulin by pancreatic βcells. However, administration of GBC at a high dose can occasionally induced the fatal hypoglycemia, especially in the patients with impaired liver function. Therefore, in order to develop an oral sustained-release preparation which reduces the side effect, microcapsulation of GBC has been carried out in this experiment. Microcapsules of GBC were prepared by coacervation-phase separation method with ethylcellulose as a wall-forming material in cyclohexane, using polyisobutylene as a coacervation-inducing agent. Different amounts of hydrophilic additives. L-arginine(ARG) and polyethylene glycol 4000 (PEG), were added to the microcapsule wall, in order to alter the porosity of the wall and hence to enhance the release of the core material. The microcapsules prepared were examined for physical properties by scanning electron microscopy (SEM) and particle size analysis. The release of the poorly water-soluble GBC was found to be very slow from the microcapsules and strongly dependent on the core to wall ratio of the microcapsules, but it was accelerated considerably with increasing amount of PEG or ARG. In general, on the basis of dissolution kinetics, GBC releases from microcapsules were followed apparent first-order kinetics, while the dissolution data of GBC powder conformed to Hixon-Crowell's cube root law. Therefore, in conclusion, it might be possible to design the sustained-release formulation by the combination of both GBC powder and microcapsules of different core wall ratio with or without hydrophilic additives.

      • 산국 (Chrysanthemum boreale) 휘발성 추출액 및 EDTA가 미생물 및 인체 치은 섬유아세포에 미치는 영향

        Chrysanthemum boreale (C. boreale) has been used for the treatment of furuncles, carbuncles, sores, conjunctivitis, headache, vertigo and swollen throats as a folk medicine in Korea, but there is little report about experiment evidence of antimicrobial activity. To investigate the antifungal and antibacterial activity of Chrysanthemum boreale(C. boreale) volatile substance and EDTA, the growth inhibition against Aspergillus nidulans (A. nidulans), Fusarium oxysporum (F. oxysporum), Alternaria mali(Al. mali), Candida albicans (C. albicans), staphylococcus aureus (S. aureus), Streptococcus pyogenes(S. pyogenes), streptococcus mutans (S. mutans), Escherichia coli(E. coli), Helicobacter pylori (H. pylori) and Porphyromonas gingivalis (P. gingivalis) was monitored by agar dilution and broth microdilution method. The antifungal effect of C. boreale on A. nidulans, F. oxysporum, Al. mali and C. albicans was shown at the concentration of 0.04%. The antibaterial effect of C. boreale on S. aureus, S. pyogenes, S. mutans and H. pylori was shown at the concentration of 0.04%. However, the growth was inhibited at the concentration of 0.04% irrespective of the presence of EDTA 0.125% was much higher than higher than that of several concentration of volatile substance. The minimum inhibitory concentrations of volatile substance were 0.16% against S. aureus, 0.04% against S. pyogenes, 0.02% against S. mutans, 0.40% against E. coli, and 0.18% against C. albicans. The growth of human gingival fibroblasts was not affected by C. boreale volatile substances at the concentration of 0.04%. These results indicate that volatile substance from C. boreale may have biologically toxic activities to the microorganisms.

      • 5일 보존용 채혈백에 따른 보존일자별 농축혈소판의 생화학 및 성상 분석

        Biochemical changes and icon of platelets on platelet concentrates preserved for a 5day term in several plastic containers were investigated. The authors evaluated the TOTM-PVC second generation container produced by Green Cross Medical Corp. and Boin medica. Blood obtained from 40 persons was manufactured for platelet concentrates within 4 hours and each divided into 20 units of platelet concentrations. Samples were taken at 0, 3, and 5days and the platelet count, pH, pO_(2), pCO_(2), HCO_(3)-, glucose, LDH, lactate, electrolytes, and mineral element were tested The number and MPV, for platelets icon, were well preserved on two different platelet concentrates during storage. A test for biochemical function appeared to be effective data. pH was maintained above 6.8 on two different platelet concentrates during storage. The results suggest that the TOTM-PVC second generation platelets storage container produced by Green Cross Medical Corp. and Boin medica was able to preserve platelets for at least five days without lesion and was it concluded to have biochemical efficacy on patients with transfusions.

        Chronic spontaneous urticaria (CSU) is defined as the occurrence of spontaneous wheals, angioedema, or both for >6 weeks in the absence of specific causes. It is a common condition associated with substantial disease burden both for affected individuals and societies in many countries, including Korea. CSU frequently persists for several years and requires high-intensity treatment; therefore, patients experience deteriorations in quality of life and medication-associated complications. During the last decade, there have been major advances in the pharmacological treatment of CSU and there is an outstanding need for evidence-based guidelines that reflect clinical practice in Korea. The guidelines reported here represent a joint initiative of the Korean Academy of Asthma, Allergy and Clinical Immunology and the Korean Dermatological Association, and aim to provide evidence-based guidance for the management of CSU in Korean adults and children. In Part 1, disease definition, guideline scope and development methodology as well as evidence-based recommendations on the use of antihistamines and corticosteroids are summarized.

      • Genotoxicity Study of Sophoricoside, a Constituent of Sophora japonica, in Bacterial and Mammalian Cell System

        Sophoricoside was isolated as the inhibitor of IL-5 bioactivity from Sophora japonica (Leguminosae). It has been reported to has an anti-inflammatory effect on rat paw edema model. To develope as an anti-allergic drug, genotoxicity of sophoricoside was investigated in bacterial and mammalian cell system such as Ames bacterial reversion test, chromosomal aberration assay and single cell gel electrophoresis (Comet) assay. As results, in the range of 1,250~40 $\mu\textrm{g}$/plate sophoricoside concentrations was not shown significant mutagenic effects in Salmonella typhimurium TA 98, TA 100, TA 1535 and TA 1537 strains in Ames test. The 80% cell growth inhibition concentration (IC/SUB 80/) of sophoricoside was determined as above 5,000 $\mu\textrm{g}$/$m\ell$ in Chinese hamster lung (CHL) fibroblast cell and L5178Y mouse lymphoma cell line for the chromosomal aberration and comet assay, respectively. Sophoricoside was not induced chromosomal aberration in CHL fibroblast cell at concentrations of 700, 350 and 175 $\mu\textrm{g}$/$m\ell$ or 600, 300 and 150 $\mu\textrm{g}$/$m\ell$ in the absence or presence of S-9 metabolic activation system, respectively. Also, in the comet assay, the induction of DNA damage was not observed in L5178Y mouse lymphoma cell line both in the absence or presence of S-9 metabolic activation system. From these results, no genotoxic effects of sophoricoside were observed in bacterial and mammalian cell systems used in these experiments.

        Induction of Oral Tolerance to Japanese Cedar Pollen

        Oral tolerance is thought to play a role in preventing allergic responses and immune-mediated diseases. An improved mouse model of the oral tolerance to Japanese cedar pollen (JCP) as antigen was developed in order to detect induction of the tolerance, and the immunological characteristics of this model were also elucidated. Oral tolerance was induced by C3H/ HeN mice given an oral administration of 10 mg JCP 7 days before immunization with an i.p. injection of 0.1 mg JCP in complete Freunds adjuvant (CFA). The effects of oral JCP on systemic immunity were assessed by enzyme-linked immunosorbent assay (ELISA) of immunoglobulin (Ig) levels in serum collected on day 7 or 14 after immunization. Oral tolerance to JCP was adequately induced on day 7 after immunization and was more effective in C3H/HeN mice than in BALB/c mice. The tolerance was primarily concerned with the decreased serum levels of antigen-specific IgG. In these mice, oral administration of JCP also suppressed various immune responses to the antigen including delayed-type hypersensitivity (DTH), total Igl level and anti-JCP IgGl level. The suppression of these immune responses by the oral antigen was associated with a significant reduction in interleukin-4 (IL-4) production. These findings therefore indicate that this C3H/HeN mice model has potential use in detecting the induction of oral tolerance by JCP and suggest that this tolerance model may be effective in the treatment and prevention of allergic responses caused by the antigen.

        Experimental Study on the Effects of GamiSamgieum (SGMX) on Hyperlipidemia

        Objectives : This study was aimed to investigate the effects of GamiSamgieum (SGMX) on hyperlipidemia using an animal model. Additionally, the underlying mechanisms were investigated by determination of gene expressions related with lipid metabolism. Methods : Forty mice were selected for use in this experiment, and then divided into five groups; Naive, Induced, SGMX 100, SGMX 200, and Lovastatin group as positive control with 8 mice each, and their blood was collected for analysis of blood and serum parameters. Results : Administration of SGMX significantly inhibited the increase of liver weight and the macrovacuolar cytoplasmic alterations in histopathologic finding. SGMX administration significantly protected the liver from pathologic elevation of AST and ALT and from lipid peroxidation. SGMX administration significantly lowered total cholesterol levels and TG, and partially upregulated the gene expression of LCAT, CYP7A1 and LDL-R receptor. Conclusion : SGMX is suggested to possess hypolipidemic effect, and thus could be a potential candidate for herbal hypolipidemic via further studies.
