Brachymetatarsia frequently affects the fourth toe, and is more common in females, The etiology is obscure but the premature closure of epiphysis results in early union and cessation of metatarsal growth. The abnormally short metatarsus is both clinically and radiographically evident with an associated shortened and contracted toe. The deformity is usually asymptomatic but produces a serious cosmetic and psychological problems. For the corrction fo brachymetatarsia, many different surgical alternatives have been presented. We adopted the modified Jinnaka's method : interposing the autogeneous cranial bone graft within the metatarsophalangeal joint of the metatarsophalangeal joint for reducing the tension dorsally on the digit. Postoperatively the short leg splont was removed two weeks after the operation. During the next four weeks, the walking cast was applied and weight bearing was begun. During recent 4 years, 9 brachymetatarsia developed in the 4th toe in 5 patients were treated with this technique and good postoperative results were achieved in cosmetic and functinal aspects. This method has following merits. 1. The cranial bone is less absorbable than the rib or iliac crest. 2. The lengthening of the dorsal skin is easily obtained by means of Z-plasty over the dorsal skin of the metatarsophalangeal joint. 3. By interposing a bone graft within the metatarsophalangeal joint, bone lengthening of two centimenters or more could be achieved and the patients could walk without fracture of the grafted bone except one toe, in which pseudojoint is formed with normal gait.

Thin amorphous carbon films were deposited by a Q-switched Nd;YAG 532nm with beam power density of ?? on the high purity graphite (99.7%). The pressure of vacuum chamber was ?? Torr. In order to estimate the quality of the Pulsed Laser Deposition films one of the most important optical properties, bandgap energy, was characterized by transmission and reflection in the range of the visible, and an optical direct bandgap energy of 2.45eV and indirect bandgap energy 0.36eV were obtained. Surface morphology of amorphous film was investigated by AFM (Atomic Force Microscopy). Its surface roughness is 70nm. Comparing our results with the published values in the literature we have confidence that our films show the good quality for optical properties.

<P>Antigenic variation in viral surface antigens is a strategy for escaping the host’s adaptive immunity, whereas regions with pivotal functions for infection are less subject to antigenic variability. We hypothesized that genetically invariable and immunologically dormant regions of a viral surface antigen could be exposed to the host immune system and activated by rendering them susceptible to antigen-processing machinery in professional antigen-presenting cells (APCs). Considering the frequent antigen drift and shift in influenza viruses, we identified and used structural modeling to evaluate the conserved regions on the influenza hemagglutinin (HA) surface as potential epitopes. Mutant viruses containing the cleavage motifs of cathepsin S within HA were generated. Immunization of mice showed that the mutant, but not the wild-type virus, elicited specific antibodies against the cryptic epitope. Those antibodies were purified, and specific binding to HA was confirmed. These results suggest that an unnatural immune response can be elicited through the processing of target antigens in APCs, followed by presentation <I>via</I> the major histocompatibility complex, if not subjected to regulatory pathways. By harnessing the antigen-processing machinery, our study shows a proof-of-principle for designer vaccines with increased efficacy and safety by either activating cryptic, or inactivating naturally occurring, epitopes of viral antigens.—Lee, Y. <P>Calcium and integrin binding protein 1 (CIB1) is a Ca(2+)-binding protein of 22 kDa that was initially identified as a protein that interacts with integrin alpha(IIb). Although it interacts with various proteins and has been implicated in diverse cellular functions, the molecular mechanism by which CIB1 regulates intracellular signaling networks has remained unclear. We now show that, by targeting apoptosis signal-regulating kinase 1 (ASK1), CIB1 negatively regulates stress-activated MAPK signaling pathways. CIB1 was thus shown to bind to ASK1, to interfere with the recruitment of TRAF2 to ASK1, and to inhibit the autophosphorylation of ASK1 on threonine-838, thereby blocking ASK1 activation. Furthermore, CIB1 mitigated apoptotic cell death initiated either by TNF-alpha in breast cancer MCF7 cells or by 6-hydroxydopamine (6-OHDA) in dopaminergic cells. Ca(2+) influx induced by membrane depolarization reversed the inhibitory effect of CIB1 on 6-OHDA-induced ASK1 activation and cell death in dopaminergic neurons. These observations thus suggest that CIB1 functions as a Ca(2+)-sensitive negative regulator of ASK1-mediated signaling events.</P>
EMI Noise Reduction with New Active Zero State PWM for Integrated Dynamic Brake Systems

Based on the application of an integrated dynamic brake (IDB) system that uses a PWM inverter fed-AC motor drive to operate the piston, a new active zero state PWM (AZSPWM) is proposed to improve the stability and reliability of the IDB system by suppressing the conducted electro-magnetic interference (EMI) noise under a wide range of load torque. The new AZSPWM reduces common-mode voltage (CMV) by one-third when compared to that of the conventional space vector PWM (CSVPWM). Although this method slightly increases the output current ripple by reducing the CMV, like the CSVPWM, it can be used within the full range of the load torque. Further, unlike other reduced common-mode voltage (RCMV) PWMs, it does not increase the switching power loss. A theoretical analysis is presented and experiments are performed to demonstrate the effectiveness of this method.

During the 10-year period up to December 1984, 176 patients with anterior communicating aneurysm (ACOMA) among total 467 patients of intracranial aneurysms were admitted to this Catholic Medical Center. Of these, 135 cases of ACOMA were operated by direct intracranial procedures. To analyse the factors influencing the mortality involving in surgery of 135 patients with ACOMA, a classification of ACOMA was attempted. The origins and projections of aneurysms, anatomical variations were analysed with the aid of angiography, intraoperative findings and intraoperative photographs which permitted the establishment of a classification of ACOMA with their direction. Our classification of operated 135 cases of ACOMA are seven types: 32 anterior(23.7%), 30 anterior-rostral(22.2%), 42 anterior-caudal (31.1%), 3 posterior(2.2%), 18 posterior-rostral(13.4%), 4 posteriorcaudal (3.0%), caudal 6(4.4%), respectively. Most of ACOMA projecting anteriorly, anterio-rostrally were situated above or between the optic nerve, and the less frequent posterior-caudal and inferior aneurysms were in close proximity to hypothalamic branches of the anterior communicating artery and A₂ segment. There was on overall operative mortality of 6%. The surgical morbidity and mortality were significantly higher in the posteriorly projecting group, garticulary in the posterior-caudal direction. In direct surgery of ACOMA, subpial resection of the gyrus rectus was sffective for not only anteriorly, but also posteriorly directing aneurysms.
