        수정체유화술에서 프리챠퍼 사용이 초음파 에너지량의 감소에 미치는 영향

        Purpose: To compare the effectiveness and safety of the phaco prechopper technique dividing lenses into 2 or 4 pieces before phacoemulsification with the effectiveness and safety of conventional phacoemulsification during cataract surgery. Methods: This study included 360 eyes, which were divided into 4 groups according to nuclear opacity each group was subdivided into 3 groups according to the lens extraction technique (control, dividing the lens into two pieces using a phaco prechopper, and dividing it into four pieces) (Table 1). The following parameters were compared between the groups that had the same degree of nuclear opacity: changes in the endothelial cell count and hexagonality 2 months postoperatively, phaco time as well as the total phaco energy used for the operation, and the complication rates. Results: There were no intra- and post-operative complications in any group. The endothelial cell loss rate was significantly less in nuclear opacity groups 2 and 3 with the prechopper technique than in the control group. Hexagonality was significantly higher in the nuclear opacity group 3 with the prechopper technique than in the control 2 months after the operation. Phaco time was significantly less in the nuclear opacity groups 2 and 3 with the prechopper technique than in the control. Total phaco energy was significantly less in the nuclear opacity groups 2, 3, and 4 with the prechopper technique than in the control. Conclusions: Phacoemulsification using the prechopper technique is safe, resulting in significantly less endothelial cell damage, and requires less phaco time and less total phaco energy compared to those of the conventional phacoemulsification technique.

        콘택트렌즈와 관련된 감염각막염

        굴절교정, 근시진행 억제, 그리고 미용 목적 등 콘택트렌즈의 사용은 꾸준히 증가하고 있다. 콘택트렌즈의 사용은 각막표면에 여러가지 영향을 주게 되는데, 눈물흐름의 방해, 각막상피세포의 미세손상, 각막의 저산소, 각막 면역 변화, 그리고 오염물질에의 노출 등으로 감염에 의한 각막감염이 발생할 수 있다. 콘택트렌즈를 착용하던 환자가 각막염으로 내원 시 감염인지 비감염인지 감별하고, 감염이라면 원인 균주를 찾고 적절한 치료를 신속하게 하는 것이 예후에 중요하다. 또한 부적절한 렌즈 관리가 감염 유발과 관련이 있으므로 적절한 환자 교육을 시행하도록 하고, 정기적인 안과 진료를 통해 콘택트렌즈로 인한 감염의 위험성을 줄여야 하겠다. The use of contact lenses for correcting refraction, suppressing myopia progression, and cosmetic purposes is increasing steadily. Contact lenses have various effects on the corneal surface and corneal infection can occur following obstruction of tear flow, micro-damage to corneal epithelial cells, corneal hypoxia, changes in corneal immunity, and exposure to contaminants. When a patient who used to wear contact lenses presents with keratitis, it is important to distinguish whether it is an infection; if it is an infection, it is important to find the causative strain and promptly treat it appropriately. Since improper lens care is related to infection, appropriate patient education is necessary, and the risk of contact lens infection should be reduced through regular ophthalmic examinations.

        비천공성 안외상의 통계적 고찰

        Purpose: To investigate the clinical and social characteristics of non-penetrating ocular injuries and use the results for treatment and prevention. Methods: A retrospective survey was performed on 468 eyes of 421 patients, who visited our clinic due to non-penetrating ocular injuries from January 2010 to December 2010. The incidence of ocular injuries, sex, age, monthly and seasonal distribution, side, cause, influence of alcohol, and change in visual acuity were reviewed statistically. Results: The incidence of non-penetrating ocular injuries was 4.9%. The incidence was more common in males (82.9%), in the 3rd decade of life (23.5%) and in summer (30.8%). Minor injuries (36.1%) were the most common caused by scratches, finger pricks, nails, or small foreign bodies. The second common cause was violence (16.4%), followed by traffic accidents (14.7%). The injuries related to alcohol represented 12.6% of all cases. When considering initial diagnoses, periorbital contusion and corneal abrasion were the most common. The initial visual acuity was less than 20/200 in 9.4% of the eyes and the final acuity after a 6-months of treatment was less than 20/200 in 3.0% of the eyes. Conclusions: This survey gathered the clinical and social characteristics as well as new insights into non-penetrating ocular injuries which can be used for treatment and prevention.

        577 nm 레이저 섬유주성형술의 단기 임상효과

        Purpose: To evaluate the pressure-lowering effects of single-spot laser trabeculoplasty and patterned laser trabeculoplasty using a 577-nm wavelength laser. Methods: A total 35 eyes of 35 patients with primary open-angle glaucoma were enrolled in this study. Eighteen eyes of 18 patients were treated with 360° single-spot laser trabeculoplasty and 17 eyes of 17 patients were treated with 360° patterned laser trabeculoplasty. All patients were evaluated after laser trabeculoplasty at 1 week, 1 month, 3 months, and 6 months using slit lamp examination and Goldmann applanation tonometry. Results: At 6 months postoperatively, the single-spot laser trabeculoplasty group had a mean IOP of 15.89 ± 3.89 mm Hg with a mean IOP reduction of 37.1%, while the patterned laser trabeculoplasty group had a mean IOP of 17.57 ± 2.64 mm Hg with a mean IOP reduction of 27.1%. Conclusions: Laser trabeculoplasty with a 577-nm optically pumped semiconductor laser was safe and demonstrated an IOP lowering effect. There were no significant differences in the IOP lowering effects between the single-spot laser trabeculoplasty and the patterned laser trabeculoplasty. J Korean Ophthalmol Soc 2014;55(4):563-569

        마이토마이신을 병용한 치료적 레이저각막절제술을 이용한 라식 후 발생한 상피내생의 치료

        Purpose: To report 2 cases of thick, late-onset and long-standing epithelial ingrowth after laser-assisted in situ keratomilusis (LASIK) using mitomycin C-assisted photorefractive keratectomy and flap suturing. Case summary: The first case involved a 29-year-old woman who had LASIK in both eyes in 2002. She was diagnosed with epithelial ingrowth OD after blunt ocular trauma in 2007. The second case involved a 30-year-old man who had LASIK in both eyes in 1999 followed by enhancements in 2004. Slit lamp examination showed dense epithelial ingrowth OS in a geographic pattern. Two eyes of the 2 patients with epithelial ingrowth had flap lifting followed by debridement of interface epithelial cells, photorefractive keratectomy, interface application of mitomycin C, and the placement of interrupted 10-0 nylon sutures. Two eyes regained better uncorrected visual acuity and showed reduced corneal astigmatism postoperatively. There was no evidence of residual or recurrent ingrowth throughout the follow-up period (case 1: 11 months, case 2: 1 month). Conclusions: Combined mitomycin C-assisted photorefractive keratectomy and flap suturing can be an effective and safe method for treating and preventing the recurrence of thick, late-onset and long-standing epithelial ingrowth involving the visual axis.         우세안과 비우세안의 망막신경섬유층 두께, 시신경유두 형태 비교

        Purpose: To investigate the comparison of retinal nerve fiber layer (RNFL) thickness and optic disc parameters measured by optical coherence tomography (Cirrus HD-OCT<sup>®) in dominant and non-dominant eyes. Methods: Seventy-one subjects without underlying ocular disease were recruited for the present study. Ocular dominance was determined using the hole-in-the-card test. Comprehensive standardized eye examinations were performed. Scans of the optic disc and RNFL were performed using OCT. Results: The mean intraocular pressure (IOP) of the dominant eye was higher than its counterpart (p = 0.025). No significant differences were observed in uncorrected visual acuity, refractive error and axial length between dominant and non-dominant eyes (p = 0.235, 0.180, 0.850). No RNFL and optic disc features were identified in the dominant from non-dominant eyes. Conclusions: Although dominant eyes tended to have higher IOP than non-dominant eyes, no consistent ocular structural differences between dominant and non-dominant eyes with the use of OCT were found.

        상대정맥증후군 환자에서 발생한 안허혈에 의한 신생혈관녹내장 1예

        Purpose: To report a case of neovascular glaucoma (NVG) with ocular ischemia in superior vena cava syndrome (SVCS). Case summary: A 57-year old male who had been treated for small cell lung cancer (SCLC) visited our hospital because of SVCS-like symptoms and decreased visual acuity in the right eye. On the initial examination, best corrected visual acuity was 0.5, intraocular pressure (IOP) was 38 mm Hg and relative afferent papillary defect was positive in the right eye. Slit lamp examination showed mild dilatation and tortuosity of the episcleral vessels and ruobeosis iridis. Gonioscopic examination showed neovascularization of the opened angle. Fluorescein angiography showed delayed choroidal filling and nevascularization of the disc. The patient was diagnosed with NVG with ocular ischemia in SVCS and was treated with chemotherapy and steroid therapy. In addition, intravitreal bevacizumab and IOP lowering eyedrops were administered to the right eye. During follow-up, neovascularizations disappeared and IOP was well controlled. Conclusions: NVG with ocular ischemia in SVCS should be considered as a possible cause of high IOP in SCLC patients. J Korean Ophthalmol Soc 2012;53(9):1346-1351

        엘립스 방식의 초음파유화기를 이용한 2.2 mm와 2.8 mm 절개 백내장 수술의 결과 비교

        Purpose: Introduction of phacoemulsification and development of foldable artificial lens has facilitated smaller incisions, even micro-coaxial incisions. However, there have been several studies showing that micro-coaxial incision has no benefit compared with the conventional small incision method. Cases where Ellips ultrasound was used have not yet been reported. Therefore, we compared the postoperative results between 2.2-mm and 2.8-mm incision groups using Ellips ultrasound. Methods: Among 49 eyes receiving cataract surgery from March, 2012 to August, 2012, 27 eyes in the 2.2-mm group and 22 eyes in the 2.8-mm group were examined to obtain cumulated dissipated energy (CDE), use of balanced salt solution (BSS), best-corrected visual acuity (BCVA), corneal endothelial cell count (ECC), corneal thickness at center and incision site, and keratometric astigmatism before and after surgery. Results: There were no statistically significant differences between the 2.2-mm and 2.8-mm groups in CDE (2.5 ± 2.0 vs. 2.5 ± 2.3) and use of BSS (188 ± 127 vs. 138 ± 43 mL) during the surgery, BCVA (-0.45 ± 0.62 vs. -0.55 ± 0.79 log MAR), ECC (-178 ± 210 vs. -99 ± 114 cells/mm²), corneal thickness at center (23 ± 23 vs. 27 ± 23 μm) and incision site (24 ± 19 vs. 27 ± 19 μm) and keratometric astigmatism before and after the surgery. Conclusions: A 2.2-mm micro-coaxial incision using Ellips ultrasound showed no statistically significant differences in BCVA, ECC, corneal thickness at center and incision site, and keratometric astigmatism compared with 2.8-mm small incision.         약제로 잘 조절되는 원발개방각녹내장에서 수정체초음파유화술이 안압에 미치는 장기적 영향

        Purpose: To investigate the long-term effect of phacoemulsification on intraocular pressure (IOP) in patients with medically controlled primary open-angle glaucoma (POAG). Methods: This is a retrospective review of 120 eyes with POAG who underwent uncomplicated phacoemulsification with longitudinal follow-up. Demographic, clinical, and biometric variables such as age, axial length, central corneal thickness, gonioscopic results, anterior chamber depth, IOP, number of glaucoma medications were evaluated at 1, 2, 3, and 4 years after surgery. Results: Postoperative IOP change through 1, 2, 3, and 4 years of -1.47 ± 2.59, -1.21 ± 2.48, -1.15 ± 1.68, and -0.98 ± 1.53 mmHg was statistically significant (p < 0.01). The number of glaucoma medications remained unchanged (p = 0.18). Higher preoperative IOP was significantly associated with greater postoperative decrease in IOP (p < 0.001). Four years after phacoemulsification, 81.3% of the POAG eyes maintained the same number of glaucoma medications while 14.6% had a decrease and 5.1% of the eyes required additional glaucoma medications. Conclusions: After 4 years of phacoemulsification in patients with medically controlled POAG resulted in a very small IOP decrease without a change in the number of glaucoma medications. These findings have important implications when considering cataract surgery in POAG eyes for IOP reduction.
